Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part I, Line 3c:EMC uses FPG and has a charity care policy to determine eligibility for charity or discounted care.
Part I, Line 7:EMC operated outpatient 1206(d) clinics that provide primary care and health services to low income families and communities.
Part II, Community Building Activities:Leadership development and training:Emanuel Medical Center provided the opportunity for students in the health care fields to learn in our facility free of charge. The education department coordinated clinical rotations and provided orientations for nursing students and high school Regional Occupational Program (ROP) students. Staff that work with these students provide one-on-one attention and teaching throughout the students' experience here. In the 2013 school year, a total of approximately 25 ROP high school students from Turlock and Hilmar were each provided 100 - 150 hours of experience in patient care or administrative areas. In the spring and fall semesters of 2013, approximately 125 total nursing students each completed an average of 100 hours of clinical rotations at our facility. Aside from the large amount of time that staff members spend with students, the education department itself provided approximately 200 hours of time devoted to coordination and orientation for these students.Physician availability and work force development:Completed in 2011, Emanuel Medical Center's latest community physician need study documents significant growth in the number of physicians required in the primary services area of the medical center. However, the community continues to have fewer than half the number of physicians per 100,000 population at the most conservative benchmarks for physician need.During the year ending January 31, 2014, Emanuel Medical Center recruited one new general surgeon who is also boarded in colorectal surgery, the first in the region and in Emanuel's service area. Additionally, a new nurse practitioner was hired to provide services in the new Patterson Primary Health Center operated by Emanuel. In FY 2014, Emanuel continued to provide a medical education program specifically designed to increase the number of physicians in the community. In the fourth year of this program, a total of 25 students from two medical schools - Midwestern and Touro Medical School Universities - began their year-long clinical rotation studies under the direction of Emanuel medical staff.
Part III, Line 2:Bad debt expense of $31,909,692 was subtracted from total expenses for purposes of calculating the percentages in Part I, Line 7. EMC separates charity care expenses from bad debt expenses on the financial statements and does not include bad debt expense in charity care. Bad debt expenses are recorded for self-pay patient accounts that are not qualified for charity care and are not collectible for services provided by EMC. The cost methodology for bad debt expense is based on cost-to-charge ratio.
Part III, Line 3:The Center maintains separate records of bad debt expenses and charity care expenses. The charity discounts and charity care provided during the year totaled $58,327,412, separate from the true bad debts totaling $31,909,692. The charity discounts and care conform to the Financial Assistance Policy guidelines.
Part III, Line 4:Patient accounts receivable: The Center reports patient accounts receivable for services rendered at net realizable amounts from third-party payors, patients and others. As a service to the patient, the Center bills third-party payors directly and bills the patient when the patient's liability is determined. Patient accounts receivable are due in full when billed. Accounts receivable are reduced by both an allowance for contractual adjustments and an allowance for doubtful accounts. Patient accounts are written off when collection efforts have been exhausted.
Part III, Line 8:The Center uses a cost to charge ratio methodology to determine the allowable costs of care related to Medicare costs. This involves using prospectively determined prices for each patient service and subsequent application given the cost of such service.
Part III, Line 9b:EMC has a charity care policy to provide patient care for all patients regardless of ability to pay. Financial assistance is extended in accordance with AB774 and the mission and values of the hospital.
Part VI, Line 2:To assess health care needs in EMC's service area, we rely on the "Stanislaus County Community Health Assessment" prepared and published regularly by the Stanislaus County Health Services Agency. In preparing the most recent assessment, the Stanislaus County Community Health Assessment Steering Committee developed more than 70 quality-of-life indicators; indicators that are understandable to the general user and the public, that respond quickly and noticeably to real changes, that are relevant for policy decisions, and that are available annually. In preparing its assessment, the assessment steering committee gathered primary data from a face-to-face survey of more than 2,800 Stanislaus County residents and used a number of secondary data sources including the U.S. Census Bureau's American Community Survey; the Centers for Disease Control and Prevention's behavioral risk factor surveillance system; the California health interview surveys for 2007, 2009, and 2011-2012; the California healthy kids survey; and the healthy people 2020 objectives.
Part VI, Line 3:EMC's charity care policy is communicated to every patient through procedures spelled out in the policy itself. Those requirements include the following:1. Every patient is presented, at time of service, with notice (in appropriate language) regarding the AB774 discount/charity care program. Patients will be asked to sign an acknowledgement attesting that notice of program and offer of applications has transpired.2. A follow-up notice regarding the AB774 discount/charity care program is included with first billing for services. A plain language summary of patients' rights pursuant to SB 350 is included in this notice.
Part VI, Line 4:EMC serves the City of Turlock and a number of smaller communities surrounding it in southern Stanislaus and northern Merced counties. This area is located in the heart of California's central valley and is generally more rural, agricultural, ethnically diverse and economically disadvantaged than California as a whole. Part I, Line 1aJohn Sigsbury, President/CEO of EMC, utilizes the Country Club membership. Housing allowances are occasionally provided on a temporary basis to new hires recruited from out of town.
Part I, Lines 4a-bDavid Canton received $113,057 in severance pay. EMC and related organizations provide certain supplemental retirement benefits to their officers and key employees. These benefits are provided through a nonqualified deferred compensation plan, under which a portion of the benefits being earned are structured to provide a retention incentive that has been determined by the compensation committee of CMB's Board (CMB is EMC's parent) to be of a substantial value to the organization. The committee approves all retirement benefits, together with all other forms of compensation and benefits for these and other executives, in a manner intended to qualify for the rebuttable presumption of reasonableness under the intermediate sanctions rules of federal income tax law.
