DescriptionNorthwestern Memorial Hospital, hereafter NMH, is a not-for-profit corporation and is part of an academic medical center in downtown Chicago, Illinois that provides a full range of adult inpatient and outpatient services in an educational and research environment. For 146 years, NMH and its predecessor institutions, Passavant and Wesley Memorial hospitals, have served the people of Chicago. The commitment to provide healthcare, regardless of the ability to pay, reaches back to the founding principles of Passavant and Wesley and continues to be integral to our Patients First mission. Importantly, it provides the foundation for our mission-driven commitment to improve the health of the communities we serve. This commitment is advanced by focusing on enhancing patient access, safety and quality through hospital-based care and services and through partnerships with community health centers that date back more than 50 years. As the primary teaching hospital for Northwestern University Feinberg School of Medicine (Feinberg), the more than 1,600 physicians on the medical staff at NMH carry faculty appointments at Feinberg and represent virtually every medical specialty. Through Northwestern Medicine TM, Northwestern Memorial shares a vision with Feinberg to work to positively impact the future of healthcare through exceptional patient care, medical education and breakthrough scientific research that can lead to improved treatments and cures. As the only acute care hospital located in Chicago's growing downtown area, more than 48,900 adult patients were admitted as inpatients in fiscal year 2010 at NMH. As the only Emergency Department (ED) in downtown Chicago with 24/7 service, NMH had more than 80,600 ED visits in fiscal year 2010, up 15 percent since fiscal year 2005. NMH also is Chicago's only academic medical center hospital participating in city and state Level I trauma networks and as a Level III neonatal intensive care unit, enabling the provision of the most extensive lifesaving care to the most seriously injured adults and the sickest infants. For more than 16 years, healthcare consumers have consistently ranked NMH the "most preferred" hospital in Chicago and the nine-county region through independent market research. Twelve clinical specialties are ranked in U.S. News & World Report's 2010 "America's Best Hospitals" list. NMH has achieved the prestigious Magnet designation from the American Nurses Credentialing Center and are among only six percent of the nation's healthcare organizations to do so. NMH was recertified with this designation in 2010. Recognized as the gold standard for nursing, Magnet designation honors excellence and an organizational commitment to quality care. In addition, NMH was ranked in the top quintile of hospitals nationwide by University HealthSystem Consortium for Quality and Accountability in 2006, 2008, 2009 and 2010. NMH was also the only hospital serving adult patients in Illinois named as one of the 100 Best Companies for working mothers for the past decade by Working Mother magazineDescription

DescriptionNorthwestern Memorial's outreach services and financial assistance programs are built on the foundation of a Patients First mission and continue to expand to serve the most vulnerable in our community. With a mission-driven commitment to providing quality medical care, regardless of the ability to pay, Northwestern Memorial has, in recent years, significantly broadened its efforts to improve the health of the most medically underserved members of its community by more than doubling the amount of charity care provided over the last five years, resulting in approximately $38 million in hospital services provided to those who could not afford to pay in fiscal year 2010. NMH is the sixth largest provider of Medicaid services among acute care hospitals in Illinois when measured by total Medicaid patient admissions and the ninth largest provider when measured by total Medicaid patient days. (Source: Illinois Department of Healthcare and Family Services for the Fiscal Year ended June 30, 2009, the latest information available). In fiscal year 2010, net unreimbursed cost of government sponsored indigent healthcare services for NMH totaled $105.6 million. Through longstanding partnerships with two of Chicago's Federally Qualified Health Centers, Near North Health Service Corporation (Near North), which includes the James and Catherine Denny Primary Care and Preventive Medicine Center at the Lawson House YMCA, and Erie Family Health Center (Erie), Northwestern Memorial has implemented a successful multiyear program to develop sustainable results in the care of indigent patients with diabetes and developed and implemented a replicable, culturally appropriate model for comprehensively addressing disparities in breast health services. Through these partnerships, community residents in need and the homeless have access to care and support services so they can receive necessary healthcare in their own communities and access to hospital-based services at NMH.Description

DescriptionIn partnership with Feinberg, Northwestern Memorial is committed to providing superior medical education, ongoing training for physicians who practice at the hospital as well as the more than 900 medical residents, fellows and students who each year learn in the comprehensive clinical training environments available through the academic medical center setting. An ongoing focus on clinical innovation, medical education and scientific inquiry allows NMH to bring the most advanced medical treatments to our patients in the safest and fastest way possible; to create new and better treatments to fight diseases such as cancer, heart disease and diabetes; and to work to improve the quality of life for future generations through training the next classrooms of physicians and creating an environment that supports physicians and scientists as they conduct pioneering research that can be widely shared throughout the medical and scientific community. In addition, through its educational offerings and affiliations with education partners, NMH provides ongoing and comprehensive training and continuing education for the nursing staff and allied healthcare professionals who are integral to providing quality patient care. In fiscal year 2010, Northwestern Memorial's net unreimbursed cost of education and research was approximately $71.5 million. In fiscal year 2010, Northwestern Memorial and Feinberg received accreditation to train obstetrics and gynecology resident physicians at both NMH and John H. Stroger, Jr. Hospital (Stroger). As a result of this expansion, the merged program ensures important physician resources remain available to provide high quality obstetric and gynecologic care to medically underserved patients in Chicago.Description

Annual Community Benefit ExplnNorthwestern Memorial HealthCare and subsidiaries (NMHC) submit a community benefit report to the Illinois attorney General according to the requirements for the state of Illinois. Northwestern Memorial Hospital's(NMH)and all other NMHC non-profit subsidiaries results are included in this report. A complete copy of the report is available on request. A summary version is also available on the nmh.org website.AnnualCommunityBenefitExpln
Costing Method UsedCharity care at cost was calculated by applying the cost-to-charge ratio from the NMH Medicare cost report for the reporting period (consistent with the state of Illinois attorney General office definition) to the self pay portion of charges on accounts identified as qualifying for charity care. The resultant calculated cost was then offset by any patient-related payments. Charity Care expense is determined as defined in the healthcare financial management association's principles and practices board statement 15: valuation and financial statement presentation of charity care and bad debts by institutional healthcare providers. Unreimbursed medicaid was calculated by applying payments received under the medicaid program against medicaid costs as calculated using the nmh overall cost-to-charge ratio. The NMH ratio was adjusted to exclude medical education and other costs that are included elsewhere on schedule H. The resultant calculated cost was then reduced by $19.8 million of net reimbursement nmh received from the Illinois Hospital Assessment Program. Other benefit expenses were calculated primarily based on direct costing methodology consistent with functional expense reporting in the footnotes to the audited financial statements.CostingMethodUsed
Subsidized Health Services ExplnNMH does not include costs attributable to physician clinics as subsidized health services. The benefits reported are primarily associated with operating losses supporting nmh's outpatient mental health program. NMH's mental health program includes outreach efforts to connect with those in the community needing services who might otherwise not interface with the healthcare system. NMH works with the Chicago Alliance to End Homelessness in programs aimed at identifying homeless people with mental illness and providing housing, social support services, medical and mental healthcare. Staff participate in the Homeless Action Committee of the Department of Human Services, which brings representatives from homeless shelter organizations together with mental health providers to educate one another and develop programs to identify and provide mental health services to the homeless. Mental health employees are also active in the efforts of HELP (Help Ease Local Poverty), a program of the Jane Addams Hull House Association, both as board members and participants. Through HELP, NMH mental health employees help to serve a weekly meal to the homeless and in this setting, can identify and reach homeless people in need of mental health services. Patients cannot persist in treatment or begin to improve in mental health status unless their basic needs are being met. For many patients, NMH's mental health programs provide a critical link to public health and social support services. For individuals who are homeless or have mental health conditions that impair cognitive abilities, the process of gaining access to social welfare programs and accessing health coverage is often beyond their capabilities. Without access to social and welfare supportive programs, individuals are unlikely to be able to manage a mental health or substance abuse problem and are at high risk for homelessness. NMH mental health employees participate in the SSI/SSDI Outreach, Access and Recovery program that provides training and tools for mental health professionals nationally, to assist the homeless or those at risk of homelessness in gaining access to health, income support and housing programs. In addition, NMH staff train mental health providers at community-based organizations who otherwise would not be able to afford training through the program. When needed, NMH provides emergency and long-term housing for mental health patients through three programs (two under federal fund match arrangements and a third mainly underwritten by Northwestern Memorial with limited state funding) at the Lawson House YMCA. NMH provides on-site support services at the Lawson House YMCA for psychiatry patients who are unable to function independently and would otherwise be at risk for homelessness.SubsidizedHealthServicesExpln
Percent Of Total Expense ExplnThe bad debt expense per Schedule IX, line 25, $26,578,536, was removed from total exepnses in determining the column f percentages.PercentOfTotalExpenseExpln
Bad Debt FootnoteFootnote: Patient accounts receivable are stated at net realizable value. Northwestern Memorial maintains allowances for uncollectible accounts and for estimated losses resulting from a payor's inability to make payments on accounts. Northwestern Memorial estimates the allowance for uncollectible accounts based on management's assessment of historical and expected net collections considering historical and current business and economic conditions, trends in healthcare coverage, and other collection indicators. Accounts receivable are charged to the allowance for uncollectible accounts when they are deemed uncollectible. Part III Line 2: Bad debt expense is determined as defined in the healthcare financial management association's principles and practices board statement 15: valuation and financial statement presentation of charity care and bad debts by institutional healthcare providers. The expense is then adjusted by the nmh cost-to-charge ratio as adjusted to exclude education and certain other costs.BadDebtFootnote
Shortfall As Community BenefitThe unreimbursed cost of Medicare is defined by the state of illinois attorney general's office annual nonprofit hospital community benefits plan report as a community benefit. The Healthcare Finance Management Association also views the unreimbursed costs of Medicare as part of a hospital's community benefit program. NMH provides medical care to medicare patients at a cost higher than the reimbursement it receives from medicare. The amounts listed for PArt III, Lines 5 thru 7, are derived from the NMH financial statements FOR FISCAL 2010.ShortfallAsCommunityBenefit
Debt Collection PolicyNMH's Credit and Collection Policy contains a provision for financial counseling. tHE POLICY STATES THAT patients with self-pay balances and without the resources to pay their obligations will be assessed FOR FREE AND DISCOUNTED CARE eligibility By the Financial Counseling Department. The assessment involves an evaluation of all levels of assistance including governmental assistance, extended pay alternatives, and free or discounted care. If THE PATIENT QUALIFIES FOR free care, THE ACCOUNT IS ADJUSTED TO ZERO SO NO COLLECTION ACTIVITY OCCURS. If financial assistance results in a discounted or reduced balance, only the reduced balance will be subject to the collection practices.DebtCollectionPolicy

Needs AssessmentNorthwestern Memorial's mission to improve the health of the communities we serve is advanced through innovative programs to better manage chronic diseases, particularly among the medically underserved. Northwestern Memorial's Community Benefit Plan focuses on addressing chronic diseases and is consistent with the strategic priorities of public health organizations at both the state and local levels. In July of 2010, the State of Illinois formally transferred its Diabetes Prevention and Control Program from the Department of Human Services to the Department of Health. This move underscores the expectation that chronic disease programs are directly integrated with the healthcare system. The Chicago Department of Public Health's (CDPH) 2006-2011 Strategic Plan prioritizes prevention of chronic diseases, promotion of health for all Chicagoans and reduction in health disparities based on socioeconomic status. We have formal and longstanding affiliations with two Federally Qualified Health Center partners, Near North and Erie. Through these partnerships, programs are collaboratively developed and implemented to address the healthcare needs of the patients in medically underserved communities throughout Chicago. One member of Northwestern Memorial Hospital's senior management team serves as a board member at Near North and two serve at Erie. Near North, a community health partner for more than 40 years, provides neighborhood-based care and support services through six primary healthcare sites and four ancillary sites to more than 47,000 primarily low-income uninsured or underinsured residents who live in some of the city's most impoverished communities, including the Near North/Cabrini Green area; West Town; Humboldt Park; West Garfield Park; Austin; Kenwood/Oakland; Douglas; and Grand Boulevard. Near North operates primary care clinics on Chicago's South, West and Near North sides. Erie was founded in the 1950s as a project of volunteer physicians from Northwestern Memorial Hospital and Erie Neighborhood House. Erie provides a variety of primary care and case management services through nine sites and a number of other partnership programs to more than 30,000 patients annually from the Chicago neighborhoods of Belmont Cragin, Hermosa, Humboldt Park, Logan Square, North Lawndale and West Town. Erie serves a population that is primarily Hispanic, the majority of which come from households with incomes that fall below the Federal Poverty Level. Both Near North and Erie have identified diabetes and women's health as priority health concerns for the populations they serve. The collaborative development of health initiatives among NMH, Near North, Erie, Feinberg and others to address chronic disease in medically underserved areas is detailed in a subsequent section of this report. Expanding access and improving the ease with which Near North and Erie patients are referred and receive patient care also is detailed later in this reportNeedsAssessment
Patient Education AssistanceThere are many ways that patients of the Hospital are informed or made aware of the availability of the Hospital's various financial assistance programs: a. To increase awareness of its financial assistance programs, the Hospital has developed brochures (in English and Spanish) that are provided to patients upon admission and available at registration points-of-entry. b. English and Spanish-language signs notifying patients that financial assistance is available are present at every patient registration area, including the emergency department. c. As part of the registration process, patients are provided with a financial assistance information brochure which describes the types of assistance available and how to qualify for one or more of the programs. d. The general consent form that every patient signs contains information about the NMH financial assistance programs, and is available in English, Spanish, Russian and Polish. e. Inpatients receive a Patient Welcome Package that includes the financial assistance information. f. Patients can learn about and assess their eligibility for the Hospital's financial assistance programs with the help of the Hospital's team of financial counseling and patient inquiry representatives. These representatives are available on a walk-in basis or through a toll-free number. g. Processes are in place to link patients with financial counselors and patient inquiry representatives when financial hardship is identified as a concern during social services assessments. h. The entry portal to the NMH website contains a prominent link to information about NMH's various financial assistance programs, the financial assistance brochure and downloadable applications in multiple languages. i. Working in conjunction with clinical staff, financial counselors visit inpatients not enrolled in government or private health plans while they are still in the hospital to assist them in determining their eligibility for both government health programs and for Hospital Free and Discounted Care programs. j. The Hospital informs uninsured patients, and patients with an outstanding balance after insurance, of the availability of various financial assistance programs, including the free care and discounted care program, and the catastrophic program offered by the Hospital, in written correspondence sent to those patients. This information includes the toll-free phone number to NMH's team of patient account representatives. k. The Hospital has on-site patient account staff who are trained and available to assist patients with financial assistance. l. The Hospital provides proactive financial counseling for self-pay patients who have a scheduled inpatient admission. Financial counseling includes assessment for publicly or privately funded insurance and the Hospital's financial assistance programs.PatientEducationAssistance
Community InformationNorthwestern Memorial's patient care, education and research programs provide broad benefit to Chicago, the region and internationally. NMH divides its total service area into three geographic areas: the primary service area (PSA), the city of Chicago and the surrounding seven-county area. The PSA, which is defined by the 22 zip codes surrounding NMH, accounts for 38 percent of inpatient admissions. The city of Chicago in total accounts for 65 percent of inpatient admissions. The community in NMH's PSA has a large and growing population and it is important for us to continue to grow so that we can respond to the need for quality healthcare services. Between 2010 and 2015, the population in NMH's PSA is projected to increase by 0.9 percent, whereas the population of Chicago is projected to decrease by 1.3 percent. (source Thomson Reuters Demographic Profile (Claritas Based)) Chicago is a diverse city, with a large African-American population and growing Latino and Asian populations. Northwestern Memorial is committed to providing culturally competent care that is responsive to the needs of all our patients. NMH has worked with community health centers in some of Chicago's medically underserved areas to identify priority health concerns and jointly develop community-based health initiatives designed to address healthcare disparities among people living in the community.CommunityInformation
Community Building ActivitiesPart II Line 8, Workforce Development. Community Training and Education programs at NMH work to ensure that a highly trained healthcare workforce of adequate capacity is in place to serve the residents of the region, that at-risk members of the community have access to jobs in the healthcare system and that the youth have access to programs that help them learn about and potentially interested in healthcare careers. Objective #1: Continue to provide training and employment opportunities for residents of the community while addressing the shortage of healthcare workers through a direct, formal training pipeline. Ongoing Initiatives: NMH offers 12 to 21-month post-baccalaureate certificate programs in nuclear medicine, radiation therapy, radiography and diagnostic medical sonography. The programs are available to employees as well as the general public. Many of the students come from the local community. Leaders of these programs visit city high schools, colleges and universities to introduce various medical fields to prospective students and increase their general knowledge of various allied health fields. NMH is an important clinical setting for the education of the next generation of healthcare workers, from physicians to nurses to skilled technicians. Through clinical affiliations with top regional universities and colleges, as well as established clinical rotation, mentoring, clinician shadowing, traditional didactic lectures and other teaching programs, NMH in fiscal year 2010 provided a clinical setting for education of hundreds of students, many of whom will become professionals in fields identified as areas of current or future workforce shortage in the U.S. healthcare system. In fiscal year 2010, NMH provided education to: a. More than 750 undergraduate and graduate nursing students b. 150 students from university-based pharmacy programs c. 35 respiratory therapy students d 4 graduate social work interns e. 4 polysomnography students f. 25 clinical pastoral students g. 4 interns and students in biomedical engineering h. Students in physical therapy, physical therapy assistant, occupational therapy and occupational therapy aide programs Objective #2: Continue to provide on-the-job training programs for at-risk members of our community. Ongoing Initiatives: Since 1997, NMH has partnered with the CARA program to help homeless and other at-risk adults in their efforts to achieve long-term employment success by providing on-the-job training skills that ready them to move into the work force. NMH has hired more than 100 employees through this partnership since it began. In fiscal year 2010, NMH hired four new employees through CARA. Objective #3: Continue to support youth education programs, helping students understand the potential for job openings and success in healthcare careers. Ongoing Initiatives: A. NMH offers ongoing, comprehensive youth programs that expose Chicago students to potential healthcare careers. - For 11 years, through the Medical and Health Careers Academy, NMH has partnered with high schools in the Chicago Public School System - Percy L. Julian on the South Side, Roger C. Sullivan on the North Side, Richard T. Crane Technical Preparatory on the Near West Side and Dunbar Vocational Academy on the South Side - to promote interest in post-high school education and healthcare careers. NMH employees speak to the students about their jobs in healthcare and students with their parents visit the hospital, where they have the opportunity for a behind-the-scenes understanding of clinical areas and potential careers. Through this program, 75 high school freshmen and 55 high school seniors visited NMH in fiscal year 2010. NMH also sponsored a workshop for high school seniors during which employees volunteered to review college applications and essays and coached students on application processes and interviews. Students in this program also become a part of other youth programs offered through NMH and are eligible for summer internships at the hospital. - NMH has hosted Medical Explorers Post 9766 since 1996. Students participate in a variety of activities designed to encourage their expressed interest in healthcare careers and expose them to the field. The program emphasizes career exploration, life skills, service learning, character development and leadership. Students participate in tours, hear guest speakers and join in discussions and projects. The program offers internships, mentorship, tutoring, networking, community service opportunities and scholarships. To date, more than 800 high school and college students have participated in NMH's Medical Explorers Post. Since the program began, many Medical Explorers have pursued careers in nursing and medicine and several are now employed at NMH, including a recent nursing Medical Explorer student who now works in the neonatal intensive care unit. - The Cristo Rey Internship Program, run collaboratively with the Cristo Rey Jesuit High School in Pilsen, on the city's Near West Side, offers students from a primarily Hispanic community an opportunity to work one day a week in an administrative role or within the Information Services department at NMH to gain valuable work experience and learn time management and organizational skills in a corporate setting. Twelve students participated in this program in fiscal year 2010. B. NMH continues to offer comprehensive internships and fellowships for college students and post-graduates. - NMH sponsors the INROADS program, which provides progressive internships, year-round academic instruction and summer workshops to prepare minority college students for healthcare careers. Initially developed under the federal "Hire the Future" program, NMH was the first Chicago hospital to participate in this program. Students benefit from mentoring and leadership training to prepare them for future positions in a healthcare career. - NMH offers administrative fellowships and internships that expose select post-graduate students to various aspects of leadership within our academic medical center hospitalCommunityBuildingActivities
Other InformationPart I, Line 7H: Research is conducted in virtually all clinical areas at NMH and each year, more than 500 new clinical research studies - many of which offer hope for patients through new diagnostic or therapeutic techniques - are approved to be conducted at the hospital. In addition, in partnership with Feinberg, Northwestern Memorial is building the infrastructure and dedicating resources to actively develop community-based research initiatives that respond to medical needs. Part I, Line 7I: During fiscal 2010, NMH provided contributions in support of the community benefit categories included in Part 1, Lines 7e - 7h. In accordance with the instructions, these amounts are reflected as cash and in-kind contributions to community groups.OtherInformation
