Part V, Line 4: Intended uses of the endowment fund.Endowment funds are designated for purposes that align within the organization's exempt purpose.
Part X : FIN48 FootnoteEMHS, its hospitals, and certain other affiliates have been determined by the Internal Revenue Service to be tax-exempt charitable organizations as described in Section 501(c)(3) or 501(c)(2) of the Internal Revenue Code (the Code) and, accordingly, are exempt from federal income taxes on related income pursuant to Section 501(a) of the Code. Accordingly, no provision for federal income taxes has been recorded in the accompanying consolidated financial statements for these organizations.Tax-exempt charitable organizations could be required to record an obligation for income taxes as the result of a tax position they have historically taken on various tax exposure items including unrelated business income or tax status. Under guidance issued by the Financial Accounting Standards Board (FASB), assets and liabilities are established for uncertain tax positions taken or positions expected to be taken in income tax returns when such positions are judged to not meet the "more-likely-than-not" threshold, based upon the technical merits of the position. Estimated interest and penalties, if applicable, related to uncertain tax positions are included as a component of income tax expense. The System has evaluated its tax position taken or expected to be taken on income tax returns and concluded the impact to be not material.Certain of the System's affiliates are taxable entities. Deferred taxes related to these entities are based on the difference between the financial statement and tax basis of assets and liabilities using enacted tax rates in effect in the years the differences are expected to reverse. The deferred tax assets and liabilities for these entities are not material.

Part V, Line 3j - Description of Other Needs Assessment3g The data gathered from the 2014 Community Health Needs Assessment (CHNA) allowed Inland Hospital to identify areas of significant need including priority concerns, gaps in healthcare services, and barriers to care. A hospital task force considered the data and identified areas of significant needs to be addressed, as well as areas of need unable to be addressed by the hospital. An implementation strategy was presented and adopted by the hospitals board of directors. The Implementation Strategy consists of actions the hospital intends to take to address the health need. Programs/resources the hospital plans to commit to address the health need were identified along with planned collaborations with other area organizations.3h To assure broad representation of the communitys interests, key public health and healthcare partners were engaged and consulted throughout the design and implementation phase of the 2014 CHNA. Qualitative Stakeholder Surveys were distributed to public and community health representatives who directly serve the communities interests. Sampling was monitored throughout the survey dissemination to assure a minimum threshold of respondents providing local insight to priority community health needs and opportunities. Survey outreach was conducted by the local hospital, partner healthcare facilities in the same region, the local public health liaison, and a local comprehensive community health coalition.3i The 2014 CHNA report presents findings of three sets of data, by county. The data sets include absolute indicators of health status as gathered in 2010 using BRFSS and other primary source data gathered using empirical methods; A qualitative stakeholder survey conducted in 2014 and 2014 County Health Rankings indicator set. The qualitative stakeholder survey was disseminated broadly to public health and community stakeholders who serve local populations. As such, the Community Health Needs Assessment does not provide for random assignment of survey respondents, nor does it attempt to function as a surveillance instrument. Limitations aside, taken together, the data provide a unique perspective on the health of Maine communities, with a focus on the social, environmental, and clinical factors which influence the ability of populations to lead healthy lives.
Part V, Line 5 - Account Input from Persons Who Represent the CommunityKey collaborators were convened as an advisory committee to review the Needs Assessment and assist in the dissemination of a survey instrument to a broad base of key informants representing healthcare, public health, and community stakeholders to identify issues and opportunities for collaborative community health improvement. These collaborators include: Local Public Health District Officials: Stacy Boucher, Aroostook District Coordinating Council; Paula Thomson, Central District Coordinating Council; Becca Matusovich, Cumberland District Public Health Council; Alfred May, Downeast Public Health Council; Jessica Fogg, Penquis District Coordinating Council; Clarisa Weber, Wabanaki Public Health. Healthy Maine Partnerships (contracted entities functioning as part of the local public health system): Jamie Comstock, Bangor Region Public Health & Wellness; Elsie Flemings, Healthy Acadia; Carol Bell, Healthy Aroostook; Anne Lang, Healthy Casco Bay; Joanne Joy, Healthy Communities of the Capital Area; Zoe Miller, Healthy Lakes; Fran Mullin, Healthy Northern Kennebec; Kristen Dow, Healthy Portland; Lucie Rioux, Healthy Rivers; Karen Hawkes, Healthy Sebasticook Valley; Malissa Boynton, Healthy Wabanaki; Vyvyenne Ritchie, Healthy Waldo County; Jane McGillicuddy, Partnership for a Healthy Northern Penobscot; Robin Mayo, Piscataquis Public Health Council; Ellen Willinghan, Power of Prevention; Bill Primmerman, Somerset Public Health; Eleody Libby, Washington County: One Community. Other Entities: Federally Qualified Health Centers, Bangor Public Health and Community Services, Portland Public Health.

Part I, Line 7, Column F - Explanation of Bad Debt Expense$5,564,188 of bad debt expense, $2,979,255 of charity care, $50,766,516 of contractual allowances is included on Form 990, Part IX, line 25, column (A).
Part III, Line 2 - Methodology Used To Estimate Bad Debt ExpenseThe costing methology used to determine the amount is cost to charge ratio.
Part III, Line 4 - Bad Debt ExpensePatient and trade accounts receivable are stated at the amount management expects to collect from outstanding balances. Management provides for probable uncollectible amounts through a charge to earnings and a valuation allowance based on its assessment of the current status of individual accounts. Balances that are still outstanding after management has used reasonable collection efforts are written off through a charge to the valuation allowance and the applicable patient accounts receivable. Credit is extended without collateral.
Part III, Line 8 - Explanation Of Shortfall As Community BenefitMedicare losses should be treated as a community benefit because the losses are incurred in performing an important public service, and Maine hospitals experience one of the lowest Medicare reimbursement rates in the country.
Part III, Line 9b - Provisions On Collection Practices For Qualified PatientsAll account guarantors who express an inability to pay inpatient and outpatient services will be screened for eligibility for charity care using an application and guidelines established by Inland Hospital. An account may be reconsidered for charity care at any time when new information is available about a patients inability to pay.
Part VI - Needs AssessmentThe 2014 Community Health Needs Assessment was conducted by Eastern Maine Healthcare Systems (EMHS, Inland Hospitals parent company) in collaboration with several member/affiliated hospitals and many public health and community organizations across eight Maine counties. The Community Health Needs Assessment identifies priority health issues across the system as well as within each member hospital's local service area. Each member hospital adopts a local implementation strategy and community health improvement plan, tailored to address the issues and meet needs at the local level.
Part VI - Patient Education of Eligibility for AssistanceAt Inland Hospital, Patient Account Representatives are available to assist and answer any questions our patients may have regarding free care. Additional interactive strategies are in places that include billing statements that have free care information added for the patient's convenience, providing all uninsured patients with free care packets at point of registration. When a patient states that they are under-insured free care information is provided. For any other patient requests we receive (via emails and phone calls) packets are mailed. All patient accessible areas, such as hospital waiting areas, contain information to direct patients on how to receive financial assistance. Additionally, Inland Hospital provides online resources and tools to assist our patients with the necessary information regarding charity/free care via www.inlandhospital.org. Inland Financial Counselors are Certified Application Counselors for the Federal Market Place and will assist customers with the Healthcare Market Place as requested. It is the policy of Inland Hospital to treat all patients requiring care without regard for ability to pay and to assist patients in arranging for settlement of their financial obligation. Under this program, medically necessary services will be provided to Maine residents at no cost. If a patient does not qualify for free hospital care, he or she is entitled to ask for a fair hearing.
Part VI - Community InformationInland Hospital is a not-for-profit, community hospital located in Waterville, Maine with a mission to care for and serve our community. We serve residents in parts of Kennebec, Somerset, and Waldo Counties (mostly Kennebec). We are a 48-bed medical-surgical facility, with Lakewood, our 105-bed continuing care facility on the hospital campus, along with 19 primary and specialty care physician practices in Waterville and our surrounding communities of North Anson, Unity, Oakland, Madison, and Augusta. We also offer our primary care patients a Weekend Care service to help them avoid unnecessary, costly emergency room visits. Inland has three rural health centers in Unity, North Anson and Madison serving medically underserved populations.Inland is the only inpatient hospital serving Waterville. The next nearest inpatient hospital is located 15 miles south of Waterville in Augusta. The nearest tertiary hospital is located 60 miles north of Waterville.2013 data shows that Kennebec County has 121,000 residents (63% ages 18-64/17% over 64.) Median household income is $47,000. 13% of individuals and 18% of children are living in poverty. Somerset County has 52,000 residents (61% ages 18-64 and 18% over 65.) Median household income is $37,000. 18% of individuals and 25% of children are living in poverty.
Part VI - Community Building ActivitiesInland Hospital gets involved and gives back to our community to improve quality of life in Greater Waterville its how we help fulfill our community service mission to care for and serve our community. Whether its sponsoring scholarships to the local colleges, supporting physical activity and nutrition initiatives in our local schools, funding local public transportation, or supporting a cultural or economic development effort, Inland is proud to support and collaborate with groups that foster health and well-being. Our Community Benefit program contributed $50,000+ in 2015 towards health and community-building organizations and initiatives. In addition, we offer free health screenings and education to the public through our health events, and contribute to health improvement through numerous community collaborations and public events.
Part VI - Explanation Of How Organization Furthers Its Exempt PurposeAt Inland we strive to live up to our non-profit mission every day by caring for community members who otherwise would have limited access to quality healthcare. Besides providing charity care and financial assistance, we believe we can also affect the health of the communities we serve beyond care inside our hospital. We agree with the World Health Organization which states, Health is a state of complete physical, mental, and social well-being, and not merely the absence of disease or infirmity. We have a vision to be a responsible corporate citizen whose actions can impact our communitys overall well-being through commitments to priority issues. A hospital task force considered priority issues from the 2014 Community Health Needs Assessment and other data, and identified areas of significant needs to be addressed, as well as areas of need unable to be addressed by the hospital. An Implementation Strategy was developed and consisted of actions the hospital took to address the health need. Programs/resources the hospital committed to address the health need were identified along with planned collaborations with other area organizations. Inland Hospital focused on four key areas:1-Obesity - Goal: Prevent the increase of childhood obesity rates for Inlands service area2-Access to Care - Goal: Increase access to healthcare services in Inlands service area3-Chronic Disease Management - Goal: Improve health status of patients with chronic disease 4-Economic Opportunity/Poverty - Goal: Stimulate economic growth, expand educational opportunities and address social issuesIn addition to efforts led by Inland and our collaborating partners on the issues above, resources such as in-kind staff time and community benefit funding were dedicated to the following secondary issues:-Domestic Violence-Substance Abuse/Prevention/Treatment-Homelessness-Dental Health-TransportationInland's 2015 Community Wellness initiatives included:1)The "World of Womens Wellness", the hospitals annual health fair for women. 450 women attended the 18th Annual event on April 4, 2015 and received 1,700 free health screenings including, blood pressure, cholesterol, blood glucose, and bone density. More than 50 exhibitors linked attendees to local resources and services to improve health.2)In an effort to combat the childhood obesity epidemic, in 2015 we continued our year-long program to reach families with children to encourage active lifestyles and healthier eating. Our "Let's Go! Family Fun Series" offers activities each month to engage families with children to be physically active and eat healthier through free and low cost resources and activities such as hiking, swimming, and ice skating. We also provided funding support for Let's Go! in 18 local schools through our Community Benefit program. 3)We utilized the Lets Go! program in the healthcare sectorspecifically our family practices. Six of Inland's family practices and our Diabetes & Nutrition Wellness department were recognized as Let's Go! Sites of Distinction in 2015 for their efforts to curb childhood obesity in our patient populations. Components of the program include displaying posters with the 5210 messages, tracking weight classification for patients ages 2-18, and using the Healthy Habits survey at all well-child visits to inspire respectful conversations around weight status and lifestyle behaviors.4)We continue to maintain and improve the Inland Woods Trail on the hospital campus (created in 2010) with support from several local organizations. The trails were developed to create a lasting way to impact health, and give residents an easy, free resource for regular activity. The trails are used regularly by residents, hospital staff, and local schools for walking, biking, and snowshoeing.5)Inland offered weekly Snowshoe Adventures at Inland Woods January 8-March 12 to encourage the community to get outdoors and be active. We also established a snowshoe loan program for hospital staff.6)We provided ongoing support for the Quarry Road Recreation Area, investing $40,000+ since 2013 in annual support, event and youth program sponsorships and in-kind contributions of staff volunteer hours and board participation.7)Inland provided $1,000 in Community Benefit funding for Veggies For All (VFA), a food bank farm in Unity that works to relieve hunger by growing vegetables for those in need, working collaboratively to distribute and increase access to quality and nutritious food. VFA works with 10 food pantries to serve 1,500 food insecure people by growing 3 acres of storage crops. In addition to collaborating with Inland Hospital, they partner with Unity College, Volunteer Regional Food Pantry, Mid-Maine Food Pantry Coalition, more than 60 volunteers, and others.8)Inland provided $1,000 in Community Benefit funding to help provide twice yearly visits to ten Head Starts in Kennebec & Somerset counties to provide dental exams, cleanings, and fluoride varnishes for children who are predominantly Maine Care recipients.9)We collaborated with the Alfond Youth Center to bring two American Red Cross blood drives to Waterville. In addition, a majority of the members of Inland Hospitals board of trustees is comprised of people who reside in our primary service area who are neither employees nor independent contractors of the organization, nor family members thereof. Inland extends medical staff privileges to qualified physicians in its community for some or all of its departments or specialties. The Inland Hospital Foundation helps by raising funds for everything from capital expansion projects to residential activities to help offset normal operating expenses.Inland Hospital utilizes surplus funds to support a medical simulation center on the hospital campus that is used for training of clinical staff. Examples of training include Advanced Cardiac Life Support, Neonatal Resuscitation Program, and Pediatric Advanced Life Support. Training is not limited to employees at Inland Hospital - we also offer courses to local Emergency Medical Technicians and to clinicians at other hospitals in our region. Inland Hospital invites men, women, and young adults to volunteer their time to further the hospital's mission. In 2015, volunteers from our community donated 11,000 hours of their time to Inland and Lakewood, our continuing care center. Some volunteers work in areas with direct contact with patients and families; others are involved in our community events and "behind the scenes" activities which enhance the care provided by the hospital. In 2015, 210 Inland and Lakewood employees engaged in our community by volunteering on local boards, committees and projects that helped contribute to a vital community.
Part VI - Affilated Health Care System Roles and PromotionInland Hospital has been part of EMHS (Eastern Maine Healthcare Systems) since 1998. As part of EMHS, we conducted the 2014 Community Health Needs Assessment in collaboration with several member/affiliated hospitals and many public health and community organizations across eight Maine counties. The Community Health Needs Assessment informs initiatives to promote community health across the system as well as within each member hospital /local service area. Inland and other EMHS members adopt a local implementation strategy and community health improvement plan, tailored to meet local needs.
