ASC Subtopic 740-10 also provides guidance on derecognition, classification, interest and penalties on income taxes, and accounting in interim periods and requires increased disclosures. As of December 31, 2021 or 2020, The Medical Center does not have a liability for unrecognized tax benefits. The Medical Center is a not-for-profit corporation as described in Section 501(c)(3) of the Internal Revenue Code and is exempt from federal income taxes on related income pursuant to Section 501(a) of the Code.

THOSE FOUR PROCESSES ARE COMMUNITY HEALTH STATUS ASSESSMENT (CHSA): FORCES OF CHANGE ASSESSMENT (FOCA); LOCAL PUBLIC HEALTH SYSTEM ASSESSMENT (LPHSA); COMMUNITY THEMES & STRENGTHS ASSESSMENT (CTSA); THESE FOUR ASSESSMENTS WERE USED TO DEVELOP THE KANKAKEE COUNTY DATA SHARING/STRATEGIC PRIORITIES. RIVERSIDE LEADERSHIP AND PARTICIPATION MORE THAN TEN REPRESENTATIVES FROM RIVERSIDE PARTICIPATED IN THE ASSESSMENT AND PLANNING PROCESS INCLUDING MATT MCBURNIE, VICE PRESIDENT, WHO SERVES ON THE STEERING COMMITTEE TO GUIDE THE PARTNERSHIP.

Schedule H, Part V, Section B, Line 11 Facility A, 1Facility A, 1 - RIVERSIDE MEDICAL CENTER. The 2021 Community Health Needs Assessment Plan was developed following the MAPP process over more than two years with a broad variety of community stakeholders. Riverside helped fund the assessments and participated as a leader on the steering committee and with strong expert representation across the various assessments. Riverside's website (www.riversidehealthcare.org) displays the full 2021 Community Health Needs Assessment, the associated four assessments that informed the plan, and Riverside Medical Center's approved three-year Implementation Plan. The Partnership for a Healthy Community utilized the MAPP process to identify three prioritized strategic areas to improve residents' health. These priorities are: I. Health and Wellness II. Behavioral Health III. Education and Employment. Riverside's Implementation Plan includes: Priority 1: Health and Wellness: Prevention and Primary Care: Riverside will increase access to care in the community by actively recruiting primary and specialty care providers to the growing 150-member Riverside Medical Group, educate and encourage preventative services for patients, and leverage technology for greater accessibility and efficiency through tools like MyChart and video visits to enhance patient/doctor communication and accessibility. Social Determinants: Riverside will continue to improve its positive impact through local sourcing which includes development and funding of health career ladders, by hiring locally when possible, and with place-based investing. We will continue to meet the needs of the community and region by resourcing the county's only clinic- based Rx Food Pantry and through providing the Rx Mobile Food Pantry monthly to the community. In partnership with the Northern Illinois Food Bank, Riverside will explore additional sites in our service area to offer this healthy resource. Chronic Disease: As the region's leader in services treating the most prolific chronic diseases, Riverside will further support the area's largest multi-disciplinary cardiovascular team and most advanced technology including cardiac catheterization labs. In 2022 we will increase advanced cancer care treatment accessibility with the purchase of a state of the art linear accelerator. Further, Riverside will continue its commitment to stroke prevention and care from our neurologists and neurosurgeons. Also, Riverside will continue to lead the community in diabetes education, treatment, and prevention efforts and with programs that encourage healthy lifestyles and reduce obesity in children and adults. Priority #2: Behavioral Health: Substance Use: Riverside will continue as a leader in securing grant funding for substance abuse initiatives. In collaboration with community partners, we will continue to lead the opioid task force to address prevention as well as provide NARCAN to community members. Through this task force, community members will be educated on the impact of substance use and abuse. Riverside will provide substance abuse specific services to meet the needs of community partners such as probation and drug court as well as community members. Mental Health: Riverside is committed to making Mental Health services more accessible to the community. We will continue to provide assessments and referrals to the community linking individuals with the appropriate type of treatment and level of care. Riverside will increase the availability and access of services and treatment modalities offered through inpatient and outpatient services through continued recruitment and streamlining of services and with strong coordination with community partners who offer adjacent services. Riverside will offer professional development and trainings to community and team members to remain current on appropriate interventions to address treatment needs. Trauma Awareness and Prevention: Riverside will continue to provide trauma- informed care services, through inpatient and outpatient programs and trauma training opportunities to community and team members to increase awareness and implement evidence-based interventions. We will partner with community providers and first responders to address their staff education and training needs to improve trauma-sensitive interventions and assessments. Riverside will host annual educational events to increase awareness regarding mental health and trauma as well as prevention. Priority #3: Employment and Education Job Skill: As the region's largest employer, Riverside will invest in job skill training for employees via internal programs and through philanthropic and organizational scholarships for formal education and certifications. Employability: Riverside will continually align our job orientation and ongoing training to improve the service level of all staff for the betterment of our patients and as an investment in the local workforce. Additionally, Riverside will continue to implement pay increases as market appropriate and consistent with job titles and duties. When possible, annual pay increases are given to all staff to address cost of living increases. Riverside has a program to scholarship staff with ongoing continuing education, further enhancing their service delivery when part of our staff and advancing their training level overall. Career Ladder: Riverside will work collaboratively with local K-12 education and higher education institutions to promote and coordinate practical, dynamic career ladders. Further, Riverside will leverage these career ladders to encourage local students to consider health careers. This includes partnership with our local high schools, the Kankakee Area Career Center, Kankakee Community College, and Olivet Nazarene University. Conclusion: It should be noted that Riverside Healthcare's broad support of the Community's Health Needs occurs every day as we pursue our mission. While we will give focus to the three prioritized areas identified by this process, Riverside will further support the community's health with the following major areas of support: 1. Annual Community Benefit, which in 2021 totaled more than $54.7 million. 2. Riverside's funding of and leadership for the CHNA and its Action Teams 3. Riverside's commitment to quality care, which regularly results in national recognition and benefits every patient through every service.

Schedule H, Part I, Line 7g Subsidized Health ServicesNET COMMUNITY BENEFIT EXPENSE FROM PHYSICIAN CLINICS WAS $17,056,116.
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceTHE HOSPITAL USES A COST ACCOUNTING SYSTEM FOR ALL PATIENT SEGMENTS, INCLUDING INPATIENT, OUTPATIENT, EMERGENCY ROOM, PRIVATE INSURANCE, MEDICAID, MEDICARE, UNINSURED, OR SELF PAY. IN ARRIVING AT THE AMOUNTS REPORTED IN SCHEDULE H, PART 1, LINE 7, THE HOSPITAL USED A COST TO CHARGE RATIO, DERIVED FROM WORKSHEET 2, RATIO OF PATIENT CARE COST-TO-CHARGES.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountBad debt expense is determined using the cost to charge ratio. Payments on patient accounts are a reduction of bad debt expense.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyNone of the amount the organization reported as bad debt was attributable to patients eligible under the organization's financial assistance policy.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteGenerally, patients who are covered by third-party payors are responsible for related deductibles and coinsurance, which vary in amount. The Medical Center also provides services to uninsured patients and offers those uninsured patients a discount, either by policy or law, from standard charges. The Medical Center estimates the transaction price for patients with deductibles and coinsurance and from those who are uninsured based on historical experience and current market conditions. In evaluating the collectibility of patients' accounts receivable, The Medical Center analyzes its past history and identifies trends for each of its major payor sources of revenue to estimate the variable consideration. Management regularly reviews data about these major payor sources of revenue in evaluating the variable consideration. The Medical Center uses a portfolio approach to account for categories of patient contracts as a collective group rather than recognizing revenue on an individual contract basis. The Medical Center believes that revenue recognized using this approach approximates the revenue that would be recognized if an individual contract approach were used. The initial estimate of the transaction price is determined by reducing the standard charge by any contractual adjustments, discounts, and implicit price concessions. Subsequent changes to the estimate of the transaction price are generally recorded as adjustments to patient service revenue in the period of the change.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsTHE COSTING METHODOLOGY USED IS COST TO CHARGE RATIO. THE COST TO CHARGE RATIO IS FIGURED BY TAKING THE TOTAL EXPENSES EXCLUSIVE OF BAD DEBT AND DIVIDING THAT BY GROSS PATIENT REVENUE AND OTHER OPERATING REVENUE. ANY SHORTFALLS THAT RESULT FROM MEDICARE ALLOWABLE COSTS EXCEEDING MEDICARE REVENUE CAN BE CONSIDERED A COMMUNITY BENEFIT IF THE ACTIVITY IS SOMETHING THAT the MEDICAL CENTER IS DOING TO CREATE THE AWARENESS AND WELL-BEING OF THE HOSPITAL COMMUNITY.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceWhen a patient has been approved under the hospital's Charity Care Policy for a partial discount, the hospital will work with the patient or responsible party to establish a reasonable payment plan. The plan will take into account available income and assets, the amount of the discounted bill(s), and any prior payments. The hospital will also advise the patient of his or her responsibilities under these collection practices guidelines. Riverside may offer prompt payment discounts to charity care patients who are eligible for partial discounts on their hospital bills. The hospital will not pursue legal action for non-payment of bills against charity care patients who have clearly demonstrated that they have neither sufficient income nor assets to meet their financial obligations. Legal action, including the garnishment of wages, may be taken by the hospital to enforce the terms of the payment plan. Such measures would only take place when there is evidence that the charity care patient or responsible party has sufficient income and/or assets to meet his or her obligation. The hospital will not place a lien on a charity care patient's primary residence. The hospital will not execute a lien by forcing the sale or foreclosure of a charity care patient's primary residence to pay for an outstanding medical bill. The hospital will not use body attachment to require the charity care patient or responsible party to appear in court, however Riverside recognizes that the court system may take this action independently. The hospital will ensure that the guidelines outlined above are followed by any external collection agency engaged to assist in obtaining payment on outstanding bills from charity care patients. Additionally, external collection agencies will identify patients that may qualify for charity care and will assist in communicating Riverside's financial assistance policy and mail applications to patients requesting assistance.

Each year the organization revisits the needs identified in the CHNA and documents progress made towards addressing these needs. For more information regarding these assessments, please see Schedule H, Part V, Section B, Line 11.

Examples include, but are not limited to: Signage Brochures Placing a note on the hospital bill and statements regarding how to request information about financial assistance. This includes statements from our bad debt agency. Designated individuals who can explain the hospital's charity care policy, including a Spanish-speaking representative. Reference in our patient handbook Notice on in-house television Lobby Kiosks Website Employee Charity Lesson Riverside also screens identified patients for eligibility for various federal, state or local government programs.
