DescRURAL CONNECTIVITY PROGRAM:The Rural Connectivity Program was designed to develop a sustainable model for affordable health information management and technology infrastructure, shared analytics, and technical support for rural healthcare facilities. The goal for the Rural Connectivity Program is to enable safety-net rural health clinics such as Rural Health Clinics (RHCs), Critical Access Hospitals (CAHs), Non-Certified/Independent Rural Health Clinics, and qualifying rural PPS hospitals to make data-driven improvements to patient care by leveraging two primary technologies: Analytics and Health Information Exchange.CRHC provides continual outreach and technical assistance to rural facilities to improve the engagement and use of analytics, which includes support of dashboards and connection to the health information exchange. CRHC emphasizes the importance of utilizing quality data to inform changes and improvements. Heres what we accomplished this year:Platform Additions and Upgrades - Launched the Child & Youth dashboard in the Community Analytics Platform Unlocking new data insights into pay for performance childrens health metrics via the Rural Well-Child KPI report. - Enhanced the Community Analytic Platforms Cancer Screening and Diagnosis report in July. - Introduced Innovaccer to empower healthcare organizations with actionable insights into cost and utilization trends. - Enrolled 12 new participating facilities to the Rural Connectivity Program, increasing the number of total facilities to 117 and total attributed members in the CAP to over 700,000 an increase of nearly 100,000 unique patients since December 2024.Expanded Technical Assistance & Shared Learning - Delivered remote and on-site technical support and training on dashboard access, usage, and visual analytics. - Facilitated Super User Group and presented on Rural Connectivity Programs impact on analytics and CRHCs role in rural quality improvement.Quality Improvement Initiatives and Financial Impact - Supported facilities projects through: - Identifying new service lines - Development and workflow optimization for cancer screenings and Chronic Care Management (CCM) programs - Improve data collection in practices - Provided Sustainability Payments to 111 eligible facilities. Each Critical Access Hospital and Rural Health Clinic who met the criteria received $100,000 and $20,000, respectively - Released two Innovaccer Modules: - Cost Management Module includes trends on cost and utilization by inpatient, outpatient, medication, and professional services - Risk Management Module includes data on payer performance, gap to potential risk, and RAF scoresDesc

DescSNAPSHOT OF RURAL HEALTH:As Colorados State Office of Rural Health and member-based association for rural health facilities, CRHC is a clearinghouse for rural health information and resources, helping providers throughout Colorado enhance healthcare services available in rural communities, and helping elected officials make informed decisions about healthcare policy. The 2025 edition of its Snapshot of Rural Health was released in January 2025. Each year, the publication highlights rural health data such as demographic trends, chronic disease, the rural healthcare delivery system, substance use, and the rural economy using infographic data visualization, case studies, text, and maps. The 2025 edition of the Snapshot is the largest volume yet. The report has expanded analysis of healthcare in rural Colorado, illustrated with useful statistics, data, and maps that can all be used for education, advocacy, and grant writing. This year, the report analyzes 29 different topics and features 55 maps, all in a digestible visual format with rigorous citations. The 2025 edition expanded coverage of chronic disease in rural Colorado and analyzes rural Emergency Medical Services (EMS) deserts while featuring unique data points for each region in Colorado. The Snapshot is designed from the ground up to be a useful tool for healthcare facilities, policy makers, providers, and members of the public. All of this is provided free of charge in the interest of advancing CRHCs vision of improving healthcare services available in rural communities to ensure that all rural Coloradans have access to comprehensive, affordable, high quality healthcare. The Snapshot of Rural Health includes data collected and analyzed by CRHCs Public Health Data Specialist. A comprehensive citations page makes sourcing each fact from the 104 page report quick and easy. The Snapshot of Rural Health is available for download at https://coruralhealth.org/snapshotof-rural-health - For additional resources, visit the CRHC resource center at http://coruralhealth.org/resources.Desc

DescADVOCACY:The 2025 Colorado legislative session convened on January 14, 2025, and ended 120 days later. This session was marked by a significant state budget shortfall, pushback to proposals from the federal Administration, and a historical battle between Colorado hospitals and the pharmaceutical industry. Of the 657 bills introduced, 476 bills (72%) passed, and Governor Polis vetoed 11 bills. The Colorado Rural Health Center (CRHC) tracked 93 bills and took positions on 25 bills with 93% of the bills CRHC supported passing. The Policy and Legislative Committee is a board lead committee with CRHC Director of Policy and Advocacy providing support for this committee including tracking bills pertinent to rural communities in Colorado. CRHC bill positions are decided by majority vote of the CRHC Policy and Legislative Committee (PLC) and approved by the CRHC Board of Directors. CRHC uses a bill tracker which contains links to Colorado Capitol Watch, where you can find the most recent bill text, fiscal note, bill history and bill sponsors. CRHC regularly publishes policy information in the monthly Special Delivery to inform members, partners and collaborators on policy related information. CRHC tracked 93 bills this session, voting to take positions on twenty-five bills, each corresponding to a CRHC policy priority. CRHC supported fourteen bills (56% of the positions taken), opposed four bills (16% of the positions taken) monitored six bills (24% of the positions taken) and an amend position on one bill (.04% of positions taken). All the bills with a CRHC position correlated with the 2025 Policy Priorities: Facility Reimbursement, Healthcare Workforce, Healthcare Access, Administrative and Regulatory Burdens, and Rural Health Investments and Sustainability.Throughout the session, CRHC testified ten times in committee hearings and coordinated numerous member testimonies, hosted 12 member-driven PLC meetings, secured numerous bill amendments, and made over 200 state policymaker touchpoints to achieve our policy goals. A summary of the 2025 bills, and their rural impact, are outlined in the final report which is available on CRHCs website.2025 was unique in the amount of effort that CRHC put into federal policy. With the Administration taking over in January 2025, we have seen an unprecedented number of Executive Orders impacting healthcare, public health and the standard operations of the federal government. These Executive Orders initially frozen federal funding impacting CRHCs ability to pull down funding of awarded grants and grants were zeroed out leaving an uncertainty of federal funding. Due to these changes CRHC made additional connections at the state and federal levels including to the CO General Assembly, Governor and HCPF.With CRHCs CEO serving as the Chair of the National Rural Health Association, this has afforded CRHC the opportunity to raise the rural voice for Colorado as well as all rural communities nationally. These meetings included Federal Democrat Delegation, HHS Secretary Kennedy and CMS Administrator Dr. Oz. Additionally, CRHC conducted many posts on social media, created a dedicated page on the CRHC website, and participated in many interviews.Desc
