DescInpatient Services - The Uniontown Hospital has160 adult, pediactic, and nursery level II beds. The Hospital delivered state-of-the-art primary inpatient services to approx. 7,900 inpatients during FY 2016. Since 1903, the Hospital has been serving the greater Uniontown and Fayette County area, providing quality healthcare regardless of the persons ability to pay. The Hospital has earned several recognitions from HealthGrades, the nations leading independent healthcare ratings organization. The Hospital draws the majority of its patients from Fayette County. Although the population of the County has been decreasing, the demand for the Hospitals services remain due to the downsizing and service cuts at nearby facilities along with the growth in both the regions uninsured and elderly populations. The Uniontown Hospital offers lines of services that are unavailable elsewhere in Fayette County. In addition to being the primary source of orthopedic care, it is the only hospital to offer obstetrics, interventional cardiology and radiation oncology services.Desc

Topic Decrease the presence of uninsured residents in the community. Action One a Implemented a program that provides real time opportunities to support and assist our patients in applying for 1 Medical Assisstance 2 SSDI 3 Explore, select, and enroll through the Affordable Care Marketplace 4 Uniontown Hopsitals Community Charity Care progam. b An outside vendor provides employees two full-time and one part-time that work 7 days a week, approximately 12 hours a day in the Emergency Department and throughout the organization. Topic Increase the appropriate use of available healthcare resources by increasing the use of affordable primary care clinics and FQHC whereby reducing the use of the Emergency Departments for non-emergent and/or ongoing primary care purposes. Action One a We have successfully reduced the number of uninsured patients presenting to the emergency department by 51 over the last three years. b We have successfully increased the number of Medicaid eligible patients presenting to the emergency department by 23 over the last three years. c The number of patients coming to the emergency department that are Medicaid eligible or uninsured has increased by 6 over the last three years. Topic To improve the health and birth outcome for women with chemical dependency. Action One a County has decided to begin a national program with grant money called Communities That Care to address these issues. b Ongoing networking nationally regarding prevalence and best evidence based treatment. c Care of the newborn withdrawing policy was changed to increase mother baby interaction and education. Involvement of the parents better prepares them for a difficult baby once discharged and enhances bonding. Parents are very pleased with this progam and there is less emotional/guilt displayed by the parents when they can participate in comforting the child. d Our Pediatric medical staff determined that mothers taking rehabilitative drugs may breastfeed but marijuana and other illegal substances not recommended, in accordance with American Academy of Pediatrics policies. e Investigating the national trend of initial treat for Neonatal Abstinence Syndrome to be performed in the newborn nursery verses transfer to tertiary care. Decision to transfer then if initial treatment not effective. This is still under reasearch and investigation.
Group Uniontown Hospital Line Part V, Section B, Line 11f Revision of policies in collaboration with Magee Womens Hospital, our partner. g Ongoing education of our staff and assurance on non-judgmental care and professional behaviors. h Participation in a Herald Standard article that was recently featured on the front page tracing a womans addiction and thoughts and tying it to the clinics and our hospital.
Group Uniontown Hospital Line Part V, Section B, Line 11The Uniontown Hospitals most recently conducted Community Health Needs Assessment and Implementation Plan adopted in tax year 2015 can be found at www.uniontownhospital.com. The health needs identified by the CHNA were reviewed and priority areas were determined by hospital management. Priority 1 Healthy Behaviors. Goal Work to improve the behaviors towards health care and lifestyle choices in the community. Strategies to be implemented FY 2017 - 2019 A Work with the local YMCA to develop and maintain a cardiac rehab/maintenance program and consider developing other related programs. B Develop an end of life/palliative care program to provide the patient/family with the reasources needed to help in decision making and communication. C Further develop patient/family centered care education model to include families in the patients care. D Develop patient educational materials. E Coordinate an annual health/wellness event for the community. F The Community Engagement team of the Uniontown Hospital will engage in fundraisers to support one charity event per year at a minimum such as, the American Cancer Societys Relay for Life. G Partner with local health care assistance programs such as Cornerstone Care and health-related charitable organizations like the American Heart Association and the March of Dimes to provide support and assistance in local health care initiatives and educational efforts.
Group Uniontown Hospital Line Part V, Section B, Line 11Priority 2 Social and Economic Factors. Goal Continue to decrease the number of uninsured people in the communtiy and those with limited insurance. Strategies to be implemented FY 2017 - 2019 A Increase financial assistance discussions in the Emergency Department with the development of a discharge discussion desk. B Annually evaluate the Uniontown Hospitals Charity Care Program and adjust related policies and procedures. C Track and improve statistics related to services provided by external resources regarding self-pay accounts and pre-service collection, charity care applications, and all financial assessments. In addition, monitor services provided to assist with Financial Assistance applications.
Group Uniontown Hospital Line Part V, Section B, Line 11Priority 3 Access to Health Services. Goal Increase access to healthcare through availability of physicians and alternative healthcare locations. Strategies to be implemented FY 2017 - 2019 A Significantly expand the availability of primary care physicians. B Increase the number of physician specialists including a, Neurologist, General Surgeon, Pulmonologist, Orthopedic Surgeon, Urologist, and Neurosurgeon. C Open additional outpatient service centers throughout the county starting with Brownsville.
Group Uniontown Hospital Line Part V, Section B, Line 11Some issues identified through the Community Health Needs Assessment have not been addressed in this plan. In initial discussion and subsequent prioritization, Uniontown Hospital considered the levels to which some needs were already being addressed in the service area. Additionally, some community needs fall out of the scope of expertise and resources of Uniontown Hospital. The following outlines how some of the needs identified in the assessment are addressed by others or in different ways A Substance Abuse - Mental health and substance abuse is not an expertise of the Uniontown Hospital. There is a small hospital on the border of the county which is dedicated to mental health and substance abuse issues and may focus on these issues. B Poor Nutrition/Access to Healthy Food Options - The Hospital is an active supporter of The Be Local Network program that is operated by a group of local media companies and community organizations who are dedicated to improving the climate of the local economy and to build a community of buyers that understands the importance of investing in local businesses and corporations. Part of this program focuses on healthy eating and consumption of healthy foods from the local farmers.
Group Uniontown Hospital Line Part V, Section B, Line 13hEligibility criteria will be based on whether the patient is uninsured or under insured. For uninsured, liquid assets may be considered for accounts over 5,000. For underinsured, liquid assets may be considered for personal account balances over 5,000.
Group Uniontown Hospital Line Part V, Section B, Line 16iThere is a paragraph on the second patient statement discussing free or discounted services and directing patients to the proper location to see the FAP. Brochures are available in the emergency waiting room, Patient Financial Services and admissions office which discuss the FAP.
Group Uniontown Hospital Line Part V, Section B, Line 22dDetermining Amounts Generally Billed - Following a determination of financial assistance eligibility, an individual will not be charged more than the amounts generally billed AGB for emergency or other medical care provided to individuals with insurance covering that care. Look Back Method - At Uniontown Hospital the AGB is determined through the look-back method which is callculated as follows 1 The AGB is calculated by reviewing all past claims that have been paid in full to the hospital facility for medically necessary care by Medicare fee-for-service together with all private health insurers paying claims to the hospital in a prior 12-month period. This amount can include co-insurance copayments and deductibles. 2 The AGB for emergency or medically necessary care provided to a financial assistance eligible individual is determined by multiplying gross charges for that care by one or more percentages of gross charges called AGB percentages. 3 The percentages are applied by the 45th day after the end of the 12-month period Uniontown Hospital used in calculating the AGB percentagess. 4 Any financial assistance eligible patient that paid over the AGB will be refunded. The Uniontown Hospitals current AGB rate is 53.3 which was calculated as of June 30, 2016.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part I Line 3cEligibility for the Uniontown Hospital Community Charity Care Program will be considered for those individuals who are uninsured, underinsured, ineligible for any government health care benefit program, and who are unable to pay for their care, based upon a determination of financial need. In addition to FPG, consideration of other resources available, such as assets and other financial obligations may be reviewed. For uninsured Liquid assets may be considered for accounts over 5,000. For underinsured Liquid assets may be considered for personal account balances over 5,000. Eligibility exceptions will exist for those patients that are eligible but failed to enroll for subsidized marketplace health plans during open enrollment periods or apply for Medicaid. Other exceptions may exist for certain other situations such as need of patients who have exhausted their insurance benefits, face extraordinary medical costs, and exceed financial eligibility criteria.
Part I Line 7Uniontown Hospital used the best available data to calculate the cost of charity care reported in line 7 for certain categories, the best available data was derived from a cost accounting system in other categories, specific cost-to-charge ratios from worksheet 2 were calculated and applied to that category.
Part I Line 7column f - The amount of expenditures used to calculate column f were reduced by bad debt expense of 3,939,476.
Part III Line 2The gross charges of Hospital provided services to individuals whom were determined to be ineligible under the Hospitals financial assistance policy for which payments were not received. These charges have been determined uncollectible by management.
Part III Line 3Line 3 was estimated by reviewing data from the PA Department of Health, Bureau of Health Statistics and Research to determine the percentage of the population in the Hospitals service area that falls below federal poverty guidelines. This percentage was then adjusted by subtracting the Hospitals average percentage of individuals eligible under the organizations financial assistance policy. The resulting percentage was applied to line 2 to estimate line 3. Rationale for including bad debt amounts in community benefit It is the Hospitals contention that the cost of bad debt should be stated in Part 1 Line 7 of Schedule H as they represent the costs for provision of services to patients for which the entity has exhausted all recourse for reimbursement. The service provided to patients who present themselves are provided regardless of a patients ability to pay as in line with the organizations charitable mission and service to our community.
Part III Line 4The audited financial statements do not contain a footnote that describes bad debt expense. They do, however, contain a footnote that describes patient accounts receivable. That footnote reads as follows Accounts receivable are reduced by an allowance for doubtful accounts. In evaluating the collectability of accounts receivable, the Health System analyzes its past history and identifies trends for each of its major payer sources of revenue to estimate the approriate allowance for doubtful accounts and provision for uncollectible accounts. Management regularly reviews data about these major payer sources of revenue in evaluating the sufficiency of the allowance for doubtful accounts. The difference between the standard rates or the discounted rates if negotiated or provided by policy and the amounts actually collected after all reasonable collection efforts have been exhausted is charged off against the allowance for doubtful accounts. See page 9 of the attached financial statements for the patient accounts receivable footnote in its entirety.
Part III Line 8Serving patients with government health benefits, such as Medicare, is a component of the community benefit standard that tax-exempt hospitals are held to. This implies that serving Medicare patients is a community benefit and that the hospital operates to promote the health of the community. The Medicare cost report was utilized to determine the amount reported on line 6.
Part III Line 9bThe Uniontown Hospitals Billing and Collection policies were approved by the Board of Directors and state that for patients who qualify for charity or are cooperating in good faith to resolve their hospital bills according to our payment options, Uniontown Hospital will not send unpaid bills to outside collection agencies.
Part VI Line 2The Fayette Regional Health System leadership team develops an operating plan every year. This operating plan establishes several priorities for the Uniontown Hospital, all of which are focused on meeting the ever changing healthcare needs of the community. The leadership team develops this operating plan with knowledge gathered from direct contact from patients patient satisfaction surveys outside consultants professional health care groups such as the Hospital Association of Pennsylvania and the Healthcare Financial Management Association and research obtained from various sources including the federal and state government and various other outreach programs and sources.
Part VI Line 3As part of the Revenue Cycle policy entitled Service Access it states under Patient Advocacy that the health care system will actively assist all individuals in seeking resolution to their financial obligations. This includes helping them understand what services are covered by their health insurance, providing assistance in obtaining health insurance and helping seek eligibility for financial assistance. Convenient office hours, in a convenient loction, will be established for patients seeking assistance. Currently this program is called the Uniontown Hospital Community Charity Care UHC3. UHC3 is available to all residents of Fayette County and patients will be assisted by Financial Care Consultants that are part of the Patient Financial Services department.
