Income Based CriteriaFroedtert Hospital reserves the right to review each application for financial assistance on its own merits and to consider other extenuating circumstances in the decision to approve or deny a patient's application for financial assistance. The applicant's gross family income will be compared to the annual Federal Poverty guidelines set forth by the U.S. Department of Health and Human Services. A patient who has an annual gross income equal to or less than 400% of the current year's poverty guidelines will not pay more than 15% of their annual gross income on any single account during the approved eligibility timeframe. Patients who meet the requirements and have a gross income equal or less than 200% of the FPL may qualify for a 100% discount. Patients who meet the requirements and have a gross income between 200% and 400% of the FPL may qualify for a discount on a sliding scale. In addition to income, Froedtert Hospital also takes certain assets into consideration. Assets protected from financial evaluation include a portion of a household's retirement assets, cash, savings assets and home equity.IncomeBasedCriteria
Costing Method UsedCharity Care and certain other community benefits costs were determined by using internal information to reduce the various activities to cost. We try to present cost information consistently in the different venues that it appears.Froedtert Hospital reports accounts receivable for services rendered at net realizable amounts from third-party payers, patients, and others. Froedtert Hospital provides an allowance for uncollectible accounts based upon a review of outstanding receivables, historical collection information, and existing economic conditions and trends. The ratio of patient care cost to charges is applied to the bad debt attributable to patient accounts to calculate the estimated cost of bad debt attributable to patient accounts that is reported on line 2. Discounts and payments on patient accounts are recorded as an adjustment to revenue, not bad debt expense. As a not-for-profit, patient care is provided to all, regardless of ability to pay for that care. Making quality patient care available to all in our community, regardless of their economic means, qualifies bad debts as a community benefit.CostingMethodUsed

Debt Collection PolicyFroedtert Hospital has established a written policy for the billing, collection and support for patients with payment obligations. Froedtert Hospital makes every effort to adhere to the policy and is committed to implementing and applying the policy for assisting patients with limited means in a professional, consistent manner. Staff members who work closely with patients (including those working in patient registration and admitting, financial assistance, customer service, billing and collections) are trained regularly about these policies with an emphasis on treating all patients with dignity and respect regardless of their insurance or their ability to pay for services.DebtCollectionPolicy

General ExplanationPart IV - Froedtert Hospital is the owner of the shares. Part VI - Community Information (continued)Comparison of High Needs vs. Average/Low Needs Zip CodesFor the purposes of Froedtert Hospital's community health need assessment, Milwaukee County has been divided into two distinct service areas; one comprised of 13 zip codes which are designated as "high needs," and another comprised of the remaining zip codes, which are designated "average/low needs." The high needs zip codes have been identified in the Community Need Index Report which showed these specific zip codes lagging behind the rest of the county in several social, economic and health related factors. The table below (Source: DataBay; Revision Date: 10/11/07) provides a basic population demographic comparison of the two service areas in Milwauke County: High Needs Average/Low Demographic Factors Zip Codes Needs Zip CodesAge 0-17 32.4% 20.8% 18-34 25.6% 22.7% 35-44 13.5% 15.5% 45-54 12.3% 15.4% 55-64 7.7% 10.7% 65+ 8.3% 15.0%Race/ White 30.5% 87.3%Ethnicity Black 52.3% 5.4% Other 17.3% 7.4%Annual Under $50,000 69.9% 48.4%Household $50,000 - $74,999 16.2% 21.7%Income $75,000+ 13.9% 29.9%Education Bachelors Degree or Above 12.9% 27.4% Technical Degree 6.1% 9.3% High School Diploma 50.4% 50.5% No High School Diploma 30.6% 12.8%Employment Employed 54.5% 65.6% Unemployed 7.3% 2.5% Out of Workforce 38.1% 31.9%High Needs Zip Codes 8,077Total IP Discharges 15,253The primary issues identified in the high needs zip codes fall into two main categories; socio-economic disparities and health disparities. Socio-economic disparities: The demographic comparison of the two zip code areas showed significant differences, with high needs zip codes having generally lower incomes, fewer highly-educated residents, and more residents either unemployed or out of the workforce. These disparities make it more likely that residents in these zip codes will be uninsured and therefore limit their access to appropriate health care, which can lead to disparities in overall health. Evidence from the Community Health Survey supports this, as only 49% of the residents from the high needs zip codes had health insurance, compared to 60% in the average/low needs zips. Considering that only 8% of the residents in the high needs zips are eligible for Medicare coverage, this leaves 43% of residents either dependent on government aid or with no health coverage at all.Health disparities:These access barriers make it more likely that residents in the high needs zip codes will develop health issues. The comparison of Ambulatory Sensitive Conditions showed that these zip codes are more likely to have cardiac, respiratory, diabetic and other and chronic disease management related issues that require hospitalization than elsewhere in the county. To address these disparities, Froedtert Hospital's community benefit strategy falls into three categories: 1. Health Improvement / Community Capacity Building 2. Education / Workforce Development 3. Research / Needs AssessmentsGeneralExplanation
Needs AssessmentAs the academic medical center serving Southeastern Wisconsin, Froedtert Hospital's primary approach to community benefit focuses on addressing the variances in health needs and social determinants of health, associated with the high poverty, "high need" zip codes identified through the Community Need Index. Community Benefit FrameworkFroedtert Hospital is committed to working to improve the quality of life in the communities we serve through healthcare programs and services that are measurable, accessible and culturally appropriate; recognizing the greatest opportunity for impact is in Milwaukee's underserved, urban population. A vast majority of these neighborhoods are considered high need zip codes. Community AssessmentCompared to other Wisconsin counties, Milwaukee has ranked in the bottom five both in resident health outcomes as well as health determinants which predict future change in health. In fact, the 2010 national census rankings show that Milwaukee is the fourth poorest city in the nation.Froedtert recognizes its greatest opportunity for community benefit impact is in Milwaukee's underserved, urban population. Froedtert & Community Health's Strategic Planning and Support department provides ongoing analysis of demographic shifts and healthcare utilization and in 2007, completed a Community Health Assessment for Froedtert Hospital. The primary focus of the assessment was on those communities with a disproportionately high degree of poverty and health needs. Research ReviewedIn developing the assessment, the following were reviewed: - Community Needs Index Report (CNI) - The CNI is a measurement tool to identify and correlate geographic areas with preventable inpatient health conditions. The CNI is based on a wide array of demographic and economic statistics, and can be used to pinpoint specific zip codes that are at greater risk for health disparities. This report compared discharge rates in the Milwaukee metro area of Ambulatory Sensitive Conditions (ASC's) against Marker Conditions, for which hospitalization cannot be avoided. The 2007 CNI Report showed significant hospitalizations for patients from high poverty, high need zip codes, which could be avoided with adequate access to primary care and preventive services. - The Community Health Survey - The Community Health Surveys were conducted by JKV Research, LLC, between 2003 and 2006 for health departments in the Milwaukee metro region. - Wisconsin Healthy People 2010 - This study cites 2005 county-level health data gathered by the Wisconsin Department of Health Services (DHS). - Wisconsin County Health Rankings - A publication of the University of Wisconsin School of Medicine and Public Health/Population Health Institute. - What Works? Policies and Programs to Improve Wisconsin's Health - (January 2009) University of Wisconsin School of Medicine and Public Health - A comprehensive analysis of overall health and social determinants of health across a variety of populations and regions in Wisconsin. - U. S. Healthy People 2010 - Healthy People 2010 outlines a comprehensive, nationwide health promotion and disease prevention agenda. It is designed to serve as a roadmap for improving the health of all people in the United States during the first decade of the 21st century. - Professional Research Consultants (PRC) National Health Assessment (2005) - Systematic, data-driven approach to identifying the health status, behaviors and needs of Americans. - Westside Healthcare Association, Inc. Community Outreach Brief 2006 - A summary of interviews with community leaders from high poverty neighborhoods regarding community healthcare and health education needs. These data sources provide comprehensive comparative data against which Froedtert Hospital plans annual health education and outreach programming to improve the overall health of the community it serves.NeedsAssessment
Patient Education AssistanceFroedtert Hospital informs and educates patients regarding financial assistance and government program eligibility in a number of ways. Its communication efforts also address special needs of patients and their families, such as hearing or visual impairment or language interpretation. Information on hospital-based financial support policies and government programs are made available to patients during the pre-registration and registration processes through brochures, signage and direct contact with financial counselors, social workers / case managers and registration staff. Patient billing statements also inform patients that financial assistance is available. The Froedtert website contains information regarding pricing, how to understand your hospital bill, and how to apply for Financial Assistance. Froedtert Hospital has made financial assistance forms and information available in Spanish. Financial counselors screen uninsured patients for government program eligibility and social services staff are available to assist patients with enrollment processes. Patients who are uninsured, those covered by government programs and those with limited financial means may also be eligible for charity care or discounts through the Hospital's financial assistance program. Financial counselors make every effort to determine a patient's eligibility prior to or at the time of admission or service. However, determination for financial assistance can be made during any stage of the patient's stay after stabilization, or the collection cycle.PatientEducationAssistance
Community InformationAs Southeastern Wisconsin's academic medical center, Froedtert Hospital considers these five counties as its primary service area: Milwaukee, Waukesha, Ozaukee, Kenosha and Racine. Froedtert recognizes that the greatest need for health services and health improvement exists in Milwaukee County. Compared to other Wisconsin counties, Milwaukee has ranked in the bottom five both in resident health outcomes as well as health determinants which predict future change in health.
