Part V, Section BFacility Reporting Group A
Facility Reporting Group A consists of:- Facility 1: St. Helena Hospital, - Facility 2: SHH Center for Behavioral Health
Facility Reporting Group - A Part V, Section B, line 5:The 2019 CHNA data collection process can be summarized as follows: 1. Review of prior CHNA reporting efforts;2. Collection of most recently available demographic, socioeconomic and health indicator data; and3. Data gathering from community members. Data gathering was accomplished by engaging with community focus groups and conducting key stakeholder interviews. Focus group participants were end-users of programs and services provided by AHSH. Populations represented by focus group members included Promotoras, seniors, low-income, homeless/at-risk and representatives from the education sector. The majority of focus group participants live in Napa County, specifically in Santa Rosa, Yountville, St. Helena, and Calistoga. Key informant interviews consisted of key leaders in our community from an array of agencies, including those that serve children, homeless populations, seniors and community at large populations. Additionally, key informant interviews were conducted with representatives from the non-profit, educational, homeless, law enforcement, and faith-based sectors. The majority of the people interviewed serve residents in Napa County, specifically in in Santa Rosa, Yountville, St. Helena, and Calistoga. Additional localities identified for the service areas were Rutherford, Ronald Park, Deer Park and Hope Valley. Most key informants hold titles such as Executive Directors or Presidents, Superintendent, Mayor, and Head Pastor.A significant portion of secondary data for this assessment was collected through reports generated through CARES Engagement Network CHNA (https://engagementnetwork.org/assessment/). Other sources include California Department of Public Health, County Health Rankings & Road maps, and California Environmental Protection Agency's Office of Environmental Health Hazard Assessment. When feasible, health metrics have been further compared to estimates for the state or national benchmarks, such as the Healthy People 2020 objectives.
Facility Reporting Group - A Part V, Section B, line 6b:St. Helena Hospital partnered with Abode Services, Napa Valley Unified School District-St. Helena School District, Rianda House and Up Valley Center to prepare the 2019 Community Health Needs Assessment.
Facility Reporting Group - A Part V, Section B, line 11:Our 2016 CHNA identified five areas of significant needs: mental health, obesity & diabetes, access to primary health & dental care, cancers, and access to shelter for the homeless. The main goal was to improve the overall health and wellness of our communities through provision of services, community collaboration and innovation. Priority Need 1 - Mental Health - the main goals: to increase access to mental health services and to increase awareness and interventions for youth.Interventions:1. In an effort to expand access and increase awareness of mental health disorders, Adventist Health St. Helena proudly partnered with Mentis, a community partner that focuses on providing bilingual professional mental health services to people throughout the county of every age, stage and income level. Through our partnership, we have been able to implement Teens Caf, a group peer and professional support program, serving over 245 teens on the St. Helena High School campus. As of January 2019, Teens Caf expanded to include middle school aged students at Robert Louis Stephenson Middle School. Mentis' School-Based Program works closely with the Napa, Calistoga, Howell Mountain and St. Helena school districts to provide counseling to children and teens in elementary, middle and high schools struggling with depression, violence, and family conflicts that are causing emotional, behavioral and academic problems. Mentis' therapists work closely with the student and their families to address and resolve problems that are having a significant impact on high risk youth in Napa County. 2. Increased outpatient mental health services by expanding available appointments and hours: Transitioned Dr. Haycraft from part-time inpatient hospital and part-time outpatient clinic setting to full-time clinic setting allowing for additional appointments in clinic and increasing access to immediate mental health resources. Community members served: 309/senior community members served: 175.Priority Need 2 - Obesity & Diabetes - the main goals: increase accessibility to healthy, affordable foods, increase opportunities for physical activity, and increase education about healthy eating and active living. Interventions:1. Partnered with and participated in Wellness Fair for St. Helena Unified School District: Adventist Health St. Helena worked together with Robert Louis Stevenson Middle School to put together a wellness curriculum that teaches students about mind, body and spirit health. The day's events included education on healthy snacks, sleep, exercise interventions, prevention of tobacco use and the importance of hydration. The day was attended by more than 285 students and was very successful in teaching children how to make healthy choices for their long-term health. 2. Utilize physicians, integrative medicine specialists, and nutritionists to educate community on health-related topics: Adventist Health St. Helena proudly sponsored a series of educational events that helped to educate the public on variety of health topics taught by physicians and dietitians. We proudly sponsor a monthly senior health education series in Calistoga that highlights different health-related and self-care topics presented by specialists in each field. 3. Safe Walk & Roll to School Days: Adventist Health St. Helena participated in the Safe Walk & Roll to School days promoting exercise and getting outdoors to students throughout St. Helena.4. Enhance diabetes education program to accommodate more of the community: Adventist Health St. Helena provided residents in our community with access to a free four-week diabetes education class in St. Helena. The free class series helps participants learn how simple lifestyle choices can make all the difference. Plus, participants have an opportunity to meet one on one with a registered dietitian where they work together to tailor a plan around the participant's health and lifestyle. Community members served: 70.5. Provide free community exercise programs to encourage physical fitness and weight management: Together with the City of St. Helena Parks and Recreation department, we were able to provide two free exercise classes where more than 30 community members gathered on a weekly basis, encouraging community members to get outside, get moving and take control of their health. Additionally, we participated in the Park Rx coalition to promote physical activity in open spaces and parks to people to reduce stress, improve blood pressure and reduce glucose. Priority Need 3 - Access to Primary Health & Dental Care - the main goals: to expand accessibility and increase awareness of resources.Interventions:1. Dare to C.A.R.E program providing free heart and vascular screening: Adventist Heart & Vascular Institute continued the Dare to C.A.R.E program, a free screening for those who qualify to detect carotid artery disease, abdominal aortic aneurysm, renal artery disease and extremity artery disease. This ultrasound screening provides the public with education about the unrecognized risks of vascular disease. Countless lives can be saved by teaching people about vascular disease and options they have for pre-emptive treatment. In 2019, 256 patients were screened, and two large aortic aneurysms were detected, preventing a rupture. One in three participants screened are found to have disease, thus far. This program is currently run out of our clinic in Calistoga and also in Lake County.2. Operation Access - Provide specialty care and surgical services to low-income, uninsured patients: Adventist Health St. Helena is a proud partner of Operation Access. Together with other community partners we are able to provide quality specialty care for low-income, uninsured patients. This work improves individual lives as well as the community as a whole. In 2019, six surgeons/ specialists provided sixty services at AHSH, in the specialties of Gastroenterology, General Surgery, Gynecology, Plastics, and Vascular Surgery. Additionally, the anesthesia, pathology, radiology, and physician groups (Adventist Health Physicians Network, Affiliated Pathology Medical Group, Lyons Huber Anesthesia, and Valley Radiological Associates Medical Group) have agreed to waive any charges associated with Operation Access services. In 2019, Adventist Health has increased service volume from 36 to 60, an increase of 67% from the prior year.3. Senior Lyft Pilot Program: Transportation is a major barrier to healthcare access for many seniors. A recent survey by HAPI (Healthy Aging Population Initiative) indicated that transportation challenges are the leading concern from patients in this population. As a note: 24% of the population of St. Helena is senior, and 22% of Calistoga. In partnership with Rianda House and Molly's Angels, Adventist Health St. Helena sponsored a pilot with on-demand ride service Lyft for seniors facing transportation challenges getting to and from their appointments at the hospital. The program was very successful and was able to provide approximately 41 rides for seniors.
Part V, Section B, Line 11 (continuation):Priority Need 4 - Cancers - the main goals: to increase access to cancer detection services and related support services and to increase awareness and interventions to prevent cancer. Interventions: 1. Partnered with ZERO Prostate Cancer to promote education and awareness of prostate cancer prevention and treatment: Adventist Health St. Helena's Martin O'Neil Cancer Center is a proud sponsor of the Zero Prostate Cancer Napa Valley, where a team of staff members, physicians and community members participate in a 5k or 10k walk/run to raise awareness and promote prostate cancer screening. All proceeds provide research for new treatments, free prostate cancer testing, and education for men and families about prostate cancer. Over 150 walkers and runners participated in the 2019 event.2. Intervention: Turkey Trot to promote the importance of early hereditary cancer screening and educate the community on the AHEAD (Adventist Health Early All Around Detection) Program for early detection and treatment of genetic cancers. Founded by Dr. Candace Westgate, an obstetrician and gynecologist, our AHEAD program sponsored the second annual Turkey Trot in St. Helena promoting health, community and the importance of early hereditary cancer screening for genetic cancers. Over 150 participants attended the event. 3. Intervention: Communitywide education through the support and education series: The Martin-O'Neil Cancer Center communitywide education and support programs throughout 2019 that focused on topics like skin cancer prevention and the importance of genetic screening. A total of 190 individuals participated in the services. During the genetic cancer discussion, the participants also had genetic screenings if indicated. In 2019, there were 1,414 mammograms completed with eight referrals to a cancer specialist. A total of 365 low dose CT screening were completed in 2019. Genetic screenings found 713 at high risk eligible patients/clients with 418 actually tested. Breakdown of those tested saw 294 with results, 82 with negative high risk and 42 with pathogenic mutations. Priority Need 5 - Access to Shelter for the Homeless Catholic Charities Nightingale House: Adventist Health St. Helena is proud to support and be a part of Catholic Charities Shelter and Housing Department's initiative to operate the Nightingale Center, a medical respite center for patients from Queen of the Valley and Adventist Health St. Helena Hospital. The center is designed to help patients who have no place to go to continue with their recovery. The Nightingale House will help patients to be released to a safe and stable environment to minimize recidivism. This facility will have 11 beds to provide temporary on-site residential medical care.Priority Areas Not Addressed:Taking existing hospital and community resources into consideration, St. Helena Hospital Napa Valley will not directly address the remaining health needs identified in the CHNA including: education, economic and housing insecurity, and substance use. The hospital cannot address all the health needs present in the community, therefore, it will focus efforts on the health needs that can most effectively be addressed given the organization's areas of focus and expertise. St. Helena Hospital Napa Valley will look for partnership opportunities that address needs not selected where it can appropriately contribute to address those needs, or where those needs align with current strategy and priorities.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part I, Line 7:The costs were determined by using a cost-to-charge ratio. The cost-to-charge computation is based on Hospital specific data included in the system-wide combined audited financial statements. The formula used for computation equals financial statement data as follows:Total expenses - (Provision for bad debts + Other revenue + Interest income)/Gross patient chargesThe Hospital is located in a medically underserved area and participates in a quality assurance fee program with the State of California to fund certain Medi-Cal coverage expansions. The state redistributes funds to hospitals that provide patient care to a higher proportion of indigent and medically underprivileged patients, who otherwise would most likely not have access to physicians and other medical services. The community benefit analysis includes receipts from this redistribution that are used to assist in partially offsetting the significant costs associated with providing patient care to this population group. The program may or may not continue in the future based on the State of California's regulations and policies and the approval of the federal government.
Part II, Community Building Activities:The hospital is involved in numerous community building activities which promote the health of the communities it serves. Numerous community concerns are addressed, including health improvement, education, poverty, workforce development and access to care. We also encourage our employees to serve on community collaboration boards, health advocacy programs, and physical improvement projects to promote the health of the communities we serve. In addition, we work with neighborhood programs, including schools, work sites and safety net providers to promote health and wellness and prevent disease. These activities are not included elsewhere on Schedule H.
Part III, Line 2:Uncollected patient accounts are analyzed using written patient financial services policies that apply standard procedures for all patient accounts. The result of the analysis is what is recognized as bad debt expense. For example, all self-pay patients receive a discount. If the discounted account is unpaid after collection efforts, the unpaid balance is classified as bad debt. The cost-to-charge ratio described for Part I, Line 7 is multiplied times the Hospital's bad debt expense. The resulting figure has been reported as bad debts at cost on Part III, Line 2.
Part III, Line 3:The portion of the bad debts attributed to charity care as reported on Part III, Line 3 was calculated by an independent third-party consulting firm. This is an estimate of additional charity care that would have been granted if patients had cooperated by furnishing family financial information. A statistically valid sampling of patient accounts written-off was evaluated. The evaluation used various factors to determine which patients would have been eligible for charity care. Had the Hospital obtained sufficient information from all patients who qualified for financial assistance, these additional accounts would have been recorded as charity care instead of bad debt.
Part III, Line 4:The system-wide audited combined financial statements do not contain a footnote describing bad debt expense.
Part III, Line 8:The Medicare cost report apportions the Hospital's costs on the basis of inpatient days and ancillary and outpatient charges to establish the costing methodology. Healthcare delivery by hospitals is a complex, highly regulated business in the United States. Healthcare unit cost inflation is driven by compliance with ever-expanding regulatory requirements, shortages of highly skilled labor and evolving medical and information technology. The health care "market basket" is unrelated to that of the average individual consumer.Since the 1997 Balanced Budget Act, Medicare annual payment updates have fallen behind actual healthcare cost inflation to the point that Medicare payments too many U.S. hospitals are well below the cost of providing care. These unreimbursed costs are a community benefit for seniors and others in the community as these individuals are continuing to receive care without which many would become dependent on other governmental resources such as Medicaid. The benefit to the community for healthier uninsured and underinsured patients who are eligible for partial and full charity care. Medicare is a safety net for seniors and others. Without Medicare coverage, many individuals would undoubtedly qualify for charity care. In addition to the mismatch between Medicare payment increases and healthcare cost inflation, the highly complex Medicare payment systems and formulas produce disparate payment levels from one hospital to another for the same service. These disparate payment levels create disparate results within groups of hospitals. Reconciliation of Medicare Revenue from the hospital's Medicare Cost Report to GL:*Medicare Cost Report Revenue 82,998,993Prior Year Settlements 624,725Cost Report Reimbursable Bad Debts 574,945Estimates and Accrual Variances (52,861)Other 8,600,343Total Medicare Revenue 92,746,145*Note: The Medicare Cost Report revenue does not include the bad debt reimbursement. The Cost Report revenue does include the patient co-pay and deductible amounts. Adding the bad debt reimbursement would have duplicated the revenue already accounted for in the co-pay and deductible amounts.
Part III, Line 9b:When a patient has requested screening for charity care, the hospital must immediately cease collection activity and place the account in a charity pending status. If 100% charity is approved, the entire account balance is written off to charity care. If the patient has a sliding scale liability based on the federal poverty guidelines, they are billed only for that liability. If the patient fails to pay their after-charity liability, they are assigned to a collection agency with an identifier that indicates to the agency that the patient is "low income and the following criteria must be followed by the agency:1. They may not report the patient to a credit bureau2. They may not file a lawsuit to recover the outstanding liability3. They may not charge interest
Part VI, Line 2:The hospital's 2019 CHNA, the 2020 Community Health Plan (CHP) Update for fiscal year 2019, and the 2020 Implementation Strategy (adopted in May 2020) are posted on the hospital's website at:https://www.adventisthealth.org/st-helena/about-us/community-benefit/The two most recent Community Health Needs Assessments, two most recent Implementation Strategy reports and the and the most recent Community Health Plan Updates are also available on the Adventist Health Corporate website at:https://www.adventisthealth.org/about-us/community-benefit/In addition to the CHNA, our hospital engages in a process of continuous quality improvement, focusing on questions such as: are our interventions making a difference in improving health outcomes; are we providing the appropriate resources in the appropriate locations; and how are we using technology to track our health improvements and provide relevant feedback at the local level? Building a healthy community requires multiple stakeholders working together with a common purpose. With that in mind, our hospital leadership serves on a variety of community boards and committees focused on addressing community-specific needs, allowing for ongoing responsiveness to the health care needs of the community and collaboration with local agencies and organizations, enabling maximum effectiveness through collective impact.
Part VI, Line 3:The plain language summary of the Financial Assistance Policy (FAP) is posted along with the complete FAP policy and FAP Application on the hospital's website at:https://www.adventisthealth.org/documents/financial-assistance/ENG_PFS-112_Financial-Assistance-Policy.pdf. These documents are available in multiple languages. At the time of registration, patients who are uninsured and underinsured are provided information about government healthcare programs. Patients are also orally informed of their right to request charity assistance. Signs are displayed in the patient business office, patient registration areas and the emergency room in multiple languages informing patients of this right as well. The hospital also provides a brochure during the registration process that explains the hospital billing and collection procedures, and how to request financial assistance. In addition, every billing statement sent to patients contains information on how to request financial assistance.
Part VI, Line 4:St. Helena Hospital Napa Valley service area is comprised primarily of communities in Napa and Solano counties. The major communities are the upper valley cities of St. Helena and Calistoga. The demographics of the communities we serve are wide scope, including urban, suburban, and rural, and from all income levels. Although we primarily serve those in the Napa and Solano counties, our destination services also bring us patients from larger and further demographics including Lake County. Napa County has a population of 140,314 with a median household income of $88,457. At the end of the 2017 school year, testing for fourth graders found that far more students scored not proficient or worse on standardized reading testing, than 'proficient or better in Lake County (76.4%), this average was higher than AHSH PSA (72.6%) and Napa County (59.1%). Across the two-county region, Napa County has a lower percentage of populations receiving Public Assistance Income at 1.7% and SNAP benefits at 5.1% as compared to Lake County and the state estimate of 3.6% and 11.2%, respectively. Napa County demonstrated a higher proportion of woman receiving prenatal care and adequate care at 79.7% in comparison to Lake County at 68.5%. Notably, Napa County estimates meet the Healthy People 2020 performance target of 77.9% and 77.6%, respectively. AHSH PSA Medicare population has the lowest rates of depression (16.4%) and heart disease (24.4%) as compared to Lake County (16.6% and 25.8%, respectively). AHSH PSA had higher rates of diabetes as compared to Lake and Napa County (21.6% and 22.2%, respectively). Food insecurity averages in Lake County for the overall population (15.6%) and children (23.2%) are higher than reported averages for the state (11% and 18.1%, respectively). These averages are higher than Napa County.
Part VI, Line 5:Our hospital's mission is, "Living God's love by inspiring health, wholeness and hope." Our community benefit work is rooted deep within our mission and merely an extension of our mission and service. We have also incorporated our community benefit work to be an integral component of improving the "triple aim." The "Triple Aim" concept broadly known and accepted within health care includes:1) Improve the experience of care for our residents.2) Improve the health of populations.3) Reduce the per capita costs of health care. Our strategic investments in our community are focused on a more planned, proactive approach to community health. The basic issue of good stewardship is making optimal use of limited charitable funds. Defaulting to charity care in our emergency rooms for the most vulnerable is not consistent with our mission. An upstream and more proactive and strategic allocation of resources enables us to help low income populations avoid preventable pain and suffering; in turn allowing the reallocation of funds to serve an increasing number of people experiencing health disparities.Hospitals and health systems are facing continuous challenges during this historic shift in our health system. Given today's state of health, where cost and heartache is soaring, now more than ever, we believe we can do something to change this. These challenges include a paradigm shift in how hospitals and health systems are positioning themselves and their strategies for success in a new payment environment. This will impact everyone in a community and will require shared responsibility among all stakeholders. As hospitals move toward population health management, community health interventions are a key element in achieving the overall goals of reducing the overall cost of health care, improving the health of the population, and improving access to affordable health services for the community both in outpatient and community settings. The key factor in improving quality and efficiency of the care hospitals provide is to include the larger community they serve as a part of their overall strategy. Population health is not just the overall health of a population, but also includes the distribution of health. Overall health could be quite high if the majority of the population is relatively healthy even though a minority of the population is much less healthy. Ideally such differences would be eliminated or at least substantially reduced.Community health can serve as a strategic platform to improve the health outcomes of a defined group of people, concentrating on three correlated stages:1) The distribution of specific health statuses and outcomes within a population; 2) Factors that cause the present outcomes distribution; and 3) Interventions that may modify the factors to improve health outcomes.Improving population health requires effective initiatives to: 1) Increase the prevalence of evidence-based preventive health services and preventive health behaviors, 2) Improve care quality and patient safety, and 3) Advance care coordination across the health care continuum. We will work together with our community to ensure the community health improvements are identified and then targeted for programs to influence behaviors to obtain improved health within the whole community.
Part VI, Line 6:The hospital is a member of Adventist Health, a health care system which provides healthcare services in diverse markets within the Western United States. A member hospital may share some services with other member hospitals in its geographic area, such as clinical, management and support services. Using today's technology, hospitals outside the geographic area are able to provide support through remote services such as telepharmacy and robotics surgery. The Corporate Office provides important shared administrative support for member hospitals' rural health clinics and home care agencies, quality of care, other clinical needs, financing and risk management, and shared clinical and financial information technology. As many experienced and new physicians search for alternatives to independent practice, there is also corporate administrative support for hospital affiliated medical groups that engage physicians through employment or other contracts. This provides stability and growth of qualified physicians across many specialties, which is very important to make healthcare services available and to maintain and improve health within the communities served by all member hospitals.aintain and improve health within the communities served by all member hospitals.

The Hospital operates as an affiliate of Adventist Health System/West.
Form 990, Part VI, Section A, line 7aThe Hospital's Bylaws define its Board of Directors to be the same individuals who are members of the Adventist Health System/West Board of Directors.
Form 990, Part VI, Section A, line 7bThe sole corporate member, Adventist Health System/West, must approve all changes to the Articles of Incorporation and Bylaws.
Form 990, Part VI, Section B, line 11bThis Form 990 including all supporting schedules was prepared by a public accounting firm, reviewed by the System Controller and Market Finance Officer, and shared by electronic communication with the Hospital's Board of Directors prior to filing.
Form 990, Part VI, Section B, line 12cDuring the first quarter of each year, the annual conflict of interest questionnaire is sent to board members, hospital officers, key employees and department directors for completion and signature. The questionnaire is accompanied by a letter of explanation, to illustrate examples of a conflict and to remind the recipient that if any perceived conflict should arise before the next annual questionnaire, he/she is to notify the CEO immediately. The Corporate General Counsel distributes, collects, and reviews for signatures the COIs for all applicable Corporate employees and the board. In addition, the hospital's administration is responsible for keeping record to ensure all hospital-based individuals with director and above positions have submitted their COIs. The Corporate General Counsel reviews all the disclosures on the board and Corporate employees' COIs. Corporate General Counsel retains the COIs for the board members. The officers and key employees' COIs are kept in their personnel files. Further inquiries on any potential significant conflicts are made as needed. Conflicts are documented and reviewed with the Board.
Form 990, Part VI, Section B, line 15The Hospital's Board of Directors has established a Human Performance Committee to oversee the executive compensation program. This committee is composed of independent directors with no conflicts of interest. The committee performs the following functions: recommends a total compensation philosophy to the board; assures compliance with the board-approved philosophy; meets annually to review comparability data from outside consultants; recommends any adjustments to current executive compensation, including salary ranges for hospital presidents and finance officers that would be indicated by the data; evaluates executive performance against annual goals; recommends appropriate incentive awards to the board for approval; follows a diligent process that meets regulatory requirements for a rebuttable presumption of reasonableness; records committee deliberations and decisions in timely minutes; selects, engages and supervises any consultant hired to advise and provide comparability data. The board-approved executive compensation philosophy specifies that salary ranges will be established for hospital executives, with midpoints aligned with the 50th percentile of comparable system hospital data, and having a 50 percent spread from minimum to maximum. A hospital CEO has a maximum potential incentive of 30 or 35 percent of base salary based upon size and scope of hospital (incentive potential is industry norm). Other hospital executives have a maximum potential incentive of 25 to 30 percent of base salary.
Form 990, Part VI, Section C, line 19The Hospital does not make its governing documents publicly available, beyond required filings of Articles of Incorporation with the Secretary of State. The Hospital does not make its Conflict of Interest Policy available upon request, but does file monthly/quarterly summary financial reports with the state health-planning agency.
Form 990, Part VII:In recognition of the time commitment directors make to serve on the Adventist Health System/West boards and committees, directors receive compensation when their employment agreements with their employers allow such payments. For those whose employers do not allow accepting compensation for board service, the funds are directly distributed to charitable organizations as mandated by their employment agreements. In addition, amounts paid for taxable benefits are also reported.
