IdentifierIdentifierExplanationExplanation
Part I, Line 3cELIGIBILTY IS BASED ON THE FEDERAL POVERTY GUIDELINES
Part I, Line 6aST JAMES HEALTHCARE PRODUCED A COMMUNITY BENEFIT REPORT IN 2010 AND IT WAS MADE AVAILABLE TO THE PUBLIC. IT WAS PUBLISHED AS AN INSERT IN THE LOCAL NEWSPAPER WITH A DISTRIBUTION OF 15,000. IT ALSO WAS PLACED IN ALL OUR WAITING AREAS AND IS ON OUR PUBLIC WEBSITE.
PART I, LINE 7GSUBSIDIZED SERVICES AT ST. JAMES HEALTHCARE THAT WERE COUNTED AS COMMUNITY BENEFIT TOTALED $597,709. THIS AMOUNT INCLUDES PROVIDING ATHLETIC TRAINERS FOR THE HIGH SCHOOLS' ATHLETIC PROGRAMS. TRAINERS ARE PROVIDED TO 7 DIFFERENT SCHOOLS IN SOUTHWEST MONTANA, AND WITHOUT THIS SUPPORT, NONE OF THE SCHOOLS EXCEPT MONTANA TECH (THE COLLEGE IN BUTTE) WOULD BE ABLE TO PROVIDE THIS SERVICE TO THE STUDENT ATHLETES. THESE TRAINERS ATTEND ALL PRACTICES, GAMES AND TOURNAMENTS FOR EACH SCHOOL, AND ST. JAMES PAYS FOR 80% OF THEIR SALARIES TO PERFORM THIS SERVICE. IN ADDITION, ST. JAMES PROVIDES LOW COST ($5.00) SPORTS PHYSICALS FOR ALL THE ATHLETES. THE TOTAL AMOUNT FOR ATHLETIC SERVICES AND THE SPORTS PHYSICALS WAS $146,563. ST. JAMES HEALTHCARE SUBSIDIZED ITS TRANSITIONAL CARE UNIT AND COUNTED $331,715 IN COMMUNITY BENEFIT DOLLARS FOR THIS SERVICE. THE TRANSITIONAL CARE UNIT PROVIDED A BENEFIT TO PHYSICIANS AND PATIENTS WHEN HOME CARE WAS APPROPRIATE BUT ASSISTANCE AT HOME WAS NOT AVAILABLE. THIS CARE WAS PERFORMED AT A LOSS TO THE HOSPITAL AND AS A BENEFIT TO THE COMMUNITY, THE PATIENTS AND THIER FAMILIES. IN MID-2010, LOCAL NURSING HOMES ACQUIRED THE STAFF NECESSARY TO TAKE OVER THIS FUNCTION. IN COLLABORATION WITH THESE NURSING HOMES, ST. JAMES HEALTHCARE CLOSED ITS TRANSITIONAL CARE UNIT IN AUGUST 2010, AND THESE PATIENTS NOW RECEIVE EXTENDED CARE IN THE NURSING HOMES. ST. JAMES HEALTHCARE ALSO PROVIDED A MOBILE PET VAN FOR PATIENTS THAT REQUIRE PET SCANS. SUCH EQUIPMENT IS NOT AVAILABLE IN THE AREA SO PATIENTS WOULD NEED TO TRAVEL TO ANOTHER FACILITY. THE VAN COMES TO ST. JAMES HEALTHCARE EVERY OTHER WEEK. SOMETIMES, THERE ARE NO PATIENTS BUT ST. JAMES STILL MUST PAY FOR THE VAN. WE CONSIDER ONLY THOSE PAYMENTS AS COMMUNITY BENEFIT DOLLARS. THE TOTAL IN 2010 WAS $63,438. ST. JAMES ALSO PAID $55,993 FOR AMBULANCE TRANSPORT SERVICES FOR THE POOR.
Part I, Line 7, COLUMN (f)THE AMOUNT IN COLUMN (F) IS BASED ON THE NET COMMUNITY BENEFIT IN COLUMN (E) DIVIDED BY THE TOTAL OPERATING EXPENSES MINUS BAD DEBT EXPENSE ($93,244,545 MINUS $8,184,480 = $85,060,065). The denominator is $85,060,065.
Part I, Line 7IN PART I, LINE 7, ST JAMES HEALTHCARE PROVIDES THE AMOUNTS OF FINANCIAL ASSISTANCE AND CERTAIN OTHER COMMUNITY BENEFITS AT COST. THE AMOUNTS REPORTED ARE FROM WORKSHEET 1 THROUGH 8.
PART IIWE EXTEND OUR CARE BEYOND OUR HOSPITAL'S WALLS TO IMPROVE THE HEALTH OF OUR COMMUNITY. IN 2010, OUR COMMUNITY BUILDING ACTIVITIES TOTALED $318,659 AND INCLUDED PROGRAMS SUCH AS DIABETES OUTREACH, FOOD FOR THE HOMELESS, PHYSICIAN RECRUITING, COMMUNITY SUPPORT GROUPS AND OTHER COMMUNITY VOLUNTEER ACTIVITIES AND WORK ON THE COMMUNTIY NEEDS ASSESSMENT. THE DIABETES OUTREACH PROGRAM IS PRIMARILY FUNDED THROUGH FEDERAL AND STATE GRANTS, BUT THE PROGRAM HAS GROWN SIGNIFICANTLY OVER THE PAST TWO YEARS AND REQUIRES HOSPITAL STAFF TIME THAT IS NOT PAID FOR BY THE GRANTS. THE GOAL OF THE PROGRAM IS TO REACH OVER 1,500 INDIVIDUALS, BOTH ADULTS AND CHILDREN, OVER THE THREE YEAR GRANT PERIOD. WEEKLY CLASSES TO EDUCATE PEOPLE ON DIABETES ARE HELD AS ARE MONTHLY STAFF GROUP MEETINGS, ONE FOR ADULTS AND ONE FOR CHILDREN AND THEIR FAMILIES. SEVERAL HEALTH-FOCUSED CLASSES AT THE LOCAL YMCA WERE IMPLEMENTED IN 2010, AND AN ANNUAL CONFERENCE FOR HEALTH CARE WORKERS THROUGHOUT MONTANA IS ALSO HELD IN CONJUNCTION WITH A PUBLIC DIABETES AWARENESS EXPO. PHYSICIAN RECRUITING IS A CRITICAL ACTIVITY TO ENSURE THE RESIDENTS OF THE COMMUNITY, WHICH IS IN A MEDICALLY UNDERSERVED AREA, HAVE ACCESS TO THE MEDICAL TREATMENT NEEDED. IN 2010, ST JAMES HELATHCARE RECRUITED THREE NEW PHYSICIANS INTO THE COMMUNITY. HOWEVER, BASED ON THE OUTCOME OF THE COMMUNITY NEEDS ASSESSMENT, BUTTE IS STILL LACKING MORE THAT 18 PHYSICIANS IN A NUMBER OF SPECIALTIES. CONDUCTING A COMMUNITY NEEDS OF ASSESSMENT IS AN IMPORTANT COMMUNITY BUILDING ACTIVITY. THIS PROCESS BROUGHT TOGETHER MORE THAN 30 SOCIAL SERVICE AGENCIES AND HEALTH PROVIDERS AND MORE THAN 100 PEOPLE. UPON COMPLETION OF THE ASSESSMENT, TEAMS WERE FORMED TO DEVELOP ACTION PLANS FOR SOME OF THE HIGHEST PRIORITY FINDINGS IN THE ASSESSMENT. THIS WORK WILL CONTINUE OVER THE NEXT THREE YEARS.
PART III, LINE 4A cost to charge ratio is used to determine the amount reported in Part III, lines 2 and 3. Discounts and payments applied to accounts reduce bad debt expense. No method is used to determine the amount that reasonably could be attributable to patients who would likely qualify for financial assistance if sufficient information had been available to make a determination of their eligibility. No portion of bad debt is included as community benefit. The organization's financial statements do not contain a footnote that describes bad debt expense. Within the financial statement of the Sisters of Charity of Leavenworth Health System, bad debt expense is included within operating expenses, at the value of uncollected and estimated uncollectible charges after contractual allowances and self-pay discounts are applied.
PART III, LINE 8St. James Healthcare believes a significant portion of the shortfall from Medicare should be treated as community benefit. In St. James' case in 2010 that shortfall was more than $4.6 million. St. James provided the resources required to care for these patients and should receive some benefit for doing so. The costing methodology used is the Cost to Charge ratio because St. James believes this methodology most accurately reflects the actual cost of providing the services.
PART III, LINE 9BAn integral component of our Mission is to be good financial stewards. This requires us to determine which patients are in need of charity care and which are able to contribute some payment for care received. We maintain a balance that enables us to continue to provide charity care to those who need it most, and to ensure that we manage our resources so that we can continue to be here when people need us most. St. James Healthcare notifies patients of its financial assistance policy prior to admission, upon admission and upon discharge. In addition, we provide communication regarding patient bills. Patients are contacted multiple times about unpaid balances prior to initiating any collection action. If a patient is determined to be eligible for financial assistance at any time during the collection process, the account is reclassified as financial assistance and debt collection efforts are ceased. In 2010, St. James provided a total of $3,409,521 in traditional charity care and $2,081,971 for the unpaid cost of Medicaid. 2. NEEDS ASSESSMENT - As part of our core value of response to need, we take steps to determine where there is the most need to provide the greatest good. St. James Healthcare has regularly participated in needs assessment surveys to identify the ongoing and changing needs of the community. The most recent survey was conducted in 2010 and finalized in early 2011. This assessment examined the population of Silver Bow County which is served by St. James Healthcare and was done by partnering with the local public health department and all the local social agencies. With the assistance of outside consultants, the assessment also determined where gaps in services exist. From the "gaps" document, we conducted focus group meetings with social agency personnel to develop objectives and strategies for the following categories of need: Aging Population; Environmental Factors; Socio-Economic; Social and Behavior; and Health Outcomes and Factors. With objectives and strategies in hand, groups have begun to develop action plans (short term and long term) for each of the five areas. Groups will meet quarterly throughout 2011 and into 2012 to report progress. 3. PATIENT EDUCATION OF ELIGIBILITY FOR ASSISTANCE - St. James Healthcare treats patients with respect and dignity regardless of their ability to pay. St. James Healthcare has a written financial assistance policy that explains eligibility criteria for financial assistance at various levels, including a 100% discount, and is based on federal poverty guidelines. We work with patients to help them understand their financial responsibility for care received, financial assistance available to them, and to establish payment programs in demonstration of our core value of respect. As part of our responsibility to educate, we inform our patients and their families of the availability of assistance, including government programs. We communicate this in a variety of ways to ensure that messages reach multiple audiences. Before, during and/or after admission, we encourage our self pay patients to visit with a financial counselor to discuss qualifications for financial assistance. The financial counselor works with the patient to complete a financial assistance form to determine the level of discount for which the patient may be eligible. We post financial assistance information in emergency and admissions areas, on billings invoices, in various areas around the hospital and clinic sites and on the hospital website. We provide written materials to patients that detail our financial assistance policy and how it is administered. We provide materials and education when patients are discharged, and include information in billing statements, including phone numbers and other methods to contact us with questions. We ensure that our financial counselors, admission employees, social workers and other employees understand our policies to be able to assist patients in the most appropriate way. We also have a partnership with Western Security Bank to provide payment plans and medical credit lines. This helps patients meet their financial obligations in a reasonable and dignified manner based on their ability to pay and allows them to continue to ensure the ongoing welfare of their families. This is done in accordance with our core value of respect. 4. COMMUNITY INFORMATION - Silver Bow County, located in Southwestern Montana in a Medically Underserved Area, is the eighth largest county in the state with a population of slightly more than 32,000. Butte-Silver Bow (Butte), the county seat, is the state's fourth largest city, but the population has decreased by 4.8% since 2000. State government, which includes higher education institutions, is the largest employer in Silver Bow County with 22.3% of the total employment. Health care is the top private sector employer at 13% with St. James Healthcare, the community's only hospital, as the largest single private employer. Additional Butte-Silver Bow statistics follow. *Median income for Silver Bow County households in 2010 was $37,346 compared to national median income of $52,175. *37% of the local population is at 200% of the federal poverty level; 15.8% of the residents have an income below the poverty level. *93.7% of Butte's population is Caucasian; 2.9% is American Indian; 2.7% is Hispanic; 1.4% is Asian and Pacific; and 0.6% fall into other categories. *Approximately 17% of Butte's population are over 65. This compares to the national average 13% over 65. *Butte-Silver Bow has more females than males. *Of the Butte-Silver Bow residents, ages 25 and over, 85.2% have a high school education or higher, 21.7% have a bachelor's degree or greater, and 6.9% have a graduate or professional degree. *14 % of the population is uninsured and 13% are covered by Medicaid. From a health perspective, Butte-Silver Bow's leading causes of death are heart disease and cancer. Rates of diabetes and COPD are high. 5. PROMOTION OF COMMUNITY HEALTH - We extend our care beyond our hospital's walls to improve the health of our community. Our community building activities demonstrate this commitment. Most of our community building activities are in the category of community support. These activities promote health in the community in a number of ways. We provide monthly food donations to the local food bank to ensure that it has a sufficient supply of food so its clients have nutritious food each month which in turn promotes better health. Each day, we provide a significant number of free meals to the community to ensure that our poor population has access to nutritious food that helps them maintain better health. Our employees are involved in a number of volunteer activities throughout the year that promote healthy living. These encompass everything from raising funds for the youth program at the local YMCA to helping distribute food at the Food Bank. Service clubs throughout the community are focused on promoting healthy life styles for both children and adults, and St. James staff actively participate in clubs such as the Kiwanis, the Chamber of Commerce, and the Breast and Cervical Cancer Group. In the area of community health improvement advocacy, we partner with public health agencies and serve on the boards and committees of these agencies that are focused on improving the health of the community. St. James is the lead agency in a community-wide diabetes network outreach. Members of our dietary staff, nursing staff, and administrative staff work many hours to provide education and support to both our adult and child diabetic population. We also provide social workers for a bi-monthly Alzheimer Caregiver Support Group. St. James and the Butte-Silver Bow Public Health Department are the lead agencies for conducting a Community Needs Assessment and driving action plans to improve the overall health of the community. As a medically underserved area, Butte needs more physicians, and St. James actively recruits physicians to the area. When potential recruits make site visits to Butte, we host meetings with local government officials, the local school district, the college, the Chamber of Commerce and other organizations to provide a positive and encompassing experience for the potential recruit. In 2010, we successfully recruited one Emergency Department physician, a pediatrician, and a family medicine physician to the community. St. James Healthcare is an important part of our community and serves in many ways, from delivering core health care, to preventive care, to support of other civic groups. In 2010, we provided community benefit totaling $6,782,251, including traditional charity care and the unpaid cost of Medicaid. Our Community Board of Directors represents medical and business professionals and all provide hours of service in support of our hospital. They are involved in our community needs assessment process, building programs and services, and community out

PART III, LINE 4APROGRAM SERVICE ACCOMPLISHMENTSSt. James Healthcare Community Benefit Report 2010 Community Diabetes Network St. James Healthcare's primary market area has an extremely high rate of diabetes among both its young and older population. Consequently, the need for diabetes education for providers and the general public was apparent. In June 2008, the St. James Foundation received a grant to develop a Community Diabetes Network to provide a continuum of care for diabetic patients in Southwest Montana. Through this grant, St. James hired a Diabetes Program Coordinator who is responsible for developing a community needs assessment, establishing both youth and adult diabetes support groups, and growing the Community Diabetes Network to ensure that a consistent diabetes education and prevention program is developed. The Butte Community Diabetes Network was formed and is currently made up of a collaborative partnership between St. James Healthcare, the Butte Family YMCA, the North American Indian Alliance, Butte Community Health Center and two local physicians. St. James Healthcare Ticker Tuner Fun Run For the past three years, St James Healthcare has co-sponsored a Ticker Tuner Fun Run for the community. In 2010, over 100 runners and walkers participated in a 10 kilometer or 5 Kilometer race or a one mile walk. Proceeds from the event were used to help purchase new equipment for the St James Healthcare Cardiac Rehabilitation Department, which is extensively used by those in the community who have undergone cardiac treatment and require follow-on rehabilitation services. The Participate for Life (PAL) Program was established to offer financial assistance to individuals in our community who need to travel out of town to be with a loved one needing specialized medical treatment not offered in Butte as well as to help with expenses for patients who need to travel to Butte from outlying towns for treatment who could not afford to do so. The PAL Program helps families with some type of financial assistance, such as gas cards, hotel expenses, bus vouchers, air plane tickets. 2010 Freedom Fest- St. James Healthcare and the Foundation joined again on July 3, 2010, to bring food and entertainment to Chester Steele Park before the annual fireworks display. This annual event is a great opportunity for friends and family to gather and enjoy the summer evening. This celebration gives us an opportunity to educate our community on the PAL program, and vendor fees help support the program. Community Share Program - The Community Share Program was established by St James Healthcare employees to help community non-profit agencies who have specific needs. Employees contribute to the program through payroll deductions. Local non-profit agencies and organizations can then submit requests for grants. Wear Red for Heart Health was to educate about heart disease and strokes. Free Screenings for Body Mass, Glucose, Blood Pressure were conducted. Cholestrol testing was done for a nominal fee and if participants were wearing red they received a $5.00 discount. Other Community Benefit Donations Including the provision of health care for the poor and uninsured (charity care) and the unpaid cost of Medicaid, St. James Healthcare also supported, through monetary or in-kind donations, a number of other community-enhancing activities. Some of the other activities included: -Ambulance transport services for the poor or uninsured -Mental health crisis response line -Prescription drug assistance for the poor or uninsured -Meals for more than 6,000 indigent people -Athletic programs, including providing athletic trainers and low-cost sports physicials ($5), to all Southwest Montana schools and colleges -Free public talks on a variety of health issues (presented by medical staff) -Preceptor training for students majoring in nursing, physical and respiratory therapy, pharmacy etc.
