AN ADDITIONAL NEED WAS IDENTIFIED AT HONORHEALTH JOHN C. LINCOLN MEDICAL CENTER AND HONORHEALTH SCOTTSDALE OSBORN MEDICAL CENTER, ADDRESSING THE SOCIAL DETERMINANTS OF HEALTH. HONORHEALTH ALIGNED THE IMPLEMENTATION STRATEGY WITH THE ORGANIZATIONS STRATEGIC PLAN, WITH A FOCUS ON POPULATION HEALTH MANAGEMENT. THE IMPLEMENTATION STRATEGY WAS APPROVED BY THE BOARD OF DIRECTORS FOR HONORHEALTH ON APRIL 26, 2016. TO ADDRESS THE MENTAL HEALTH AND SUBSTANCE ABUSE NEEDS OF THE COMMUNITY, HONORHEALTH IS WORKING WITH A NATIONAL PROVIDER OF BEHAVIORAL HEALTH SERVICES TO DEVELOP A COMPREHENSIVE, INTEGRATED BEHAVIORAL HEALTH SERVICE PLAN. THIS PLAN WILL BE IMPLEMENTED IN THREE PHASES TO ADDRESS THE MENTAL HEALTH NEEDS OF HONORHEALTH PATIENTS AT ALL ACUTE CARE HOSPITALS AND THE HONORHEALTH MEDICAL GROUP. HONORHEALTH HAS SEVERAL INITIATIVES TO COORDINATE CARE FOR PATIENTS AND COMMUNITY MEMBERS ACROSS THE CONTINUUM OF CARE. THESE INITIATIVES WILL ENSURE PATIENTS RECEIVE THE RIGHT CARE AT THE APPROPRIATE TIME, WITH THE GOAL OF REDUCING HOSPITAL READMISSIONS. HONORHEALTH WILL CONTINUE TO PROVIDE SCREENINGS FOR EARLY DETECTION OF DISEASE. IN ADDITION, HONORHEALTH IS DEVELOPING INITIATIVES TARGETED TOWARDS SENIORS. THESE INITIATIVES WILL PROVIDE SCREENINGS AS WELL AS SUPPORT SERVICES AND PROGRAMS TO HELP OLDER COMMUNITY MEMBERS WITH HEALTHY AGING. IN ADDITION, HONORHEALTH HAS PARTNERED WITH SCOTTSDALE FIRE DEPARTMENTS TO CREATE A MOBILE INTEGRATED HEALTHCARE PRACTICE. THIS PROGRAM HELPS TO MEET THE SOCIAL AND HEALTHCARE NEEDS OF INDIVIDUALS WHO OVER UTILIZE 911 AND EMERGENCY DEPARTMENT CARE. IN ADDITION TO ADDRESSING MENTAL HEALTH NEEDS, THIS PROGRAM WILL ADDRESS OTHER NEEDS IDENTIFIED IN THE COMMUNITY HEALTH NEEDS ASSESSMENT. HONORHEALTH WILL ALSO ADDRESS THE NEEDS THROUGH THE PROGRAMS OF DESERT MISSION, INC. AND THROUGH THE PARTNERSHIP WITH NOAH. DESERT MISSION, INC. PROGRAMS WILL ADDRESS THE HEALTHCARE NEEDS OF THE COMMUNITY AT ALL STAGES OF LIFE, THROUGH THE LINCOLN LEARNING CENTER AND THE ADULT DAY HEALTH CARE PROGRAM. ALTHOUGH THERE ARE OTHER NEEDS WITHIN THE COMMUNITY, THESE FIVE WERE IDENTIFIED AS PRIORITY NEEDS BASED ON THE MAGNITUDE OF THE PROBLEM, HEALTH DISPARITIES, COMMUNITY FEEDBACK, FEASIBILITY OF HONORHEALTH TO ADDRESS THESE NEEDS AND THE CONSEQUENCES OF INACTION. THE FIVE IDENTIFIED NEEDS ARE NOT PRIORITIZED FURTHER BECAUSE EACH NEED HAS A DIFFERENT LEVEL OF IMPORTANCE BASED ON THE DIFFERENT CRITERIA USED TO IDENTIFY THE PRIORITY NEEDS. HONORHEALTH ACKNOWLEDGES THAT THERE ARE OTHER COMMUNITY NEEDS THAT ARE NOT BEING ADDRESSED IN THIS COMMUNITY HEALTH NEEDS ASSESSMENT. THESE INCLUDE COMMUNICABLE DISEASE PREVENTION, WOMENS AND CHILDREN HEALTH, PRENATAL CARE, UNINTENTIONAL INJURY PREVENTION, AND VIOLENCE. HONORHEALTH WILL CONTINUE TO PROVIDE PROGRAMS AND SERVICES THAT ADDRESS THESE NEEDS AS PART OF NORMAL OPERATIONS, BUT WILL FOCUS SPECIFICALLY ON DEVELOPING AND IDENTIFYING STRATEGIES AND SERVICES TO ADDRESS THE FIVE PRIORITY NEEDS. HONORHEALTH HAS ATTEMPTED TO ESTABLISH ROUTINE METRIC GATHERING FOR FUTURE EVALUATION OF THE IMPLEMENTATION STRATEGY. HOWEVER, DURING THE COLLECTION PERIOD, BARRIERS HAVE ARISEN. FOR EXAMPLE, DESPITE MULTIPLE ATTEMPTS, SOME OF THE METRICS IDENTIFIED IN THE IMPLEMENTATION STRATEGY HAVE NOT BEEN COLLECTED. IN OTHER INSTANCES, CHANGES IN STAFFING HAVE RESULTED IN THE DELAY OR CHANGE TO PROGRAMS TIED TO THE IMPLEMENTATION STRATEGY. DESPITE THESE SETBACKS, HONORHEALTH WILL CONTINUE TO IDENTIFY WAYS TO ADDRESS THE NEEDS OF THE COMMUNITY THROUGH PROGRAM DEVELOPMENT, PARTNER IDENTIFICATION AND COLLABORATION WITH THE COMMUNITY.

SCHEDULE H, PART III, LINE 2NET PATIENT ACCOUNTS RECEIVABLE AND NET PATIENT SERVICE REVENUE HAVE BEEN ADJUSTED TO THE ESTIMATED AMOUNTS EXPECTED TO BE RECEIVED. THESE ESTIMATED AMOUNTS ARE SUBJECT TO FURTHER ADJUSTMENTS UPON REVIEW BY THIRD-PARTY PAYORS. BAD DEBT IS DETERMINED BY THE PATIENT'S OUTSTANDING ACCOUNT BALANCE ON THE DAY THEIR ACCOUNT IS TRANSFERRED TO A BAD DEBT STATUS. THE OUTSTANDING ACCOUNT BALANCE CONSISTS OF GROSS REVENUE LESS ANY CONTRACTUAL ADJUSTMENTS AND PAYMENTS POSTED TO THE ACCOUNT. PAYMENTS MADE AFTER THE ACCOUNT IS IN A BAD DEBT STATUS ARE CONSIDERED RECOVERIES AND WILL REDUCE THE BAD DEBT AMOUNT WHEN PAYMENTS ARE RECEIVED. SCHEDULE H, PART III, LINE 3 THE COST OF BAD DEBT EXPENSE IS DETERMINED USING THE NETWORK'S CALCULATED COST TO CHARGE RATIO APPLIED TO REPORTED GROSS CHARGES WRITTEN OFF DURING THE YEAR. USING A SAMPLE OF ACCOUNTS WRITTEN OFF DURING THE YEAR, THE NETWORK HAS ESTIMATED THAT APPROXIMATELY 42% OF BAD DEBT WAS ATTRIBUTED TO PATIENTS ELIGIBLE UNDER THE NETWORK'S FINANCIAL ASSISTANCE POLICY. A NUMBER OF PATIENTS ARE TRULY UNABLE TO PAY THEIR OUT-OF-POCKET LIABILITY, BUT DO NOT COMPLETE THE PROCESS REQUIRED TO APPLY FOR FINANCIAL ASSISTANCE UNDER THE HOSPITAL'S FINANCIAL ASSISTANCE POLICY. THESE PATIENTS WOULD QUALIFY FOR CHARITY CARE IF THEY COMPLETED THE PAPERWORK, SO THE BAD DEBT EXPENSE ASSOCIATED WITH TREATING THEM IS TREATED AS COMMUNITY BENEFIT.
SCHEDULE H, PART III, LINE 4THE FOOTNOTE THAT DESCRIBES BAD DEBT IS ON PAGE 10 OF THE ATTACHED AUDITED FINANCIAL STATEMENTS.
SCHEDULE H, PART III, LINE 8THE AMOUNT ON PART III, LINE 6 IS FROM THE MEDICARE COST REPORTS FILED BY THE ORGANIZATION. THE ENTIRE SHORTFALL REPORTED ON PART III, LINE 7 IS A COMMUNITY BENEFIT. THE RATIONALE FOR INCLUDING MEDICARE LOSSES AS COMMUNITY BENEFIT LIES IN THE NETWORK'S BELIEF THAT, BASED ON IRS REVENUE RULING 69-545, SERVING PATIENTS WITH GOVERNMENT HEALTH BENEFITS SUCH AS MEDICARE IS AN INDICATOR THAT THE NETWORK'S HOSPITALS OPERATE TO PROMOTE THE HEALTH OF THE COMMUNITY AND THEREFORE PROVIDES A COMMUNITY BENEFIT. THE NETWORK ALSO BELIEVES THAT TAX-EXEMPT HOSPITALS PLAY A VITAL ROLE IN PROVIDING THE ELDERLY WITH ACCESS TO HEALTHCARE SERVICES THEY MIGHT OTHERWISE BE DENIED BY FOR-PROFIT AND SPECIALTY HOSPITALS THAT FOCUS ON HIGH-MARGIN SERVICES OR THAT WOULD HAVE TO BE PROVIDED DIRECTLY BY THE FEDERAL GOVERNMENT.
SCHEDULE H, PART III, LINE 9BPURSUANT TO HONORHEALTH'S FINANCIAL ASSISTANCE POLICY, HONORHEALTH WILL NOT PURSUE LEGAL ACTION FOR NON-PAYMENT OF BILLS AGAINST FINANCIAL ASSISTANCE PATIENTS WHO HAVE CLEARLY DEMONSTRATED THAT THEY HAVE NEITHER SUFFICIENT INCOME NOR ASSETS TO MEET THEIR FINANCIAL OBLIGATION. HONORHEALTH WILL NOT EXECUTE A LIEN BY FORCING THE SALE OR FORECLOSURE OF A FINANCIAL ASSISTANCE PATIENT'S PRIMARY RESIDENCE TO PAY FOR AN OUTSTANDING MEDICAL BILL. HONORHEALTH WILL NOT USE BODY ATTACHMENT TO REQUIRE THE FINANCIAL ASSISTANCE PATIENT OR RESPONSIBLE PARTY TO APPEAR IN COURT. HONORHEALTH MAY REPORT FINANCIAL ASSISTANCE PATIENTS TO CREDIT RATING AGENCIES WHEN THEY FAIL TO HONOR THEIR REDUCED PAYMENT PLANS AND THERE IS EVIDENCE THAT THE FINANCIAL ASSISTANCE PATIENT HAS SUFFICIENT INCOME OR ASSETS TO SATISFY HIS OBLIGATION. AS OF 2018, HONORHEALTH NO LONGER ENGAGES IN ANY EXTRAORDINARY COLLECTION ACTIONS.

SCHEDULE H, PART VI, LINE 7HONORHEALTH PUBLISHES A COMMUNITY BENEFIT REPORT ONLY IN THE STATE OF ARIZONA. THE STATE OF ARIZONA DOES NOT REQUIRE FILING.
Disclosures in accordance with Revenue Procedure 2015-21:HonorHealth Rehabilitation Hospital (Hospital) is compliant with the Section 501(r) statute and has made a good-faith effort to be fully compliant with the requirements of the Section 501(r) regulations. As part of this effort and with the assistance of an outside accounting firm, the Hospital reviewed its Financial Assistance Policy (FAP), plain language summary of the FAP (PLS), the FAP application, and 2015 Community Health Needs Assessment (CHNA) for any areas of improvement in September of 2018. During this self-evaluation process, the Hospital identified some temporary gaps in compliance with the regulations. As of the time of filing, the Hospital has revised its FAP, PLS and FAP application in order to address all of the identified issues. The Hospital has not identified any persons directly affected by the TEMPORARY GAPS IN COMPLIANCE. THEY related primarily to the specificity of information contained in documents that are not typically referenced by patients. the Hospital was providing financial assistance as part of a formal program that was clearly communicated to patients, and the patients of the Hospital were patients of HonorHealths other hospital facilities WHO received information about HonorHealths financial assistance program as part of their prior visits. Through THE Hospitals review of its FAP, IT identified the following gaps in compliance and areas where information could be more clearly communicated. (1) The FAP did not contain language specifying that individuals eligible for financial assistance under the policy would not be charged more for medically necessary care thaN the amount generally billed (AGB) to individuals who have insurance covering that care. The FAP also did not include a description of the method used to calculate AGB. (2) The FAP or FAP application did not list A telephone number and physical location where an individual can get information about, and request assistance with completing, the FAP application. (3) The FAP did not fully describe the actions, including the process, reasonable efforts taken and timeframes, Hospital or its collection agencies may take if a bill is not paid. The FAP also did not explicitly state the department responsible for ensuring the reasonable efforts requirements were met. (4) The FAP did not specify the individual name, practice group name or category of care of providers that GIVE medically necessary care in the hospital facilities and whether or not the providers are covered by Hospitals FAP. (5) A PLS was not available and thus did not contain all the required language. (6) The FAP, PLS and the FAP application, as well as their translations, were not all posted on the Hospitals website. (7) The PLS was not provided to all hospital patients as part of the intake or discharge process. (8) a PLS was not provided at least 30 days prior to initiating ECAS. The Hospital corrected the items above as follows. The Hospital began the process of revising the FAP and PLS for the above listed items immediately following the September review. A revised FAP and PLS containing improved information and correcting the areas of non-compliance was formally adopted by the Board in September of 2018. In addition, the PLS is being offered to all patients at registration, is included with all billing statements, And is available upon request and without charge in public locations in the hospital and by mail. In 2018, the hospital (as well as all hospital facilities in reporting group A) no longer engages in any ECAs. The revised FAP, PLS and related translated documents ARE available at the below website: https://www.honorhealth-rehab.com/referral-sources/financial-assistance/ As part of its review of the 2015 CHNA, the following gap in compliance was identified. (1) The most recently conducted CHNA was posted on the main HonorHealth website but was not posted on the Hospitals website. Only one CHNA has been conducted due to the effective date of when Hospital became subject to Section 501(r). The Hospital corrected the item by posting a link to CHNA on its website. https://www.honorhealth-rehab.com/about/ The Hospital will continue to monitor its compliance with the Section 501(r) regulations and procedures have been put in place to maintain compliance through periodic reviews.

Schedule J, Part I, Line 4BHONORHEALTH OFFERS CERTAIN EXECUTIVES A NON-QUALIFIED SUPPLEMENTAL EXECUTIVE RETIREMENT PLAN (SERP). IT IS INTENDED THAT THIS PLAN BE AN INELIGIBLE DEFERRED COMPENSATION PLAN UNDER THE PROVISIONS OF CODE SECTION 457(F) AND BE OPERATED IN COMPLIANCE WITH CODE SECTION 409A. THE DESIGN OF THE SERP IS SUCH THAT IT PROVIDES A MECHANISM FOR RESTORATION OF DEFERRED RETIREMENT THAT OTHERWISE WOULD BE LOST TO THE EXECUTIVES DUE TO MANDATORY CAP ON DEFERRALS WITHIN THE QUALIFIED RETIREMENT PLAN OFFERED TO OTHER EMPLOYEES OF HONORHEALTH. THE SERP IS ALSO DESIGNED TO DISCOURAGE EXECUTIVE TURNOVER, WHICH COULD HAMPER ORGANIZATIONAL STABILITY AND SUSTAINABILITY, THROUGH THE AGE AND SERVICE REQUIREMENTS THAT AN EXECUTIVE MUST MEET IN ORDER TO RECEIVE BENEFITS FROM THIS PLAN. THE ANNUAL VALUE OF EACH EXECUTIVE'S PARTICIPATION IN THE PLAN IS TAKEN INTO CONSIDERATION AS PART OF THE CALCULATION OF TOTAL COMPENSATION WHEN TESTED AGAINST THE MARKET FOR REASONABLENESS.
