Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Schedule H, Part V, Section B, Line 5-CRAIG HOSPITALKey informant interviews were conducted with subject matter experts and community partners. These interviews provided qualitative information for primary data analysis. Organizations participating in these structured interviews are: Brain Injury Alliance of Colorado (BIAC), Gavin Atwood; CCAD/adapt, Josh Winkler, Chanda Plan, Chanda Hinton; Colorado RSVP Clinic, Jeff Berliner, MD; Easterseals Colorado, Roman Krafczyk; Home Builders Foundation, Beth Forbes, and Personal Assistance Services of Colorado (PASCO), Home Health and Community Based Services, Damian Rosenberg. Colorado Health Institute (CHI) conducts the Colorado Health Access Survey (CHAS) every other year to survey health care coverage, access and utilization in Colorado. Questions from the CHAS were used on the Craig Hospital survey to compare its population with CHAS' data from the 2017 survey. CHI reviewed the draft survey of Craig patients and provided feedback to improve the survey format. CHI provided comparative data from the CHAS for the general population of Colorado and a subset identified as disabled. Rebecca Silvernale, director of operations, was the CHI staff member who provided the consultation. Colorado Department of Public Health and Environment (CDPHE) collects health statistics in Colorado obtained from the Behavioral Risk Factor Surveillance System questionnaire (BRFSS). This questionnaire has been used by the state to develop a public health improvement plan. Data from the 2017 BRFSS survey was provided on specific questions for the general and disabled population and compared with Craig survey responses. Jackie Gardner, population health data analyst with CDPHE is the staff member who provided the data on behalf of CDPHE. A team of three independent public health professionals provided consultation on primary data collection and reviewed and made recommendations on the final CHNA report. These three individuals have over 50 years of combined experience in public/population health, health care, data collection and analysis, and community health. The team members are Caila Aube, MA; Eliza lanman, MS; Amy Pulver, MBA, MA.
Schedule H, Part V, Section B, Line 11-CRAIG HOSPITALCraig Hospital completed our most recent CHNA survey in September 2019. The CHNA identified two high priority health needs: access to care and social/emotional wellness and two medium priority needs: healthy eating/active living (heal) and independence. We are working to address the two high priority needs identified and do not plan to work on the two medium priority needs at this time. Access to care - people with disabilities have higher rates of many chronic health conditions, including arthritis, asthma, cardiovascular, diabetes, high blood pressure, and high cholesterol when compared to those without a disability. Those living with physical disabilities often face enormous difficulty finding medical practices with wheelchair-accessible equipment or clinicians who are cognizant of their unique health care needs and risks which can lead to life-threatening infections. Further, individuals with disabilities often encounter professionals unprepared to identify and treat their primary and secondary conditions and any other health and wellness concerns as health care providers often focus on their disabling condition rather than on other health issues that might be of concern to the individual. Craig Hospital's aim is to reduce barriers to healthcare, promote wellness, and prevent injury and disease. This will be done by implementing the following strategies: increase knowledge of gaps in preventative care for the BI/SCI community and identify and collaborate with community partners and programs already working to address the identified gaps; serve as a consultant to clinics in our community to make their environment, facilities, and services more accessible and inclusive; educate healthcare providers on how to best treat those living with BI/SCI. Social/emotional wellness - national data (National Health Interview Survey - NHIS) show that 14.4 percent of adults 18 years of age or older with disabilities experienced serious psychological distress; the healthy people 2020 target is a 10 percent improvement to 13 percent. Social relationships play an important role in the mental health and wellbeing of people living with BI/SCI. The community served by Craig Hospital is particularly impacted by increased social isolation after their injury. The goal is to reduce isolation, increase social-emotional support for our community, and support increased quality of life opportunities. The plan is to achieve this goal by creating a pipeline for individuals living with BI/SCI to serve in positions of influence that can impact accessibility of physical, cultural, and institutional environments; develop and support internal and external programs that support engagement in meaningful activities with the BI/SCI community. This fiscal year was an extremely challenging year due to the COVID pandemic, however Craig Hospital continued to make strides on meeting our goals for improving access to care and addressing social/emotional wellness in our community. For access to care, Craig developed and conducted a six-part webinar series for health care professionals on caring for the psychological issues of those with a brain injury. The hospital also hosted community vaccination clinics for COVID-19. For social/emotional wellness, Craig completed the development of a community college program for students with acquired brain injuries and welcomed the first students in September 2021. Craig Hospital does not plan to address the two medium priority health needs: healthy eating/active living and independence. Focusing resources on the high priority health needs will allow the Craig CHNA committee to engage the entire Craig staff in the implementation as well as permit the team to build strong and targeted relationships with other community partners to achieve significant impact. In addition, because the issues identified in the CHNA process are interrelated, some of the activities planned to be undertaken that address the high priority needs may also address some aspects of the medium priority health needs. Other organizations in the community include healthy eating/active living in their CHNAs and community health improvement plans and present opportunities for Craig staff to collaborate with these organizations to provide Craig's unique perspective without taking on leadership of additional health needs.

Schedule H, Part I, Line 3cIn addition to the FPG, Craig Hospital evaluates providing discounted care based on the patient's asset level, medical indigence, the patients insurance status as well as the other factors described in schedule H, Part V1, Line 3 detailing the admissions departments evaluation of each new patient.
Schedule H, Part I, Line 7Costs were determined by using either actual costs or calculated costs based off of a cost-to-charge ratio.
Schedule H, Part I, Line 7, Column fDue to the COVID-19 pandemic, Craig Hospital limited the number of students/scholars/visitors allowed within the hospital to help mitigate the chance of spreading the virus as well as follow the restrictions imposed by the CDPHE and the State of Colorado.
Schedule H, Part III, Section A, Line 2The Hospital continued to follow the presentation and disclosure of patient service revenue, provision for bad debts and allowance for doubtful accounts as pronounced in the accounting standards updates 201107 and Accounting Standards Codification topic 954. This requires organizations that do not have access to the collectability of a receivable prior to recognizing revenue to present their provision for bad debt related to patient service revenue as a deduction from revenue, net of any contractual allowances and discounts on their statement of operations. The adoption on this did not impact the financial statements as the hospital does not recognize significant revenue without assessing a patient's ability to pay.
Schedule H, Part III, Section A, Line 3The Hospital does not consider any of its bad debt expense to be attributable to patient's eligibility under the organization's financial assistance policy.
Schedule H, Part III, Section A, Line 4There is no footnote in the audited financial statements that describes bad debt expense. Please refer to page 17 of the audited financial statements that addresses patients and uninsured payors.
Schedule H, Part III, Section B, Line 8The source of the data is the most recent filed Medicare cost report using the ratio of cost to charges method. The payment mechanism received for care provided to Medicare patients is not negotiated but set by federal regulations. The payment currently received for treatment is not sufficient to cover costs and although we don't recognize this shortfall as charity, we believe it should be included in the calculation of community benefit. All patients receive the appropriate level of care and services regardless of payment source.
Schedule H, Part III, Section C, Line 9aThere is not a specific written debt collection policy as those procedures are addressed in the financial assistance policy.
Schedule H, Part VI, Line 2In addition to conducting its CHNA every 3 years, the hospital assesses community needs through a variety of different programs. As a condition of federal funding, Craig Hospital's research department conducts ongoing longitudinal studies on the long-term effects and needs of persons with spinal cord injuries and/or brain injuries. Craig Hospital also operates a toll-free nurse advice line and collects data on why former patients are calling for assistance. Craig hospital collaborates with the Brain Injury Alliance of Colorado, operation TBI freedom, The Hangout, The Christopher & Diane Reeves Peer Network Program and other community organizations to assess and meet the long-term needs of persons with disabilities. Craig also developed two research funded pilot programs into hospital funded programs over the past year; the discharge program focuses on helping patients and families successfully transition from their rehabilitation program back into their home community and the peer mentoring program provides peer support to individuals with spinal cord injury.
Schedule H, Part VI, Line 3The admissions department identifies patients who have limited benefits and compare the benefits to the estimated length of stay. When there is a shortage of days allowed by the insurance, the admissions director meets with the patient and/or family to determine alternative coverage. This conversation encompasses the patient's ability to pay for the care, potential eligibility for federal and state programs and the hospitals charity care policy. Upon admission, the case manager completes a thorough psycho-social assessment of the patient/family specifically analyzing their financial situation. The case managers consider household income post-injury, family size, assets, current and future living expenses, and financial obligations including future care needs. The case managers, along with the patient/family, assess other potential resources/programs for which the patient may qualify. Resources and programs evaluated include but are not limited to Medicaid, victim's assistance, liability and liquidity, accidental death and dismemberment, and The Hospital's charity care program. If it appears as though the patient will qualify for state or federal programs, the case manager will assist the patient/family in completing the applications, obtaining the required documentation and advising them on how to position themselves to qualify. If patients do not meet the criteria for other state or federal programs, the financial assistance eligibility discount guideline is used to determine if the patient's bill will be discounted. If appropriate, the case manager will initiate and assist the patient/family in completing the financial assistance application for the charity care program. Applications may also be obtained on the hospital's website, completed over the phone or by mail. If it is not clear that the patient/family will have a balance prior to discharge (copays, coinsurance, etc.) the patient/family will receive information on the charity care program which is noted on the monthly billing statements and the copy of the application along with the letter for pre-collection. At this time the patient can complete the application. All applications are reviewed by the A/R manager, CFO and/or the controller. When it is determined that the charity application is appropriate, it is communicated to the patient/family by the case manager or the A/R manager.
Schedule H, Part VI, Line 4As one of the most highly-rated rehabilitation hospitals in the United States for spinal cord injuries (SCI) and brain injuries (BI), Craig treats patients from virtually every state including some from outside of the United States. Based in Englewood, Colorado, historically approximately 45 percent of all inpatients are from Colorado. The other top five states would include California, Oregon, Kansas, Missouri and Oklahoma. During the fiscal year, Craig treated inpatients from 41 states. Since becoming a specialty hospital in 1956, Craig has treated over 35,000 patients with an SCI or BI. With an average of 500 inpatients and 2,000 outpatients a year, approximately 45 percent of patients are treated for SCI and 47 percent of patients are treated for brain injury. The average age of Craig's patient population is 37, ranging from 15 to 89 years old and 80 percent of all patients are male coming from varied socio-economic backgrounds.
Schedule H, Part VI, Line 5Craig Hospital's mission is to provide exemplary patient care, education and research to people with spinal cord (SCI) and brain injuries (BI). The Board of Directors, consisting of 20 members of which 19 are independent, has ultimate authority in extending privileges only to professionally competent physicians, dentists and podiatrists, licensed to practice in the State of Colorado. There are 195 physicians on the medical staff of Craig Hospital, each assigned to one of four separate medical staff categories. The medical staff of Craig Hospital is organized and operates under the guidance of the medical staff bylaws, medical staff rules and regulations, and medical staff policies and procedures. The organized medical staff is accounting to the governing body for the quality of care, treatment, and services provided to patients by the practitioners with privileges. The medical staff is responsible for establishing and maintaining patient care standards, credentialing and delineation of clinical privileges. Additionally, there are approximately 72 allied health professionals, categorized as either independent or physician directed who function within one or more health related professions or occupations. The hospital uses surplus funds to fund capital improvements, equipment for patient programs, and patient and staff education. In the current fiscal year, Craig reinvested over $3.5 million back into the infrastructure of the hospital for building improvements, equipment and information technology. Craig provides significant support to the families of patients including providing 30 days of free housing for families of inpatients who live further than 60 miles from the hospital. Craig also provides significant support to the community, supporting the Brain Injury Alliance of Colorado, conducting free annual outpatient clinics in Pueblo, Grand Junction and Casper, Wyoming (only Casper, Wyoming being held this fiscal year due to the COVID pandemic). Our leaders serve on multiple boards for state agencies and organizations including the Chanda Center for Health, Brain Injury Alliance of Colorado, Denver Metro Chamber of Commerce, Access Unlimited, National Sports Center for the Disabled, and others. Craig financially supports many local, regional, and national organizations each year and collaborates on patient research and technology with several local colleges and universities. This fiscal year, Craig provided $182,000 in funding to local and national community partners. Craig is pioneering adaptive technology and cutting-edge treatment including operating the PEAK Center at Craig Hospital, a state-of-the-art wellness and fitness program for inpatients, out patients, and community members. In fiscal year 2021, over 320 individuals utilized the PEAK Center facilities, including 152 new members. Other programs that Craig provides to the community include the RSVP free clinic that is held the first Sunday of each month offering rehabilitative care to the underinsured, adaptive adventure trips and activities for Craig graduates, community blood drives, and community liaisons who travel to assess and educate patients and providers. Craig also hosts rotations for RN's from four local nursing schools, and offers and provides community outreach educational presentations to local hospitals and colleges on treatment of SCI and BI, and has a medical resident program for 2 medical school residents and 1 fellow which it maintained throughout the COVID 19 pandemic. Craig provides long-term community support through multiple programs including telehealth through its nurse advice line, free educational materials through the resource library on Craig Hospital's website, and social media including Facebook, Twitter, LinkedIn, YouTube, Instagram and a blog. Craig staff volunteer with numerous organizations like Camp Discover, Homebuilders Foundation, AMI Wellness Professional Advisory Committee, FVA Mountain States, and serve on the boards of hundreds of community organizations including the Brain Injury Alliance of Colorado, National Sports Center for the Disabled, Academy of SCI Professionals, American Congress of Rehab Medicine, Phamaly Theatre Company, Cathy Scott Foundation, Center for Women's Health Research, Colorado Hospital Association, and more. During the fiscal year, Craig provided financial assistance, community support and subsidized health services for government programs totaling $12.8 million. Craig also gives back to the SCI and BI communities through research that is changing the field of rehabilitative medicine. The Hospital's research department spent in excess of $5.6 million in federal, state and foundation-sponsored grants for SCI and BI rehabilitation research on 64 research projects with a staff of 27 employees. In 2016, Craig assumed responsibility of managing operation TBI Freedom (OTF), a military case management program located in Colorado Springs, Colorado. OTF provides support for veterans and active duty personnel with brain injuries through case management services including crisis management, medical and mental health referrals, employment/education assistance, psycho-social classes, and emergency assistance for immediate needs and services not covered by the VA. According to the Department of Defense 450,000 U.S. Service members have been diagnosed with a BI since 2000. Based on these numbers it is estimated that more than 10,000 have returned to Colorado. Since 2016 Craig Hospital's military programs have served more than 800 Service members and 1600 family members, with OTF specifically supporting more than 3,000 services members and 8,000 family members (since inception in 2008). What makes Craig unique is the team approach towards treatment and working together to empower patients to achieve their highest possible level of independence and quality of life. Craig has been recognized for the past 31 consecutive years as a top 10 rehabilitation hospital by U.S. News and world report and received the NDNQI award in 2009, 2012 through 2015, and 2020 for the highest quality outcomes in nursing care in a rehabilitation facility. In 2021, Craig Hospital also earned its 4th Magnet designation for excellence in nursing by the American Nurses Credential Center (ANCC) Magnet Recognition Program. The hospital is one of only two free standing rehabilitation hospitals to ever receive such recognition. Craig was voted by employees as a Top Workplace in Denver in 2012 through 2016, 2018 and 2020 and earned the Top Workplaces USA recognition in 2021.
Schedule H, Part VI, Line 6Craig Hospital is not part of an affiliated health care system.
