Schedule H, Part V, Section B, Line 5-Craig HospitalCommunity partners and individuals with special expertise in public health provided comparative data on our patient population. Health Data Analyst from the Health Surveys and Analysis Program of the Colorado Department of Public Health and Environment provided data on the Behavioral Risk Factor Surveillance System survey. Policy Analyst from the Colorado Health Institute provided data on the Colorado Health Access Survey.
Schedule H, Part V, Section B, Line 11-Craig HospitalThe CHNA identified several needs that are being addressed as follows: 1. Develop a decision tree to help patients find transportation resources in their community through public, private, or non-profit organization. Continue development of resources to help disabled persons access medical services in their community through the Hospital's Nurse Advice Line and the Hospital website. The Nurse Advise Line staff is experienced in helping callers find a doctor in their community and guide patients in how to get an appointment with their PCP when needed. 2. Continue using social media to raise awareness about the importance of receiving a flu shot each year. We will also use social media to promote resources available on our website such as the list of dentists who provide affordable care and have accessible offices for disabled persons. The video we created addressing how patients can be assertive when advocating for their needs in the community will be available in shorter segments on the Hospital website. 3. Pilot a transition program for inpatients discharged into the community. Hospital staff to accompany patients home and help establish community resources for ongoing health needs including transportation and PCP identification. Over the last fiscal year the hospital did a number of things to improve our patients' health after discharging from Craig and re-entering their community. We vaccinated more than 95% of our inpatients against influenza during flu season and offered the vaccine to our outpatients as well. The hospital implemented a soft-landing pilot program to better support some of our most complicated discharges. Craig staff go home with the patient and their family whether they live in Colorado or out of state for a couple of days to help the patient settle into their community again and make sure they are set for success. Another initiative the hospital implemented was our patient portal. The portal allows patients to access their health information electronically after going home. This can greatly assist their primary care physician as they begin caring for our complex patients.
Schedule H, Part V, Section B, Line 13h-Craig HospitalSize of household.
Schedule H, Part V, Section B, Line 16j-Craig HospitalThe financial assistance application is sent to patient with pre-collection letter. Patient billing statements also reference the policy and contact information is given if they require additional information or want to apply.
Schedule H, Part V, Section B, Line 20e-Craig HospitalSent a final notice letter and a Financial Assistance Application to patient.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Schedule H, Part I, Line 7Costs were determined by using either actual costs or calculated costs based off a cost-to-charge ratio.
Schedule H, Part III, Section A, Line 2Part III Line 2: The Hospital continued to follow the presentation and disclosure of patient service revenue, provision for bad debts and allowance for doubtful accounts as pronounced in the accounting standards updates 201107 and accounting standards codification topic 954. This requires organizations that do not have access to the collectability of a receivable prior to recognizing revenue to present their provision for bad debt related to a patient service revenue as a deduction from revenue, net of any contractual allowances and discounts on their statement of operations. The adoption of this did not impact the financial statements as the Hospital does not recognize significant revenue without assessing a patient's ability to pay.
Schedule H, Part III, Section A, Line 3The Hospital does not consider any of its bad debt expense to be attributable to patients eligible under the organization's financial assistance policy.
Schedule H, Part III, Section A, Line 4Part III Line 4: There is not a footnote in the audited financial statements that describes bad debt expense. Please refer to page 9 of the Audited Financial Statements that addresses Patient Accounts Receivable.
Schedule H, Part III, Section B, Line 8Part III, Line 8: The source of the data is the most recent filed Medicare Cost Report using the ratio of cost to charges method. The payment mechanism received for care provided to Medicare patients is not negotiated but set by federal regulations. The payment currently received for treatment is not sufficient to cover costs and although we don't recognize this shortfall as charity, we believe it should be included in the calculation of community benefit. All patients receive the appropriate level of care and services regardless of payment source.
Schedule H, Part III, Section C, Line 9aThere is not a specific written debt collection policy as those procedures are addressed in the Financial Assistance Policy.
Schedule H, Part VI, Line 2It addition to conducting its CHNA every 3 years, the Hospital assesses community needs through a variety of different programs. As a condition of federal funding, Craig Hospital's research department conducts ongoing longitudinal studies on the long-term effects and needs of persons with spinal cord injury and brain injury. Craig Hospital also operates a toll-free Nurse Advise Line and collects data on why former patients are calling for assistance. Craig Hospital also collaborates with the Brain Injury Alliance of Colorado , Operation TBI Freedom, The Hangout, the Christopher & Dana Reeve Peer Network Program and other community organizations to assess and meet the long term needs of citizens with disabilities. Craig also launched the Peer Mentor Soft Landing Program-a 3 year pilot program aimed at improving the success of patients as they are discharged home and gaining new insight on the challenges these patients face.
Schedule H, Part VI, Line 3The Admissions Department identifies patients who have limited benefits and compare the benefits to the estimate length of stay. When there is a shortage of days allowed by insurance, the Admissions Directors meets with the patient and/or family to determine alternative coverage. This conversation encompasses the patient's ability to pay for the care, potential eligibility for Federal and State programs and the Hospital's Charity Care program. Upon admission, the case manager complete a thorough Psycho-Social Assessment of the patient/family specifically analyzing their financial situation. The case managers consider household income post-injury, family size, assets, current and future living expenses, and financial obligations including expected future care needs. The case managers, along with the patient/family, assess other potential resources/programs for which the patient may qualify. Resources and programs evaluated included but are not limited to Medicaid, Victim's Assistance, Liability and Liquidity, Accidental Death and Dismemberment, and the Hospital's Charity Care program. If it appears as though the patient will qualify for State or Federal Programs, the case manager will assist the patient/family in completing the applications, obtaining the required documentation and advising them on how to position themselves to qualify. If patients do not meet the criteria for other State or Federal programs, the Financial Assistance Eligibility Discount Guideline is utilized to determine if the patient's bill will be discounted. If appropriate, the case manager will initiate and assist the patient/family in completing the Financial Assistance Application for the Charity Care Program. Applications may also be obtained on the Craig website, completed over the phone or by mail. If it is not clear that the patient/family will have a balance prior to discharge (co-pays, out of pocket etc) the patient/family will receive information on the Charity Care Program which is noted on the monthly billing statements and the copy of application along with letter for pre-collection. At this time the patient can complete the application. All applications are reviewed by the A/R Manager, VP of Finance and/or the Controller. When it is determined that the charity application is appropriate, this is communicated to the patient/family by the case manger or A/R Manager.
Schedule H, Part VI, Line 4As one of the most recognized specialty rehabilitation and research hospitals in the world for patients with spinal cord and brain injury, Craig Hospital contributes to the local, state, and national communities in a myriad of ways in the areas of rehabilitation, education, disability awareness training, handicap accessibility, and other forms of support. During FY 2017, the Hospital provided financial assistance, community support and subsidized health services from government programs totaling $7.9 million. As a non-profit free standing hospital, Craig Hospital has 1,132 full-time and part time employees and utilizes 225 active volunteers from the community. Craig Hospital is the largest medical/rehabilitation hospital in Colorado exclusively serving patients with spinal cord injury (SCI) and traumatic brain injury (TBI) and serves the largest percentage of the 150 newly injured SCI and 5,000 TBI patients per year in Colorado, with inpatient, outpatient, and long term follow-up services. There are approximately 2,400 people with SCI and 175,000 with TBI living in Colorado. Approximately 50% of Craig Hospital's patients are Colorado residents while the remaining come from virtually every other state, and even some from foreign countries. The average age of our patients is 42 years old, 75% are male, and all come from varied socio-economic backgrounds. Craig Hospital provides significant support to the community, supporting the Brain Injury Alliance of Colorado, conducting annual outpatient clinics in Pueblo and Grand Junction, Colorado and Casper, Wyoming, serving on multiple boards for state agencies and organizations (TBI Mindsource Program, Division of Workers Compensation TBI Medical Treatment Guidelines, Colorado Brain Injury Professionals Network, the Hangout Network and others), and financially supporting more than 100 local, regional, and national organizations each year. Craig Hospital collaborates on patient research and technology with several local colleges and universities. In FY 2016, Craig took over the operation of Operation TBI Freedom (OTF), a military case management program located in Colorado Springs, Co. that provides support for veterans and active duty personnel with TBI. OTF provides case management services including crisis management, medical and mental health referrals, employment/education assistance, psychosocial classes, and emergency assistance for severe needs. There are an estimated 339,000 U.S. service members who have been diagnosed with TBI since 2000 of which approximately 7,200 have returned to Colorado. Of that amount, OTF has provided services to more than 1,300 service members. During the past five years, Craig Hospital has served nearly 2,500 inpatients with catastrophic spinal cord and brain injuries, of which approximately one half were from Colorado. In the same time period, Craig Hospital served more than 11,000 outpatients, of which three quarters were from Colorado. These numbers do not include patient support systems and community members, families, friends, co-workers, and employers. Craig Hospital's patient outcomes are superior in comparison to virtually every regional and national average in the most important measures of rehabilitation outcomes. On average, Craig graduates are more dependent when they are admitted, functionally make greater gains during their initial treatment, are discharged home at higher rates, return to work or school in greater numbers, are more functionally independent and require less attendant care, have fewer re-hospitalizations, have higher levels of community reintegration and satisfaction with life, and have higher earning power than their national peer group treated in other rehabilitation programs. Our exceptional outcomes validate that rehabilitation is an investment in long-term societal value. When we help patients become as independent and productive as possible, it is in their best interest as well as in the best interest of their family, insurance carriers, taxpayers, and to the community as a whole. Craig has received the prestigious Magnet Recognition for quality in nursing for 3 consecutive 5 year periods covering 2005 through 2020 and the Magnet Prize Award in 2012. Craig has also been ranked as a top ten rehabilitation center by U.S News and World Report for 28 consecutive years. Each year, Craig Hospital staff and physicians receive national recognition and in 2017, Craig Hospital staff and physicians won 5 major awards. Craig Hospital is pioneering adaptive technology and cutting edge treatment and in 2011, Craig Hospital founded the Peak Center at Craig Hospital, a state-of-the art wellness and fitness program for inpatients, outpatients, and community memberships. Additionally in 2017, Craig launched a pilot for the Soft Landing Program which helped assess patient needs and challenges when patients arrived home after discharge. Other programs that Craig provides to the community include the RSVP Free Clinic that is held the first Sunday of each month that offers rehabilitative care to the underinsured; Outreach Clinics that are held annually in 3 regional cities providing outpatient reevaluations; Therapeutic Recreation adaptive adventure trips and activities; community blood drives; community liaisons who travel to assess and educate patients and providers; 30 days free lodging for all family members plus free meals and additional lodging for those that cannot afford that expense; hosting student rotations for RN and CNS students from 8 local schools; hosting two Denver Public School Career expos for students interested in a career in healthcare; hosted six community outreach educational presentations to local hospital and colleges on treatment of SCI and TBI; Residents Program for2 medical school residents and 1 fellow.
