DescInpatient Services - The Uniontown Hospital is a 179 bed community hospital that delivered state-of-the art primary inpatient services to approx. 8,400 inpatients during the FYE 6/30/15. Since 1903, the Hospital has been serving the greater Uniontown and Fayette County area, providing quality healthcare regardless of the persons ability to pay. The Hospital has earned several recognitions from HealthGrades, the nations leading independent healthcare ratings organization. The Hospital draws the majority of its patients from Fayette County. Although the population of Fayette County has been decreasing, the demand for the Hospitals services remain due to the downsizing and service cuts at nearby facilities, the growth in the regions uninsured population, and the increase in the regions elderly population. The Uniontown Hospital offers lines of services that are unavailable elsewhere in Fayette County. In addition to being the primary source of orthopedic care, it is the only hospital to offer obstetrics, interventional cardiology and radiation oncology services.Desc

Group Uniontown Hospital Line Part V, Section B, Line 11The Uniontown Hospitals full Community Health Needs Assessment and Implementation Plan can be found at www.uniontownhospital.com. Community Health Needs Assessment Implementation Plan Progress Year 2 FYE 6/30/15. Topic Decrease the presence of uninsured residents in the community. Actions One a We helped 6 of all the self pay patients seen at our facility to become enrolled in medical assistance in FY 15. b There was a decrease of 4 in the Medical Assistance and self-pay patients seen in the emergency department from FY 14 to FY 15. Two a Identified opportunity to enhance the Financial Care Services Department by outsourcing consultant activities to create a higher level and a more timely patient interaction model. To be implemented in Year 3. b Identified opportunities to interact with self pay patients during their emergency department visit in order to help them determine eligibility for insurance options. To be implemented in Year 3. c Determined a redesign of our charity care program was necessary to meet the needs in the new healthcare coverage environment. To be implemented in Year 3. d Year 1 activities continue with the above opportunities addressed and ready for implementation. Three a There was a 30 decrease in self pay patients from FY 14 to FY 15. b Even though the volume of self pay patients decreased FY 15, we increased the percentage of the number of patients that we helped successfully apply for Medical Assistance by 1 from FY 14.
Group Uniontown Hospital Line Part V, Section B, Line 11Topic Increase the appropriate use of available healthcare resources by increasing the use of affordable primary care clinics and FQHC whereby reducing the use of the Emergency Departments for non-emergent and/or ongoing primary care purposes. Actions One a Continued partnership with Cornerstone Care, Centerville Clinics and Wesley Healthcare Center to provide resources for self-pay patients to seek affordable healthcare. b The number of non-emergency patients seen in the emergency department decreased by 14 between FY 14 and FY 15. Two Efforts established in year 1 continue. Three There was a 23 decrease of self pay patients seen in the emergency department in FY 15 as there was in FY 14.
Group Uniontown Hospital Line Part V, Section B, Line 11Topic Improve the health and birth outcomes for women with chemical dependecny. Actions One The county has decided to begin a national program with grant money called Communities That Care to address these issues. Uniontown Hospital personnel have attended several meetings for the initiation process. Two Networking nationally regarding prevalence and best evidence based treatment. Three Care of newborn withdrawing policy was changed to increase the mother baby interaction and education. Involvement of the parents better prepares them for a difficult baby once discharged and enhances bonding. Four Our pediatric medical staff determined that mothers taking rehabilitative drugs may breastfeed but marijuana and other illegal substances not recommended, in accordance with the American Academy of Pediatrics policy. Five FBBC is investigating the national trend of initial treat for Neonatal Abstinence Syndrome to be performed in the newborn nursery verses transfer to tertiary care. Decision to transfer them if initial treatment is not effective.
Group Uniontown Hospital Line Part V, Section B, Line 11Three needs identified as not being met 1. Lifestyle choices of consumers While hospital leaders are interested in this issue, there is a Healthy Lifestyles Committee that is participating in the planning process from the Fayette County CHIP Program. The Hospital collaborates and works closely with the CHIP program, which has chosen to take responsibility for addressing the choices of residents that impact health. Additionally, the hospital will study and identify methods for addressing substance abuse issues among pregnant women, which is as a component of lifestyle choices and an area the hospital may have a measure of influence. 2. Dental Care The hospital does not have access to the resources needed to provide dental care to residents in Fayette County. The hospital does not offer dental care and has limited leverage to influence dental providers in the community. 3. Socio-economic barriers The hospital does not have access to the resources needed to address the socia-economic issues in Fayette County. The hospital has helped facilitate the formulation of a community development committee, which has chosen to address the socio-economic issues in Fayette County. A seperate community development plan was developed during this planning process and may begin to address some of these issues.
Group Uniontown Hospital Line Part V, Section B, Line 16iThere is a paragraph on the patient bill discussing free or discounted services and directing patients to the proper location to see the FAP. Brochures are available in the emergency waiting room, Patient Financial Services and admissions office which discuss the FAP.
Group Uniontown Hospital Line Part V, Section B, Line 22dCharges are established using cost as the principle basis. A relevant cost basis is established for each service on an annual basis. The charge will be initially established based upon 150 of the cost of the service. Prior to eligibility for financial assistance, a patient will be billed gross charges. Individuals eligible for assistance under the Hospitals financial assistance program will not be charged more than the amount generally billed to individuals having insurance coverage. At the Uniontown Hospital the AGB is determined through the lool-back method which is calculated as follows The AGB is calculated by reviewing all past claims that have been paid in full to the hospital facility for medically necessary care by Medicare fee-for-service together with all private health insurers paying claims to the hospital in a prior 12-month period. This amount can include co-insurance copayments and deductibles. The AGB for emergency or medically necessary care provided to a financial assistance eligible individual is determined by multiplying gross charges for that care by one or more percentages of gross charges called AGB percentages. The percentages are applied by the 45th day after the end of the 12-month period Uniontown Hospital used in calculating the AGB percentages. Any financial assistance eligible patient that paid over the AGB will be refunded. Additional information Financial assistance free or discounted adjusted amounts will be applied to gross charges. Based on the poverty income guidelines, free assistance will be provided at 100 for incomes less than 200 of the poverty income guidelines. Discounted assistance will be provided at 66 for incomes greater than 200 but less than 250 and 33 for incomes greater than 250 but less than 300. There will be a minimum patient liability amount required for each service. Payment of the following minimum patient liability amounts will be required for each service 10 Outpatient diagnostic testing 25 Emergency department 50 Same Day Surgery 100 Observation and Inpatient services.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part I Line 7Uniontown Hospital used the best available data to calculate the cost of charity care reported in line 7 for certain categories, the best available data was derived from a cost accounting system in other categories, specific cost-to-charge ratios from worksheet 2 were calculated and applied to that category.
Part I Line 7column f - The amount of expenditures used to calculate column f were reduced by bad debt expense of 3,464,221.
Part III Line 2The gross charges of Hospital provided services to individuals whom were determined to be ineligible under the Hospitals financial assistance policy for which payments were not received. These charges have been determined uncollectible by management.
Part III Line 3Line 3 was estimated by reviewing data from the PA Department of Health, Bureau of Health Statistics and Research to determine the percentage of the population in the Hospitals service area that falls below federal poverty guidelines. This percentage was then adjusted by subtracting the Hospitals average percentage of individuals eligible under the organizations financial assistance policy. The resulting percentage was applied to line 2 to estimate line 3. Rationale for including bad debt amounts in community benefit It is the Hospitals contention that the cost of bad debt should by stated in Part 1 Line 7 of Schedule H as they represent the costs for provision of services to patients for which the entity has exhausted all recourse for reimbursement. The services provided to patients who present themselves are provided regardless of a patients ability to pay as in line with the organizations charitable mission and service to our community.
Part III Line 4The audited financial statements do not contain a footnote that describes bad debt expense. They do, however, contain a footnote that describes patient accounts receivable. That footnote reads as follows Accounts receivable are reduced by an allowance for doubtful accounts. In evaluating the collectability of accounts receivable, the Health System analyzes its past history and identifies trends for each of its major payer sources of revenue to estimate the approriate allowance for doubtful accounts and provision for uncollectible accounts. Management regularly reviews data about these major payer sources of revenue in evaluating the sufficiency of the allowance for doubtful accounts. The difference between the standard rates or the discounted rates if negotiated or provided by policy and the amounts actually collected after all reasonable collection efforts have been exhausted is charged off against the allowance for doubtful accounts. See page 9 of the attached financial statements for the patient accounts receivable footnote in its entirety.
Part III Line 8Serving patients with government health benefits, such as Medicare, is a component of the community benefit standard that tax-exempt hospitals are held to. This implies that serving Medicare patients is a community benefit and that the hospital operates to promote the health of the community. The Medicare cost report was utilized to determine the amount reported on line 6.
Part III Line 9bThe Uniontown Hospitals Revenue Cycle Principles and Policies include seven policies approved by their Board of Directors. One of the policies is titled Financial Need Discounting and establishes discounts ranging from 0 to 100 based upon the patients income as compared to the federal poverty guidelines after the payment of a Minimum Patient Liability. The Minimum Patient Liability ranges from 10-100 depending on the service. According to the policy titled Patient Payment Expectations, patients are given four monthly billing statements to resolve their financial obligation. Each statement will indicate that the patient needs to resolve their financial obligation to avoid the potential for future service delays. The patient has 120 days from the date of the initial billing statement to resolve their financial obligation. Patients are given multiple options to make payment including major credit/debit cards. The organization actively pursues collection of accounts in default unresolved for greater than 120 days in a way consistent with a relationship built on trust and compassion. Insensitive collection practices are avoided and collection agencies are only used in situations where contact cannot be made with the patient. Patients that have accounts in default may be asked to take financial accountability before scheduling non-emergency services. Collection activities do not include judgments.
Part VI Line 2The Fayette Regional Health System leadership team develops a strategic plan every three years. This strategic plan establishes several priorities for the Uniontown Hospital, all of which are focused on meeting the ever changing healthcare needs of the community. The leadership team develops this strategic plan with knowledge gathered from direct contact from patients patient satisfaction surveys outside consultants professional health care groups such as the Hospital Association of Pennsylvania and the Healthcare Financial Management Association and research obtained from various sources including the federal and state government and various other outreach programs and sources.
Part VI Line 3As part of the Revenue Cycle policy entitled Service Access, it states under Patient Advocacy that the health care system will actively assist all individuals in seeking resolution to their financial obligations. This includes helping them understand what services are covered by their health insurance, providing assistance in obtaining health insurance and helping seek eligibility for financial assistance. Convenient office hours, in a convenient location, will be established for patients seeking assistance. Currently this program is called the Safety Net Access Program SNAP. SNAP is available to all residents of Fayette County and patients will be assisted by Financial Care Consultants that are part of the Patient Financial Services department. The Hospital has brochures located in its waiting rooms referencing all financial services and patient obligations. In addition, the Patient Access staff makes the brochures readily available to the population at the time of registration.
Part VI Line 4The Uniontown Hospital is a 190 bed community hospital which is located 50 miles southeast of Pittsburgh in the City of Uniontown, Fayette County. Since 1903 the Hospital has been serving the greater Uniontown and Fayette County area, providing quality healthcare regardless of a persons ability to pay. The approximate population of Fayette County is 136,000. The current unemployment rate is 9.3. Approximately 19 of the population is over 65 with 18.7 of the population below the poverty level and 25.2 of the population eligible for Medical Asistance. Source Pennsylvania Department of Health 2014 Fayette County Health Profile.
Part VI Line 5The general health and well being of the community residents is central to the mission of Uniontown Hospital. The Hospital responds to the healthcare needs of the community with a number of community education and outreach programs. These outreach services include a Community Outreach department, support groups to help patients and their families cope with debilitating disease and injury and health fairs offering free health information and screenings. Support groups include Caner Support, Diabetes Support, Breast Feeding Support, Pregnant Moms Club, Patient Support group for Pregnancy Infant Loss and Caregivers Support. Additionally numerous specialized education classes are offered free of charge to the public and the Hospital sponsors Blood Drives with the American Red Cross and outreach activities with the American Heart Association. Also, in June 2004 the Hospital joined forces with health organizations and other community groups to organize the Fayette County Community Health Improvement Partnership CHIP. CHIP has initiated numerous programs to benefit the health of the public such as a student immunization program, a program to reduce teen pregnancy and smoking, and serveral health issue related task forces. Under the auspice of CHIP is Fayette County STEPS initiative which is a national pilot program funded by the Centers for Disease Control which targets chronic disease prevention. The STEPS program builds partnerships among area school districts, workplaces, doctor offices, and neighborhood coalitions to educate and promote healthier lifestyles.
Part VI Line 6Fayette Regional Health System FRHS is a registered 501c 3 Pennsylvania nonprofit charitable organization that serves as the parent company for Uniontown Hospital Hospital.
