ExplanationHSHS and the Foundation are Illinois not-for-profit organizations as described in Section 501(c) (3) of the Internal Revenue Code (the Code) and are exempt from federal income taxes on related income pursuant to Section 501(a) of the Code. Kiara, Inc. is an Illinois for-profit corporation that recognizes income taxes under the asset-and-liability method. Deferred tax assets and liabilities are recognized for the future tax consequences attributable to differences between the consolidated financial statement carrying amounts of existing assets and liabilities and their respective tax basis and operating loss and tax credit carryforwards. Deferred tax assets and liabilities are measured using the enacted tax rates expected to apply to taxable income in the years in which those temporary differences are expected to be recovered or settled. The effect on deferred tax assets and liabilities of a change in tax rates is recognized in income in the period that includes the enactment date. The net operating loss carryforwards result in a deferred tax asset of $3,548 and $3,650 at June 30, 2022 and 2021, respectively. In assessing the realizability of deferred tax assets, management considers whether it is more likely than not that some portion or all of the deferred tax assets will not be realized. The ultimate realization of deferred tax assets is dependent upon the generation of future taxable income during the periods in which those temporary differences become deductible. Management considers projected future taxable income and tax planning strategies in making this assessment. Based upon the level of historical taxable losses and projections for future taxable losses over the periods for which the deferred tax assets are deductible, management believes it is more likely Kiara, Inc. will not realize the majority of the benefits of these deductible differences. Full valuation allowances have been applied against the deferred tax assets attributable to the net operating loss carryforwards not realized as of June 30, 2022 and 2021 in the accompanying consolidated financial statements due to the uncertainty of realization. HSHS recognizes the tax benefit from an uncertain tax position only if it is more likely than not the tax position will be sustained on examination by the taxing authorities, based on the technical merits of the position. As of June 30, 2022 and 2021, HSHS does not have any liabilities for unrecognized tax benefits.ExplanationTxt

Schedule H, Part V, Section B, Line 3EYes, the significant health needs are a prioritized description of the significant health needs of the community and were identified through the CHNA process.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - HSHS Sacred Heart Hospital. Hospital Sisters Health System and HSHS Sacred Heart Hospital are committed to promoting and defending human dignity, caring for persons living in poverty and other vulnerable persons, promoting the common good, and stewarding resources wisely. Consequently, we strongly believe the assessment process must be informed by input from the poor and vulnerable populations we seek to serve. During FY2021, HSHS Sacred Heart Hospital conducted the CHNA for Eau Claire County (WI) in conjunction with HSHS St. Joseph's Hospital, Marshfield Clinic Health System, Mayo Clinic Health System, Eau Claire City-County Health Department, Chippewa County Department of Public Health, United Way of the Greater Chippewa Valley, and Eau Claire Healthy Communities (a local health coalition). The CHNA partners in turn worked together to gather input from a broad array of sources, ranging from hospital systems and educational institutions to government and service agencies to community groups, as well as from area residents. These efforts included, but were not limited to, outreach to and through the Aging and Disability Resource Center, The Community Table, Eau Claire Area Hmong Mutual Assistance Association, Eau Claire County Jail, El Centro, Feed My People Food Bank, and Joining Our Neighbors Advancing Hope (JONAH), as well as churches, schools, municipalities, and other organizations. All of these organizations serve under-resourced individuals in our community - such as low-income seniors, children living in poverty, families who struggle with shelter and food insecurity, justice-involved individuals, and other vulnerable populations - and possess extensive knowledge regarding the needs of those they serve. Actively including these organizations in the CHNA process was critical to ensure consideration of diverse needs in both the assessment itself and the development of related implementation strategies. The FY2021 CHNA process involved several key components. These included a Community Health Survey (distributed via SurveyMonkey and paper copies), online Community Conversations with area residents and stakeholders, and a virtual discussion with members of Eau Claire Healthy Communities. The feedback collected through these means, in combination with secondary data comparisons of local and national health indicators, were used to identify community health needs and priorities. The hospital subsequently used this data to develop a community health implementation plan incorporating health improvement strategies. The health priorities identified in the CHNA report were formally approved by the HSHS Sacred Heart Hospital Board of Directors on May 20, 2021.
Schedule H, Part V, Section B, Line 6a Facility , 1Facility , 1 - HSHS Sacred Heart Hospital. Yes, the FY2021 CHNA was conducted with sister-hospital HSHS St. Joseph's Hospital, Chippewa Falls, WI, and Mayo Clinic Health System and Marshfield Clinic Health System of Eau Claire, WI.
Schedule H, Part V, Section B, Line 6b Facility , 1Facility , 1 - HSHS Sacred Heart Hospital. HSHS Sacred Heart Hospital also conducted the FY2021 CHNA in conjunction with the Eau Claire City-County Health Department, Chippewa County Department of Public Health, United Way of the Greater Chippewa Valley, and Eau Claire Healthy Communities.
Schedule H, Part V, Section B, Line 7 Facility , 1Facility , 1 - HSHS Sacred Heart Hospital. Hard copies of the CHNA are available free of charge through the hospital's Community Health Department, as well as through the community health coalition, Eau Claire Healthy Communities.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - HSHS Sacred Heart Hospital. As detailed in the FY21 CHNA Report, HSHS Sacred Heart Hospital, in collaboration with Eau Claire Healthy Communities and other community partners, identified several health priorities for Eau Claire County. These included: * Drug Use * Mental Health * Alcohol Misuse * Obesity * Healthy Nutrition. Due to close collaboration with HSHS St. Joseph's Hospital and the Chippewa Health Improvement Partnership in Chippewa County (which is contiguous to Eau Claire County), as well as overlapping service areas and resources and the interrelationship between these issues, HSHS Sacred Heart Hospital further refined the focus of its efforts to: * Mental Health * Alcohol Misuse * Chronic Disease Prevention and Management * Drug Use * Healthy Nutrition/Obesity. Mental Health HSHS Sacred Heart Hospital in Eau Claire (SHEC) has engaged in multiple efforts to improve the mental health and emotional wellbeing of residents of the area that the hospital serves. The SHEC Community Health Implementation Plan includes several evidence-based strategies and other actions to positively impact mental health. These strategies will benefit both those we serve as patients and the community at large. Strategy 1: Work with community partners to increase access to prevention and early intervention services. Action 1: Train and partner with facilitators to provide suicide prevention, behavioral health "first aid, and trauma/resiliency training to school staff, students, and the general public. FY2022 Outcomes Due to continued COVID-19 concerns, training remained on the backburner during FY2022, although those efforts have since begun to resume. Nonetheless, the Community Health Outreach Specialist took an active role in Mental Health Matters, a collaborative, grant-funded effort between Chippewa and Eau Claire counties to promote youth mental health. During FY2022, she provided leadership to the Education and Awareness Team, which worked with a local production company to develop a 30-minute training video on adverse childhood experiences and resiliency (ACE/R). SHEC also provided funding to help meet unbudgeted expenses related to video edits. The ACE/R video is in final production and will be ready for public showing in the near future. In order to better understand the evidence-based curricula Question, Persuade, Refer and Mental Health First Aid and help prepare for future instructor certification, the Community Health Outreach Specialist also participated in community-level trainings for each of these courses. Finally, the Community Health Outreach Specialist actively participated in the Eau Claire Healthy Communities Mental Health Action Team, which also discussed training and other suicide prevention strategies. Action 2: Work with community partners to ensure early identification of pregnant and postpartum moms with behavioral health needs. FY2022 Outcomes The Community Health Outreach Specialist facilitated quarterly meetings of the Substance-free Pregnancy and Recovery Coalition (SPaRC) for Eau Claire and Chippewa counties. The coalition involves a broad range of state and local government agencies, health care partners, and service organizations and seeks to stop the intergenerational impacts of substance use by identifying the behavioral health needs of pregnant and postpartum women and connecting them with available resources. Screening recommendations and tracking data were discussed internally with HSHS and Prevea colleagues. Additionally, SHEC's Women and Infants Department offers the Maternity onto Motherhood (MOM) Program during pregnancy and after birth to help connect families with needed resources, including mental health and substance use services. Prenatal providers refer families to the program, and the coordinator initiates contact during the second trimester of pregnancy. Strategy 2: Work with internal and external partners to improve access to care and services. Action 1: Expand inpatient psychiatric bed capacity for adults. FY2022 Outcomes SHEC learned in FY2022 that it would receive $15 million in state funding to help expand its ability to provide local inpatient psychiatric care. This funding will be used in part to create a separate, 18-bed adolescent psychiatric bed unit at HSH St. Joseph's Hospital in Chippewa Falls, allowing HSHS Sacred Heart to dedicate its current psychiatric unit to adult care and expand the total number of beds for the latter population from 29 to 33. The contract for this funding recently was signed, paving the way for further planning in FY2023. Action 2: Continue to partner with Prevea Health to provide/expand school-based mental health services. FY2022 Outcomes SHEC worked through its Foundation and outpatient behavioral health partner, Prevea Health, to provide school-based mental health services in Eau Claire County. Action 3: Provide grief support to Chippewa Valley residents through The Healing Place. FY2022 Outcomes SHEC resumed grief support services on a limited basis during FY2022, balancing requests for these services with ongoing concerns about transmission of COVID-19. The Community Health Department worked with two previous group facilitators to resume two groups in November: Suicide Loss and Loss of a Child. Both groups were subject to masking, social distancing, and other precautions to help safeguard the health of participants while meeting in person off hospital grounds in November and December. The groups then reverted to virtual meetings in January and February due to increased transmission levels; however, the groups then disbanded due to declining interest in meeting online. In addition to the facilitators, five unique individuals attended the Suicide Loss support group in November and December, while three other community members attended the Loss of a Child support group during those months. Meanwhile, six different individuals attended one of the Suicide Loss support groups in January and February, while only two community members participated in the Loss of a Child group during those months. Neither group met in March due to lack of interest in this format. Finally, the Community Health Outreach Specialist fielded calls and e-mails weekly from individuals seeking grief support for themselves or others and provided resource information. Action 4: Streamline behavioral health service access for pregnant and postpartum moms. FY2022 Outcomes The Community Health Outreach Specialist met with the Executive Director of Behavioral Health for HSHS Sacred Heart Hospital and St. Joseph's Hospital to discuss ways to streamline services for pregnant and postpartum women; these discussions will continue in FY2023. As noted above, the MOM Program Coordinator also works with moms/families to connect them with needed resources. Strategy 3: Work with community partners to address other social determinants of health (SDOH), including economic stability. Action 1: Provide funding and other support to homeless resources to help promote housing and economic stability, as well as facilitate access to health care services. FY2022 Outcomes SHEC participated in multiple efforts within Eau Claire County to support individuals struggling with housing insecurity. Activities included: - Providing funding to help purchase an AED unit for the new, temporary Warming Center in Eau Claire. This center provided a safe, warm place for individuals who lack housing to go during the day when local shelters are closed. - Providing funding for the Salvation Army's "Lighten Your Load" program, which offers laundromat gift cards to homeless and other low-income individuals who lack access to laundry facilities. SHEC also worked to secure support for the program from other local health systems. - Participating in a Day Resource Center Steering Committee to help plan for a permanent, full-time replacement for the temporary Warming Center in Eau Claire. As envisioned, this center will help facilitate access to health care and other needed services. - Participating in the Dairyland Housing Committee Gaps Committee to identify and prioritize other needs (e.g., transportation and bathroom access) for unhoused individuals and pursue solutions.
Schedule H, Part V, Section B, Line 11 Facility , 2Facility , 2 - HSHS Sacred Heart Hospital. Alcohol Misuse HSHS Sacred Heart Hospital in Eau Claire (SHEC) also included strategies for addressing alcohol misuse in both its patients and the general public through the Community Health Implementation Plan. Strategy 1: Work with community partners to increase access to prevention and early intervention services. Action 1: Work with schools and other community partners to provide Mental Health First Aid training for youth and adult populations. FY2022 Outcomes: Due to continued COVID-19 concerns, training remained on the backburner during FY2022, although those efforts have since begun to resume. Nonetheless, the Community Health Outreach Specialist participated in community-level training in Mental Health First Aid (both Adult and Youth) in order to better understand this evidence-based curricula in preparation for future certification as an instructor. Action 2: Work with the Mental Health Matters grant project to provide training on adverse childhood experiences and resiliency (ACE/R) to school staff and other organizations. FY2022 Outcomes: As noted above, trainings large remained on the backburner during FY2022 due to COVID-19 concerns; however, the Community Health Outreach Specialist took an active role in Mental Health Matters, a collaborative, grant-funded effort between Chippewa and Eau Claire counties to promote youth mental health. During the fiscal year, she provided leadership to the Education and Awareness Team, which worked with a local production company to develop a 30-minute training video on adverse childhood experiences and resiliency (ACE/R). SHEC also provided funding to help meet unbudgeted expenses related to video edits. The ACE/R video is in final production and will be ready for public showing in the near future. Action 3: Work with the Eau Claire Healthy Communities (ECHC) High-Risk Drinking Prevention Action Team and Alliance on Substance Abuse Prevention (ASAP) to provide community education and engage in other prevention activities. FY2022 Outcomes: The Community Health Outreach Specialist participated in the ECHC High-Risk Drinking Action Team and Alliance on Substance Abuse Prevention (name since changed to the Alliance on Substance Misuse Prevention; also known simply as the Alliance), each of which worked extensively on strategic planning during this time period. Due to the loss of its chair and a degree of duplication between these two entities, the work of the High-Risk Drinking Action Team eventually reverted to the Alliance. Action 4: Work with community partners to ensure early identification of pregnant and postpartum moms with behavioral health needs. FY2022 Outcomes: The Community Health Outreach Specialist facilitated quarterly meetings of the Substance-free Pregnancy and Recovery Coalition (SPaRC) for Eau Claire and Chippewa counties. The coalition involves a broad range of state and local government agencies, health care partners, and service organizations and seeks to stop the intergenerational impacts of substance use by identifying the behavioral health needs of pregnant and postpartum women and connecting them with available resources. Prenatal providers refer families to the program, and the coordinator initiates contact during the second trimester of pregnancy. Strategy 2: Work with internal and external partners to improve access to care and services. Action 1: Ensure consistent use of Screening, Brief Intervention, and Referral to Treatment (SBIRT) by providers. FY2022 Outcomes: The Executive Director of Behavioral Health for HSHS Sacred Heart Hospital and HSHS St. Joseph's Hospital and the Community Health Outreach Specialist began discussions around SBIRT implementation. This topic will be revisited during FY2023. Action 2: Streamline behavioral health service access for pregnant and postpartum moms. FY2022 Outcomes: The Community Health Outreach Specialist met with the Executive Director of Behavioral Health for HSHS Sacred Heart Hospital and St. Joseph's Hospital to discuss ways to streamline services for pregnant and postpartum women; these discussions will continue in FY2023. As noted above, the MOM Program Coordinator also works with moms/families to connect them with needed resources. Action 3: Continue partnership with Prevea to provide/expand school-based mental health services. FY2022 Outcomes: SHEC worked through its Foundation and outpatient behavioral health partner, Prevea Health, to provide school-based mental health services in Eau Claire County. Strategy 3: Work with community partners to address other social determinants of health (SDOH), including economic stability. Action 2: Work with the Eau Claire Healthy Communities (ECHC) High-Risk Drinking Prevention Action Team and Alliance for Substance Abuse Prevention (ASAP) to provide alcohol-free events, activities, and opportunities throughout the county. FY2022 Outcomes: As noted above, the work of these two groups primarily centered around strategic planning during the fiscal year. Chronic Disease Prevention and Management Strategies and actions related to Chronic Disease Prevention and Management for patients and other community members are detailed below. Strategy 1: Work with community partners to increase access to prevention and early intervention services. Action 1: Provide health education, screenings, and referrals to care. FY2022 Outcomes: The Stroke Coordinator for HSHS Sacred Heart Hospital and HSHS St. Joseph's Hospital worked with HSHS and Prevea colleagues to provide blood pressure, blood sugar, and cholesterol checks, as well as a stroke risk assessment, at the Academy for Lifelong Learners in Eau Claire in June 2022. Individuals at risk also were provided health counseling This event for older adults attracted more than 400 participants from the Chippewa Valley region and beyond. Approximately 50 individuals took advantage of these free screenings. Action 2: Work with providers to ensure regular screenings and patient education. FY2022 Outcomes: Prioritization of other strategies/actions and time constraints prevented work in this area during the fiscal year. Action 3: Work with local Farmers Markets to supplement the purchase power of FoodShare recipients to buy fresh produce.

THE IRS PROVIDED WORKSHEET 2 WAS USED TO CALCULATE A COST TO CHARGE RATIO USING INFORMATION FROM THE ORGANIZATION'S GENERAL LEDGER AND COST REPORTS. THAT RATIO WAS THEN USED IN CONJUNCTION WITH IRS WORKSHEET 1 TO CALCULATE THE COST OF FINANCIAL ASSISTANCE PROVIDED AND WORKSHEET 3 TO CALCULATE UNREIMBURSED MEDICAID AMOUNTS. IMPLICIT PRICE CONCESSIONS INCLUDES BAD DEBTS. THEREFORE, BAD DEBTS ARE INCLUDED IN NET PATIENT REVENUE IN ACCORDANCE WITH HEALTHCARE FINANCIAL MANAGEMENT ASSOCIATION STATEMENT NO. 15 AND BAD DEBT EXPENSE IS NOT SEPARATELY REPORTED AS AN EXPENSE.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteThe text of the footnote to the organization's financial statements that describes bad debt expense can be found on pages 16-17 in the HSHS Consolidated Audit Report.
Schedule H, Part III, Line 8 Community benefit methodology for determining medicare costsThe Hospital continually strives to provide excellent patient care in the most cost effective fashion. Nonetheless, the Medicare program, in many cases, does not provide payment that covers the full cost of the care provided. Since it is the mission of the hospital to respond to community need, hospital leadership continually advocates for improved Medicare payment so that the cost of quality care to those patients who are not able to afford it is not compromised and is fairly subsidized by all payers. The IRS does not classify this shortfall in Medicare payments as Community Benefit. However, we believe it is an important contribution made by the hospital to the health and well-being of the community. If the Medicare program did not exist, many Medicare patients would be eligible for financial assistance or other means-tested government programs. Further, by absorbing this shortfall and providing care below cost to these individuals, the Hospital is relieving the burden of the government. Accordingly, this shortfall restricts the Hospital's ability to make funds available to provide for financial assistance and other Community Benefit. The hospital's Medicare shortfall at cost for fiscal year 2022 was $43.6M.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceEvery effort is made prior to, during and after provision of medical services to determine whether a patient is eligible for charity/community care and to assist the patient in completing the application and provide adequate documentation. If the patient qualified for charity/community care for the full balance of their account, the entire amount is written off to charity/community care and hence no debt collection is pursued. If the patient qualified for charity/community care for a portion of their account balance, that portion is written off to charity/community care, with the patient being responsible for the remainder of the balance. Additionally, reasonable effort will be made to obtain third-party or government payer reimbursement on behalf of the patient. If those efforts are not fruitful, an offer will be extended to the patient to make installment payments on their balance. Only at such point that the patient defaults on installment payments or refuses to cooperate with the hospital's efforts to be reimbursed will the account be sent to collections. The hospital will make reasonable efforts to determine whether a patient is eligible under the financial assistance policy before it engages in an ECA. To the extent a patient is determined to be eligible for financial assistance under the FAP, the hospital will take all reasonably available measures to reverse any ECA taken against the patient to obtain payment for the care.

Schedule H, Part VI, Line 2 Needs assessmentIn addition to the triennial CHNA, HSHS Sacred Heart Hospital (SHEC) uses many approaches to continually assess the health care needs of the community. SHEC uses Press Ganey Patient Experience Solutions to collect patient feedback to understand patient preferences and develop improvement plans. SHEC also utilizes face to face opportunities with patients to gauge community perception through regular leader rounding. SHEC engages in quarterly discussions about community health needs with the hospital's board of directors and advisory council. Membership consists of community members who represent varying sectors of the Chippewa Valley including non-profit agencies, schools, and businesses. SHEC has a robust Volunteer Partners program whose members are a valuable source of feedback from the community to the hospital and back to the community. SHEC leadership and Community Health staff participates in community groups and coalitions that address the needs of the underserved and minority populations. These groups include the Eau Claire YMCA, the Chippewa Valley Free Clinic, the Family Resource Center, Big Brothers/Big Sisters, the Mission Coalition, Eau Claire Healthy Communities, and Feed My People Food Bank.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceThe Registration Department provides brochures to patients upon registration regarding the Financial Assistance Program available to them at the Hospital. The brochures are also supplied in waiting and ancillary areas for easy access by the patient. To better serve our patient population, the Hospital offers information on our financial assistance program in Spanish. This includes the Financial Assistance Policy, Brochure and Application, which allows the hospital to serve populations in its county and the surrounding area for whom Spanish is their primary language. Patients are also provided a Fair Billing Act notification at the point of registration regarding separate physician billing they can expect to receive, plus information about the patient's financial responsibility if their insurance plan is out of network. One-on-one conversations to educate patients about financial assistance often stem from patients receiving a statement or a pre-collection letter. At that point, the customer service representative or cashier, or other qualified staff, will inform/educate the caller about financial assistance options. The first pre-collection letter reminds the patient their account is in delinquent status. It also informs them they may be eligible for financial assistance through the organization's charity care program. The letter provides direct contact information to begin the application process for charity care and lists the Internet address where the policy can be obtained. A third-party vendor also assists individuals in determining their eligibility for state health plans or insurance on the open health insurance marketplace. They can also assist with the application process.
Schedule H, Part VI, Line 4 Community informationFor purposes of the CHNA, the community served by HSHS Sacred Heart Hospital is defined as Eau Claire County, Wisconsin, a primarily urban county located in the west-central part of the state. HSHS Sacred Heart Hospital does not exclude MUAs, low income, or minority populations from its service area. As of 2019, the U.S. Census Bureau estimated the county population at 104,646, with more than two-thirds of residents living within the boundaries of the county seat, the City of Eau Claire. Per the County Health Rankings, 23% of residents live in a rural area. Slightly more residents are female (50.5%) versus male (40.5%). The median age is 35, and life expectancy is approximately 80. In regard to race, nearly 92% of Eau Claire County residents report being White alone; more than 4% Asian alone; just over 1% Black or African American alone; less than 1% American Indian and Alaska Native alone or Native Hawaiian and Other Pacific Islander alone; and ; 3% Asian alone; and 2% two or more races. Fewer than 3% of residents report Hispanic ethnicity. The county is also home to a significant Amish population. Median household income within the county is $59,476, with almost 11% of the population estimated to live in poverty. Eleven percent of children also fall within this threshold (source: County Health Rankings). Major industries include health care, social services, retail trade, manufacturing, and education. As of June 2021, 25,195 Eau Claire County residents were covered by BadgerCare Plus or another Medicaid program. Per the 2021 County Health Rankings, 6% of the county's population is uninsured. Of the 72 counties in Wisconsin, Eau Claire County ranks 21st in overall health outcomes and 8th in overall health factors. Both rural and non-rural components of Eau Claire County have been designated medically underserved areas (MUAs) for primary care. In total, three not-for-profit general medical surgical hospitals (HSHS Sacred Heart, Marshfield Clinic Medical Center-Eau Claire, and Mayo Clinic-Eau Claire) and one private, for-profit surgical hospital (OakLeaf Surgical Hospital) operate within the county.
Schedule H, Part VI, Line 5 Promotion of community healthAs a healing ministry of the Catholic Church and an affiliate of Hospital Sisters Health System (HSHS), HSHS Sacred Heart Hospital is committed to delivering high quality, compassionate, and cost-effective health care services to all. Founded in 1889, the hospital brings a healing presence and improves the health of our community, especially those persons who are sick, poor, and disadvantaged. Because of the hospital's mission and heritage, the hospital is dedicated to promoting the health of Eau Claire and surrounding areas. The hospital is governed by a Board of Directors, most of who reside in the hospital's primary service area and are neither employees nor independent contractors of the hospital (nor family members thereof). The Board ensures that HSHS Sacred Heart Hospital is responding to community need. At every Board of Directors meeting, a written or oral report on community benefit activities was provided. There are also other bodies that assist Sacred Heart Hospital's connection with the community, such as the hospital Advisory Council, Patient and Family Advisory Council, The Foundation of HSHS Sacred Heart Hospital, and Volunteer Partners Board. Consistent with its exempt purpose, HSHS Sacred Heart Hospital has an open medical staff with privileges available to all qualified physicians in the area. The hospital also operates an Emergency Department that is open 24 hours to all persons regardless of their ability to pay. As a not-for-profit hospital, HSHS Sacred Heart Hospital reinvests surplus funds to support the mission of the organization and promote the health of the community rather than distributing surplus funds as profits to shareholders or individuals. Funds not committed to ongoing operations are generally used to upgrade facilities, secure new technologies, improve patient care, and support initiatives designed to promote health and ensure access for all. For example, HSHS Sacred Heart Hospital continues to partner with Prevea Health, a multi-specialty health care organization based in Green Bay, Wisconsin that has been a part of the western Wisconsin region since 2015. Prevea Health works extensively with HSHS Sacred Heart Hospital and sister hospital HSHS St. Joseph's Hospital in Chippewa Falls. HSHS Sacred Heart Hospital provides a range of Community Benefit initiatives and programs that further our mission and long-standing commitment to our community. In many cases, these Community Benefits would not exist without the leadership role played by HSHS Sacred Heart Hospital and they often relieve a burden that would otherwise be carried by government. HSHS Sacred Heart Hospital also devotes significant resources to access for patients who cannot afford care, along with other Community Benefits. In FY2022, HSHS Sacred Heart Hospital provided $32,754,336 in Community Benefit, including financial assistance at cost, unpaid costs of Medicaid and other public programs, and a range of diverse programs designed to enhance access and improve community health.
Schedule H, Part VI, Line 6 Affiliated health care systemHeadquartered in Springfield, Illinois, Hospital Sisters Health System (HSHS) is a highly integrated, multi-institutional health care system comprised of 15 hospitals and more than 200 physician practice sites in Illinois and Wisconsin. HSHS is sponsored by Hospital Sisters Ministries (HSM), which is a public juridic person of the Roman Catholic Church. HSHS continues the health care ministries begun by its founders, the Hospital Sisters of St. Francis, more than 140 years ago, in rural and midsized communities throughout Illinois and Wisconsin. HSHS provides compassionate, holistic, high quality, and cost-effective health care through its acute care hospitals, physician practices, home health programs, palliative and hospice programs, and community outreach services. In addition, HSHS affiliated hospitals include critical access facilities that offer essential health services that otherwise would not be available in many communities. Our mission calls us to reveal and embody Christ's healing love for all people, and we extend our high quality Franciscan health care ministry beyond the walls of our hospitals by providing community health improvement services with special emphasis on the poor and vulnerable. In addition to providing community health improvement services to the broader community, HSHS cares for everyone who comes to its facilities, regardless of their ability to pay. In tandem with a variety of cross sector community partners, HSHS's Community Benefit initiatives are strategically planned and professionally implemented to improve access to health care services; enhance the health of the community; advance medical or general health care knowledge; and relieve or reduce the burden of government to improve health. The COVID-19 pandemic caused an unprecedented disruption in healthcare services across the United States. HSHS responded with services that benefited our communities, including free telehealth screenings, free testing, and coordination with other healthcare providers in the communities we serve to best ensure access to needed care. In FY2022, HSHS collectively provided $218.9 million in Community Benefit (8.3% of total hospital expenses). Of the total Community Benefit provided, HSHS spent $11.6 million provided for Financial Assistance (aka Charity Care) and $146.1 million for unreimbursed care provided as part of the Medicaid program. In addition, HSHS hospitals committed significant resources to treat Medicare patients. The cost of providing services to primarily elderly beneficiaries of the Medicare program - in excess of governmental and managed care contract payments - was $23.2 million. HSHS hospitals also recorded $68.7 million in uncollectible accounts. While HSHS does not count the latter two amounts as Community Benefit, they nonetheless reflect our commitment to serving all persons in need of care. In addition to the dollars invested in our Community Benefit programs, HSHS continues to reinvest any surplus revenue from operations and investments into new medical technology, facility infrastructure and health care services in our communities. By doing so, we ensure our ability to meet the ongoing demand for high quality, efficient and easily accessible health care.
