DescAcute - InpatientProvidence Saint John's Health Center has been serving the Santa Monica and westside Los Angeles county communities since 1942, and since that time has earned a reputation for clinical excellence and award-winning care in a compassionate and tranquil healing environment.Today, Providence Saint John's is a nationally renowned 266-bed hospital with physicians, nurses, volunteers and support staff who work as a team to provide the best possible medical care to its patients and the community. Providence Saint John's provides the latest in diagnostic technology and specialty care with a strong commitment to its core service lines: heart and vascular care, oncology, orthopedics and women's health. Saint John's also is home to the world-renowned John Wayne Cancer Institute, dedicated to clinical research and medical advancements in cancer care.Providence Saint John's reputation for academically-based care guided by research and evidence-based medicine is due to the commitment to excellence by our physicians, nurses and caregivers. Our cancer surgeons care for patients in the Cancer Center and hold faculty positions at the John Wayne Cancer Institute and other teaching institutions in the community. Nearly every core service line at Providence Saint John's participates in research or clinical trials.Founded by the Sisters of Charity of Leavenworth, who in 2014 passed its sponsorship to Providence Health & Services, Providence Saint John's is rooted in the Catholic health care tradition, which is devoted to providing leading-edge medicine with unwavering compassion and personalized care. In line with both its Catholic Mission and its responsibilities as a non-profit health care provider, Providence Saint John's seeks to create healthier communities by investing in community benefit programs, with an emphasis on the poor and vulnerable.Providence Saint John's continually has achieved top ratings and distinguished hospital awards for clinical excellence and patient safety by the nation's leading accreditation organizations, such as Healthgrades, The Joint Commission and Leapfrog Group. Within the past decade, Providence Saint John's has received multiple acclamations by these organizations for a variety of services including cardiac care, coronary intervention, critical care, orthopedics, pulmonary care, joint replacement, neurosurgery, and women's health.Through the dedication and collaborative efforts of the community, friends, patrons, physicians, nurses, staff and volunteers, Providence Saint John's continues its world-class tradition established more than 70 years ago by providing breakthrough medicine in an environment of inspired healing.Year after year, Providence Saint John's Health Center along with its staff, physicians and nurses are recognized by the nation's leading healthcare rating organizations, the medical community and the media for clinical excellence and exceptional patient care. DescGrants & Allocations - Saint John's motto, "...and the greatest of these is charity...," reminds us that love and charity are the same. Caritas is the Latin word for both "love" and "charity." Caritas Benefits are the focus and priority for the Providence Saint John's community benefit program. These benefits are the goods, services and financial resources that Saint John's provides, manages or coordinates for those living in poverty in our community. Our community is fortunate to have many outstanding organizations and agencies delivering vital services for those living in poverty. OPCC, Saint Joseph Center, Venice Family Clinic, and Westside Family Health Center are just a few examples of these partners. Providence Saint John's commitment to care for the poor and underserved is a commitment to their work. Our care of those living in poverty is built on yet another partnership. The dedicated physicians who provide professional services in Providence Saint John's Laboratory, Imaging Department, and Heart Institute are our essential partners in caring for patients referred to Saint John's for diagnostic and other services by community agencies. These physicians volunteer their time and professional skills, as Providence Saint John's volunteers its staff, resources and facilities, to provide these services at no charge to those living in poverty.Providence Saint John's has been blessed by many extraordinary partners, such as Santa Monica-Malibu Unified School District (SMMUSD) and St. Anne's School. Providence Saint John's supports SMMUSD's School Nurse Program in order to enhance the health status of our community's school age children. Saint John's partners with St. Anne's School by funding a school nurse and providing a school counseling program, thereby extending its community health mission into local parochial schools.Providence Saint John's is a major employer and a not-for-profit corporate citizen on the Westside. As such, we support community organizations such as the Venice Family Clinic, the Santa Monica-Malibu Unified School District, the Westside Family Health Center, OPCC, Saint Joseph's Center, the Saint John's Child and Family Development Center, WISE & Healthy Aging, Step Up on Second, Upward Bound House, St. Anne's School, Pico Youth & Family Center, The City of Santa Monica, Westside Shelter & Hunger Coalition, the Santa Monica Chamber of Commerce, the Santa Monica Firefighters Association, and the American Red Cross.Desc

Saint John's Health CenterPart V, Section B, Line 5: The needs assessment is validated by Saint John's Community Advisory Committee, composed of members of local community based organizations, and local governmental agencies.
Saint John's Health CenterPart V, Section B, Line 11: In the 2013 CHNA, there were four priority needs/issues identified from this assessment. The first need was affordable and accessible mental health services. Providence Saint John's Health Center operates the Child and Family Development Center (CFDC). This program offers free and low cost mental health services to children and youth at the facility in Santa Monica and through partnerships with area schools and nonprofit organizations. The CFDC also offers a therapeutic preschool to assist children that might be dealing with learning and behavioral issues. The second priority need was assistance for homeless patients using the emergency department at the hospital. PSJHC is working closely with several homeless organizations in the area to better address the needs of these patients. PSJHC helps to fund some of the projects at two of the area's homeless organizations. The organization also funds homeless health care services through a program operated by Venice Family Clinic. Also, the hospital will be initiating a Homeless Care Navigator Program in the emergency department in the fourth quarter of 2015. This person will work closely with hospital staff, physicians, and the community to ensure that the patients are getting connected with the resources and services that they need. The third priority need was improved access to affordable and convenient primary and specialty care. PSJHC is working in partnership with UCLA Health System, Venice Family Clinic, Westside Family Health Center, Kaiser Permanente and Cedars-Sinai Health System to look at ways to expand medical access on the Westside. The organizations serve on a collaborative committee and the group is focusing on ways to contract with Medi-Cal managed care to improve specialty access. The group has also identified the top medical specialties that are in demand in the area, and work is underway to try and improve access in these areas including gastroenterology and dermatology. The fourth priority need was expanded health education and wellness programs for the community. PSJHC offers a very robust health education program for the community. Four health education symposia are offered each year for the community that focus on various health and wellness topics (e.g. nutrition, cancer prevention, chronic disease, women's health, etc.). Staff and physicians from PSJHC participate as expert speakers at these educational events that are very well received by the community with 70+ people attending on average. Not selected at this time were dental care and assistance in enrollment in the new insurance programs - Saint John's does not have the resources to respond to the need at this time.
Saint John's Health CenterPart V, Section B, Line 22d: The hospital uses a sliding scale based on income levels of the federal poverty guidelines based on full billed charges and approves financial assistance based on the scale of the income of the family.

Part I, Line 3c:The PH&S sliding fee scale will be used to determine the amount to be written off as charity care for guarantors with income between 201% and 400% of the current federal poverty level after all funding possibilities available to the guarantor have been exhausted or denied and personal financial resources and assets have been reviewed for possible funding to pay for billing charges.
Part I, Ln 7 Col(f):The Bad Debt expense included on Form 990, Part IX, Line 25(A), but subtracted for purposes of calculating the percentage in this column is $ 9,415,889.
Part II, Community Building Activities:We extend our care beyond our hospital's walls in order to improve the health of our community. Our community building activities demonstrate this commitment. During this year, our community building activities included environmental stewardship, presentations to educational professional by child & family development center mental health providers, participation in the faith committee of the Westside Shelter & Hunger Coalition. We are an important part of our community and serve in many ways, from delivering core health care to preventive care to support of other civic groups. Our executives serve on the boards of many local nonprofit agencies throughout the community and our board of directors represents medical and business professionals, and all provide hours of service in support of our hospital. They are deeply involved in our needs assessment process, building programs and services, and community outreach to ensure that people know about services available to them through our hospital. When Saint John's has excess revenue over operating expenses, we use those funds to obtain current health care technologies and equipment, improve patient care, provide medical training education and research, and to expand access to points of care. These investments ensure we'll be here to care for future generations. We have an open medical staff, we provide several educational events designed to promote healthy behaviors, and access to medical care, and continue to donate unused supplies to nonprofit corporations in the community and those that serve international needs. We also support our employees in volunteering for community organizations, including serving on community boards, and provide opportunities for them to support causes through hospital events such as:Autism Advisory Board California Professional Society on the Abuse of Children (CAPSAC) California State College, Los Angeles Child Abuse Professional Providers Association (CAPPA) Clare Foundation Hospital Association of Southern California Interagency Council on Abuse and Neglect (ICAN) Institute of Contemporary Psychology John Adams Middle School Student Success Team Meetings Los Angeles Unified School District (LAUSD) Los Angeles Service Area 5 Provider Advisory Committee Los Angeles County Department of Mental Health Networking Meetings Los Angeles United School District Fair for Parents of Deaf and Hard of Hearing Children Los Angeles Chapter of the Hearing Loss Association Lifelong Learning Community Partnership Lincoln School Counselor Meetings McKinley Elementary School English Language Advisory Committee OPCC* (formerly Ocean Park Community Center) Organization of Community Human Service Agencies Pico Neighborhood Project and Pico Youth and Family Center Rand Corporation Meetings (as invited)Rideshare Santa Monica Chamber of Commerce Santa Monica Child Care Task ForceSanta Monica College - Child Development Department Santa Monica Early Childhood Task Force Santa Monica Rotary Club Santa Monica Malibu Unified School District Santa Monica College Foundation Search to Involve Pilipino Americans Southern California Bioethics Consortium St Joseph Center Unity Fair at Virginia Ave Park Upward Bound House Venice Family Clinic West Service Planning Area Westside Child Trauma Council Westside Children's Center Westside Diabetes Task Force Westside Directors (UCLA/Connections for Children) Westside Domestic Violence Coalition Westside Family Health Center Westside Meals on Wheels West Los Angeles Department of Mental Health Child Mental Health Providers Group Westside Health Coalition Westside Transportation Collaborative Wise & Healthy Aging Youth Resource Team of Virginia Avenue Park YWCA of Santa Monica We are good citizens and partner with other organizations and agencies to support a thriving community, including: Santa Monica Chamber of Commerce Santa Monica College Foundation Santa Monica Police Activities League Santa Monica Fire Department Westside YMCA and YWCA Westside Domestic Violence Network Prior to 3/1/2014, Saint John's Health Center was part of the Sisters of Charity of Leavenworth Health System (SCLHS). As part of SCLHS, we promote the shared mission that "we will, in the spirit of the sisters of charity, reveal God's healing love by improving the health of the individuals and communities we serve, especially those who are poor or vulnerable". We are committed to living and demonstrating our core values of excellence, respect, response to need, stewardship and wholeness. SCLHS supports its hospitals by providing guidance, oversight, and resources to help them accomplish initiatives that improve health in all our communities. This includes coordinating community benefit processes, providing guidance with community needs assessments, and establishing consistent financial assistance and charity care policies and procedures. Other ways SCLHS benefits its hospitals include quality improvement and performance excellence initiatives, system-wide IT implementation and infrastructure, strategic and operations direction and oversight, supply chain management and purchasing, benefits administration (including a wellness program free for employees that promotes their health and well-being), risk management, disaster planning and crisis assistance. By sharing the work across our system we are able to lighten the burden for all in order to fulfill our mission of improving health in our communities. SCLHS improves overall health in our communities by providing infrastructure to support and sustain four clinics for the uninsured in Kansas and Colorado. The SCLHS clinics for the uninsured are often the only safety net clinic in their community and provide valuable services for those who are most vulnerable. Each hospital is supported by a foundation to access and optimize local philanthropic organizations and individuals. These foundations raise funds through outreach, special events, and building donor relations throughout their community to support the needs of the hospital. In March of 2014, Saint John's Health Center affiliated with Providence Health & Services. As a not-for-profit Catholic health care ministry, Providence Health & Services embraces its responsibility to provide for the needs of the communities it serves - especially the poor and vulnerable. Providence's not-for-profit, tax-exempt status enables Providence to serve its communities, to solicit donations through its foundations and to access capital to respond to community needs that otherwise would go unmet.
Part III, Line 2:Bad debt expense represents the amount of gross charges for patients who do not have insurance and which Providence was unable to qualify for assistance under either government programs or our internal charity care policy. 2014 expense is $ 9,415,889.
Part III, Line 4:The Health System provides for an allowance against patient accounts receivable for amounts that could become uncollectible. The Health System estimates this allowance based on the aging of accounts receivable, historical collection experience by payor, and other relevant factors. There are various factors that can impact the collection trends, such as changes in the economy, which in turn have an impact on unemployment rates and the number of uninsured and underinsured patients, the increased burden of copayments to be made by patients with insurance coverage and business practices related to collection efforts. These factors continuously change and can have an impact on collection trends and the estimation process used by the Health System. The provision for bad debts in 2014 has decreased from 2013 as a result of the expansion of Medicaid programs and initiatives to assist patients in their Medicaid enrollment. The Health System records a provision for bad debts in the period of services on the basis of past experience, which has historically indicated that many patients are unresponsive or are otherwise unwilling to pay the portion of their bill for which they are financially responsible.
Part III, Line 8:It is the organization's policy to exclude any Medicare shortfall from Community Benefit information. The amount reported on Part III, Section B, line 6, was determined by applying the Cost-to-Charge Ratio to the Medicare revenue.
Part III, Line 9b:Saint John's Health Center has billing and collection practices in place that take into account each patient's ability to pay, and that demonstrate our core value of respect. We work with each patient to help them understand their financial responsibility for care received, financial assistance available to them, and to establish payment programs. An integral component of our mission is to be good financial stewards. This requires us to determine which patients are in need of charity care and which are able to contribute some payment for care received. This balance enables us to continue to provide charity care to those who need it most, and to ensure that we will be here when people need us. Saint John's Health Center notifies patients of financial assistance policy upon admission, discharge and in communication regarding patient bills. Patients are contacted multiple times about unpaid balances prior to initiating any collection action. If a patient is determined to be eligible for financial assistance at any time during the collection process, the account is reclassified as financial assistance and debt collection efforts are ceased.
Part VI, Line 2:As part of our core value of response to need, we take steps to determine where there is the most need in order to provide the greatest good. Saint John's Health Center has regularly participated in needs assessment surveys to identify the ongoing and changing needs of the community. The most recent survey was conducted in 2013. This assessment studies a defined community served by Saint John's Health Center, and is done by partnering with local government and social agencies, combining efforts with other local hospitals and health care organizations, and conducting surveys and assessments with the assistance of outside consultants. We also continuously assess the needs of the community through close working relationships and partnerships with service agencies in the community, and by evaluating state and county health statistics. Saint John's Health Center makes the entire community needs assessment available on request. Based on 2013 needs assessment survey, Saint John's Health Center chose the following needs as prioritized needs:- Mental health services- Inebriated homeless patients in the emergency department- Access to affordable and convenient primary and specialty care- Health education and wellness promotionSaint John's chose these areas because they were the most frequently mentioned by survey respondents, responses can be targeted to the areas of greatest need, and because in each of these areas there is a structured collaboration in place that can facilitate effective action.
Part VI, Line 3:Saint John's Health Center treats patients with respect and dignity regardless of their ability to pay. Saint John's Health Center has a written financial assistance policy that explains eligibility criteria for financial assistance at various levels, including a 100% discount, and is based on federal poverty guidelines. We work with patients to help them understand their financial responsibility for care received, financial assistance available to them, and to establish payment programs in demonstration of our core value of respect. As part of our responsibility to educate, we inform our patients and their families of the availability of assistance, including government programs. We communicate this in a variety of ways to ensure that messages reach multiple audiences. Before, during and/or after admission, we encourage our self pay patients to visit with a financial counselor to discuss qualifications for financial assistance. The financial counselor works with the patient to complete a financial assistance form to determine the level of discount for which the patient may be eligible. We post financial assistance information in emergency and admissions areas, on billings invoices, in various areas around the hospital and clinic sites and on the hospital website. We provide written materials to patients that detail our financial assistance policy and how it is administered. This covers eligibility, steps to follow to determine if a patient qualifies for assistance, typical charges a patient may expect for routine procedures, and assistance available based on a patient's income level. We provide materials and education when patients are discharged, and include information in billing statements, including phone numbers and other methods to contact us with questions. We ensure that our financial counselors, admission employees, social workers and other employees understand our policies to be able to assist patients in the most appropriate way.
Part VI, Line 4:Saint John's Health Center determined our community by reviewing the addresses reported by our patients. 78% used a zip code within the Service Planning Area (SPA) 5.A Service Planning Area, or SPA, is a specific geographic region within Los Angeles County. A variety of demographic and health information was gathered from both primary and secondary sources for our most recent community needs assessment conducted in 2013. Secondary data for Saint John's Community Health Needs Assessment involved collection and analysis of demographic and health status information for SPA 5. The following types of information were used: local health department indicators of health status, city reports of health and development assessment, homeless counts, population, death statistics, designated health professional shortage areas, large scale telephone surveys designed to measure health access, utilization of health services and health behaviors. The second phase of the needs assessment was qualitative primary research. Telephone, online and in-person surveys with representatives from public and private community organizations were conducted. Respondents were asked to indicate how they wanted to provide feedback. This was selected as an efficient, convenient and cost effective way to gain understanding of important health issues and potential opportunities to collaborate. Part I, Line 1aProvidence Health & Services Expense Reimbursement Procedures include the following policies: First Class Travel or Charter Travel or Travel of Companions Air travel is reimbursable for tourist or economy class and should be at the least expensive airfare; which permits departures and arrivals at reasonable times and reasonable distance traveled. Employees are encouraged to plan in advance to get available discounts. Airline frequent flyer upgrades will never be reimbursed. First class air travel will only be reimbursed when tourist or economy class air travel is not available and business travel is mandated by a supervisor. In the rare circumstance that an executive must fly on a first class full fare ticket, their senior level supervisor must approve this expense. Companion travel will only be reimbursed by the organization for travel related to relocation, and should not exceed two relocation-related visits, unless approved by the Executive Vice President/Chief People and Experience Officer. Spouse or Companion Travel. Travel expenses incurred by a PH&S employee's spouse or companion will not be reimbursed by PH&S unless the spouse or companion is required to, or invited to attend a PH&S System-sponsored meeting. These expenses may be considered a taxable benefit by the IRS and if so, will be included on the employee's W- 2. During 2014, there were three First Class tickets utilized by Officers, Directors or Key Employees listed on Form 990, Part VII. Tax Indemnifications or Gross-Up Payments Providence Health & Services follows the federal and state taxation laws related to relocation expenses paid to the employee or to a third party on the employee's behalf. They are considered income and are therefore subject to payroll taxes. Based on the way Providence has chosen to pay the relocation expenses, Providence reports reimbursements and payments to vendors as income and these expense payments are reflected on the executive's Form W-2. Providence will gross-up the relocation benefits to offset the personal tax burden to the employee for IRS allowable expenses. During 2014, the following Listed Persons received gross-up payments: Debra Canales Lisa Vance Craig Wright, MD The amounts reported for these gross-up payments are included on Schedule J, Part II, Column B (iii) - Other Reportable Compensation. Housing Allowance or Residence for Personal Use Providence Health & Services provides housing allowances for purposes of relocation assistance only. Providence may pay temporary living expenses for the employee up to a maximum of 90 calendar days. Covered expenses are rent (excluding "rent" which may be paid in order to occupy a new permanent residence until the title clears) and utilities, including heat, electricity, gas, water, local internet and local telephone and garbage services. The Executive Vice President/Chief People and Experience Officer may approve temporary housing assistance for up to six months when family relocation is delayed to accommodate the school year or equivalent circumstances. Only in extenuating circumstances is housing extended beyond this six month period. During 2014, the following Listed Persons received relocation/housing program payments: Debra Canales Lisa Vance Craig Wright, MD The amounts reported for these relocation/housing payments are included on Schedule J, Part II, Column B (iii) - Other Reportable Compensation.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Form 990, Part VI, Section A, line 4The governing documents were amended naming Providence Health System - Southern California as the sole Corporate Member effective March 1, 2014.
Form 990, Part VI, Section A, line 6The sole Corporate Member is Providence Health System - Southern California, a California nonprofit religious corporation.
Form 990, Part VI, Section A, line 7aThe Corporate Member has the power to: 1) Appoint or remove, with or without cause, after consultation with the Corporation's Board of Directors, any member of the Board of Directors. 2) Appoint or remove, with or without cause, after consultation with the Corporation's Board of Directors and the President/CEO of the Corporate Member, the President/CEO of this Corporation.
Form 990, Part VI, Section A, line 7bPowers of the Corporate Member include the power to: 1) Change the Mission and Philosophy. 2) Adopt, amend or repeal the Articles of Incorporation or Bylaws. 3) Implement corporate goals, policies and procedures. 4) Approve the acquisition of assets, incurrence of indebtedness or the lease, sale, transfer, assumption, or encumbering of assets. 5) Approve the merger, dissolution or corporate restructuring. 6) Approve all long-range and strategic plans and operating and capital budgets and deviations thereto. 7) Appoint the auditors.
Form 990, Part VI, Section B, line 11The Form 990 is prepared internally by experienced staff and reviewed by the internal Director of Taxes and external tax advisors. The Board of Directors reviewed the Form 990 prior to filing with the IRS.
Form 990, Part VI, Section B, line 12cProvidence Health & Services maintains a conflict of interest policy that applies to board members and management of all Providence-related organizations. The purpose of the policy is to guide and direct those serving the Providence Health & Services' corporations and other legal entities so they can (1) fulfill their fiduciary responsibilities and exercise stewardship in ways that promote and protect the best interests of Providence and, (2) avoid situations that create a conflict, or the appearance of a conflict, between the interests of an individual associated with Providence and Providence. On an annual basis, each board member and management level employee must complete and submit an updated conflict of interest statement. Conflict of interest disclosures are reviewed by the System Integrity Department working in conjunction with the Department of Legal Affairs. If it is determined that an actual conflict exists, appropriate follow-up action is taken with the individual to rectify the conflict.
Form 990, Part VI, Section B, line 15It is Providence's intention to make financial information accessible and transparent. Although the filing of Form 990 provides insight into how Providence achieves its Mission, delivers its programs and stewards its finances, deciphering the information directly from Form 990 can be challenging. The following paragraphs provide further information about the process we use to determine compensation for top management, officers and key employees. Providence has a single fiduciary Board, with responsibility for financial oversight associated with fulfillment of the Providence Mission, developing system policies, protecting the assets entrusted to the organization and overseeing the strategic and operational affairs of Providence's legal entities. Providence also maintains a network of community ministry boards with responsibility for quality of care oversight, community relations, advocacy and community needs assessments. Providence has a consistent compensation philosophy for all of its employees, including our senior executives. Salaries for senior executives are reviewed by the Providence Board's Human Resources Committee and approved by the full Board of Directors, none of whom is a Providence employee. The Board retains an independent consultant each year to review salaries of those in the most significant leadership roles in the organization. Part of the consultant's role is to review an extensive array of compensation surveys of large, not-for-profit health care systems in the United States. Providence is one of the larger health systems in the country, and as such, the Board benchmarks executive compensation against other large, not-for-profit health systems whose revenue is similar to that of Providence. Base salaries for Providence executives are set at the median level of the market, as identified by the independent consultant and reviewed with the Human Resources Committee. Each year, the Board Chair conducts a formal performance evaluation of the President/CEO that considers input from the other directors and senior leaders reporting to the President. The evaluation is discussed with the Human Resources Committee and then a recommendation is made by the committee to the full Board. The Board Chair and the Chair of the Human Resources Committee also meet with an independent consultant to develop a salary recommendation; which is reviewed and approved first by the committee and then by the Board of Directors. Additionally, the President/CEO utilizes the market information provided by the consultant along with formal performance evaluations, to determine salary recommendations for other senior executives. This process includes a rigorous analysis of those recommendations with the Human Resources Committee as a part of the review and approval process. Performance incentives allow executives to earn additional compensation if they achieve specific organizational goals for furthering Providence operating commitments and strategic objectives - advancing the Providence Mission and core values, meeting benchmarks for charity care, achieving quality targets, delivering top-rated patient satisfaction, meeting employee satisfaction goals and reaching financial performance objectives. The Board of Directors conducts a thorough process to ensure performance incentives are aligned with appropriate practices for not-for-profit health care systems. The Board's process for executive compensation fully complies with IRS standards and mirrors the best practices recommended in the "Report to Congress and the Nonprofit Sector on Governance, Transparency, and Accountability" submitted to the Senate Finance Committee by the Panel on the Nonprofit Sector.
Form 990, Part VI, Section C, line 19Public disclosure of governing documents, conflict of interest policy and 990 filings are made available to the public upon written request. The consolidated financial statements are available on our public Internet site www2.providence.org. All governing policies including the conflict of interest policy, as well as 990 filings are available to employees on the Intranet site.
