Schedule D, Part V, Line 4 Intended uses of endowment fundsTo fund the treatment and prevention of blindness in the state of Louisiana.
Schedule D, Part X, Line 2 FIN 48 (ASC 740) footnoteThe Foundation has received a favorable determination letter from the Internal Revenue Service, stating that it is exempt from federal income taxes under the provisions of Section 501(c)(3) of the Internal Revenue Code (IRC) of 1986, as amended, except for income taxes pertaining to unrelated business income. The Financial Accounting Standards Board issued guidance that requires tax effects from uncertain tax positions to be recognized in the financial statements only if the position is more likely than not to be sustained if the position were to be challenged by a taxing authority. Management has determined that there are no material uncertain positions that require recognition in the financial statements. Additionally, no provision for income taxes is reflected in these financial statements, as the Foundation's unrelated business taxable income is expected to be offset by net operating losses carried forward from prior years. There is no interest or penalties recognized in the financial statements.
Schedule D, Part XII, Line 2(d) Other expenses in audited financial statements not in form 990RECOVERIES OF PRIOR YEAR GRANTS - NETTED AGAINST GRANT EXPENSE ON AUDITED FINANCIAL STATEMENTS - -3458108

Form And Line ReferenceSchedule I, Part I, Line 2 Procedures for monitoring use of grant funds.FormAndLineReferenceDesc
ExplanationGrant awards are subject to board approval and recipients must meet requirements necessary to receive said grant. LCIF staff monitors the requirements on a regular basis to ensure all awards are proper. After grants are disbursed in part or in full, the recipient must provide an interim or final report, which includes financial documentation of the use of grant funds and local matching funds and a narrative of the actions taken in developing the project. The interim or final report is initially reviewed and approved by the grant administrator. The report is then reviewed and approved by the district cabinet (or multiple district council) before submission to LCIF.ExplanationTxt
