ExplanationThe hospital is exempt from federal and state income taxes under section 501(c)(3) of the internal revenue code and therefore is not subject to federal income tax under current income tax regulations. Current accounting standards define the threshold for recognizing uncertain income tax return positions in the financial statements as "more likely than not" that the position is sustainable, based on its technical merits, and also provide guidance on the measurement, classification and disclosure of tax return positions in the financial statements. Management believes there is no impact on the accompanying financial statements related to uncertain income tax positions. The hospital is subject to routine audits by taxing jurisdictions; however, there are currently no audits for any tax periods in progress.ExplanationTxt

Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - Boone Memorial Hospital, Inc.. ASSEMBLED AN ADVISORY PANEL COMPRISED OF REPRESENTATIVES FROM COMMUNITY LEADERS, HEALTH CARE PROVIDERS, AND LOCAL CITIZENS TO ASSIST WITH THE ASSESSMENT PROCESS.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - BOONE MEMORIAL HOSPITAL, INC.. Boone Memorial Health (BMH) conducted its 2025 Community Health Needs Assessment (CHNA) covering its service area of Boone, Logan, Lincoln, and Kanawha counties in West Virginia. The CHNA report was adopted by the BMH Board of Directors in June 2025. The corresponding 2025-28 Community Health Improvement Plan (CHIP) was adopted by the Board in September 2025 and is available to the public at bmh.org. Based on CHNA findings, BMH identified three priority areas for the 2025-28 reporting cycle: 1. Behavioral Health and Substance Use Disorder. BMH operates Brighter Futures, a comprehensive substance use disorder program offering prevention, medication-assisted treatment (MAT), case management, therapy, peer recovery coaching, and career readiness services. BMH reduced barriers to treatment through MAT induction in the emergency department, mobile MAT services, and the Reverse the Cycle Comprehensive ED Substance Use Response Program. Peer recovery coaches are embedded in the ED, and BMH supports community crisis intervention services including HELP4WV and the Quick Response Team. BMH also invests in youth development and resilience programming and community-based mental health and stigma-reduction initiatives. 2. Chronic Disease Prevention and Management. BMH extended access to care through four rural health clinics, a specialty clinic, telehealth, mobile health services, and a Meds to Beds point-of-care medication education program. BMH implemented universal screening for social drivers of health (SDOH) and deployed care managers to connect patients with wraparound supports including transportation, medications, and nutrition assistance. Community benefit programs include diabetes education, the BMH Healthy Lifestyle Program, chronic disease self-management programming, and mobile community health screening events. BMH removes financial barriers to physical fitness for chronically ill patients by providing no-cost gym access to individuals enrolled in the Chronic Care Management program. This program supports evidence-based lifestyle interventions for conditions including diabetes, hypertension, heart disease, and obesity - conditions that are disproportionately prevalent in BMH's rural Appalachian service area. BMH also serves as a catalyst for grant funding and regional partnerships to bring healthcare investment to underserved communities. 3. Community Building. BMH launched the Market on Main, a farmers market and greenhouse initiative designed to address food insecurity, support local growers, and promote agritourism and nutrition education. The FARMacy program provides fresh, locally grown produce to patients experiencing food insecurity at no cost. The BMH Foundation for Community Health advances community and economic development by addressing social drivers of health through grantmaking, programming, and partnerships with local organizations. BMH also supports workforce development initiatives, including healthcare career exposure programs for students and clinical rotation partnerships with colleges and universities.

Schedule H, Part I, Line 3cPatients must provide their "Total Yearly Income" in order to be considered for financial assistance. a. Adults: If the patient is an adult, the term "Total Yearly Income" means the sum of the total yearly gross income of the patient and the patient's spouse. b. Minors: If the patient is a minor, the term "Total Yearly Income" means the sum of the total yearly gross income of the patient, the patient's mother and the patient's father. The patient, immediate family member or patient representative shall verify the patient's reported Total Yearly Income, as provided on the Application. The Total Yearly Income may be verified through any of the following mechanisms: a. Income Indicators. By the provision of third party financial documentation including IRS Form W-2, Wages and Tax Statement; pay check remittance; individual tax return; telephone verification by employer; bank statements; Social Security payment remittance; Worker's Compensation payment remittance; unemployment insurance payment notice; Unemployment Compensation Determination Letters; or other appropriate indicators of the patient's Total Yearly Income. b. Participation in a Benefit Program. By the provision of documentation showing current participation in a public benefit program such as Medicaid; Food Stamps; WIC; or other similar means tested programs. Proof of Participation in any of the above programs indicates that the patient has been deemed Financially Indigent and therefore, is not required to provide his or her income on the Assistance Application. c. In cases where the patient or responsible party is unable to provide third party verification of patient's Total Yearly Income, verification of the patient's Total Yearly Income can be done in either of the following ways: 1. Obtaining the patient's or responsible party's Written Attestation. By obtaining an Application signed by the patient or responsible party attesting to the veracity of the patient's Total Yearly Income information provided: or 2. Obtaining the patient's or responsible party's Verbal Attestation. Through the written attestation of the BMH employee completing the Assistance Application that the patient or responsible party verbally verified the patient's Total Yearly Income information provided. 3. In instances where the patient or responsible party is unable to provide the requested third party verification of patient's Total Yearly Income, the patient or responsible party is required to provide a reasonable explanation of why the patient or responsible party is unable to provide the required third party verification. Sound business practices will be employed to verify patient's attestation and supporting information. Unmarried expired patients may be deemed to have no income for purposes of calculation of Total Yearly Income. Documentation of Total Yearly Income is not required for expired patients; however, documentation of estate assets may be required. If an expired patient is married, the surviving spouse may apply for financial assistance. Patients, immediate family members or patient representatives applying for financial assistance must verify the number of people in the patient's household "Number in Household"). a. Adults: In calculating the Number in Household, include the patient, the patient's spouse, and any dependents (as defined by the Internal Revenue Code). b. Minors: In calculating the Number in Household, include the patient, the patient's mother, the patient's father, dependents of the patient's mother, and dependents of the patient's father. During the verification process, while information to determine a patient's Total Yearly Income is being collected, the patient may be treated as a private pay patient.

Schedule H, Part VI, Line 4 Community informationBMH serves primarily Boone, Logan, Lincoln, and Kanawha counties in West Virginia, with a combined service area population of approximately 251,506 as of 2023. Boone County, the organization's home county, had an estimated population of 21,312 in 2023, reflecting a decline of 14.3% since 2010 consistent with broader regional population trends. The median household income in Boone County was $56,152, with approximately 17.3% of residents living below the federal poverty level and 20.8% of children experiencing poverty. Approximately 21.6% of the population is aged 65 or older - a cohort that grew by more than 31% since 2010 - and the median age is 45.0 years, significantly older than the national median of 38.7 years. The region faces disproportionate health burdens relative to state and national benchmarks, including lower life expectancy, higher rates of chronic disease, and elevated social vulnerability, driven largely by the legacy of the coal economy, persistent poverty, and limited access to healthcare and economic opportunity.
Schedule H, Part VI, Line 5 Promotion of community healthTHE MAJORITY OF THE BOARD OF DIRECTORS ARE LOCAL, LIFELONG RESIDENTS. THE EMERGENCY ROOM IS ALWAYS AVAILABLE TO ALL PATIENTS, REGARDLESS OF ABILITY TO PAY. BOONE MEMORIAL HEALTH HAS MADE THE MOST RECENT COMMUNITY NEEDS ASSESSMENT REPORT AVAILABLE ON ITS WEBSITE FOR ALL PATIENTS TO HAVE EASY ACCESS TO READ. BMH OFFERS PATIENTS ACCESS TO STATE OF THE ART MRI EQUIPMENT SEVEN DAYS A WEEK WHICH PROVIDES TOP QUALITY CARE CLOSE TO HOME - A DIFFERENTIATING SERVICE LINE WHEN COMPARED TO OTHER CRITICAL ACCESS HOSPITALS THROUGHOUT WV. WE HAVE A FULLY DIGITAL RADIOLOGY DEPARTMENT WITH A PACS SYSTEM. OUR LAB HAS RECENTLY EXPANDED TO INCORPORATE ADDITIONAL TESTING EQUIPMENT. WE ALSO OPERATE MULTIPLE RURAL HEALTH CLINICS THROUGHOUT THE REGION WITH EXTENDED AND WEEKEND SERVICE HOURS TO HELP SERVE THE HEALTHCARE NEEDS OF OUR COMMUNITY. BMH IS COMMITTED TO ADDRESSING THE SOCIAL DETERMINANTS OF HEALTH THROUGH THE BMH FOUNDATION FOR COMMUNITY HEALTH. OUR COMPREHENSIVE HEALTH PREVENTION AND COMMUNITY BENEFIT PROGRAMS INCLUDE: THE BMH FARMACY PROGRAM, WHICH PROVIDES PATIENTS AND FAMILIES WITH FRESH, LOCALLY-GROWN FRUITS AND VEGETABLES AT NO COST TO SUPPORT FOOD SECURITY AND CHRONIC DISEASE PREVENTION; AND THE BMH FOODBOX PROGRAM, WHICH DELIVERS NUTRITIOUS FOOD DIRECTLY TO COMMUNITY MEMBERS FACING FOOD INSECURITY.
