| PART III, LINE 2 | WINCHESTER HEALTHCARE AND AFFILIATES GRANT CREDIT WITHOUT COLLATERAL TO PATIENTS, MOST OF WHOM ARE LOCAL RESIDENTS AND ARE INSURED UNDER THIRD PARTY ARRANGEMENTS. ACCOUNTS RECEIVABLE ARE REDUCED BY AN ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS. IN EVALUATING THE COLLECTABILITY OF ACCOUNTS RECEIVABLE, THE HOSPITAL ANALYZES ITS PAST HISTORY AND IDENTIFIES TRENDS FOR EACH OF ITS MAJOR CATEGORIES OF REVENUE TO ESTIMATE THE APPROPRIATE ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS AND PROVISION FOR BAD DEBTS. MANAGEMENT REGULARLY REVIEWS DATA ABOUT THESE MAJOR CATEGORIES OF REVENUE IN EVALUATING THE SUFFICIENCY OF THE ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS. THROUGHOUT THE YEAR, THE HOSPITAL, AFTER ALL REASONABLE COLLECTION EFFORTS HAVE BEEN EXHAUSTED, WILL WRITE OFF THE DIFFERENCE BETWEEN THE STANDARD RATES AND THE AMOUNTS ACTUALLY COLLECTED AGAINST THE ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS. IN ADDITION TO THE REVIEW OF THE CATEGORIES OF REVENUE, MANAGEMENT MONITORS THe WRITE OFFS AGAINST ESTABLISHED ALLOWANCES AS OF A POINT IN TIME TO DETERMINE THE APPROPRIATENESS OF THE UNDERLYING ASSUMPTIONS USED IN ESTIMATING THE ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS. THE ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS APPROXIMATES 15.39% FOR THE YEAR ENDED SEPTEMBER 30, 2018 and 12.3% FOR THE YEAR ENDED SEPTEMBER 30, 2017. WINCHESTER HEALTHCARE MANAGEMENT, INC. AND AFFILIATES UTILIZED WORKSHEET A: ESTIMATED BAD DEBT EXPENSE (AT COST) TO DETERMINE THE AMOUNT FOR SCHEDULE H, PART III, LINE 2. WINCHESTER HEALTHARE MANAGEMENT INC. AND AFFILIATES USED ER BAD DEBT WRITE OFFS NET OF ANY HEALTH SAFETY NET PAYMENTS TO REASONABLY ESTIMATE THE AMOUNT IN BAD DEBT THAT COULD BE ATTIBUTABLE TO PATIENTS WHO LIKELY WOULD QUALIFY FOR FINANCIAL ASSISTANCE UNDER OUR CHARITY CARE POLICY HAD SUFFICIENT INFORMATION BEEN SUBMITTED IN ORDER TO DETERMINE ELIGIBILITY. |

| PART III, LINE 4 | WINCHESTER HOSPITAL GRANTS CREDIT WITHOUT COLLATERAL TO PATIENTS, MOST OF WHOM ARE LOCAL RESIDENTS AND ARE INSURED UNDER THIRD-PARTY ARRANGEMENTS. WINCHESTER HOSPITAL RECOGNIZES PATIENT SERVICE REVENUE ASSOCIATED WITH SERVICES PROVIDED TO PATIENTS WHO HAVE THIRD-PARTY PAYOR COVERAGE ON THE BASIS OF CONTRACTUAL RATES FOR THE SERVICES RENDERED. FOR UNINSURED PATIENTS THAT DO NOT QUALIFY FOR CHARITABLE CARE, REVENUE IS RECOGNIZED ON THE BASIS OF ITS STANDARD RATES (SUBJECT TO DISCOUNTS) FOR SERVICES PROVIDED. ON THE BASIS OF HISTORICAL EXPERIENCE, A SIGNIFICANT PORTION OF THE WINCHESTER HOSPITAL'S UNINSURED PATIENTS ARE UNABLE OR FAIL TO PAY FOR THE SERVICES PROVIDED. CONSEQUENTLY, A PROVISION FOR BAD DEBTS IS RECORDED RELATED TO UNINSURED PATIENTS IN THE PERIOD THE SERVICES ARE PROVIDED. |
| PART III, LINE 8 | ALL MEDICARE COSTS ARE ALLOCATED BY COST CENTER IN ACCORDANCE WITH MEDICARE REGULATIONS. THIS IS A SIGNIFICANT BENEFIT TO THE COMMUNITY SINCE REIMBURSEMENT FROM THE FEDERAL GOVERNMENT'S MEDICARE PROGRAM IS LESS THAN THE ACTUAL COST FOR THE HEALTHCARE SERVICES PROVIDED TO THE INDIVIDUALS RECEIVING IT. |
| PART III, LINE 9B | THE HOSPITAL HAS A FIDUCIARY DUTY TO SEEK REIMBURSEMENT FOR SERVICES IT HAS PROVIDED FROM INDIVIDUALS WHO ARE ABLE TO PAY, FROM THIRD-PARTY INSURERS WHO COVER THE COST OF CARE, AND FROM OTHER PROGRAMS OF ASSISTANCE FOR WHICH THE PATIENT IS ELIGIBLE. TO DETERMINE WHETHER A PATIENT IS ABLE TO PAY FOR THE SERVICES PROVIDED AS WELL AS TO ASSIST THE PATIENT IN FINDING ALTERNATIVE COVERAGE OPTIONS IF THEY ARE UNINSURED OR UNDERINSURED, THE HOSPITAL FOLLOWS THE FOLLOWING CRITERIA RELATED TO BILLING AND COLLECTIOns FROM PATIENTS. THE HOSPITAL MAKES THE SAME REASONABLE EFFORTS AND FOLLOWS THE SAME REASONABLE PROCESS FOR COLLECTING ON BILLS OWED BY AN UNINSURED PATIENT AS IT DOES FOR ALL OTHER PATIENTS. THE HOSPITAL WILL FIRST SHOW THAT IT HAS A CURRENT UNPAID BALANCE THAT IS RELATED TO SERVICES PROVIDED TO THE PATIENT AND NOT COVERED BY A PRIVATE INSURER OR A FINANCIAL ASSISTANCE PROGRAM. THE HOSPITAL FOLLOWS REASONABLE COLLECTION/BILLING PROCEDURES, WHICH INCLUDE MULTIPLE PROGRESSIVE BILLING NOTICES THAT CONTAIN INFORMATION ABOUT THE AVAILABILITY OF FINANCIAL ASSISTANCE PROGRAMS, DOCUMENTATION OF CONTINUOUS BILLING EFFORTS, CHECKING THE MASSACHUSETTS ELIGIBILITY VERIFICATION SYSTEM TO ENSURE THE PATIENT IS NOT A LOW INCOME PATIENT, AND OUTSIDE COLLECTION AGENCIES. |
| PART VI, LINE 2, 4, 5 | Purpose and Approach In FY16, Winchester Hospital completed a Community Health Needs Assessment in conjunction with the three other hospitals in the Lahey Health System: 1.) Lahey Hospital & Medical Center, Burlington 2.) Northeast Health System - Beverly Hospital, Beverly & Addison Gilbert Hospital, Gloucester The purpose of the CHNA is to inform and guide the hospitals selection of and commitment to programs and initiatives that address the health needs of the communities it serves. The assessment was conducted in partnership with John Snow Inc., public health consulting and research organization. Winchester Hospital Community Benefits Service Area: WHs community benefits investments are focused on expanding access, addressing barriers to care and improving the health status of residents living in nine cities and towns including Medford, North Reading, Reading, Stoneham, Tewksbury, Wakefield, Wilmington, Winchester and Woburn. Methodology: The CHNA was conducted in three phases, allowing Winchester Hospital to: - Compile an extensive amount of quantitative and qualitative data - Engage and involve key internal and external stakeholders - Develop a report and detailed Community Health Improvement Plan (CHIP) - Comply with all state and federal IRS community benefits requirements To ensure a comprehensive understanding of the issues, data was collected from a number of quantitative and qualitative sources. Quantitative Data Sources: - Massachusetts Community Health Information Profile (MassCHIP) - U.S. Census Bureau, American Community Survey 5-Year Estimates (2009-2013) - Behavioral Risk Factor Surveillance System (BRFSS) (2012-2013 aggregate) - CHIA Inpatient Discharges - Massachusetts Health Data Consortium (MHDC) ED Visits - MA Hospital IP Discharges (2008-2012) - MA Cancer Registry (2007-2011) - MA Communicable Disease Program (2011-2013) - MA Hospital ED Discharges (2008-2012) - Massachusetts Vital Records (2008-2012) - Massachusetts Bureau of Substance Abuse Services (BSAS) (2013) - Massachusetts Board of Health Qualitative Data Sources: In order to obtain targeted data and understand the health issues currently perceived by the community, interviews and listening sessions were conducted: - Informant interviews with external stakeholders (28 completed) - Random household surveys (1,022 completed in the WH service area) - Community listening sessions (two sessions, 100 attendees total) Public Reporting of Information: The results of the CHNA were made publicly available in various ways including on the WH website, distribution to internal and external community stakeholders and presentations to hospital leaders and staff. In addition, report out sessions for key stakeholders in each municipality WERE HELD IN FY 18 The goal of the sessions WAS to present the findings of the CHNA and to engage them in discussion to gain better understanding of their perceptions of the current health needs, social determinants of health, barriers to care in their communities and discuss potential opportunities for collaboration. Addressing the Identified Needs: In response to the priority health needs identified in the 2016 CHNA, WH developed a three year Community Health Improvement Plan (CHIP). The CHIP was developed with input from the Winchester Hospital Community Benefits Advisory Board and approved by the Winchester Hospital board of directors. In addition, feedback from community report out sessions was taken into consideration when developing the plan. The community benefits programs offered in FY18 were designed to address the priority health needs identified in the 2016 CHNA, along with the FY18 Massachusetts Statewide Health Priorities identified by the Executive Office of Health and Human Services. Priority Health Issues Identified in FY16 CHNA: - Behavioral Health/Substance Use Disorder - Elder Health Socialization, Isolation, Depression, Falls Prevention - Wellness/Chronic Disease Prevention & Management - Cancer Prevention, Management and Support FY18 Statewide Health Priorities: - Address Unmet Health Needs of the Uninsured - Manage chronic disease - Promoting Wellness of Vulnerable Populations - Reducing Health Disparities - Support Health Care Reform FY18 Key Community Benefit Programs & Outcomes: In collaboration with various community partners, listed below are some of the key programs WH provided in FY18: I. Behavioral Health/Substance Use Disorder: - Substance Use Disorder Community Outreach Initiative - Winchester Hospital worked collaboratively with community partners to develop and execute a variety of programs to address substance use prevention, early identification, recovery and support. Winchester Hospital staff members also participated in local and regional substance abuse coalitions and task forces whose missions are to develop strategies to prevent substance abuse, reduce stigma, eliminate barriers to treatment and improve access to care. - Boys & Girls Club Screening, Brief Intervention, Referral to Treatment - This program utilizes an innovative approach to screening and identifying youths who have or are at risk for developing mental health and substance use disorders, and providing intervention as early as possible. Administering the program onsite at the Boys & Girls Club provides a non-authoritarian, safe environment resulting in more natural, open and honest dialogue between the staff members and the participant. In FY18 300 youths were screened, resulting in 35 referrals to treatment or counseling. - Substance use in the workplace: In response to the priority needs identified in the 2016 Winchester Hospital Community Health Needs Assessment, Winchester Hospital awarded a mini-grant to the Winchester Coalition for a Safer Community to implement the Substance Use in the Workplace Program. The program, launched in October, 2017, was designed to educate employers and employees about substance use issues in the workplace, and the important role staff can play in helping employees and/or family members who are struggling with substance misuse or addiction. The program included one educational session targeted towards department heads and supervisors, followed by four educational sessions that were open to all employees of the Town, and included: -An overview of substance use in the workplace -The incidence/prevalence of the problem -Identifying Helpful Resources (Internal & External/Community) -Review of Legal Policies/Procdures -Accessing Care/Health Insurance FAQs -Goal: To increase awareness of substance use in the workplace and provide participants with helpful information and resources for addressing issues among employees and/or their families.
