| Schedule H, Part V, Section B, Question 5 | In its most recently conducted Community Health Needs Assessment ("CHNA") Aria Health ("Aria") took into account input from persons who represent the broad interested of the community served by the hospital facilities. According to its CHNA, Aria obtained primary and secondary data from multiple sources. Primary data was collected through interviews with leaders of community-based organizations who represent the needs of the community, local public health officials and through health-related surveys of community residents. Members of the community were asked to provide their insight into the health issues affecting the people living in Arias service area. These included members of the health departments, physicians, insurers, and community service organizations. Included as Appendix A in the CHNA, the organization identifies the following individuals/organizations who were interviewed in an effort to obtain input from persons who represent the broad interests of the community served: 1) Bucks County Health Department Expertise in health needs of the Bucks County residents; 2) Philadelphia Department of Public Health Expertise in the health needs of Philadelphia County residents; 3) Bucks county Ara agency on Aging Expertise in health needs specific to older adults; 4) Bucks County Health Improvement Partnership Expertise in health needs of the uninsured in Bucks County and barriers to receiving appropriate care; 5) Community Care Center of the Northeast Expertise in the health needs specific to older adults and resources available to provide assistance to low-income older adults in Northeast Philadelphia; 6) Community Physicians Expertise in the needs of patients (especially low-income and disadvantaged populations) in Northeast Philadelphia and Lower Bucks County; and 7) Keystone First Expertise in the health needs of Philadelphia County residents and health resources in Philadelphia County. In addition to performing key information interviews described above, Aria also sent out a Community Health and Wellness study survey to over 5,900 residents of Arias service area in an effort to obtain additional insight and information. |
| Schedule H, Part V, Section B, Question 6A | Aria Health includes three hospital facilities, Aria Health Frankford, Aria Health Bucks and Aria Health Torresdale. These three hospital facilities worked together to create the organizations CHNA. Therefore, Arias CHNA takes into account the primary service area of these hospital facilities and defines the Aria Health community as the service area that stretches from Kensington and Port Richmond through Northeast Philadelphia and into Lower Bucks County. This primary service area is further defined within the organizations CHNA. |
| Schedule H, Part V, Section B, Question 11 | Through the CHNA process, Aria identified a number of health needs in the community. High rates of obesity, diabetes and hypertension, poor nutrition, and low exercise were prevalent, along with access to care issues, and high-risk behaviors such as smoking. Additionally, Aria also serves a number of disadvantaged communities that are disproportionately affected by these issues. After the completion of the CHNA needs were prioritized across the service area and accounted for by level of need, barriers to care, known disparities and resources available. Additionally, Aria took into account the expertise and resources that Aria can provide. The following were identified as the significant needs to be addressed by the organization: - Nutrition Education & Counseling (including general nutrition, obesity, hypertension & eating behaviors); - Diabetes Management; - Preventative Screenings; - Smoking Cessation; and - Older adult needs. In order to improve community health, Aria Health formed an Implementation Team to streamline population and community health strategies and design plans and initiatives to address the CHNA identified priorities. The team developed the following 3-year plan to implement programs to address these health priorities, including anticipated impact, committed resources and partners, and goals and measures to achieve those goals. In its CHNA Implementation Plan, the organization addresses the following items for each identified health need: - Organizational response; - Target Population; - Rationale; - Anticipated Impact; - Resources and Partners; - Goals; and - Measures. Aria Health is attempting to address all needs identified in the CHNA. Please refer to the Implementation Plan attached to this Federal Form 990 for additional information explaining how the hospital facility is addressing those significant needs. |

| Schedule H, Part I, Line 3C | Aria Health is committed to providing high quality and affordable healthcare to all individuals who seek services, include those uninsured individuals who lack the means to pay for such services. Aria provides discounts to patients similar to discounts provided to third party insurance and governmental payers. Such discounts are established at rates which cover or contribute toward the cost to provide the services. As reflected in Schedule H, Part V, Section B, Question 13, in addition to Federal Poverty Guidelines ("FPG") Aria uses the following criteria when determining a patients eligibility for free or discounted financial assistance: - Medical Indigency; - Insurance Status; - Underinsurance Status; and - Residency. |
| Schedule H, Part I, Line 6A | Not applicable. |
| Schedule H, Part I, Line 7 | Worksheet 2 was used for the Cost to Charge ratio ("RCC"). This methodology is consistent with the guidelines established by HFMA Principle & Practice Statement #15. |
| Schedule H, Part II | Community building activities undertaken by this organization improve the medical and socio-economic well-being of the communities in our care. This is accomplished through numerous activities which are not a part of Part I Charity Care and other community benefits, and are not included elsewhere on Schedule H. These activities include programs that address the root causes of health problems such as education, poverty, unemployment, access to care, health advocacy and economic development. Aria promotes numerous Community Based & Health Professional Programs and Services throughout the year, including Community Health Fairs and screenings and Community Service Activities, High School Sports Physicals, Health Professional Education programs, including but not limited to, the Aria Health School of Nursing, and education respiratory programs. |
| Schedule H, Part III, Lines 2, 3 & 4 | Bad debt expense was calculated using the organizations bad debt expense from the audited consolidated financial statements, net of accounts written off at charges. Aria Health prepares and issues audited consolidated financial statements. The text below was obtained from the footnote to the audited consolidated financial statements of Aria Health System, Inc. and affiliates. Provision for Bad Debt Expense ------------------------------ The provision for bad debt expense is based on managements assessment of excepted net collections considering economic conditions, historical experience, trends in health care coverage and other collection indicators. Periodically throughout the year, management assesses the adequacy of the allowance for uncollectible accounts based upon historical write-off experience by payer category, including not covered by insurance, and history of cash collections. The results of this view are than used to make modifications to the provision for bad debt expense to establish an appropriate allowance for uncollectible accounts and is recorded in the period of service. No significant modifications were made for fiscal year 2016 and 2015. After satisfaction of amounts due from insurance and reasonable efforts to collect from the patient have been exhausted, AHS follows established guidelines for placing certain past-due patient balances with collections agencies, subject to terms of certain restrictions on collection efforts as determined by AHS. Account receivables are written off after collection efforts have been followed in accordance with AHS policy. Charitable Medical Care Provided -------------------------------- The Hospital provides services to all patients regardless of ability to pay. Although all patients are ultimately responsible for those hospital services rendered that are not covered by insurance, some patients qualify for charity care based on guidelines established by the Hospital and are therefore not responsible for payment of all or a part of such services. The Hospitals policy recognizes charity care based on the zip code where the patient resides. Charges for services rendered to patients who meet the Hospitals guidelines for charity care are not reflected in the accompanying consolidated financial statements. The cost for these services totaled $13.7 million and $16.5 million in 2016 and 2015, respectively. These amounts do not include the provision for bad debts, which is reflected separately in the accompanying consolidated statements of operations. Aria Health received charity care from the Commonwealth of PA totaling $0.3 million and $0.3 million for the years ended June 30, 2016 and 2015, respectively. In addition, disclosure of the method used to identify or determine such costs is required. Aria Health estimates the cost of charity care using the ratio of cost to charge methodology. Total charity care charges are multiplied by a ratio of cost to charges. |
| Schedule H, Part III, Line 8 | Medicare costs were derived from the Medicare cost report filed by the organization. Medicare underpayments and bad debt are community benefit and associated costs are includable on the form 990, Schedule H, Part I. The organization feels that Medicare underpayments (shortfall), bad debt and associated costs are community benefit and are includable on the form 990, Schedule H, Part I. As outlined more fully below, the organization believes that these services and related costs promote the health of the community as a whole and are rendered in conjunction with the organization's charitable tax-exempt purposes and mission in providing medically necessary healthcare services to all individuals in a non-discriminatory manner without regard to race, color, creed, sex, national origin or ability to pay and consistent with the Community Benefit Standard promulgated by the IRS. The Community Benefit Standard is the current standard for a hospital for recognition as a tax-exempt and charitable organization under internal revenue code ("IRC") 501(c)(3). The organization is recognized as a tax-exempt entity and charitable organization under 501(c)(3) of the IRC. Although there is no definition in the tax code for the term "charitable" a regulation promulgated by the Department of the Treasury provides some guidance and states that "the term charitable is used in Section 501(c)(3) in its generally accepted legal sense, provides examples of charitable purposes, including the relief of the poor or unprivileged; the promotion of social welfare; and the advancement of education, religion, and science. Note it does not explicitly address the activities of hospitals. In the absence of explicit statutory or regulatory requirements applying the term "charitable" to hospitals, it has been left to the IRS to determine the criteria hospitals must meet to qualify as IRC 501(c)(3) charitable organizations. The original standard was known as the Charity Care Standard. This standard was replaced by the IRS with the Community Benefit Standard which is the current standard. Charity Care Standard In 1956, the IRS issued Revenue Ruling 56-185, which addressed the requirements hospitals needed to meet in order to qualify for IRC 501(c)(3) status. One of these requirements is known as the "Charity Care Standard." Under the standard, a hospital must provide, to the extent of its financial ability, free or reduced-cost care to patients who cannot pay for such services. A hospital that expected full payment did not, according to the ruling, provide charity care based on the fact that some patients ultimately failed to pay. The ruling emphasized that a low level of charity care did not necessarily mean that a hospital had failed to meet the requirement since that level could reflect its financial ability to provide such care. The ruling also noted that publicly supported community hospitals would normally qualify as charitable organizations because they serve the entire community and a low level of charity care would not affect a hospital's exempt status if it was due to the surrounding community's lack of charitable demands. Community Benefit Standard In 1969, the IRS issued revenue ruling 69-545, which "removed" from revenue ruling 56-185 "the requirements relating to caring for patients without charge or at rates below cost." Under the standard developed in revenue ruling 69-545, which is known as the "community benefit standard," hospitals are judged on whether they promote the health of a broad class of individuals in the community. The ruling involved a hospital that only admitted individuals who could pay for the services (by themselves, private insurance, or public programs such as Medicare), but operated a full-time emergency room that was open to everyone. The IRS ruled that the hospital qualified as a charitable organization because it promoted the health of people in its community. The IRS reasoned that because the promotion of health was a charitable purpose according to the general law of charity, it fell within the "generally accepted legal sense" of the term "charitable," as required by The Department of Treasury Reg. 1.501(c)(3)-1(d)(2). The IRS ruling stated that the promotion of health, like the relief of poverty and the advancement of education and religion, is one of the purposes in the general law of charity that is deemed beneficial to the community as a whole even though the class of beneficiaries eligible to receive a direct benefit from its activities does not include all members of the community, such as indigent members of the community, provided that the class is not so small that its relief is not of benefit to the community. The IRS concluded that the hospital was "promoting the health of a class of persons that is broad enough to benefit the community" because its emergency room was open to all and it provided care to everyone who could pay, whether directly or through third-party reimbursement. Other characteristics of the hospital that the IRS highlighted included the following: its surplus funds were used to improve patient care, expand hospital facilities, and advance medical training, education and research; it was controlled by a board of trustees that consisted of independent civic leaders; and hospital medical staff privileges were available to all qualified physicians. Medicare underpayments and bad debt are community benefit and associated costs are includable on the form 990, Schedule H, Part I. The American Hospital Association ("AHA") feels that Medicare underpayments (shortfall) and bad debt are community benefit and thus includable on the form 990, Schedule H, Part I. This organization agrees with the AHA's position. As outlined in the AHA's letter to the IRS dated August 21, 2007 with respect to the first published draft of the new Form 990 and Schedule H, the AHA felt that the IRS should incorporate the full value of the community benefit that hospitals provide by counting Medicare underpayments (shortfall) as quantifiable community benefit for the following reasons: - Providing care for the elderly and serving Medicare patients is an essential part of the community benefit standard. - Medicare, like Medicaid, does not pay the full cost of care. From the latest data provided by the American Hospital Association, as of 2012, Medicare reimburses hospitals only 86 cents for every dollar they spend to take care of Medicare patients. The Medicare Payment Advisory Commission ("MEDPAC"), in its March 2013 report to congress, states that beneficiaries will grow notably faster in the next 10 years than in the past decade as the baby-boom generation ages into the program. In addition, the population aging into the Medicare program will present a new set of challenges since rising obesity levels put this population at a greater risk than previous generations for chronic disease. - Many Medicare beneficiaries, like their Medicaid counterparts, are poor. More than 42 percent of Medicare spending is for beneficiaries whose income is below 200 percent of the federal poverty level. Many of those Medicare beneficiaries are also eligible for Medicaid -- so called eligible." There is every compelling public policy reason to treat Medicare and Medicaid underpayments similarly for purposes of a hospital's community benefit and include these costs on form 990, Schedule H, Part I. Medicare underpayment must be shouldered by the hospital in order to continue treating the community's elderly and poor. These underpayments represent a real cost of serving the community and should count as a quantifiable community benefit. Both the AHA and this organization also feel that patient bad debt is a community benefit and thus includable on the form 990, Schedule H, Part I. Like Medicare underpayment (shortfalls), there also are compelling reasons that patient bad debt should be counted as quantifiable community benefit as follows: - A significant majority of bad debt is attributable to low-income patients, who, for many reasons, decline to complete the forms required to establish eligibility for hospitals' charity care or financial assistance programs. A 2006 congressional budget office ("CBO") report, nonprofit hospitals and the provision of community benefits, cited two studies indicating that "the great majority of bad debt was attributable to patients with incomes below 200% of the federal poverty line." - The report also noted that a substantial portion of bad debt is pending charity care. Unlike bad debt in other industries, hospital bad debt is complicated by the fact that hospitals follow their mission to the community and treat every patient that comes through their emergency department, regardless of ability to pay. Patients who have outstanding bills are not turned away, unlike other industries. Bad debt is further complicated by the auditing industry's standards on reporting charity care. Many patients cannot or do not provide |
| Schedule H, Part III, Line 9B | The facilities and providers that are a part of Aria Health provide inpatient, outpatient, and emergency services without regard to a patient's ability to pay. To fulfill its mission of providing compassionate, high quality care to all patients it serves, Aria Health must also ensure its own financial viability. In order to secure reimbursement of costs for services provided, every effort is made to assist uninsured patients in obtaining insurance coverage through Medical Assistance (MA), Children's Health Insurance Program (CHIP) or other federal, state, or city care coverage sources. Aria provides Charity Care to those patients who are unable to pay based upon the eligibility criteria included in their financial assistance policy. The organization provides discounts to patients similar to discounts provided to third party insurance and governmental payers. Such discounts are established at rates which cover or contribute toward the cost to provide the services. In order to conserve healthcare resources, Aria will seek payment from patients who do not qualify for Charity Care at its discounted rates. It is the policy of Aria Health to differentiate between uninsured patients who are unable to pay from those who are unwilling to pay for all or part of their care. For an uninsured patient to receive non-emergent care at discounted rates, the patient must make a payment or make an arrangement for payment prior to services being rendered. Billing -------- The hospital billing system will reduce charges to the Self-Pay Fee Schedule by posting the difference to an "Uninsured Allowance." This process is enacted when a final bill is generated. A first Self-pay final bill is produced showing the Uninsured Allowance. The bill will inform the patient that: (1) All financial arrangements made before, during or shortly after a visit may not be reflected on the first bill; and (2) If the patient is low income and enrolled in a government program (e.g., Medicare or Medicaid) or without insurance, he or she may qualify for Aria Healths charity care program. Consistent with normal business office practices, all unpaid Self-Pay accounts that have an uninsured allowance may be forwarded to outside vendors for pre-admission and post discharge collection assistance. Self-pay accounts with an open balance remaining after the pre-collection period effort shall be sent to a collection agency. Collection agencies shall extend to the patient a copy of Arias charity care policy in conjunction with its normal collection efforts. |
| Schedule H, Part VI, Question 2 | As the largest healthcare provider in Northeast Philadelphia and Lower Bucks County, Aria Health is committed to caring for the community and improving population health of community members. Promoting healthy lifestyles, offering preventive care, and engaging the community are central to Aria's mission. Aria Health believes that in order to effectively care for a community, it is necessary to understand the health conditions, behaviors and needs of the individuals who live there. This is achieved through the CHNA process, which includes defining the community, analyzing primary and secondary data related to the communitys health status and health needs, identifying the greatest needs, and designing a plan to meet those needs. The purpose of the CHNA was threefold: 1. Identify common health conditions and concerns that are affecting the members of the community; 2. Identify barriers faced by members of the community that impact their ability to maintain good health and well-being; and 3. Engage healthcare providers and community members in joint efforts to improve the health of the community. Through initiatives following Arias previous CHNA, the hospital was able to reach hundreds of community members with programs targeting Type 2 diabetes, obesity, smoking cessation, general health management and programs specific to the health of older adults. Feedback from initiative participants, as well as knowledge and biometric assessments, showed positive impacts of these programs on health and well-being. The organization assessed the healthcare needs of its community by obtaining primary and secondary data from multiple sources. As described in Schedule H, Part V, Section B, primary data was collected through interviews with leaders of community-based organizations who represent the needs of the community, local public health officials and through health-related surveys of community residents. In addition, secondary data was obtained from the Public Health Management Corporations Southeastern Pennsylvania Household Health Survey, Pennsylvania Department of Health, Division of Health Informatics, Healthy Communities Institute, National Cancer Institute and the American Community Survey. The data obtained was analyzed by region and compared to county, state and national level indicators and Healthy People 2020 objectives (where available). Data was also analyzed using the Healthy Communities Institutes Data Scoring Tool, which ranks indicators based on systematic comparison that assigns the community a score based on comparison to other communities, health targets, and trends over time. Socioeconomic impact and disparities between groups (such as age, gender, or race) are also accounted for in the HCI SocioNeeds Index which ranks areas based on socioeconomic variables that are known to result in poor health outcomes. After the completion of the CHNA needs were prioritized across the service area and accounted for by level of need, barriers to care, known disparities and resources available. The findings of the assessment are then used to set goals for community health improvement and to develop strategic initiatives to achieve those goals. |
| Schedule H, Part VI, Question 3 | Aria Health is committed to providing high quality and affordable care to all individuals who seek services, including those uninsured individuals who lack the means to pay for such services. Aria has a financial assistance policy which sets forth the guidelines and processes by which patients are provided Financial Assistance through individual financial counseling, help in seeking federal and state care coverage, based upon eligibility providing a discount from established medical care charges and applying for Charity Care. Aria Healths financial assistance policy is posted on the organizations website and available upon request, free of charge. Aria Health will provide on-site financial counselors. The financial counselors are individuals who are trained to assist patients in identifying sources of care coverage, determining eligibility for such coverage, and assisting in completing necessary applications. Financial counselors may either be employees of Aria Health, an Aria Health affiliate, or a third party engaged by Aria Health or one of its affiliates to assist in its billing and collections processes. The financial counselors also help to assist patients with the application process for government insurance, including, but not limited to, State Medicaid, government programs, CHIPS, and Crime Victims. At the time of scheduling, pre-registration, or registration of non-emergent services (whichever are applicable); the patient will be asked for insurance coverage. The patient will be informed of any co-payments that will be expected at the time of payment. The financial counselor will assist uninsured patients in obtaining Medical Assistance, Childrens Insurance Program, Special Care, COBRA, or other Governmental Care Coverage. If the patient is likely to qualify for Governmental Care Coverage, the Financial Counselor will assist the patient in applying for appropriate coverage or refer the patient to the local County Assistance Office. Additionally, the uninsured patient will be: (1) Informed of the discounted Self Pay Fee Schedule, (2) Registered as a Self-Pay patient, (3) Informed of the availability of Charity Care and (4) Provided a "Financial Assistance Program" summary description. After medical screening and stabilization of the emergent medical condition and within one (1) business day following admission, a financial counselor will arrange a meeting with uninsured patients, to be held during the inpatient admission. The financial counselor will work with patients and their families to obtain information necessary to complete a Medical Assistance Application. If the Medical Assistance application cannot be completed during the patient admission process, the financial counselor will follow up with the patient by telephone and request additional information. Alternatively, patients may make an appointment with the County Assistance Office to complete the Medical Assistance application. All patients will be treated in accordance with the requirements of the Federal Emergency Medical Treatment and Labor Act ("EMTALA"). All patients will be triaged and receive a medical screening exam and their emergency medical condition stabilized by Emergency Department staff prior to registration or requesting or obtaining information on insurance coverage or ability to pay. After medical screening and stabilization, the Emergency Room Registrar, upon learning a patient has no insurance, shall inform the patient of Aria Healths Self-Pay Fee Schedule and the Charity Care Policy. If the patient wishes to apply for Charity Care, the registrar will provide a Charity Care Application. The patient shall be instructed to complete and return the application to the financial counselor. Should the patient decline the charity care option, the Registrar shall request from patients without insurance an upfront discounted price which includes both hospital and physician services. Should the patient be unable or unwilling to pay the discounted price, no further action is taken and treatment proceeds through release of the patient from emergency care. Additionally, Aria Health provides financial counseling services to go over patient bills and answer any questions a patient may have. Hospital stays can be stressful, and at Aria Health, we want to help make patient experiences as comfortable as possible. One way to do that is to offer assistance to patients or families regarding their medical bills and payment options, through the Aria Financial Counseling Program. |

The following demographics were included in the organizations 2015 CHNA: Lower Bucks County (West) Minority 8.8%, Poverty 4%, Poverty age 65+ 5.1%, No high school diploma 5.6% and unemployed 8.1% Lower Bucks County (East) - Minority 13.9%, Poverty 5.7%, Poverty age 65+ 5.4%, No high school diploma 6.9% and unemployed 8.2% Lower Bucks County (South) & Far Northeast Philadelphia - Minority 21.5%, Poverty 8.3%, Poverty age 65+ 7.4%, No high school diploma 11.1% and unemployed 10.4% Northeast Philadelphia - Minority 37.5%, Poverty 16.4%, Poverty age 65+ 15%, No high school diploma 16.1% and unemployed 14.1% Lower Northeast Philadelphia - Minority 64.1%, Poverty 34.7%, Poverty age 65+ 19.8%, No high school diploma 28.7% and unemployed 21.6% Additional demographic information is included on Arias website. Aria Health is recognized by the Internal Revenue Service ("IRS") as an Internal Revenue Code 501(c)(3) tax-exempt organization. Pursuant to its charitable purposes, Aria Health provides healthcare services to all individuals in a non-discriminatory manner regardless of race, color, creed, sex, national origin or ability to pay. Moreover, Aria Health operates consistently with the following criteria outlined in IRS revenue ruling 69-545: 1) Provides healthcare services to all individuals regardless of ability to pay, including Charity Care, Self-Pay, Medicare and Medicaid patients; 2) Operates 3 active emergency departments for all persons; which are open 24 hours a day, 7 days a week, 365 days per year, including a trauma emergency department on the Torresdale campus; 3) Maintains an open medical staff, with privileges available to all qualified physicians; 4) Control of Aria Health rests with its board of directors and the board of directors of Aria Health System, Inc. Both boards are comprised of independent civic leaders and other prominent members of the community; and 5) Surplus funds are used to improve the quality of patient care, expand and renovate facilities and advance medical care, programs and activities. The operations of Aria Health as shown through the factors outlined above and other information contained herein, clearly demonstrate that the use and control of Aria Health is for the benefit of the public and that no part of the income or net earnings of the organization inures to the benefit of any private individual nor is any private interest being served other than incidentally. Mission Statement ================= To provide exceptional patient care and customer service to members of the communities served by the Hospitals, and to recognize a patient's right to considerate and respectful care, regardless of ability to pay. Vision ====== - To be the premier integrated healthcare delivery system serving Northeast Philadelphia and nearby Bucks and Montgomery counties; - To be recognized for high quality, commitment to service and caring behavior by all members of the hospital family; - To be committed to constantly seeking improvements in service to our patients, our community and to each other; - To be the organization to which people, businesses and insurers first turn to provide or arrange for the provision of health-related services; and - To be the standard by which hospital and other health-related services provided by others are measured. Principles & Values =================== We commit to holding ourselves, each other and our organization to the following core values: - Service; - Integrity; - Quality; - Compassion and Dignity; and - Safety. History ======= The story of Aria Health System began in 1902, when a resident of the Frankford section of Philadelphia contracted typhoid fever. The patient's physician, Dr. Joseph Ball, couldnt find a single hospital in the city to admit his seriously ill patient. Not one of the downtown hospitals had an available bed. When Dr. Ball learned other Northeast physicians were also having their patients turned away, he began an effort to establish a community hospital that would provide convenient, high-quality medical care to his neighbors. Named after the neighborhood it would first serve, Frankford Hospital - Frankford (now Aria Health - Frankford Campus) opened its doors on the fourth of July, 1903. During the decades that followed, Frankford expanded upon Dr. Balls founding vision. For over a century, the Hospital introduced many new services to better meet the health needs of its growing communities. For example, as the Hospitals service area grew and flourished, Frankford responded by opening a second site in 1977. Almost immediately, Frankford Hospital Torresdale (now Aria Health - Torresdale Campus) was operating at full capacity, meeting the healthcare needs of families from the Far Northeast and Lower Bucks County. When many families Frankford Hospital had been serving for generations began relocating to Bucks County in the 1980s and 1990s, the Hospital again sought better ways to serve them. In March 1999, Frankford acquired Delaware Valley Medical Center and turned it into its third inpatient site Frankford Hospital Bucks County (now Aria Health - Bucks County). To reflect an expanded, regional vision for health care service delivery in Northeast Philadelphia and Bucks County, Frankford Hospital changed its name to Aria Health in May 2009. Today, Aria Healths focus on its founding philosophy is clearer than ever before. Now, our physicians, nurses and support personnel provide advanced medicine and personal care at three modern inpatient facilities and three outpatient sites all of which are equipped with highly sophisticated technology. Campus Locations and Outpatient Sites ===================================== Aria Health has three hospital campuses, two outpatient centers, and a growing network of physicians that serve the Northeast Philadelphia and Bucks County communities. | CORE FORM, PART III; STATEMENT OF PROGRAM SERVICE ACCOMPLISHMENTS | School of Nursing/Nursing at Aria ================================= School of Nursing ----------------- Aria Health School of Nursing continues to distinguish itself, building on its roots and history as Frankford Hospital School of Nursing and poised now to grow across another hundred years of success in providing quality academics and preparation in the field of nursing. In the fall of 1904, a training school for nurses was established in Northeast Philadelphia, just a block away from Aria Health - Frankford Campus, which had been founded in 1903. Under the direction of Anna C. Garrett, who was also Aria Health - Frankford Campus's first superintendent, the Frankford Hospital School of Nursing set forth to provide exceptional education to students interested in pursuing the nursing profession. The Frankford Hospital School of Nursing developed into a distinguished co-educational institution, and over a century-plus provided more than 2,500 students with comprehensive learning experiences in professional nursing. In 2004, Aria Health School of Nursing, under its former name of The Frankford Hospital School of Nursing, celebrated its 100-year anniversary. It continues to carry on the long tradition of educating caring and professional nurses who will become a vital part of the healthcare community and the community in which they live. In April of 2012, Aria Health School of Nursing relocated to the Aria Corporate Center at Neshaminy Interplex in Trevose, PA. The new facility provides our students with a professional setting in which to pursue their studies. Year after year, Aria Health School of Nursing graduates are just as unanimously satisfied with their education as their employers are with their performance. The leadership of Thomas Jefferson Universitys Jefferson College of Nursing and Aria Healths School of Nursing are planning to open a Bachelor of Science in Nursing (BSN) program at the Aria Campus in Trevose, Pa, pending approval by the Pennsylvania Board of Nursing. The decision to move from a diploma program to a BSN-degree granting program was the result of a thoughtful and deliberate process involving an integration team that included nursing and nursing education leaders. Nursing at Aria --------------- The Department of Nursing at Aria Health supports and empowers its nurses in fulfilling the hospital's mission of providing exceptional patient care. Aria Health utilizes a nursing practice model based on Relationship Based Care ("RBC"). The RBC model provides a conceptual framework as well as a vision for delivering care with strong underlying values and principles. It also provides the practical infrastructure for putting into action the education and leadership needed to make Aria Health Nursing successful and provide an effective means to measure that success. This model is comprised of three relationships of care providers: relationship with patients and families, relationship with self and relationships with colleagues. The most crucial relationship is the healing relationship with the patient and their families or significant others. Aria nurses are committed to providing compassionate care, through conveyed a kind act, through competent clinical interventions, or through listening with empathy. They convey respect and personal concern for the patient, strive to understand what is most important to them, safeguard their dignity and well-being and actively engage in all aspects of the patients care. To support the relationship with self, our nurses are provided with opportunities to learn and grow as a nursing professional and to explore opportunities to support development and wellness. Learning begins in a comprehensive orientation program, in which you are teamed with an experienced nurse, many of whom have been trained in our Preceptor Workshop. Our Department of Nursing Education provides additional learning opportunities, including the specialty courses, clinical updates and Nursing Grand Rounds. Staff nurses are encouraged to continue their professional growth and receive recognition and rewards for their achievements through several different programs, including our Professional Nurse Recognition Program. We celebrate the achievements of our professional nurses at Aria, whether they are making valuable contributions in direct patient care, research, community service, in professional development or participation professional organizations. To support the relationship with our colleagues, our Nursing Shared Governance Councils empower bedside nurses, alongside other nurse professionals, to exercise autonomy and accountability as they address issues related to professional nursing, including clinical policy development, informatics, professional development, retention, recruitment, and the performance improvement process. Nurses are also members of hospital committees, where collaboration with interdisciplinary health team members occurs to develop evidence based care processes. Aria Health, located in Philadelphia and Bucks County, is dedicated to providing a positive environment to practice the art of Nursing. Graduate Medical Education/Residency Program ============================================ Choosing a residency program is a milestone in a medical education. Individualized instruction, under the tutelage of our dedicated clinical and teaching faculty, ensures that each individual's needs are met. The Department of Medicine Grand Rounds, Daily morning report and noon conferences are also part of the educational experience. |
| CORE FORM, PART III; STATEMENT OF PROGRAM SERVICE ACCOMPLISHMENTS | In-house rotations include standard Floor/ Telemetry teaching Teams at Bucks and 2 excellent but vastly different ICU experiences. The Bucks ICU allows for individual growth and autonomy with oversight from 2 different critical care attendings as well as the admitting attending. The Torresdale ICU is a much larger and busier ICU with a wide mix of medical, surgical, and trauma patients. Residents will round with an organized critical care team led by a full-time Intensivist. Both ICU's allow for significant procedural experience. Multiple large university hospitals in the area provide excellent academic and clinical experiences for completing elective and selective requirements, and we place very few restrictions on outside rotations. This allows for valuable "face time" at institutions where you may be pursuing a fellowship. Residents also complete a weekly half-day ambulatory medicine experience in the hospital's urban Internal Medicine Clinic at the Frankford campus. The residents are able to develop their own panel of patients that they manage throughout their residency career. Monthly patient information sessions are held by the residents to educate the Clinic patients about common medical topics, like tobacco cessation and diabetes. We schedule patients so residents will have sufficient time to spend with each patient and be comprehensive in their individual care plan. The traditional call schedule has been replaced with a Night-Float system which allows for better continuity in patient care. The Night-Float system also eliminates missing time away from rotations when residents traditionally would have been post-call. The Aria Health Internal Medicine residency program is committed to providing our residents with the best educational experience found in a community hospital. The friendly environment allows for growth at all stages of training. We welcome residents to find out more about our program by contacting our Program Directors or Chief Residents and/or scheduling a visit to our hospital. 3. Osteopathic Emergency Medical Residency The Aria Health's Emergency Medicine Residency Program offers a unique experience for education in Emergency Medicine. Academic excellence is combined with the hospital's mission to provide comprehensive and accessible health care to the communities of Philadelphia and Bucks County. The diversity within the Aria Health system creates an atmosphere of enhanced educational and clinical opportunities, while affording the development of individual interests. The Emergency Medicine Residency Program supplements the clinical rotation experience with affiliations with other academic centers of the Philadelphia region such as Children's Hospital of Philadelphia, Temple University Hospital, St. Christopher's Hospital for Children, Cooper Hospital University Medical Center and Albert Einstein Medical Center. Through Aria Health's unique three campus system and affiliates, residents have the opportunity to experience all aspects of the medical community. This diversity of clinical experience prepares the residents for careers in all areas of Emergency Medicine. Clinical, administrative, teaching and research skills are developed and cultivated, creating the next generation of leaders and mentors in their field. Because of Aria's diverse three-campus system with an annual emergency department volume of over 130,000, the residents gain exposure to both urban and community medicine. The philosophy of the emergency medicine residency program is based on the triad of high quality patient care, patient safety, and education. While patient care remains a departmental priority, all three elements are integral components to a comprehensive education experience. Residents will rotate through the Emergency Departments of a brand new state-of-the-art high volume 40+ bed Level II Trauma Center at the Torresdale Campus, a newly expanded busy urban Emergency Department at the Frankford Campus, and a growing suburban Emergency Department at the Bucks Campus. Each resident will spend approximately 26 rotations in our Emergency Departments with the remainder of their rotations devoted to experiences in pediatric emergency medicine, emergency ultrasonography, anesthesia, obstetrics & gynecology, orthopedic surgery, adult & pediatric critical care, toxicology, burn care, and trauma. A variety of elective rotations and a research experience are available to allow residents to meet their individual needs. Weekly academic conferences with faculty and guest lectures, resident presentations, simulation, and board review sessions are an integral part of our educational curriculum. Both the didactic and clinical curriculums have been designed to foster an environment of academic inquiry, and challenge the residents to develop clinical, teaching, administrative and research skills. The residency works within a framework of evidence based medicine and graduated clinical responsibility. As the residents progress through the program, clinical responsibilities reflect their broadening knowledge and experience. Having established a high level of expectations for our residents, our graduates leave with the skills and confidence necessary to become leaders in the field of Emergency Medicine. Aria Health is committed to providing our residents with the best educational experience in a friendly community environment. We welcome residents to find out more about our program by contacting our Program Directors or Chief Residents and/or scheduling a visit to our program. 4. Osteopathic Combined Internal Medicine/Emergency Medicine The combined Emergency Medicine/Internal Medicine residency training prepares residents for board certification in both fields. The academic year consists of equally distribution of rotations in each field. The weekly academic conference of both programs is integrated into all rotational blocks. Participation in the weekly half-day ambulatory Internal Medicine Clinic occurs during the Internal Medicine rotational blocks each year. The skills and knowledge gained by this unique dual certification program enhances the resident's abilities in both fields. The broad base of internal medicine subspecialty knowledge acquired is complemented by the diverse medical, pediatric, and traumatic patient presentations that the resident will manage in the Emergency Department. Aria Health's Combined Emergency Medicine/Internal Medicine Residency Training Program is committed to providing our residents with the best educational opportunities in an academic community hospital setting. The unique patient care experiences found at each of the Aria Health facilities is combined with the access to the resources of the larger university hospitals in the area making our program special.
