Shortfall As Community BenefitThe Medicare allowable costs were obtained from the organization's Medicare cost report for the year ended September 30, 2010. The organization believes that Medicare underpayments should be treated as a community benefit since these services and related costs promoted the health of the community and were rendered as part of the organization's policy of providing care to any patient in need of medical care, regardless of the patient's ability to pay for such care.ShortfallAsCommunityBenefit
Debt Collection PolicyThe policy addresses both how the organization helps individuals who do not have health insurance find assistance when it is available and how these accounts are processed based on the insurance and type of claims involved.DebtCollectionPolicy
Needs AssessmentAs needed, the organization will contract for the services of well-known professionals for the completion of a comprehensive assessment that explores both the need and demand for services from the community as well as the supply of services and the strength of the existing service system. The emphasis will be on the uninsured, low income, and/or underserved populations in the service area and the safety net system in place for this target population. The issues of the capacity for culturally competent healthcare in the service area will be explored in focus groups with current patients and minority group member from the service area. Consumer focus groups will help identify significant but subtle barriers to healthcare and we expect that our consumer representatives on the FQHC Team will be able to translate their experiences so that we may develop a responsive CHC model. Interviews with individual providers of social support and healthcare, as well as providing organizations and CBOs, will also inform this part of the Needs Assessment. The Needs Assessment process will encompass access to oral health, mental health, and substance abuse related services as well as primary medical care and will examine the needs and methods for outreach, case management and other enabling services. Methods include data analysis, interviews, surveys and focus groups. It will identify further detail on the service area and its population as follows: Health status and health indicators for the target population including subpopulations of the homeless, MSFWs, refugees, immigrants and Latinos. Data sources include state databases: MassCHIP (DPH), the Uniform Data System (including the UDS Mapper tool) and data from hospitals and state agencies. Economic factors will be examined through the American Community Survey data from the US Census Bureau, MassCHIP, and the Massachusetts Institute for Social and Economic Research (MISER), current unemployment data (MA DET) and health insurance data from the Division of Healthcare Finance and Policy (DHCFP) . Socio-demographic factors will be analyzed from U.S. Census data, MassCHIP, MISER, MassHealth Data Consortium and the HRSA Geospatial Warehouse. Consultants will conduct an assessment that explores both the need and demand for services from the community as well as the supply of services and the strength of the existing service system. The emphasis will be on the uninsured, low income, and/or underserved populations in the service area and the safety net system in place for this target population. The issues of the capacity for culturally competent healthcare in the service area will be explored in focus groups with current patients and minority group members from the service area. Consumer focus groups will help identify significant but subtle barriers to healthcare and we expect that our consumer representatives on the FQHC Team will be able to translate their experiences so that we may develop a responsive CHC model. Interviews with individual providers of social support and healthcare as well as providing organizations and CBOs will also inform this part of the Needs Assessment.NeedsAssessment
Patient Education AssistanceThe organization has established a charity program with the Greater Westfield Free Clinic. Currently we are working to include this program in the hospitals credit and collection policy. We are currently working on enhancing this program to those patients who are in need of medical services and do not qualify for state funded programs but have no financial means to pay. These patients will be screened using the state eligibility tools and will be approved on a case by case basis.PatientEducationAssistance
Community InformationWestfield is an urban area that is relatively densely populated. The city of Westfield is bordered by West Springfield on the east, Southwick on the south, Granville on the southwest, Russell and Montgomery on the west, Southampton on the north and Holyoke on the northeast. Westfield is 99 miles from Boston and 85 miles from Albany, N.Y. Westfield has the second largest land area in the Commonwealth of Massachusetts, covering 30,000 acres. Westfield has a population of 42,133, of which 93.6% is white, 1.6% is Black, 4.9% is Latino and 1.6% is Asian/Pacific Islander. The per capita income is $25,436. Westfield's educational attainment profile is slightly better than the state's, except for a lower rate of post-college education. Twelve percent of Westfield residents live below the Federal Poverty Level and 23% live below 200% of the FPL. Over 43% of renters pay 35% or greater of their household income on rent. Westfield was a resettlement site for individuals from the former Soviet Union and over 2,400 have settled in the area. There is a growing Latino population as well as low income student population that study and work in the city.CommunityInformation
Other InformationThe organization provides services to all. The organization provided $1,400,000 in Free Care gross charges to the community during the year. Some of the organization's and related organization's providers served the Westfield Public Schools and worked at the Westfield Free Health Clinic. The organization has an active Patient and Family Advisory Council. The organization shared internal newsletter space with community events, fundraisers and information that was of interest to our employees. The organization allowed outside groups to use our facilities, including meeting and conference rooms, for free. Hospital departments and staff members participate in many different community events, school activities, and health fairs in the greater Westfield area during the year. These staff members are able to give out educational materials, talk to visitors, and even perform blood pressure, blood glucose, skin analyses, and other informational tests. We are able to reach not only visitors of the event, but also the other vendors/displayers as well. Many times, visitors at events might feel more comfortable with a "stranger" taking their blood pressure or glucose level, versus asking their doctor questions about these conditions. Many of our brochures and pamphlets are now available in English, Spanish, and Russian thanks to in-house translation and printing so we are able to give information out to more people in our community. We also partner with other for-profit and non-profit organizations in our area to enhance the awareness of good health maintenance and the services that the Hospital has to offer. Hospital staff also visit private homes, nursing homes, and other facilities for "Noble Conversations" where they answer questions about healthcare and Hospital servicesOtherInformation
