Part v, Line 5In 2016, NewYork-Presbyterian Hudson Valley Hospital worked as a member of the Westchester County Health and Hospital Planning Team (WCHHPT) under the guidance of the Westchester County Department of Health to plan and execute a county-wide Community Health Needs Assessment survey, and to collaborate on the identification of priority areas and planned health interventions for 2016-2018. Members of the WCHHPT included the Department of Health and representatives from 12 hospitals and healthcare providers who met seven times throughout the year. As a result of this collaboration, survey results, and internal planning based on our hospitals strengths, resources and local community needs, NewYork-Presbyterian Hudson Valley Hospital developed its 2016-2018 Community Service Plan. In our efforts to meet the Agendas requirements, the Westchester County Health & Hospital Planning Team (WCHHPT), including NewYork-Presbyterian Hudson Valley Hospital, conducted a Community Health Needs Assessment Survey to sample residents and providers in the County and each hospitals or agencies service areas to determine which Prevention Agenda initiative(s) it plans to address during this three year period. Surveys yielded 1,318 responses and 326 were from within the subset of NewYork-Presbyterian Hudson Valley Hospitals primary and secondary service areas. Significant socioeconomic gaps exist among the communities served by NewYork-Presbyterian Hudson Valley Hospital. Over a series of meetings in 2016, the Westchester County Health & Hospitals Planning Team crafted and disseminated throughout Westchester County community assessment and provider surveys. In addition to surveying the community at large, the team sought to identify a wide and varying range of providers of health care services to ensure that as many areas of health care as possible could be included in the surveys. Surveys were distributed to community members, community agencies and providers both electronically via a Survey Monkey on-line form, and in hard copy paper format. Completed paper surveys were manually entered into the data sets prior to analysis. NewYork-Presbyterian Hudson Valley Hospital completed a stand-alone Community needs assessment report. Part V, Line 7a: www.nyp.org/documents/hudsonvalley/about/CSP/HVH-CSP-2016-2018.pdf Part V, Line 10a: www.nyp.org/documents/hudsonvalley/about/CSP/HVH-CSP-2016-2018.pdf
Part V, Line 11The hospital is addressing the significant needs identified in its most recently conducted CHNA, namely Overweight and Obesity by its Community Service Plan Priority Prevent Chronic Disease programming. The hospital also provides community outreach to address the other health factors that were raised as health concerns, namely, nutrition/eating habits and care for the elderly. The original 2014 Intervention Healthy Heart Cooking Classes expanded to become the Healthy Heart Program. In 2014, participants expressed their interest in learning more about how physical activity could benefit their overall heart health. The program broadened from cooking classes to include a membership to The Wellness Center, the hospitals in-house fitness facility. Local physician practices and Hudson River Health Care, a not-for-profit, New York State licensed, federally qualified health center, helped to identify participants. The patient profile was minorities, ages 45-65, at risk of heart disease due to a previous heart attack, other cardiac issues or predisposition due to heredity, high blood pressure, diabetes or being overweight. In 2017, the program included enrollment in 10 Healthy Heart cooking classes spanning five months at NewYork-Presbyterian Hudson Valley Hospital Chef Peter X. Kelly Teaching Kitchen and membership to the hospitals Wellness Center, an onsite exercise facility. The participants were from minority populations, with ten African-Americans and two Latinos. Cooking classes were held every month from March through July 2017 with 12 participants enrolled in the program. The cooking classes are led by the chef, whose focus is teaching participants how to better manage their health through diet and lifestyle changes. The program aims to provide long-term solutions to improving health. Long term, the program emphasizes an increase in consumption of fruits and vegetables, and increase in heart health food preparation techniques, and increase in exercise habits. Moreover, the program seeks to empower participants, increasing their confidence in preparing heart health foods, and promotes mindful food consumption. Most indicative of the success of this program is how participants report feeling after the program. In our starting survey, they felt a certain way. The goal of the Healthy Heart program is to have most participants undergoing change and remaining motivated, and this goal was realized. After the program, four out of five participants embraced significantly new attitudes and corresponding behaviors, while one out of five was just behind them, taking action and deepening a commitment to the next level. This marked difference in attitude and behavior demonstrates how empowering this program is, and transformative for participants. Biometrics Biometric measurements of blood pressure, weight, and body mass index were recorded for the first class and last class at completion of the 10 week series: 83% of participants (10 people) had a reduction in blood pressure, 58% (7 people) lost weight and 58% (7 people) reduced their body mass index. Exercise component Tracking participants exercise sessions during the 5-month Healthy Heart program at the Wellness Center showed that the aggregate amount of the entire group is a total of 71 hours exercised and 32,783 calories burned. In addition to the Healthy Heart Program, community members have access to Hudson Valley Hospital Center DBA NYP/Hudson Valleys bi-monthly Farmers Market offered from May through November, making healthier food choices more readily available. Healthy Heart participants were encouraged to use the knowledge learned in the cooking classes about the nutritional value of fresh fruits and vegetables, ask questions directly of the farmers and purchase fresh produce. Priority/Focus Area #2: Prevent Chronic Disease; Reduce obesity in children and adults. Addressed by expanding the role of health care and health service providers in obesity prevention, specifically to increase breastfeeding rates at discharge to 90%. This is accomplished by training all Obstetrical Services staff on the benefits of breastfeeding and by providing breastfeeding education and support to mothers within one hour of birth; teaching mothers how to maintain lactation; and encouraging breastfeeding on demand. The 2016 update to Priority/Focus Area #2, Prevent chronic disease by reducing obesity in children and adults, underscores the importance of breastfeeding in reducing obesity, and increasing breastfeeding at discharge. The link between breastfeeding and reducing obesity is underscored by various sources including: The Centers for Disease Control website (www.cdc.gov) states that breastfeeding protects against childhood obesity and the success rate among mothers who want to breastfeed can be improved through active support from their families, friends, communities, clinicians, health care leaders, employers, and policymakers. The New York State Prevention Agenda Dashboard for Westchester County reflects that the percentage of infants exclusively breastfed in a hospital has significantly worsened from 48.7% in 2013 to 44.9% in 2016. As the statewide prevention agenda goal for 2018 is 48.1%, it was deemed by Westchester County Department of Health that focusing on the intervention of increasing breastfeeding at discharge is aligned with and supports the overall Prevention Agenda goals for 2018. The Obstetrical Services Department at Hudson Valley Hospital Center DBA NYP/Hudson Valley has been certified Baby-Friendly by Baby-Friendly USA and the World Health Organization since 2013, for its superior care and commitment to making sure that mothers who choose to breastfeed have the best chance at success. (The hospital is seeking recertification in 2018.) The policies implemented by the hospitals Obstetrical Services Department directly correspond to the World Health Organizations Ten Steps to Successful Breastfeeding. In addition, Hudson Valley Hospital Center DBA NYP/Hudson Valley, works with Hudson River Health Care, a New York State Federally-Qualified Health Center, that delivers comprehensive primary, preventive, and behavioral health services, to successfully maintain a dedicated lactation and breastfeeding room for its staff. In 2017, Hudson Valley Hospital Center DBA NYP/Hudson Valley achieved breastfeeding rates at discharge of 89%, compared to a monthly average of 78% in 2016. The hospital is striving to achieve monthly rates of 90% by the end of 2018, and will maintain its focus on the importance of breastfeeding to prevent chronic disease and reduce obesity in children and adults, with other initiatives as well: In 2017, Hudson Valley Hospital Center DBA NYP/Hudson Valley served the community by providing bi-monthly breastfeeding education and support groups to 330 new mothers. A grand-parenting support workshop was offered quarterly with 75 participants to date. To further serve the community, the hospital has provided education to 35 students enrolled at Dominican Westchester Community and Concordia Colleges as part of their clinical education. Hudson Valley Hospital Center DBA NYP/Hudson Valley continues to work with Hudson River Health Care (HRHC), offering classes and support. During the first week of August, New York-Presbyterian Hudson Valley Hospital celebrated 2017 World Breastfeeding Week by sponsoring in part a local event with HRHC for The Global Big Latch On(held annually with the goal of breaking the previous years record of the number of babies being breastfed at the same moment worldwide). At the Peekskill event, 40 women gathered to breastfeed and offer peer support; the event and media presence also contributed to community awareness. The hospitals Chef Peter X. Kelly Teaching Kitchen offered one Do It Yourself Baby Food class in April 2017, attended by 10 people. In addition, another class, Foods to Support Healthy Lactation, was held in November 2017, attended by 17 people, some of whom also brought their babies. The goal of each class was to share insights and techniques with mothers and partners, to assist them in preparing healthier food items for their children. The 2017 community outreach and education to address the other health factors that were raised in the CHNA, namely nutrition/eating habits and care for the elderly, include the following initiatives: At the hospitals Chef Peter X. Kelly Teaching Kitchen, the Physician in the Kitchen lectures and cooking demonstrations features a NewYork-Presbyterian Medical Group Hudson Valley physician and the Teaching Kitchen chef and coordinator. This unique program highlights a health topic for the physician to lecture about, and then our chef aligns a cooking class to reflect the topic. For example a lecture on eating for heart health and reducing cholesterol is complemented by a cooking demonstration on the Mediterranean diet. Many of our cooking classes at the Chef Peter X. Kelly Teaching Kitchen are offered at no

ExplanationPart I, Line 3: N/A Part I, Line 6a: N/A PART I, LINE 7, COLUMN (F) - Explanation for Percent of Total Expense: Bad debt expense in the amount of $4,220,658 was not included in the total functional expenses of $205,098,935 located on schedule H, Line 7, column (F). PART I, LINE 7 the following is a detail of the sources used for determining the amounts reported on schedule H: Line 7A - Adjusted Ratio of Patient Care Costs to Charges Line 7B - Adjusted Ratio of Patient Care Costs to Charges Line 7E - Actual Expenses PART III, LINE 2 - BAD DEBT EXPENSE EXPLANATION: For patients who were determined by the Hospital to have the ability to pay but did not, the uncollectable amounts are bad debt expense. The process for estimating the ultimate collection of receivables involves significant assumptions and judgments. The Hospital has implemented a monthly standardized approach to estimate and review the collectability of receivables based on the payer classification and the period the receivables have been outstanding. Past due balances over 90 days from date of billing and over a specified amount are considered delinquent and are reviewed individually for collectability. Account balances are written off against the allowance when management feels it is probable the receivable will not be recovered. Historical collection and payer reimbursement experience is an integral part of the estimation process related to reserves for doubtful accounts. In addition, the Hospital assesses the current state of its billing functions in order to identify any known collection or reimbursement issues in order to quantify the impact, if any, on reserve estimates, which involves judgment. The Hospital believes that the collectability of its receivables is directly linked to the quality of its billing processes, most notably those related to obtaining the correct information in order to bill effectively for the services provided. Revisions in reserve for doubtful accounts estimates are recorded as an adjustment to provision for bad debts. Please see the Audited Financial Statements, footnote #2, regarding Accounts Receivable and Allowance for Uncollectible Accounts. PART III, LINE 3: TO CALCULATE THE AMOUNT OF BAD DEBT EXPENSE WHICH SHOULD BE ALLOCATED TO PATIENTS ELIGIBLE FOR CHARITY CARE THE FOLLOWING PROCESS WAS PERFORMED. THE PATIENT ACCOUNTS FILES WHICH WERE USED FOR THE 2017 COST REPORT WERE FILTERED FOR THOSE PATIENTS WHO RECEIVED CHARITY CARE IN 2017 PER THE 2017 CHARITY CARE REPORTS DETAIL AND THEN ADDED THE BAD DEBT EXPENSE WHICH WAS RECORDED FOR EACH OF THESE PATIENTS. THIS PROCESS WAS COMPLETED FOR ALL PATIENT ENCOUNTER TYPES BOTH INPATIENT AND OUTPATIENT. PART III, LINE 4 - BAD DEBT EXPENSE FOOTNOTE TO FINANCIAL STATEMENTS: PLEASE SEE THE AUDITED FINANCIAL STATEMENTS FOOTNOTE #2 REGARDING ACCOUNTS RECEIVABLE AND ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS. PART III, LINE 5,6 & 7: THESE DO NOT INCLUDE CERTAIN MEDICARE PROGRAM REVENUE AND COSTS AND THUS DO NOT REFLECT ALL OF THE ORGANIZATIONS REVENUES AND COSTS WITH THE PARTICIPATION IN MEDICARE PROGRAMS. THE REVENUE AND COSTS EXCLUDES MEDICARE MANAGED CARE ACTIVITY. IF ALL THESE REVENUE AND COSTS WERE INCLUDED THE MEDICARE LOSS OF ($17,362,980) WOULD RESULT IN A GREATER MEDICARE LOSS OF (34,783,767). MEDICARE NET SURPLUS PER SCHEDULE H (17,362,980) MEDICARE MANAGED CARE NET COSTS (17,420,787) TOTAL NET COSTS ASSOCIATED WITH THE MEDICARE PROGRAM (34,783,767) "Net is defined as revenue net of costs" PART III, LINE 8 - MEDICARE EXPLANATION: The Hospital provides both inpatient and outpatient services to the community in spite of this shortfall. As stated by the Hospital's mission, Hudson Valley Hospital Center DBA New York-Presbyterian/Hudson Valley Hospital is dedicated to serving the health care needs of the community and to providing quality, comprehensive medical care in a compassionate, professional, respectful manner, without regard to race, religion, national origin or disease category. Offering state-of-the-art diagnostic treatment, education and preventive services, the Hospital is committed to improving the quality of life in the community. In fulfilling this mission, the Hospital will strive to continuously improve the care provided and develop and offer programs, facilities, systems and alliances that most effectively respond to community health care needs. The Medicare Cost Report was used to determine the 2017 shortfall. PART III, LINE 9B - COLLECTION PRACTICES EXPLANATION: If a patient qualifies for charity, all open accounts are written off at that time. Charity applications are good for one year from receipt. If a patient is applying for charity care, the account is put on hold while the documents are gathered and completed. If the patient does not return documents requested within 60 days, a letter is sent to them reminding them of the request. If there is still no response after 30 days, the account may be referred for collections stating no cooperation from the patient. Per 501(r) (5), Hudson Valley Hospital Center Dba New York-Presbyterian/Hudson Valley Hospital has established written financial assistance and emergency care policies. The policies include a limit on amount charged for those who qualify. All reasonable efforts are made to determine whether an individual is eligible for assistance prior to sending an account to collection. This policy was updated in June 2018. PART VI, LINE 2 - NEEDS ASSESSMENT: In keeping with the mission of the Commissioner of Health of New York, Hudson Valley Hospital Center DBA NYP/Hudson Valley Hospital, working in conjunction with our community partners, has continued assessing our present initiatives, strategic plans and prevention agenda priorities. Form And Line ReferencePart VII and Schedule J:FormAndLineReferenceDesc
ExplanationCertain officers and key employees of the New York and Presbyterian Hospital that are identified in part VII as officers or trustees of the Hudson Valley Hospital DBA NYP/Hudson Valley Hospital are responsible for executing the mission and management of the New York and Presbyterian Hospital (NYP) and its affiliated entities. Compensation for 2017 of these upper level executives includes the payout of an annual incentive plan and a long-term incentive plan. This performance-oriented program conditions payments upon the achievement of multiple individual and group performance measures. Measures to monitor performance include: operational and financial strength, patient quality and safety, patient satisfaction, advancement of patient care, and people development and partnership. Incentive awards may only be granted if the organization achieves a financial surplus. Even if all relevant performance measurements are achieved, the NYP Board of Trustees retains full discretion to make or not make any incentive awards, or to reduce the amount of any incentive award. This initiative is critical to assuring that NYP has the requisite leadership to create and manage a highly motivated and engaged workforce, to drive superior performance throughout the organization and to achieve top tier medical center status. As a separate matter, due to restrictions imposed by the Internal Revenue Code, upper level executives are limited in the amount of benefits received under a tax-qualified retirement plan. Like many employers, NYP supplements these executives' pension benefits through a supplemental ("nonqualified") retirement plan. The supplemental executive retirement plan (SERP) is subject to a multi-year vesting requirement (commencing after five years of participation in the SERP, in prorated amounts through age 65) which places an executive's supplemental retirement benefit at risk of forfeiture if the vesting requirements are not satisfied. Once vested, however, provisions of the Internal Revenue Code require that the vested executive include in current income the value of his or her vested supplemental retirement benefit. Notwithstanding the legal requirement to recognize the vested value of the supplemental retirement benefit as current income, the supplemental retirement benefit will not be distributed to the executive until the executive actually retires from NYP (although, as permitted by the Internal Revenue Code, the supplemental retirement plan will effect a distribution of an amount necessary to satisfy the executive's tax liability resulting from the income recognition upon vesting). As noted, this supplemental retirement benefit will not be distributed to the executive until the executive actually retires from NYP. There are constantly changing legal, tax, accounting, and public disclosure rules for a SERP (supplemental executive retirement plan) in not-for-profit organizations. The executive Compensation Committee continuously monitors these changes and incorporates any changes into the overall SERP plan design. As in past years, the executive Compensation Committee of NYP requires a third party complete a review of the organization's compensation program to ensure its effectiveness in terms of government regulations, market conditions and the need to continually elevate organizational performance. The report also serves to meet the regulatory obligations to ensure that all elements of the executive compensation programs are reasonable. The individuals listed in part VII that are compensated by New York Presbyterian Hospital devote an average of sixty hours per week to perform their responsibilities for the New York Presbyterian Hospital and other related organizations in the aggregate. Schedule J, Line 3: Compensation decisions for the President was determined by a related organization following that organization's compensation policy. Part I, Line 4A: Severance Pay: John Federspiel received 750,009 in severance Pay. Debbie Neuendorf received 266,292 in severance Pay.
