HAMMER HOMES ARE CONTINUALLY ADAPTED TO MEET THE CHANGING PHYSICAL NEEDS OF OUR RESIDENTS. HAMMER ALSO OFFERS SKILLED, PART-TIME IN-HOME SUPPORT STAFF TO FAMILIES WHO HAVE DECIDED TO KEEP THEIR LOVED ONE, MOST OFTEN A CHILD, IN THE HOME. HAMMER ALSO OFFERS CASE MANAGEMENT SERVICES FOR HENNEPIN COUNTY CLIENTS, HELPING FAMILIES FIND SERVICES THAT BEST MEET THEIR NEEDS. HAMMER PROGRAMS ARE DESIGNED TO MEET THE PSYCHOLOGICAL, SOCIAL, RELIGIOUS, EDUCATIONAL, VOCATIONAL AND RECREATIONAL NEEDS OF EACH INDIVIDUAL RESIDENT.
