Schedule H, Part V, Section B, Line 5-Craig HospitalCommunity partners and individuals with special expertise in public health provided comparative data on our patient population. Health Data Analyst from the Health Surveys and Analysis Program of the Colorado Department of Public Health and Environment provided data on the Behavorial Risk Factor Surveillance System survey. Policy Analyst from the Colorado Health Institute provided data on the Colorado Health Access Survey.
Schedule H, Part V, Section B, Line 11-Craig HospitalThe CHNA identified several needs that are being addressed as follows: 1. We have developed resources to help patients search for transportation providers and public transit in their community. These resource are available on our website. 2. We continue development of resources to help disabled persons access medical services in their community through the Nurse Advice Line and the hospital website. The Nurse Advice Line staff is experienced in helping callers find a doctor in their community and guide patients in how to get an appointment with their PCP when needed. 3. We continue to use social media to raise awareness about the importance of receiving a flu shot each year and to promote the resources available on our website such as the list of dentists who provide affordable care and have accessible offices for disabled persons. 4. We previously developed a video with a variety of scenarios experienced in daily lives and how to advocate for accessibility needs for patients who experience barriers in their community after they return home. The video was made available on the hospital website for patients to access from home. 5. We created education regarding the rapid changes in Opiate prescriptions guidelines and the dangers with opiate addiction. The article was published on our blog and social media. 6. Over the last fiscal year the hospital did a number of things to improve our patient's health after discharging from Craig and re-entering their community. We vaccinated more than 95% of our inpatients against influenza during flu season and offered the vaccine to our outpatients as well. 7. The hospital continued development of a soft-landing pilot program to better support some of our most complicated discharges. Craig staff go home with the patient and their family whether they live in Colorado or out of state for a couple of days to help the patient settle into their community again and make sure they are set for success. 8. We implemented a pill-box loading program helping patients prepare their prescriptions prior to discharge from Craig and re-entering their community. This program prevents patients from being overwhelmed by the need to fill prescriptions when they arrive home from the hospital. It also helps patients prevent and identify medications errors in their homes and community pharmacies. The CNHA and implementation strategy is reviewed and approved by the Medical Care and Administrative Committee and submitted to the Board of Directors for approval.
Schedule H, Part V, Section B, Line 13h-Craig HospitalSize of household.
Schedule H, Part V, Section B, Line 16j-Craig HospitalThe financial assistance application is sent to patient with pre-collection letter. Patient billing statements also reference the policy and contact information is given if they require additional information or want to apply.
Schedule H, Part V, Section B, Line 20e-Craig HospitalSent a final notice letter and a Financial Assistance Application to patient.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Schedule H, Part I, Line 7Costs were determined by using either actual costs or calculated costs based off a cost-to-charge ratio.
Schedule H, Part III, Section A, Line 2The Hospital continued to follow the presentation and disclosure of patient service revenue, provision for bad debts and allowance for doubtful accounts as pronounced in the accounting standards updates 201107 and accounting standards codification topic 954. This requires organizations that do not have access to the collectability of a receivable prior to recognizing revenue to present their provision for bad debt related to a patient service revenue as a deduction from revenue, net of any contractual allowances and discounts on their statement of operations. The adoption of this did not impact the financial statements as the Hospital does not recognize significant revenue without assessing a patient's ability to pay.
Schedule H, Part III, Section A, Line 3The Hospital does not consider any of its bad debt expense to be attributable to patients eligible under the organization's financial assistance policy.
Schedule H, Part III, Section A, Line 4There is no footnote in the audited financial statements that describes bad debt expense. Please refer to page 9 of the Audited Financial Statements that addresses Patient Account Receivable.
Schedule H, Part III, Section B, Line 8The source of the data is the most recent filed Medicare Cost Report using the ratio of cost to charges method. The payment mechanism received for care provided to Medicare patients is not negotiated but set by federal regulations. The payment currently received for treatment is not sufficient to cover costs and although we don't recognize this shortfall as charity, we believe it should be included in the calculation of community benefit. All patients receive the appropriate level of care and services regardless of payment source.
Schedule H, Part III, Section C, Line 9aThere is not a specific written debt collection policy as those procedures are addressed in the Financial Assistance Policy.
Schedule H, Part VI, Line 2It addition to conducting its CHNA every 3 years, the Hospital assesses community needs through a variety of different programs. As a condition of federal funding, Craig Hospital's research department conducts ongoing longitudinal studies on the long-term effects and needs of persons with spinal cord injury and brain injury. Craig Hospital also operates a toll-free Nurse Advise Line and collects data on why former patients are calling for assistance. Craig Hospital also collaborates with the Brain Injury Alliance of Colorado , Operation TBI Freedom, The Hangout, the Christopher & Dana Reeve Peer Network Program and other community organizations to assess and meet the long term needs of citizens with disabilities. Craig also launched the Peer Mentor Soft Landing Program-a 3 year pilot program aimed at improving the success of patients as they are discharged home and gaining new insight on the challenges these patients face.
Schedule H, Part VI, Line 3The Admissions Department identifies patients who have limited benefits and compare the benefits to the estimate length of stay. When there is a shortage of days allowed by insurance, the Admissions Directors meets with the patient and/or family to determine alternative coverage. This conversation encompasses the patient's ability to pay for the care, potential eligibility for Federal and State programs and the Hospital's Charity Care program. Upon admission, the case manager complete a thorough Psycho-Social Assessment of the patient/family specifically analyzing their financial situation. The case managers consider household income post-injury, family size, assets, current and future living expenses, and financial obligations including expected future care needs. The case managers, along with the patient/family, assess other potential resources/programs for which the patient may qualify. Resources and programs evaluated included but are not limited to Medicaid, Victim's Assistance, Liability and Liquidity, Accidental Death and Dismemberment, and the Hospital's Charity Care program. If it appears as though the patient will qualify for State or Federal Programs, the case manager will assist the patient/family in completing the applications, obtaining the required documentation and advising them on how to position themselves to qualify. If patients do not meet the criteria for other State or Federal programs, the Financial Assistance Eligibility Discount Guideline is utilized to determine if the patient's bill will be discounted. If appropriate, the case manager will initiate and assist the patient/family in completing the Financial Assistance Application for the Charity Care Program. Applications may also be obtained on the Craig website, completed over the phone or by mail. If it is not clear that the patient/family will have a balance prior to discharge (co-pays, out of pocket etc) the patient/family will receive information on the Charity Care Program which is noted on the monthly billing statements and the copy of application along with letter for pre-collection. At this time the patient can complete the application. All applications are reviewed by the A/R Manager, VP of Finance and/or the Controller. When it is determined that the charity application is appropriate, this is communicated to the patient/family by the case manger or A/R Manager.
Schedule H, Part VI, Line 4As one of the most highly-rated rehabilitation hospital in the United States for spinal cord (SCI) and traumatic brain injuries (TBI), Craig treats patients from virtually every state including some from outside the US. Based in Englewood, Colorado, approximately 60% of all patients are from Colorado. The other top five states in order would include Wyoming, New Mexico, Kansas, and California. These five states account for approximately 74% of all patients treated during FY 2018. During FY 2018, Craig treated patients from 37 states. Since becoming a specialty hospital in 1956, Craig has treated over 33,000 patients with an SCI or TBI. With an average of 500 inpatients and 2000 outpatients a year, 51% are SCI patients and 49% BI patients The average age of Craig's patients is 38, ranging from 15 to 70 years old and 78% of all patients are male coming from varied socio-economic backgrounds. 80% of Craig's patients are Caucasian and 10% are Hispanic/Latino. During FY 2018, 9% of inpatients were Medicaid recipients and 5% were on Medicare. Craig treats all levels of spinal cord injuries, traumatic brain injuries, stroke, aneurysm, encephalopathy, anoxia, and ventilator patients, as well as neuromuscular disorders like Transverse Myelitis, Guillain-Barre Syndrome or other conditions. The leading causes of injuries in our patient population are vehicular accidents (43%), falls (21%), medical complications (18%) and sports injuries (14%). Craig works very closely with all the hospitals in the Rocky Mountain Region and receives referrals from Hospitals throughout the US. Although many of the critical care hospitals have rehabilitation units, specialty hospitals like Craig have access to extraordinary expertise, resources and therapy programs that aren't often available at general rehabilitation centers. Craig has years of experience treating individuals with SCI and TBI and can offer patient and family support programs designed for SCI and TBI patients. In addition the staff at Craig knows how to help families navigate complex SCI/TBI care at an emotionally challenging time.
Schedule H, Part VI, Line 5Craig Hospital's mission is to provide exemplary patient care, education and research to people with spinal cord (SCI) and traumatic brain injuries (TBI). The Board of Directors of Craig Hospital, consisting of 16 members of which 15 are independent, has ultimate authority in extending privileges only to professionally competent physicians, dentists and podiatrists, licensed to practice in the State of Colorado. There are 194 physicians on the medical staff of Craig Hospital, each assigned to one of four separate medical staff categories. The medical staff of Craig Hospital is organized and operates under the guidance of the medical staff bylaws, medical staff rules and regulations and medical staff policies and procedures. The medical staff is responsible for establishing and maintaining patient care standards, credentialing and delineation of clinical privileges. Additionally, there are approximately 82 allied health professionals, categorized as either independent or physician directed whom function within one or more health-related professions or occupations. The Hospital uses excess funds to fund capital improvements, equipment for patient programs, and patient and staff education. In FY2018, Craig reinvested over $13 million back into the infrastructure of the Hospital for building improvements, state-of-the-art equipment and information technology. Craig Hospital provides significant support to the families of patients including 30 days of free housing for families of inpatients who live further than 60 miles from the hospital.
