| Schedule H, Part V, Section B, Line 5 Facility , 1 | Facility , 1 - ST. JOSEPH'S AREA HEALTH SERVICES. St. Joseph's Area Health Services' Hospital Board of Directors approved the initiation of the Assessment and was overseen by the St. Joseph's Community Benefit committee. It was completed in collaboration with our Community Health Department, which serves as the Public Health agency for Hubbard County, and a team of people with expertise, interest, and knowledge of community health issues. This project brought together representatives of many health and human services provider agencies and the voices of our residents to complete a collaborative health assessment so we would better understand crucial health issues in the community. Personal invitations were sent to over 60 agency staff, and five focus groups were conducted with 54 area participants, including our Hospital Board of Directors. To reach the broader community, a survey was offered at various locations, and nearly 450 surveys were completed. Healthy Park Rapids Area 2020, a community collaborative entity of health and human service providers with a goal to improve community health and well-being, served as an advisory group through the assessment and implementation process. In December 2014, the board met to review progress made on the implementation plan and recieved a detailed update from the Community Benefit Plan Chairperson. |
| Schedule H, Part V, Section B, Line 11 Facility , 1 | Facility , 1 - ST. JOSEPH'S AREA HEALTH SERVICES. There were five (5) items identified as the highest priority under the 2013 CHNA and they are listed as follows: 1. Community Wellness 2. Economic factors to influence wellness 3. Substance and chemical dependency 4. Mental Health and Violence 5. Subsidized Health Services Below is a detailed update on these needs and we are addressing all five at this time. Evaluation of the CHNA Implementation Strategies: Community Wellness: The Minnesota Department of Health through their Statewide Health Improvement Program (SHIP) has developed a menu of strategies that has demonstrated effectiveness based on research and proven best practices. Our chosen strategies are designed to address obesity and commercial tobacco use and exposure effectively and sustainably at the community, school, and worksite. |
| Schedule H, Part V, Section B, Line 11 Facility , 2 | Facility , 2 - ST. JOSEPH'S AREA HEALTH SERVICES. * Formalize & Enhance Wellness Initiatives: - We have promoted comprehensive employee wellness initiatives that provide health assessments with follow-up coaching, ongoing health education and policies that promote healthy weight and healthy strategies at worksites. - Amy Erickson serves as the CHI St. Joseph's Health's worksite wellness coordinator for interested community worksites. - We provided consultation programs for the schools on Farm to School, Safe Routes to School, and Active School Days. - Raeann Mayer wrote a USDA Farm to School planning grant, and the Park Rapids and Laporte Schools received $45,000 to create a sustainable Farm to School program. - SHIP provided funding to the Park Rapids Farmer's Market for Market Bucks to low-income families to allow them to purchase locally grown foods and for their hand washing station. They also provided funding of up to $5,000 to the Laporte School for a School garden, and monies for Hubbard Country to purchase supplies for a Lactation room. We have promoted community programming for healthier eating (farmer's markets, community gardens), increased non-motorized transportation (walk and bike plans and walkability and bikeability), and healthier eating and increased physical activities in child care. - Through our SHIP grant, we promoted the Bike to Work day and offered bike helmets and water bottles to registered adults and children. - We have purchased six bicycles for our employees to use in the community while at work. |
| Schedule H, Part V, Section B, Line 11 Facility , 3 | Facility , 3 - ST. JOSEPH'S AREA HEALTH SERVICES. * Our Community Health WIC Department : - Has offered food vouchers and nutritional counseling to 530 pregnant women, new mothers, and young children up to age five in Hubbard County. - Has offered healthy eating information to community groups, upon request. - Has shared food from the garden to WIC participants through 2014. Staff were unable to devote time to the garden starting the summer of 2015. * St. Joseph's Health Radiology Department continues to provide mammograms for low income women through the SAGE program. * We have supported the Food Angels Program, a weekend back- to-back food program for K-4th grade. In year one, 64 children received weekend meals, and in year two, 178 children participated. * We received a grant from Prime West Health to allow for a Lifestyle Coach and Navigator position to assist consumers in finding the program and/or offering that is right for the person, i.e. MOVE, Personal Trainer, etc. We have been able to offer free transportation to medical and health appointment's in an effort to remove transportation barriers. Economic Factors to Influence Wellness: With education being the gateway to employment, we have created initiatives to promote the social connectedness of our students to their schools and to assist the clients in our FATHER project with educational and employment programming. - St. Joseph's has financially supported the Park Rapids School District with their after-school programming and the after school transportation. These initiatives create a safe place for students after school and a way to stay connected with after school activities. - Our FATHER project has an employment counselor that has offered comprehensive employment services, including job search support and resume development to over 65 Dads throughout the past three years. |
| Schedule H, Part V, Section B, Line 11 Facility , 4 | Facility , 4 - ST. JOSEPH'S HEALTH SERVICES. Substance and Chemical Dependency: St. Joseph's received a 5-year Planning and Implement Grant to focus on youth alcohol prevention. From this grant, the Hubbard in Prevention Community Coalition strives to reduce the use of alcohol, tobacco, and other drugs by youth in Hubbard County. Some key strategies used to achieve goals are: 1. We have had collaboration of community sectors for a comprehensive prevention plan. -An Evidence based curriculum has been offered in schools -Alcohol Compliance Checks with law enforcement have been completed. -Responsible Beverage Server Training for local establishments has been held. 2. The program has progressed through a Scientific Prevention Framework - This planning process consisted of guides for the selection, implementation, and evaluation of prevention activities. These steps included needs assessment, capacity building, planning, implementation, and evaluation. 3. This was a primary prevention intervention through environmental activities. -Environmental prevention strategies are aimed at changing the surroundings in which individuals and families live. This approach takes into account that individuals do not become involved in substance abuse solely on the basis of individual characteristics but rather are also influenced by other factors in their environment. Examples of these factors included rules and regulations, norms or perceived norms of substance use, media messages, and accessibility. Since the beginning of the program, they have collected student data showing a 6 percent increase of Park Rapids High School students who choose NOT to drink in a typical month over the past 4 years. |
| Schedule H, Part V, Section B, Line 11 Facility , 5 | Facility , 5 - ST. JOSEPH'S AREA HEALTH SERVICES. Mental Health and Violence: There is a growing understanding of the relationship between violence and mental health, along with the science base that confirms that violence is preventable. St. Joseph's has utilized the Spectrum of Prevention from the Prevention Institute to develop a comprehensive workplan with strategies that not only prevent violence but also foster good mental health. - We contributed $1,000 to the local mentoring program, Kinship, ensuring the most at-risk children have connections with non-judgmental, caring adults as Mentors. - We have offered positive emotional and social development by offering the Growing Great Kids curriculum for 40 at-risk families, the SAFE program to 20 Park Rapids After-school program participants, and the Second Step curriculum at the Nevis and Laporte Schools for their 5th grade students. - We have increased the nurturing of children by the fathers who are parenting by offering the Nurturing Father curriculum to over 265 dads that have enrolled in our FATHER project over the past 3 years. - We have offered trainings for providers of family programming to increase their knowledge of evidence-based programming, with the Nurturing FATHERhood program to 10 providers and the Adverse Childhood Experiences training for over 60 providers. - We explored a partnership with the Community Paramedic program to seek their assistance for clients with unmet mental health needs i.e. care assistance or transportation. However, this did not appear to be a program for our community. Specialized Health Services: St. Joseph's continues to look to ways to meet gaps in specialized health services. - Our Community Dental Clinic continues to offer a full-range of dental services to clients on the Minnesota public insurance, and we have hired a Dental Therapist to provide mid-level services. - We will offer evidence based, Tai Chi fall-prevention classes to community groups. Our Tai chi Program has been offered twice a year with an average of 8-10 participants per session. - A trained facilitator will offer I Can Prevent Diabetes Classes, a collaborative, community-based, lifestyle change program designed for people with pre-diabetes with 8 people completing the 16 week series this past year. - We continue to have trained staff who will perform Foot Care for our elderly, health screenings, and immunizations to adults at the Community Health clinic. |

| Schedule H, Part VI Lines 2, 4, & 5: Needs assessment & community benefit information | ORGANIZATION'S MISSION, VISION, AND TAX-EXEMPT PURPOSE CHI St. Joseph's Health was founded to alleviate a shortage of hospital beds in the community in the late 1940s. As the first Catholic hospital to serve the community, CHI St. Joseph's Health embraced the mission of the founding religious congregation, which has since joined with other religious congregations to form Catholic Health Initiatives. Our mission is to develop a healthy community and provide care to all in need. It also has a board of directors that is comprised of independent community representatives. CHI St. Joseph's Health operates a 24-hour emergency department 365 days per year. The emergency department is open to all individuals regardless of ability to pay. We are also home to a Center for Weight Management, named an ASMBS Center of Excellence; a Community Dental Clinic; Community Health; and Hospice departments; and offer a full range of Diagnostic Services and Rehabilitation Services. Community service has always been at the core of CHI St. Joseph Health's activities. Each year, services and programs are expanded to promote a healthy community. We have an open medical staff, participate in Medicare and Medicaid, and have an active charity care program. The programs and services described throughout this report not only serve the community, but also reduce the burdens on the government. For example, if we did not provide charity care, the burden of providing charity care would fall on a government-supported institution. COMMUNITY BENEFIT APPROACH CHI St. Joseph's Health is located in Park Rapids, a rural town of 3,903 in north central Minnesota that serves as the county seat for Hubbard County. Approximately 200 miles northwest of Minneapolis, We are the sole area provider of hospital services for the 20,658 (2013) residents of Hubbard County, as well as portions of Becker, Cass and Wadena counties (~ 3,800 square miles). Our population increased 311 residents this past year. According to the Minnesota Department of Health (MDH) County Population Estimates for 2013, of the 20,658 residents: 5.7% (1,185) are children under the age of 5 23.8% (4,480) are children under the age of 18 23.0% (4,754) are 65 years of age and over (down from 30.4% in 2012) 50% are female MDH estimates that in 2013, 94.7% (19,579) of the residents reported as being white; 0.1% (75) Black/African American; 2.6% (534) American Indian and Alaskan Native; 0.01% (92) Asian/Pacific Islander; and 1.8% (378) reported two or more races. 1.9% or 402 residents identified their ethnicity as Hispanic/Latino. This demographic remains stable from previous years. Citizens of Hubbard County are generally poorer than the state averages. 34.5% of our residents lived at or below 200% of poverty from 2009-2013 as compared to 27.1% of the total Minnesota population living at or below 200% poverty from 2009-2013. The Per Capita Income for Hubbard County 2009-2013 was $24,880. The Minnesota Per Capita Income was $30,915 for the same time range. The Hubbard County Median Household Income for 2009-2013 was reported as $45,961 (MN-$59,836). According to the Minnesota Department of Health, the annual unemployment average was 8.3% in 2012, 9.2% for 2010, and 10.3% in 2009. The Minnesota Department of Employment and Economic Development lists the Unemployment Rate for November, 2015 as 5.8%. Our MinnesotaCare average monthly enrollment in 2013 for all families was 763, and the Medical Assistance average monthly enrollment in 2013 for all families was 2,280. Since 1987, St. Joseph's has contracted with the Hubbard County Commissioners to perform the Public Health function for Hubbard County. As the Public Health entity, they have performed a community needs assessment on an annual basis. The department has compiled county health data, identified health issues, set priorities, and plans for improvement. We continue our deep commitment and involvement in community health collaboration today. Process CHI St. Joseph's Health's Community Benefit Plan is based on our understanding of unique communities' needs derived from collaborative needs assessments, focus groups, and surveys conducted with our community partners. Healthy Park Rapids Area 2020 was formed in 2002 as a collaborative entity of health and human service providers with a general desire to improve community health and well being. Additionally, it was generally recognized that the health of the community was not the responsibility of any one entity, but rather required the coordinated efforts of many entities - public and private - to realize an improved health status of a community. The community collaboration has been working for several years to measure and improve the health status of the community. The Hubbard County Community Health Assessment was completed in-depth in 2013. The report reflects the compilation and analysis of extensive data sources to identify issues having a significant impact on the health and general well being of area residents. Summary of Assessment Findings Five areas of concern were identified as having the greatest impact on the overall wellness of the area emerged in the Community Health Status review. The issues were identified and then summarized. Community stakeholders were invited to give input on the issues of most importance to them, keeping in mind how aware the community was of each issue and how difficult the issue was to change. The five key themes identified as areas of unmet need in our service area and opportunities for St. Joseph's to partner in meeting gaps in services are: 1. Community Wellness 2. Substance and Chemical Abuse 3. Economic Factors to Influence Wellness 4. Mental and Health Violence Prevention 5. Specialized Health Services Our Community Benefit plan included implementation strategies to bridge gaps in areas of need identified. The Community Benefit Plan included ongoing, new, and expanded services in each of the five areas for us to focus on in order to improve the health and quality of life of residents in Hubbard County. Monitoring and implementation of the community health needs Implementation Plan occurs in two ways. First, an internal committee of leaders and stakeholders from St. Joseph's Area Health Services meets every other month to discuss the assessment and progress towards our implementation plan. Periodically, the Board of Directors receives an update on the implementation plan to assist in monitoring and strategic direction. The entire process of conducting a Community Health Needs Assessment and developing an implementation strategy to address community health needs has presented an excellent opportunity to forge new relations with other providers, agencies, and community organizations and to strengthen existing relationships. Collaboration holds the promise of allowing progress on issues that any one or two parties alone could never accomplish. The following section outlines our Community Benefit Plan programs. |
| Schedule H, Part VI Lines 2, 4, & 5: Needs assessment & Community Benefit Information | QUALITATIVE DESCRIPTION OF COMMUNITY BENEFIT Community Outreach for the Poor Community Dental Clinic Our Community Dental Clinic provides dental care to the publicly funded populations throughout north central Minnesota. This past year, they provided 6,903 dental visits. The "Give Kids a Smile" day provided hygiene and exams to 85 children and provided 14 classrooms with dental education. Healthy Families-Healthy Homes The Healthy Families-Healthy Homes program was developed to address the needs of at-risk mothers and children living in our county. The program includes prenatal education, parenting education and support. CHI St. Joseph's Health provides the oversight of the grants and the additional funding for this program. Temporary Assistance for Needy Families (TANF) This non-medical home visiting program is partially funded by grant dollars. CHI St. Joseph's Health has worked to expand the number of families served through the TANF program which focuses on family violence prevention and child abuse prevention. Weekly home visits are made to at-risk families by a trained family home visitor to provide parenting education. Families are visited until the child reaches three years of age. Immunization Clinics Vaccinations are one of the most effective ways to prevent disease and also one of the most cost efficient medical interventions available. We offer low cost or free vaccinations to eligible children from birth to age 19 through our participating schools. Annual flu shot clinics are provided to area businesses. Minnesota Care Outreach CHI St. Joseph's Health provides outreach to uninsured individuals in our service area. We assist them in the enrollment to Minnesota Care or make referrals for other insurance services. CHI St. Joseph's Health has been certified as an MNCAA agent (Minnesota Community Application Agent) since 2010 to help assist eligible patients with their Medical Assistance applications. Transportation CHI St. Joseph's Health will pay the cost of transportation home from the emergency department or the hospital to individuals that have no options for a ride home. Free Mammography Services CHI St. Joseph's Health provides mammograms at no cost to eligible women enrolled in the Sage Program through the Minnesota Department of Health. Violence Prevention CHI St. Joseph's Health implemented programs for violence prevention. This programming has centered on mentoring, the promotion of fatherhood and on family resources in the community. Community Outreach for the Broader Community CHI St. Joseph's Health is recognized as a progressive community and regional provider of health care services. The board of directors and staff volunteer their time and resources to numerous regional and local boards, advisory councils, community organizations and collaborations. Community Building Activities CHI St. Joseph's Health continues its commitment to improve the health of our community. We coordinate a collaborative effort that is known as Healthy Park Rapids 2020. This group's purpose is to create a community infrastructure that will ensure that the physical, mental, social, and spiritual health needs of the community are met. The group recruited over 50 community stakeholders to create a shared vision of health and an action plan to create healthier communities. They have sponsored community gardens throughout the County providing locally grown food opportunities for all residents. Healthy Park Rapids 2020 collaborates with other community groups by endorsing and providing support for or partnering with organizations in activities that promote health and wellness, such as improvements to a local park and the promoting of safe bike riding. The FATHER Project Our FATHER project offers case management and training opportunities for non-custodial fathers. Their case plan includes parenting classes, couple-strengthening courses, leadership training, and employment readiness classes. We collaborate with Hubbard County Social Services to re-engage the dad in the child support efforts and with the Park Rapids GED program to assist with completing their high-school equivalency. A Service Collaborative Team advises the staff on the services and community resources needed by the enrollees. Collaborative Efforts to Improve Community Health Immunization Registry CHI St. Joseph's Health participates in and provides training on an internet-based registry for all children in Hubbard County. The registry is part of a 13 county registry and allows parents, clinics, and schools to access vaccination dates. Health Classes, Screenings and Fairs CHI St. Joseph's Health has offered I CAN Prevent Diabetes classes. This 16-week evidence-based program is a collaborative, community-based, lifestyle change program designed for people with pre-diabetes. We also have offered Tai Chi classes, which is an evidence-based fall prevention program. |
| Schedule H, Part VI Lines 2, 4, & 5: Needs Assessment & Community Benefit Information | We also provide blood pressure checks and foot care to senior citizens and flu shots to employees of participating work sites throughout Hubbard County. We will offer health promotion educational booths for local agencies and business throughout our service area, upon request. Support Groups CHI St. Joseph's Health provides staff for the following support groups: Grief Support, Diabetes Support, Gastric Bypass Support, Stroke Support, and Caregivers Support. Hubbard County Drug and Alcohol Task Force CHI St. Joseph's Health staff is active participants in this group of like-minded community members with representatives from local law enforcement, health care, education, local governments and the community at large, addressing issues related to youth and substance use/abuse. We have had a five-year program to work with youth to prevent youth alcohol use, of which we currently completed our fourth year. Parish Nurse Program CHI St. Joseph's Health sponsors parish nurse training for congregations in our service area. Our parish nurse coordinator facilitates regular meetings with this group of individuals who have chosen a ministry of nursing in their faith community. Presently 9 parish nurses are being supported in their role as health care advocates in their area faith communities. Parish nurses function in a variety of ways to meet the health needs of their church members. This may include accompanying a parishioner to a doctor appointment or coordinating a health fair. Senior Companion Program CHI St. Joseph's Health is a sponsor of the senior companion program, which finds its origins with Lutheran Social Services. It provides volunteer opportunities for seniors to help other seniors live independently by providing friendship and assisting with errands. Volunteers are paid a tax-free stipend and mileage. They are offered the opportunity for an annual health screen and meals in our cafeteria on the days they volunteer. This program meets the needs of providing meaningful work for the volunteers and helps senior adults remain independent in their own homes rather than relying on institutional care, give family caregivers some time off from their duties, and in many instances a social connection for those they serve. Car Seat Giveaway CHI St. Joseph's Health offers an infant car seat to all parents that deliver their babies at our facility. With 100 births last year, we are committed to offering a safe method for those infants to leave the hospital. Additionally, we offer car seat education to our Prime West Medical Assistance enrollees through our Community Health department. Newborn Hearing Screening CHI St. Joseph's Health screens all the infants born at the hospital for hearing loss before they are one month of age. The results are then forwarded to the family physician and the infants that do not pass the screening are referred to an audiologist. Bili Blanket CHI St. Joseph's Health provides a bili blanket for babies with elevated blood bilirubin levels upon discharge from our facility. Community Support Groups CHI St. Joseph's Health provides a location for groups to meet. Meeting locations and times are advertised in several different publications. The following groups currently use meeting space within our facility: Childbirth Education Classes Senior Providers Gastric Bypass Support Group Diabetes Support Group Grief Support Group |
| Schedule H, Part I, Line 3c Eligibility criteria for free or discounted care | When Catholic Health Initiatives (the ultimate parent organization to St. Joseph's Area Health Services established its financial assistance policy it was determined that establishing a household income scale based on the HUD very low income guidelines more accurately reflects the socioeconomic dispersions among urban and rural communities in 18 states served by CHI hospitals and health care facilities. In comparing HUD guidelines to the Federal Poverty Guidelines ("FPG"), we find that on average HUD guidelines compute to approximately 200% to 250% (and sometimes 300%) of FPG. St. Joseph's Area Health Services bases its financial assistance eligibility on HUD's 130% of Very Low Income Guidelines based on geography, and affords the uninsured and underinsured the ability to obtain financial assistance write-offs, based on a sliding scale, ranging from 25%-100% of charges. An individual's income under the HUD guidelines is a significant factor in determining eligibility for financial assistance. However, in determining whether to extend discounted or free care to a patient, the patient's assets may also be taken into consideration. For example, a patient suffering a catastrophic illness may have a reasonable level of income, but a low level of liquid assets such that the payment of medical bills would be seriously detrimental to the patients' basic financial (and ultimately physical) well-being and survival. Such a patient may be extended discounted or free care based upon the facts and circumstances |
| Schedule H, Part I, Line 7g Subsidized Health Services | Included in subsidized health services is the following clinic: St. Joseph Community Dental Clinic The total expenses associated with operating this clinic were $1,336,880. Revenue offset was $1,130,450. Accordingly, the total subsidized cost of these necessary services was $206,430. The clinic meets a community need for primary care services outside of the emergency room setting. |
| Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistance | The approach taken for the community benefit calculation was as follows: *Medicaid community benefit was calculated based upon using the Medicaid estimated Inpatient and Outpatient cost to charge ratios of 50.1% and 37.1%, respectively times the charges for Inpatient and Outpatient Medicaid Patients. Additionally the Minnesota Care Tax and Medicaid Surcharge Tax were also included as line item costs. This was then compared to projected reimbursement for Medicaid Services. The total community benefit is estimated at $1,216,141. *Subsidized health services was calculated using Estimated Net Revenue as compared to direct costs and overhead allocated. Overhead allocated follows the Medicare Cost Report assignment of allocating overhead to cost centers. The Medicare Cost Report is not yet prepared at time of calculation of the Community Benefit for financial statement purposes. As a result, we utilize an internal calculation model to project this benefit in July. The community benefit from subsidized health services was $442,986, plus an additional $1,610 for women infant and children (WIC) services. *The remaining community benefit was taken by compiling the cost of traditional charity care $120,786; community health improvement services $71,752; community building activities $171; and Financial and in-kind contributions $51,178. The cost-to-charge ratio for the year ended 6/30/15 was computed using the following formula: Inpatient allowable costs divided by Inpatient revenue by medicare and outpatient allowable costs divided by outpatient allowable revenue by medicare. Based on that formula the Inpatient calculations, $18,111,357 / $36,157,714 results in a 50.1% cost-to-charge ratio and for Outpatient $17,879,665/ $48,215,499 a 37.1% cost to charge ratio. For subsidized health services, a cost accounting methodology combined with an approach of including assigned indirect overhead based on Medicare cost report methodology was utilized. Worksheet 2 was not used to derive the cost-to-charge ratio. |
| Schedule H, Part II Community Building Activities | SEE ADDITIONAL COMMUNITY BUILDING INFORMATION IN THE SUMMARY OF NEEDS ASSESSMENT, COMMUNITY INFORMATION, AND PROMOTION OF COMMUNITY HEALTH. |
| Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amount | Costing methodology for amounts reported on line 2 is determined using the organization's cost/charge ratio of 42.5%. When discounts are extended to self-pay patients, these patient account discounts are recorded as a reduction in revenue, not as bad debt expense. |
| Schedule H, Part III, Line 3 Bad Debt Expense Methodology | St. Joseph's area health services does not believe that any portion of bad debt expense could reasonably be attributed to patients who qualify for financial assistance since amounts due from those individuals' accounts will be reclassified from bad debt expense to charity care within 30 days following the date that the patient is determined to qualify for charity care. |
| Schedule H, Part III, Line 4 Bad debt expense - financial statement footnote | St. Joseph's Area Health Services does not issue separate company audited financial statements. However, the organization is included in the consolidated financial statements of Catholic Health Initiatives (CHI). The consolidated footnote reads as follows: "The provision for bad debts is based upon management's assessment of historical and expected net collections, taking into consideration historical business and economic conditions, trends in health care coverage, and other collection indicators. Management routinely assesses the adequacy of the allowance for uncollectible accounts based upon historical write-off experience by payor category. The results of these reviews are used to modify, as necessary, the provision for bad debts and to establish appropriate allowances for uncollectible net patient accounts receivable. After satisfaction of amounts due from insurance, CHI follows established guidelines for placing certain balances with collection agencies, subject to the terms of certain restrictions on collection efforts as determined by each facility. The provision for bad debts is presented on the consolidated statements of operations as a deduction from patient services revenues (net of contractual allowances and discounts) since CHI accepts and treats substantially all patients without regard to the ability to pay." |
| Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costs | St. Joseph's Area Health Services is designated as a Critical Access Hospital (CAH). CAHs are rural community hospitals that are certified to receive cost-based reimbursement from Medicare. The reimbursement that CAHs receive is intended to improve their financial performance and thereby reduce hospital closures. CAHs are certified under a different set of Medicare Conditions of Participation (COP). Shortfalls are created when a facility receives payments that are less than the costs of caring for program beneficiaries. Because shortfalls are based on costs, not charges, St. Joseph's Area Health Services, due to their designation as a CAH, received cost-based reimbursement for inpatient and outpatient services. For these services, there were no medicare shortfalls. For Dental Clinic Services and Public/Community Health Service areas, St. Joseph's does not receive cost based reimbursement. There were shortfalls under these programs. Although not presented on the Medicare cost report, in order to facilitate a more accurate understanding of the "true" cost of services (for "shortfall" purposes) the CHI Workbook allows a health care facility not to offset costs that Medicare considers to be non-allowable, but for which the facility can legitimately argue are related to the care of the facility's patients. In addition, although not reportable on the Medicare cost report, the CHI workbook includes the cost of services that are paid via a set fee-schedule rather than being reimbursed based on costs (e.g. outpatient clinical laboratory). Finally, the CHI Workbook allows a facility to include other health care services performed by a separate facility (such as a physician practice) that are maintained on separate books and records (as opposed to the main facility's books and records which has its costs of service included within a cost report). True costs of Medicare computed using this methodology: Total Medicare Revenue: $16,028,141 Total Medicare costs: $16,165,382 Surplus or Shortfall $ (137,241) St. Joseph's Area Health Services believes that excluding Medicare losses from community benefit makes the overall community benefit report more credible for these reasons: Unlike subsidized areas such as burn units or behavioral-health services, Medicare is not a differentiating feature of tax-exempt health care organizations. In fact, for-profit hospitals focus on attracting patients with Medicare coverage, especially in the case of well-paid services that include cardiac and orthopedics. Significant effort and resources are devoted to ensuring that hospitals are reimbursed appropriately by the Medicare program. The Medicare Payment Advisory Commission (MedPAC), an independent Congressional agency, carefully studies Medicare payment and the access to care that Medicare beneficiaries receive. The commission recommends payment adjustments to Congress accordingly. Though Medicare losses are not included by Catholic hospitals as community benefit, the Catholic Health Association (CHA) guidelines allow hospitals to count as community benefit some programs that specifically serve the Medicare population. For instance, if hospitals operate programs for patients with Medicare benefits that respond to identified community needs, generate losses for the hospital, and meet other criteria, these programs can be included in the CHA framework in Category C as "subsidized health services." Medicare losses are different from Medicaid losses, which are counted in the CHA community benefit framework, because Medicaid reimbursements generally do not receive the level of attention paid to Medicare reimbursement. Medicaid payment is largely driven by what states can afford to pay, and is typically substantially less than what Medicare pays. |
| Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistance | St. Joseph's Area Health Services debt collection policy provides that St. Joseph's Area Health Services will perform a reasonable review of each inpatient account prior to turning an account over to a third-party collection agent and prior to instituting any legal action for non-payment, to assure that the patient and patient guarantor are not eligible for any assistance program (e.g. Medicaid) and do not qualify for coverage through St. Joseph's Area Health Services community assistance policy. After having been turned over to a third-party collection agent, any patient account that is subsequently determined to meet the St. Joseph's Area Health Services community assistance policy is required to be returned immediately by the third-party collection agent to St. Joseph's Area Health Services for appropriate follow-up. St. Joseph's Area Health Services requires its third-party collection agents to include a message on all statements indicating that if a patient or patient guarantor meets certain stipulated income requirements, the patient or patient guarantor may be eligible for financial assistance. All of Catholic Health Initiatives' (CHI) hospitals' contracts with third party collection agencies include the following standards: *Neither CHI hospitals nor their collection agencies will request bench or arrest warrants as a result of non-payment; *Neither CHI hospitals nor their collection agencies will seek liens that would require the sale or foreclosure of a primary residence; and *No CHI collection agency may seek court action without hospital approval. Finally, collection agencies are trained on the Catholic Health Initiatives Mission, Core Values and Standard of Conduct to make sure all patients are treated with dignity and respect. |

| Schedule H, Part VI, Line 2 Needs assessment | St. Joseph's Area Health Services followed the CHA Guidelines in conducting the community health needs assessment. This consists of a six step process and includes: 1. Planning and Preparing for the Assessment, 2. Determining the Purpose and Scope of the Needs Assessment, 3. Identifying Data that Describes the Health and Needs of the Community, 4. Understanding and Interpreting the Data, 5. Defining and Validating Priorities, and 6. Documenting and Communicating of the Results. This process was last completed during the fiscal year ended 6/30/2013 with the board approving the plan before the year end. See additional information in the summary of Needs assessment, community information, and promotion of community health. |
| Schedule H, Part VI, Line 3 Patient education of eligibility for assistance | St. Joseph's Area Health Services includes information concerning its financial assistance policy on its website. In addition, St. Joseph's Area Health Services prominently displays its financial assistance policy in both English and Spanish in obvious locations throughout the hospital, including the emergency rooms and other patient intake areas, as well as in St. Joseph's Area Health Services outpatient facilities. St. Joseph's Area Health Services registration clerks are trained to provide consultation to those who have no insurance or potentially inadequate insurance concerning their financial options including application for Medicaid and for financial assistance under St. Joseph's Area Health Services' Financial Assistance Policy. Upon registration (and once all EMTALA requirements are met), patients who are identified as uninsured (and not covered by Medicare of Medicaid) are provided with a packet of information that addresses the financial assistance policy and procedures including an application for assistance. St. Joseph's Area Health Services' registration clerks read the organization's medical assistance policy to those who appear to be incapable of reading, and provide translators for non English-speaking individuals. St. Joseph's Area Health Services' staff will also assist the patient/guarantor with applying for other available coverage (such as Medicaid), if necessary. Counselors assist Medicare eligible patients in enrollment by providing referrals to the appropriate government agencies. |
| Schedule H, Part VI, Line 4 Community information | ST. JOSEPH'S AREA HEALTH SERVICES, LOCATED IN PARK RAPIDS, MINNESOTA IS THE SOLE AREA PROVIDER OF HOSPITAL SERVICES FOR RESIDENTS OF HUBBARD COUNTY, AS WELL AS POrTIONS OF BECKER, CASS, AND WADENA COUNTIES IN MINNESOTA. | Schedule H, Part VI, Line 6 Affiliated health care system | St. Joseph's Area Health Services, along with its affiliated outpatient facilities, are part of Catholic Health Initiatives. Catholic Health Initiatives (CHI) is a national faith-based nonprofit health care organization with headquarters in Englewood, Colorado. CHI's exempt purpose is to serve as an integral part of its national system of hospitals and other charitable entities, which are described as market-based organizations, or MBOs. An MBO is a direct provider of care or services within a defined market area that may be an integrated health system and/or a stand-alone hospital or other facility or service provider. CHI serves as the parent corporation of its MBOs which are comprised of 101 hospitals, including four academic medical centers, and 30 critical access facilities; community health service organizations; accredited nursing colleges; home health agencies; and other facilities that span the inpatient and outpatient continuum of care. Together, these facilities provided $981 million in charity care and community benefit in the 2015 fiscal year, including services for the poor, free clinics, education and research. CHI provides strategic planning and management services as well as centralized "shared services" for the MBOs. The provision of centralized management and shared services - including areas such as accounting, human resources, payroll and supply chain -- provides economies of scale and purchasing power to the MBOs. The cost savings achieved through CHI's centralization enable MBOs to dedicate additional resources to high-quality health care and community outreach services to the most vulnerable members of our society. St. Joseph's Area Health Services operates with its wholly owned affiliates and community partners, along with its fundraising arm, the St. Joseph's Area Health Services foundation, to serve the health care needs of the Park Rapids, Minnesota communities. |

ANY SUCH CHANGES ARE NOT RE-SUBMITTED TO THE BOARD. ONCE FILED, THE BOARD CHAIR REVIEWS A COPY OF THE FORM 990 AT A SEPARATE MEETING OF THE BOARD OF DIRECTORS TO ALLOW ADEQUATE TIME FOR QUESTIONS. |
| Form 990, Part VI, Line 12c Conflict of interest policy | Catholic Health Initiatives ("CHI") has a Conflicts of Interest ("COI") policy in place to maintain the integrity of all of its activities. The policy applies to CHI Board of Stewardship Trustees and members of its committees; all board and board committee members of CHI Entities; all CHI employees; all CHI physicians (both employed and non-employed) and all physician administrators and leaders; advanced practice clinicians (both employed and non-employed); and all CHI research personnel (both employed and non-employed). Disclosure, review and management of perceived, potential or actual conflicts of interest are accomplished through a defined COI disclosure process. Each person has a general ongoing obligation to promptly and fully report to his/her direct manager, supervisor, medical staff office, board or board committee chair any situation or circumstance that may create a conflict of interest. The person must report the actual or potential conflict as soon as she/he becomes aware of it. In any situation where the person may be in doubt, a full disclosure should be made to permit an impartial and objective determination. In addition to the general ongoing obligation, there are initial disclosure obligations. The board, board committee members, and new employees are required to make disclosures at the time of their initial hiring/appointment. All non-employed, credentialed or contracted physicians are required to make disclosures at the time of their credentialing and during any subsequent reappointment or recredentialing. All researchers are required to make disclosures upon consideration of affiliation with a research sponsor. In addition to the general ongoing and initial disclosure obligations, there is an annual disclosure obligation. All corporate officers, board and board committee members, employees at the level of manager and above, researchers, supply chain employees, employed physicians, physician administrators and leaders, and employed advanced practice clinicians must complete a new conflict of interest disclosure annually. Disclosures of perceived, potential or actual conflicts involving financial interests are forwarded to the Conflicts of Interest Review Committee ("C-CIRC") or Legal Services Group for review depending on the position of the person involved. The C-CIRC reviews COI questionnaires containing disclosures of perceived or possible conflicts for employees at a level of manager or above, supply chain employees, researchers and physicians, physician administrators and leaders, and advanced practice clinicians (both employed and non-employed). In the determination of a conflict, a COI management plan will be developed for that person. With respect to those audiences for which the C-CIRC has review responsibility, the C-CIRC will facilitate development of any such conflict of interest management plan in collaboration with local CRP staff. A designated CHI Entity staff will be responsible for monitoring the COI management plan and for documenting monitoring activities. At its sole discretion, a CHI Entity may reject a Person's request to enter into the relationship in question, or require the relationship be sufficiently altered to avoid a potential COI. If the C-CIRC determines that there is a potential or actual conflict of interest that does not currently have appropriate controls to address the conflict of interest, it may recommend that the disclosing person be allowed to participate in the activity or transaction subject to restrictions as outlined in the COI management plan. If a Person does not agree with a determination made by the C-CIRC, its interpretation of the Policy or Addenda, or seeks an exemption or exception, the following steps should be followed. The Employee disputing the review decision, interpretation of the Policy, or seeking exemption or exception must present the matter to the Employee's immediate direct manager or supervisor for review and determination. If the Employee and the manager do not agree with the review decision, interpretation of the Policy, or seek exemption or exception, the manager shall consult with the manager's Vice President (or higher if the manager is a Vice President) to reach a determination. If the matter remains unresolved, it shall be referred to the CHI Vice President of Human Resources and the CHI Corporate Responsibility Officer. If they are unable to reach agreement, the matter shall be referred to the CHI General Counsel, whose decision shall be final. Reviews and determinations involving board and board committee members and corporate officers will be the responsibility of the board, board executive committee, or board chair, with guidance from the Legal Services Group (LSG). Annual COI disclosures of all trustee and corporate officers will be reviewed by the CHI Senior Vice President, Legal Services, and General Counsel or his or her designee who will report potential conflicts to the applicable Board Chair. The Board Chair or designee shall make such further investigation of any conflict of interest disclosures as he or she may deem appropriate. If the conflict involves the Board Chair, the Vice Chair will assume the Chair's role. Based on review and evaluation of the relevant facts and circumstances, the Board Chair will make an initial determination as to whether a conflict of interest exists and whether, pursuant to the COI Policy, review and approval or other action by the Board is required. A written record of the Board Chair's determination, including relevant facts and circumstances, will be made. The Board Chair shall then make an appropriate report to the Executive Committee of the Board concerning such review, evaluation and determination. If a difference of opinion exists between the Board Chair and another Trustee as to whether the facts and circumstances of a given situation constitute a conflict of interest or whether Board review and approval or other action is required within the COI Policy, the matter shall be submitted to the Board's Executive Committee, which shall make a final determination as to the matter presented. Such determination, including relevant facts and circumstances, will be reflected in the Executive Committee minutes and will be reported to the Board. When any conflict of interest is considered by the board, the trustee or corporate officer, as appropriate, must disclose all of the material facts to the Board. The trustee shall not vote and the trustee or corporate officer shall not use his or her personal influence on the matter. The trustee or corporate officer shall be excused from the meeting during discussion and vote on the conflict of interest. In reviewing such transactions between CHI or CHI Entities and vendors or other contractors who are, or are affiliated with, Trustees or Corporate Officers, the Board will act as it would in reviewing transactions with unrelated third parties. The transaction is not be approved unless the Board determines that the transaction is fair to CHI or the CHI Entity. The Board must approve the transaction by a majority of the Trustees on the Board, without counting the vote of any individual who has an interest in the transaction. All determinations of conflicts of interest are reported as required by law, regulations, and CHI policy. |
| Form 990, Part VI, Line 15a Process to establish compensation of top management official | The organization's top management official's compensation is paid by Catholic Health Initiatives (CHI), a related organization. CHI has a defined compensation philosophy. Both the executive and non-executive compensation structures and ranges are reviewed annually in comparison to market data. CHI uses The Hay Group as the independent third party to assess executive compensation programs and to ensure the reasonableness of actual salaries and total compensation packages. Compensation of the senior most executives is reviewed annually. The Hay Group reviews both cash and total compensation for overall reasonableness, for adherence to CHI's compensation philosophy, and for comparability to the not-for-profit healthcare market. This independent review is delivered by Hay Group to the HR committee of the CHI Board of Stewardship Trustees annually at their September meeting and minutes are shared with the full board at the December meeting. The last review was September 14, 2015. In addition, Hay Group completed a comprehensive review of all positions at the level of vice president and above in the fall of 2014 to determine and validate appropriate compensation levels. These levels have been reviewed annually since and revised based on market data, where applicable. |
