Form And Line ReferencePart I, Line 2b, Column (V):FormAndLineReferenceDesc
ExplanationFunds submitted directly to Children's Miracle Network by sponsor partners are remitted to member hospitals on a quarterly basis by Children's Miracle Network. Funds remitted directly to member hospitals as a result of a Children's Miracle Network fundraising program are reported back to Children's Miracle Network for the purposes tracking fundraising results but those are deposited by the member hospital. Member hospitals pay an annual membership fee, which differs for each member hospital and depends largely on its market territory population. These fees are analogous to franchise fees, where hospitals "own" market territory in which they fundraise using the Children's Miracle Network brand. Children's Miracle Network furnishes fundraising materials that are essential to the campaigns. Such materials include the paper icons, collateral materials, and coin canisters. Materials are sent directly to locations of sponsor partners (such as an individual Costco warehouse). It is the responsibility of each hospital to pay for the cost of these materials. Materials costs are billed separate from the annual membership fee and are sent intermittently depending on the frequency of the campaigns.ExplanationTxt

Children's Hospital Los AngelesPart V, Section B, Line 5: The Community Health Needs Assessment was conducted by the Center for Nonprofit Management (CNM) and the Office of Community Affairs at Children's Hospital Los Angeles. Other institutions, organizations and agencies as well as members of the Children's Hospital Community Benefit Advisory Committee also contributed time and resources to assist with this assessment. The CNM team has extensive experience through being involved in and conducting more than 30 CHNA's for hospitals throughout Los Angeles County and San Diego County. In 2013, CNM conducted CHNA's for three Kaiser Foundation Hospitals (Baldwin Park, Los Angeles and West Los Angeles), Citrus Valley Health Partners, The Glendale Hospitals Collaborative (Glendale Adventist Medical Center, Glendale Memorial Hospital and US Verdugo Hills Hospital) and the Metro Hospitals Collaborative (California Hospital Medical Center, Good Samaritan Hospital and St. Vincent Medical Center) and assisted an additional two Kaiser Foundation Hospitals (Panorama City and San Diego) in community benefit planning based on the needs assessments. More recently, the CNM team conducted the 2014 CHNA for CASA Colina Hospital and Centers for Healthcare, and for Hope Street Family Center. The CNM team is currently in various stages of conducting 2016 CHNAs for two Kaiser Foundation Hospitals (West Los Angeles and Baldwin Park), Citrus Valley Health Partners, The Glendale Hospitals Collaborative and The Metro Hospitals Collaborative.CHLA has conducted a Community Health Needs Assessment (CHNA) in an effort to understand the health and social needs of the community and as required by state and federal law. The CHNA is a primary tool used by the Hospital to determine its community benefit plan. This assessment incorporates components of primary data collection and secondary data analysis that focus on the health and social needs of the service area. Sources of data include the U.S. Census 2010 Decennial Census and American Community Survey, California Health Interview Survey, California Department of Public Health, California Employment Development Department, Los Angeles County Health Survey, Los Angeles Homeless Services Authority, CDC National Health Statistics, National Cancer Institute, U.S. Department of Education, and others. When pertinent, these data sets are presented in the context of California State. The 2016 Community Health Needs Assessment methodology and process involved the collection of both secondary data and primary data. Approximately 300 secondary data indicators on a variety of health, social, economic, and environmental topics were collected by zip code, service planning area (spa), county, and state levels (as available). In addition, primary data collection included an online survey, a youth-led photovoice project and a community forum with individuals who represent the broad interests of the community served by the Hospital. Additionally, input was obtained from Los Angeles County Department of Public Health Officials. For the community forum, community stakeholders identified by key departments at Children's Hospital Los Angeles were contacted and asked to participate in the prioritization of needs process. Participants included individuals who are leaders and representatives of medically underserved, low-income, minority and chronic disease populations, or regional, state or local health or other departments or agencies that have current data or other information relevant to the health needs of the community served by the hospital facility.

Children's Hospital Los AngelesPart V, Section B, Line 11: As part of the Community Health Needs Assessment (CHNA), health and social needs were identified through an examination of primary and secondary data and then prioritized through a structured process using the relative worth method. This method is a ranking strategy where each participant in the process received a fixed number of points to rank each identified need. The priority areas that are being addressed are: access to care, health promotion and prevention, health and wellness and economic development. Other health and social needs that were identified, such as mental health, chronic disease conditions, community safety, and early childhood development, are already being addressed by other local and regional community organizations. The Hospital is dedicated to making a difference in the lives of children, adolescents and their families and is addressing the priority areas identified in the CHNA by providing programs and services that are family centered and community based.-- Access to Care - Children's Hospital Los Angeles is conducting community education and outreach regarding California's Health Insurance Exchange and the changes in health access programs. The Hospital is also expanding access to health care resources and information regarding transition and transfer of care for young adult patients living with chronic illness and disability.-- Health Promotion and Prevention - The Hospital is promoting healthy behaviors and prevention of disease through outreach and education at local schools, community events and expositions. CHLA has partnered with community organizations to coordinate child health and safety campaigns.-- Health and Wellness - Children's Hospital Los Angeles has collaborated with community clinics to promote pediatric health and wellness and make available information and resources to providers who care for families in underserved areas and collaborate with local community based organizations to work on initiatives that design and inform the development of interventions that address health inequities, particularly in underserved communities.-- Economic Development - The Hospital is working to advance current efforts to expand internships, mentorships and work experience opportunities for young adults and unemployed individuals and will collaborate with community and civic stakeholders to maximize economic opportunity strategies that address health disparities and the social determinants of health.

Part I, Line 7:The Hospital will charge the greater of the maximum amounts that can be charged to FAP-eligible individuals determined by the following methods: lowest negotiated commercial insurance rate, average of its three lowest negotiated commercial insurance rates, and Medicare rates.
Part II, Community Building Activities:The Hospital sponsors various community services to benefit the physically, mentally and genetically disabled as part of its charitable mission. These services include parental counseling, educational seminars, family support groups, and an outreach organization for families administered by the Hospital in an agency relationship and funded by the State of California. Additionally, a large number of health-related educational programs are provided for the benefit of the community, including health enhancements and wellness, telephone information services, and programs designed to improve the general standards of the health of the community.
Part III, Line 2:The total bad debt is equal to the total bad debt write offs less the bad debt recoveries.
Part III, Line 3:The Hospital recognizes patient service revenue on the basis of contractual rates for the services rendered for those patients who have third-party payor coverage. For uninsured patients who do not qualify for charity care, the Hospital recognizes revenue on the basis of its standard rates (or on the basis of discounted rates, if negotiated or provided by policy). Patients covered by insurance, but required to pay deductibles or copayments, are considered to be uninsured for those portions. Based on historical experience, the Hospital believes that a significant portion of its patient accounts will be uncollectible. Thus, it records a significant provision for bad debts related to patient accounts in the period the services are provided.Bad debts are NOT included as community benefits and contain -0- charity cost. Bad debts are calculated based on uncollectible accounts net of contractuals.
Part III, Line 4:Net patient service revenue is reported at the estimated net realizable amounts from patients, third-party payors, and others, including estimated retroactive adjustments under reimbursement agreements with third-party payors. Retroactive adjustments are estimated and accrued in the period in which the related services are rendered, and adjusted in future periods as final settlements are determined.Patient accounts receivable are recorded on an accrual basis at established billing rates. The allowances for contractual adjustments and doubtful accounts are recorded on an accrual basis, using historical experience and established billing rates. In evaluating the collectability of accounts receivable, the Hospital estimates the allowance for doubtful accounts by major payor type, based on historical experience for each type. The difference between the standard rates (or the discounted rates, if negotiated) and the amounts actually collected after all reasonable collection efforts have been exhausted is charged off against the allowance for doubtful accounts. Management regularly reviews data about these major payor sources of revenue in evaluating the sufficiency of the allowance for doubtful accounts.
Part III, Line 8:CHLA is not including any Medicare Shortfall as part of its Community Benefit. Further, CHLA uses Federal Medicare Cost allocation methodology, as reported in its Medicare Cost Report, to determine the allowable costs of care relating to Medicare payments received.
Part III, Line 9b:The Hospital's Policy states that if the patient/guarantor is unable to pay for services due to their current financial situation, they could be eligible for uncompensated care, in which case the Hospital's applicable policy and procedure is further referenced. If the patient/guarantor does not qualify, the Hospital follows all applicable federal and state guidelines for debt collection, including the requirement of fair treatment, the prohibition of making false statements and restrictions on the time of day that debt collectors may contact the debtor.
Part VI, Line 2:Children's Hospital Los Angeles recognizes the importance of knowing and understanding the key indicators of its community's health and the issues that affect patients, parents, and community organizations. The Community Health Needs Assessment was conducted by The Advancement Project Healthy City, Biel Consulting and the Office of Community Affairs at Children's Hospital Los Angeles. Other institutions, organizations and agencies as well as members of the Children's Hospital Community Benefit Advisory Committee also contributed time and resources to assist with this assessment. Conducting the Community Health Needs Assessment is one of the many ways that Children's Hospital Los Angeles strengthens its commitment to understanding the health needs of the communities it serves. Needs assessments are the primary tools used to determine a hospitals Community Benefit plan, that is, how the Hospital will address unmet community needs through the provision of community health services.Health needs were identified based on a review of the secondary data (indicators) and the primary data (survey). Each health need was confirmed by more than one indicator or data source (i.e., the health need was suggested by more than one source of secondary or primary data). In addition, the health needs were based on the size of the problem (number of people per 1,000, 10,000, or 100,000 persons); or the seriousness of the problem (impact at individual, family, and community levels). To determine size or seriousness of a problem, the health need indicators identified in the secondary data were measured against benchmark data, specifically California state rates or Healthy People 2020 objectives. Indicators related to the health needs that performed poorly against these benchmarks were considered to have met the size or seriousness criteria. Additionally, primary data sources were asked to identify community and health issues based on the perceived size or seriousness of a problem. On April 6, 2016, Children's Hospital Los Angeles convened a meeting that engaged Hospital leaders and community representatives to prioritize the identified health needs. Attendees were provided with an overview of CHNA process, presented with a list of the identified health needs and Data Indicator Scorecard (Appendix I), which summarized approximately 300 secondary data indicators on a variety of health, social, economic, and environmental topics by Service Planning Area (SPA), county, and state levels (as available). Attendees were allowed an opportunity to familiarize themselves with the data and review it before prioritizing the health needs via voting. Each attendee voted using ten sticker dots to indicate which health needs they believed most severely affect the community surrounding the KFH-West Los Angeles service area. The outcomes from the voting exercise in the prioritization meeting were put into a matrix along with other factors, including observed population disparities by ethnicity, age, gender, and geography through secondary or primary data.
Part VI, Line 3:The Office of Community Affairs at Children's Hospital Los Angeles helps families access available community resources, including low-cost health insurance coverage and health programs. California was the first state to pass legislation to establish a health insurance exchange under the federal health care reform law, the Patient Protection and Affordable Care Act (PPACA) commonly referred to as the ACA or Obamacare. Through CHLA's Health Insurance Assistance Program, part of the Office of Community Affairs, families find help getting access to Covered California resources and learn about other low-cost health insurance programs in Los Angeles County. To increase access to health care resources and provide awareness of Covered California benefits, the Program has partnered with a local collaborative to conduct outreach and education regarding federal, state, and local access programs. Certified Covered California Educators are available on site at Children's Hospital Los Angeles to assist families in obtaining health coverage through the exchange. In addition, the Program's goal is to expand access to healthcare resources and information regarding mental health, oral health care, early childhood development, and transition and transfer of care.
Part VI, Line 4:Our hospital services reach thousands across Southern California, with a primary service area of Los Angeles County--a region that spans 4,057 square miles and includes vast urban communities, suburban areas and rural neighborhoods. At the time of the 2010 Census, the population for Los Angeles County was 9,818,605. From 2010 to 2015, it is estimated the population grew 3.3% to 10,136,509. For 2015, the population in Los Angeles County represents just over one fourth of the population in all of California (26.1%). It is the most populous county in the nation and one of the most ethnically and radically diverse as well. Children (ages 0-11) represented 15.5% of the population in Los Angeles County, while adolescents (12-17) represented 7.9%. The most populous group by age was adults (18-64), which represented 64.5% of all residents in Los Angeles County. The majority population race/ethnicity in LA County is Hispanic or Latino (47.7%). Whites make up 27.8% of the population. Asians comprise 13.5% of the population, and African Americans are 8.3% of the population. Native Americans, Hawaiians, and other races combined total 2.7% of the population. In LA County, Spanish is spoken in 39.4% of homes; this is higher than the number of Spanish speaking households in the state (28.7%). In Los Angeles County, 43.2% of the residents speak English only, while 10.8% speak an Asian language. In Los Angeles, almost a quarter of the population lived at or below 100% of the FPL (24.1%), which is higher than California (22.3%). Poverty decreases for the population of Los Angeles County at or below 200% of the FPL with 13.3%, almost that of California (13.8%).The Hospital is physically located in the metro area of the city of Los Angeles. It is a regional, tertiary healthcare facility that serves patients throughout L.A. County and beyond and is the only Level 1 pediatric trauma center between San Francisco and San Diego.
Part VI, Line 5:At Children's Hospital Los Angeles, our commitment to patients and their families extends well beyond the walls of our Hospital. Our community benefit services and activities ensure we remain responsive to the needs of our community and build on our community network of care. Our community benefit investment helps make a difference in the lives of the thousands of children, adolescents and families we serve throughout the Los Angeles County region, as well as the thousands reached through our national and international efforts.Community benefit services and activities are designed to provide treatment and promote health as a response to identified community needs. Our objectives are to improve access to health care services, enhance public health of the community, advance medical or health care knowledge that provides public benefit, and relieve or reduce the burden of government or other community efforts. -- Mental Health Disparities in Transgender Youth Populations - In an editorial published in JAMA Pediatrics, Johanna Olson-Kennedy, MD, medical director of The Center for Transyouth Health and Development at CHLA discusses disparities in mental health in young transgender populations. A recent study, also published in JAMA Pediatrics, found over 40% of the nearly 300 young transgender women studied had at least one mental health or substance abuse issue. Over a third of the participants had major depressive episodes, and 20% had contemplated suicide at some point over the prior 30 days. While the new study confirmed findings from previous studies, what makes this study unique is how researchers went about sampling the transfeminine youth population. Instead of recruiting participants from transgender-specific healthcare sites, researchers reached out into the community to include those without access to transgender-specific healthcare. The need for more accessible and improved mental health care for transgender youth is clear.-- The Survivorship and Supportive Care Program - The Survivorship and Supportive Care Program was launched on March 2015 building on over 25 years of pioneering efforts to support the medical and psychosocial needs of children and adolescents with cancer and blood diseases. The Program's mission is to provide or facilitate clinical care and related research aimed at optimizing health-related quality of life across the continuum of survivorship for patients treated for cancer or serious blood diseases in the Children Center for Cancer and Blood Diseases (CCCBD).At CHLA we recognize that adolescents and young adults (AYA's) constitute a unique group that requires services that are specially tailored to their needs. AYA's have specific needs due to their age, and developmental life stage that are different than those of adults and younger children, which bring about specific challenges when facing cancer or a blood disease. Two specific areas are remarkably impacted during the cancer experience: social and peer interactions and social and psychological impact of body changes. Focus areas for our Teen and Family Support Service are addressing AYA's communication/information needs, early identification and triaging of psycho-social needs, creating opportunities for social interactions, enhancing coping skills, and supporting parents to best support assist their teens.-- CHLA Salud - engaging the "corazon" of the community - The Office of Community Affairs and Government and Public Policy secured an innovative workforce development grant from the Office of Statewide Health Planning and Development (OSHPD) and the California Endowment to advance the significance of the promotor(a) role (also known as community health workers) in the delivery of health promotion, education and prevention. Promotor(a)s are lay members of communities who work in association with a local health care system and usually share ethnicity, language, socioeconomic status, and life experiences or "Corazon" with the community members being served. (HRSA)CHLA Salud "Promotor(a)s" provided tailored outreach and education regarding hand hygiene and flu prevention in our local communities. In addition, "promotor(a)s" promoted efforts and initiatives to advance health knowledge and raise awareness of pediatric health and related safety and social issues in the community. The CHLA Salud Promotor(a)s reached over 10,000 families, children, and representatives from various agencies promoting CHLA infection control and wellness efforts in both English and Spanish. -- Healthy Ties - Children's Hospital Los Angeles (CHLA) is proud of its partnership with St. Mary's Academy's Health Careers Program, which links high school students with professional mentors at CHLA. The aim of the Program is to support St. Mary's Academy Health Careers Program students in the development of educational goals related to health careers. Program activities include mentorship, health career discussions, community engagement, and a population health project. The Program will allow students to explore careers in the health care industry, as well as reinforce the importance of giving back to the community. -- Pedestrian Safety Initiative - Heads up, phones down - The CHLA Injury Prevention Program has implemented the "Heads Up, Phones Down" Campaign to help raise awareness for distracted walking. Sidewalk stencils reading "Heads Up, Phones Down" have been installed at various building entrances, crosswalks, and other areas of high foot traffic to promote pedestrian safety of all CHLA faculty, staff, and families. Distracted walking includes texting, talking on the phone or listening to music. Studies have shown that people who text and walk are 60% more likely to veer off course. Experts estimate that one in ten pedestrian injuries are related to distracted walking. The Injury Prevention program is currently conducting observations of pedestrian behavior at multiple locations around the CHLA campus.

Community Benefit Report:Our HistoryFounded in 1901, Children's Hospital Los Angeles is one of the nation's leading children's hospitals and is acknowledged worldwide for its leadership in pediatric and adolescent health. Children's Hospital Los Angeles is one of only 11 children's hospitals in the nation, and the top-ranked in California, to be named to the prestigious U.S. News & World Report Honor Roll of children's hospitals for 2016-17.The Marion and John E. Anderson PavilionChildren's Hospital Los Angeles provides inpatient care for children in the Marion and John E. Anderson Pavilion. Named in honor of philanthropists Marion and John E. Anderson, this building was designed to further expand services as well as increase the ability to provide family-centered care for young patients. Opened in July 2011, this hospital building is allowing us to transform the practice of pediatric medicine, benefiting our patients, families and hospital caregivers for generations to come. AltaMed Community Health ClinicThe AltaMed Community Health Clinic of the Division of General Pediatrics at Children's Hospital Los Angeles serves as a preventive and primary medical care center for children and adolescents in our community. AltaMed is a Federally Qualified Health Center (FQHC), with emphasis on delivering high-level preventive and primary medical care, which makes this clinic an essential community partner in providing first-class medical care for families in underserved areas of our community, where the need is greatest. The Saban Research Institute The Saban Research Institute comprises basic, translational and clinical research at Children's Hospital Los Angeles - one of the few freestanding pediatric hospitals in the country where scientific inquiry is combined with clinical care devoted exclusively to children. Children's Hospital Los Angeles is also a premier teaching hospital and has been affiliated with Keck School of Medicine of the University of Southern California since 1932. The Institute's interdisciplinary research is organized around areas of focus that together fully explore the developmental origins of health and disease and address the most pressing national child health issues. Research is performed at the lab bench, in the clinic and in the community. Collaborating with other organizations, institutions and members from our community strengthens the Institute's ability to spearhead research innovation. The Next Generation of Industry Leaders - Camp CHLA Camp CHLA is a health careers exploration program at CHLA that provides high school students a behind-the-scenes look at the health care industry. In this five-day program, students have the opportunity to "job shadow" clinicians, observe surgeries, participate in skills labs and learn about the steps needed to become our future health care professionals. This program has been successful due to the collaborative efforts of the Office of Community Affairs at Children's Hospital and the Camp CHLA committee, a multidisciplinary advisory group of clinical experts and leaders at the hospital. Past campers who were seniors in high school or enrolled in college courses were eligible to apply to volunteer with the program and develop leadership opportunities as Camp CHLA counselors.CHAMP - Igniting a Passion for Healthcare AdministrationCHAMP is one of the Hospital's pipeline workforce development programs for students and early careerists seeking exposure in healthcare administration. Through CHAMP, interns participate in a series of career readiness workshops focused on, but not limited to, understanding hospital operations and infrastructure, finance, and maintaining a healthy work-life balance. All interns are then matched with one department within the Hospital, where they will gain practical experience through a project-based internship with an administrative healthcare professional. The internship will provide the opportunity to learn essential skills for entry-level employment, build significant professional relationships, and establish a defined career The program components reinforce the significance of administrative and leadership roles in healthcare. Following the month of workshops, "CHAMPers" complete a two month administrative internship in various departments at the Hospital. After completing the internship, CHAMP interns received job transitioning and placement services, coordinated by RightSourcing at Children's Hospital. The RN Residency JourneyFounded at Children's Hospital Los Angeles in 1999 and now replicated at more than 100 hospitals nationwide, the Versant RN Residency in Pediatrics combines classroom instruction with hands-on opportunities to help new nurses master the clinical skills they recently developed in nursing school. The RN Residency also provides ample psychosocial support and professional guidance to help new nurses successfully provide care to their young patients. The RN Residency in Pediatrics at Children's Hospital Los Angeles bridges the gap between the academic setting of nursing school and the real-life challenges of caring for acutely ill children by providing new nurses with supervised clinical experience, classroom and skills lab instruction, personalized mentoring, and psychosocial support. In addition, the RN Residents also complete an evidence-based practice project during the course of the residency program. This valuable experience is producing new nurses who are equipped to lead the organization in identification of innovative research-based nursing interventions to better care for their patientsAdditional information on the organization's programs is provided in the Community Benefits Report (which is available upon request).

Form 990, Part VI, Section B, line 11The Form 990 is prepared by Deloitte Tax LLP, working in conjunction with CHLA's finance department. CHLA's Director of Accounting has direct responsibility for this effort, subject to supervision by the Chief Financial Officer. After an initial draft of the Form 990 is prepared, it is circulated for review and comment by relevant members of the executive team who have responsibility and/or knowledge about the various matters disclosed and/or described in the Form. The Chief Financial Officer and General Counsel, in particular, review the Form 990 and ensure accuracy of descriptions and that disclosure is complete. Additionally, the Legal Department of CHLA reviews contractual relationships entered into by CHLA. Furthermore, Board members and officers are asked to disclose any potential conflicts of interest on a continuous basis throughout the year. If a potential conflict exists, the board or committee determines whether the board member or officer should be excluded from voting on that particular matter.
Form 990, Part VI, Section B, line 15The process for determining the compensation of the Chief Executive Officer of CHLA is conducted by the Compensation Committee of the Board of Trustees of CHLA, acting on behalf of the Board of Trustees of the Organization. Their reports were based on a review of the executive compensation practices of a variety of hospitals and healthcare systems that are considered comparable to CHLA based on various metrics such as hospital type and revenue. The Compensation Committee deliberated on the issue of the Chief Executive Officer's compensation package in light of these reports and questions were asked of, and answered by, SullivanCotter regarding such reports and other relevant matters. Based on such deliberations, the Compensation Committee negotiated a written contract with the CEO. SullivanCotter's report was based on a review of executive compensation practices of a variety of hospitals and healthcare systems that are considered comparable to CHLA based on various metrics such as hospital type and revenue. In addition, the Chief Executive Officer made a recommendation to the Compensation Committee with respect to each of such employees' compensation package in light of the SullivanCotter report and in light of the executive's performance.
