ExplanationSCHEDULE H, PART V, SECTION B, LINE 5 The broad interests of the community were reflected quantitatively by the distribution and promotion of a regional survey called the Long Island Health Assessment Survey, which was available online and in paper format. Distribution and promotion occurred throughout a wide range of social service locations including hospital websites and educational programs, social media outlets, doctors offices, health departments, libraries, schools, insurance enrollment sites, community-based organizations, and more. The interests of the community are also reflected qualitatively, through a facilitated summit of community-based organizations (CBOs) that occurred on February 2, 2016. Representatives from CBOs shared valuable information about the health issues faced by their clients, barriers to care and other issues recognized as the social determinants of health -- citing poverty, housing, job losses, and food insecurities as problematic. SCHEDULE H, PART V, SECTION B, LINE 11 Winthrop ranked priorities based on the burden, scope and urgency of the health need across the service area, health disparities in select communities, resources, and the importance the community placed on addressing the needs. As stated in the 2016 CHNA, among the most identified pressing needs of the community that will be targeted include: 1. Prevention and management of chronic disease 2. Reduce obesity in children and adults 3. Cancer Prevention & Screening 4. Healthy & Safe Environment (Fall Prevention) These will be addressed through educational programs, screenings, and various community outreach activities. Plan revisions, resource limitations and re-prioritization of goals may impact individual programs. Two additional identified needs include 1) Mental Health and Substance Abuse and 2) women & childrens Health. Due to limited resources, Winthrop will not address these two initiatives formally, however, please see the following: Mental Health Winthrop has expanded its psychiatric division and maintains a strong referral system. We are in the stages of investigating Mental Health stress management initiatives that will be introduced to the community at Hospital-based educational programs. Women & Childrens Health Issues such as pediatric asthma and low birth weight babies in underserved communities will continue to be addressed, but not formally. SCHEDULE H, PART V, SECTION B, LINE 13B Winthrop extends discounts for financial assistance to patients based upon FPG levels for family size and household income up to 500% of the FPG. ExplanationWinthrop-University Hospital uses FPG to determine eligibility for free and discounted care. Part I, Line 7 The Hospital utilized a cost-to-charge ratio derived from Worksheet 2, Ratio of Patient Care Cost-to-Charges, of Form 990. Part I, Line 7g There are no subsidized health service costs attributable to physician clinics in the amount reported. Part I, Line 7, Column (f) A total of $33,262,248 was removed from the total expenses used in calculating the percentages of community benefit. Part II Winthrop-University Hospital provides Community Support through the Adopt-A-Family and the Meals on Wheels programs. Advocacy for Community Health Improvement/Safety is provided through the Brooklyn-Queens-Long Island Area Health Education Centers Summer Internship Program which grants healthcare shadowing opportunities for high school and college students from minority communities. Part III, Line 2 The total bad debt expense shown on this line represents actual amounts written off during the year related to balances due on patients accounts for which payment was expected and pursued by the Hospital and/or it collection agencies but never received. Part III, Line 3 The Hospital utilizes a process to identify amounts that are in a bad debt status but could qualify for financial assistance under the Hospitals financial assistance policy. These accounts are removed from bad debt and reviewed. If the criteria for financial assistance are met then they are reclassified from bad debt. These amounts are included in Part I of Schedule H. Part III, Line 4 Per page 11 of the Audited Financial Statements, the Hospital presents the provision for bad debt expense, net totaling $33,262,248 and $29,101,831 for the year ended December 31, 2016 and 2015, respectively, as a reduction of net patient service revenue in the consolidated statement of activities. For the Form 990 bad debt expense has been reclassified from revenues to expenses. Part III, Line 8 Medicare cost and charge information was extracted from the Medicare cost report and an RCC methodology was applied. The shortfall represents Unreimbursed costs incurred in providing services to Medicare recipients and therefore should be treated as a community benefit. Part III, Line 9b Patients identified as being eligible for consideration of financial assistance are referred by the debt collector to the Credit and Collection Department of the Hospital for review; Debt collection activities are suspended. 2) Needs Assessment Community needs are based on assessments conducted by Federal, State, and County agencies as well as studies performed by the Hospital and community organizations. 3) Patient Education of Eligibility for Assistance Notification is provided on the Hospitals website, through signage posted at all access points, through notification on hospital bills, and through counseling during the intake and discharge processes. 4) Community Information Winthrop University Hospital serves primarily Nassau County, which is located on Long Island in New York. Nassau County is home to 1.3 million people and makes up 6.9% of the State of New Yorks total population. Nassau is comprised of, 60.8% of White persons (not Hispanic), 16.8% persons of Hispanic or Latino origin, 12.8% Black persons, and 9.7% Asian persons based on census data. 5) Promotion of Community Health The Board of Directors of Winthrop-University Hospital is comprised of community members who are not compensated for their duties. Winthrop extends medical staff privileges to all qualified physicians in its service area. The hospital provides an array of educational seminars and preventive health services to the community including a smoking cessation program, diabetes education, health fairs, free blood pressure screenings, fall prevention programs, cancer care programs, prostate screenings, genetic counseling, health updates for seniors, Meals on Wheels, and other programs. Surpluses are reinvested into Winthrops operations, research, medical education and capital improvement initiatives to enhance its abilities to provide quality health care to its service areas. 6) Affiliated health care system The Winthrop South Nassau University Health System is the sole member of Winthrop-University Hospital and South Nassau Communities Hospital. Both hospitals provide services to their respective communities and collaborate to ensure that their communities are provided a full array of inpatient and outpatient services of the highest quality. 7) State filing of Community Benefit Report New YorkExplanationTxt

ExplanationLINE 4B THE ORGANIZATION MAINTAINS A SUPPLEMENTAL EXECUTIVE RETIREMENT PLAN (SERP) FOR THE SENIOR AND LONG-TERM EXECUTIVES OF THE ORGANIZATION, INCLUDING MR. COLLINS. THIS PLAN SEEKS TO PROVIDE A REPLACEMENT INCOME AT RETIREMENT EQUAL TO 60% OF THE EMPLOYEE'S BASE SALARY, WHEN CONSIDERING ALL ORGANIZATION-SPONSORED RETIREMENT INCOME SOURCES, AFTER 20 YEARS OF SERVICE. THE BENEFITS UNDER THIS PLAN ARE FORFEITABLE IN THE EVENT OF VOLUNTARY TERMONATION BEFORE RETIREMENT AGE. THE ORGANIZATION'S OUTSIDE COMPENSATION CONSULTANT HAS OPINED THAT THIS PLAN IS WITHIN MARKET NORMS. WINTHROP ADOPTED A SUPPLEMENTAL EXECUTIVE RETIREMENT PLAN ("SERP II") FOR 3 EXECUTIVES WHO WERE NOT ELIGIBLE UNDER THE SERP DESCRIBED IN THE PREVIOUS PARAGRAPH BUT WHO WERE NEGATIVELY IMPACTED BY LIMITS UNDER WINTHROP'S STANDARD RETIREMENT PLANS (LIMITS NOT APPLICABLE TO EMPLOYEES GENERALLY). TWO OF THE 3 EXECUTIVES ARE INCLUDED IN PART VII AND SCHEDULE J. THE PRIMARY PURPOSES OF SERP II ARE TO PROVIDE A SUPPLEMENT TO THE STANDARD RETIREMENT PLAN AND TO ENCOURAGE RETENTION. SERP II COVERS MS. CATALIOTTI AND MS. PFEFFER AND THE 2016 CONTRIBUTION AMOUNTS FOR THEM WERE $63,678 AND $77,312, RESPECTIVELY. SERP II IS FORFEITABLE BY THE EXECUTIVES IF THEY TERMINATE EMPLOYMENT VOLUNTARILY BEFORE THE RETIREMENT DATE SET FORTH IN THE PLAN (GENERALLY AGE 65 OR 20 YEARS OF SERVICE) (AND ADDITIONAL SERVICE REQUIREMENTS MAY APPLY). PART 1, LINE 7 THE ORGANIZATION MAINTAINS AN INCENTIVE COMPENSATION PROGRAM THAT PAYS VARIABLE COMPENSATION BASED ON THE SATISFACTION BY THE ORGANIZATION AND THE EXECUTIVE, IF AND TO THE EXTENT SATISFIED, OF PRE-ESTABLISHED QUALITY OF CARE, PROGRAM, FINANCIAL AND SIMILAR GOALS ESTABLISHED AT THE BEGINNING OF THE YEAR PRIOR TO PAYMENT. THE INCENTIVE COMPENSATION PROGRAM AWARDS ARE MADE BY THE COMPENSATION COMMITTEE IN ITS DISCRETION.ExplanationTxt

MISSION STATEMENTMISSION STATEMENT FORM 990, PART III, LINE 1 It is the mission of Winthrop-University Hospital (referred to as "Winthrop"the Hospital")to provide high-quality, safe, culturally competent, and comprehensive healthcare services in a teaching and research environment, which improve the health and well-being of the residents of Nassau County and contiguous county areas...based on a profound commitment to an enduring guiding principle - "Your Health Means Everything."The Hospitals Community Service Plan is guided by and reflects its mission statement to improve the health and well-being of the residents it serves.
COMMUNITY PARTNERSHIPSWinthrop remains engaged with the community by maintaining relationships with community- and faith-based organizations, private organizations, local school districts, and legislators from the village, town, county, state and federal government. Our partnerships have been strengthened through quarterly Community Cultural Advisory Committee meetings addressing the needs of the underserved, as well as increased efforts to reach out to the community to achieve goals and objectives identified through the 2013 Community Needs Assessment. On a broader scale, Winthrop is also an active member of the Long Island Health Collaborative, a coalition that includes the Nassau and Suffolk County health departments, all Long Island hospitals, health and social service community-based organizations, health plans, academic institutions, local government, and other groups that are working together toward a healthier Long Island. By using the collective impact model to enhance the quality of work being pursued in Population Health efforts, it is hoped that public awareness and education will help motivate individuals to make the changes necessary to develop a healthy lifestyle and improve outcomes. Leading a healthy lifestyle (e.g., avoiding tobacco use, being physically active and eating well) reduces the risk for chronic disease.
HEALTH INITIATIVES SUPPORTING THE NYS PREVENTION AGENDAPREVENT CHRONIC DISEASE NATIONAL DIABETES PREVENTION PROGRAM Winthrop offered the National Diabetes Prevention Program at no cost at the Diabetes Education Center. It is important to note that approximately 28% of participants were from communities with health disparities. Participation involves a commitment for one year and includes classes held weekly during the first six months. It then transitions to every other week and then monthly during the second six months of the year. Six new diabetes prevention classes were started in 2016 a total of 242 individuals participated in the Diabetes Prevention Program in 2016. TAI CHI FOR ARTHRITIS Targeting Chronic Disease, Stress Management & Fall Prevention Based on health trends and opportunities for training through a grant, Winthrop reviewed the above evidenced-based program at the end of 2015 and decided to pilot it in the community. Medical studies have shown that Tai Chi relieves pain and improves the quality of life for people living with arthritis, though participants do not need to have arthritis to participate. This program: Includes flexibility exercises which reduce pain and stiffness leading to increased mobility. Helps improve muscular strength (reduces pain and protects joints). Helps improve relaxation, balance, posture, and immunity. Winthrop implemented the free eight-week, twice a week program in January of 2016. It has been extremely successful. Since then, six programs have been offered four at the Welcome Center and two at the Yes We Can Community Center in Westbury, a community with health disparities. Approximately 200 people have participated. Evaluation of Impact The response to the program has been tremendous, with community members calling and requesting classes. Anonymous post-evaluation forms are collected as part of the program and submitted to QTAC-NY for evaluation. In addition, Winthrop distributes a post-satisfaction form to individuals who return for the refresher portion of the workshop. This form collects information regarding the impact the program has on participants over a longer period. CHILDHOOD OBESITY SCREENINGS IN THE COMMUNITY Winthrop collaborates with the Head Start communities located in Hempstead and Westbury to bring the 5-2-1-0 healthy lifestyle program to their families. The plan includes giving each child a packet of information about the program to share with their parents, documenting BMIs and providing nutritional counseling for families of children identified as having an unhealthy weight. The healthy lifestyle program was introduced at Head Start parent meetings, and encourages 5 servings of fruits and vegetables each day, no more than 2 hours of screen time, 1 hour of exercise and 0 sugary drinks. The program was implemented in 2016. Each child received a packet of information about the program, including printed forms where they track their food and activity level by coloring in the appropriate boxes. During 2016, 238 children were measured in Hempstead and 16.5% were in the high range; 18.9% in the very high range. In Westbury, 212 children were measured; 15.5% of children were in the high range and 19.3% were in the very high range. Evaluation of Impact This is a new program. Process measures involve the number of children weighed and sent home with educational packets. Administrators at both Hempstead and Westbury locations have praised the partnership and would like us to expand to other Head Start locations. More parental buy-in is needed. Although we offered nutritional counseling in the form of a support group, parents did not respond. Going forward, we hope to attend parent meetings during 2017 to explain the program more fully and encourage participation. PROMOTE EXCLUSIVE BREASTFEEDING Since breastfed infants are less likely to develop medical problems such as childhood obesity, respiratory and gastrointestinal infections and are at lower risk for childhood cancers, asthma and Sudden Infant Death Syndrome (SIDS), Winthrop developed a strategy in 2013 to achieve Baby-Friendly Designation and improve the percentage of newborns who only receive breast milk when discharged from the Hospital. Evaluation of Impact Winthrop was awarded Baby-Friendly Designation by Baby-Friendly, USA, in 2014. NYS Prevention Agenda Goal is 48.3% of women exclusively breastfeed upon hospital discharge. During 2016, 50% of babies were only receiving breast milk when discharged from Winthrop. In addition to the above, Winthrop offers a breastfeeding support group, led by a lactation consultant. Over 110 women attended the support group during 2016.
FALL PREVENTION PROGRAMWinthrops robust Fall Prevention Program for seniors includes a beginner four-part fall prevention workshop, followed by exercise classes to improve balance, flexibility and strength. During 2016, a total of 21 beginner workshops were conducted, with 18 at the Winthrop Welcome Center and three out in the community at the Yes We Can Community Center in Westbury, a community with health disparities. Two hundred and fifty-four attendees completed the four-week sessions. A total of 95 follow-up exercise classes were offered at the Welcome Center and 35 classes were offered out in the community at the Yes We Can Center in Westbury. In addition to the four-part workshop and follow-up exercise classes, Winthrop added a balance assessment to the program. Evaluation of Impact Participants fill in post-satisfaction forms. They acknowledge that the classes have given them more confidence, make them more aware of their posture, have made modifications at home, see the eye doctor once a year, and review their medications. Feedback from the workshops inspired Winthrop to develop the exercise classes as a reinforcement to improve balance, flexibility and strength. Since their inception, approximately 140 follow-up exercise classes have been held. People noticed that the program helped create an awareness of what to do to prevent a fall.
TOBACCO CESSATIONThe Tobacco Cessation Program is an interactive, four-session workshop offered over a four-week period; the program is free-of-charge and conducted by a registered nurse. A total of 8 four-week workshops were conducted, with a total of 49 people attending three out of the four workshops. Attendees were contacted via e-mail or phone calls periodically throughout the year. Of the 49 contacted, 17 responded; 11 out of the 17 have stopped smoking.Six have decreased the number of cigarettes smoked daily and two out of six have enrolled to attend the classes again. One support group was held with a total of seven attendees. Of the seven attending, four were no longer smoking.
IMPROVING ACCESS TO CAREWOMENS HEALTH Winthrops Womens Wellness Center in Hempstead addresses the health disparities of this select community. They offer a wide variety of obstetrical and gynecological services, as well as on-site risk assessment, smoking cessation support, genetic counseling, nutritional counseling along with gestational diabetes education. Additional services include: psychosocial counseling, depression screening at each visit, postpartum depression screening and referrals, bereavement counseling, education on domestic relationship abuse/sexual abuse, assessment of living conditions and cultural diversity. To meet the needs of the community, the site features a Medicaid Enrollment Specialist for uninsured patients; Charity Care Application and self-pay rates are available as well. They participate with NYS Medicaid and most Medicaid Managed Care HMO plans. Evaluation of Impact - During 2016, the Center averaged 2200 visits per month with a total of 26,000 encounters a 47% increase from 2015. Financial services department met with over 375 patients at the Center during this time period; this does not include those already enrolled in Medicaid services. There are more patients insured now; no appointment is needed for Medicaid enrollment. CHILDRENS HEALTH SERVICES The Hempstead High School Health Center (HHHC) is a school-based Health Center in a community with health disparities. During 2016, there were 3,078 visits to the Center by approximately 1,500 students. Comprehensive preventive, episodic and confidential healthcare was provided. Winthrop includes specific programs needed by the schools population, including: Youth support groups in English and Spanish that address common adolescent issues such as acculturation, adolescent adjustment, alternative lifestyles, teen pregnancy and prevention, parenting teens, and violence prevention, including gang violence. An Asthma Management Program that monitors and tracks students with asthma. It includes quarterly asthma workshops. This effort is particularly significant in Hempstead, where asthma rates are nearly double those of adjacent neighborhoods. Both staff nurses in the HHHC are certified asthma educators. Collaboration with primary care physicians in the community facilitates treatment that meets current medical standards, trains high school nurses and staff about asthma, offers access to medications, and assists with obtaining health insurance for uninsured students. There is a noticeable difference among students with asthma, in that fewer students are presenting at the clinic with difficulties. An Oral Health Program that assesses dental needs and applies fluoride varnish. It also provides dental supplies: toothbrushes, paste, floss and referral to local dental providers. Comprehensive preventive, episodic and confidential healthcare was provided. The Winthrop Hempstead Pediatric Clinic, a Patient-Centered Medical Home since 2014, draws patients primarily from the Hempstead community but also serves children, from newborn to age 21, from the surrounding neighborhoods experiencing health disparities (Elmont, Freeport, Uniondale and Roosevelt). In 2016, a total of 19,009 patients were treated at the facility, an increase of 8.5% from 2015. In addition, Winthrops Pediatric Specialty Center in Mineola sends a Pediatric Pulmonologist and a Genetic Counselor to the Hempstead location on a regular basis to allow for easier access to these services. The practice focuses on four important conditions: obesity, asthma, 18 month well-check and the first month of life. To reduce obesity, the Clinic utilizes the 5-2-1-0 program as a way of teaching parents and children a healthy lifestyle. The objective is to teach children the basics of a healthy lifestyle; the intended goal is to achieve a long-term positive impact on their health. Providers are encouraged to consistently document Body Mass Index (BMI), provide lifestyle counseling, and develop individual care plans and follow-up measures. During 2016, counseling documentation rate was 90%. Statistics and quality monthly meetings review the impact of asthma education on the patient population.
