Northeastern Vermont Regional HospitalPart V, Section B, Line 5: The driving purpose of the NVRH Community Health Needs Assessment (CHNA) for 2024-2027 is to identify, prioritize, address, and communicate the current health needs of the broader community served by NVRH and the surrounding region. The Patient Protection and Affordable Care Act (ACA) of 2010 required all tax-exempt hospitals in the United States to conduct a CHNA every three years, and the 2024 NVRH CHNA is the fifth assessment achieved according to ACA federal guidelines.NVRH engaged several partner organizations to engage in the "Northeast Kingdom Coordinated CHNA". A Steering Committee formed in January 2024 and led this inaugural CHNA approach. The participating member organizations include NVRH, North Country Hospital, Northeast Kingdom Human Services, Northern Counties Health Care, Northeast Kingdom Community Action, Northeast Kingdom Council on Aging, and the Vermont Department of Health. Between February and May, extensive secondary data analysis occurred. Primary data collection approaches were developed and implemented between April and August, including a community survey with 414 responses, and 4 focus groups for a total 20 participants. Final data analysis and development of the HSA-level report occurred between August and September 2024, including various forms of input from residents and professionals who have working knowledge and/or reside in the health service area. The NVRH Board of Trustees adopted this report on September 25, 2024. NVRH made this report publicly available following Board adoption. The entire CHNA and methodology is available at: www.nvrh.org/community-health-needs-assessment/
Northeastern Vermont Regional HospitalPart V, Section B, Line 11: An update and current evaluation to the CHNA Implementation Plan is available atwww.nvrh.org/community-health-needs-assessment/ In recognition of the importance of the social drivers of health in overall health and well-being, NVRH had taken the lead in creating NEK Prosper in 2014. NEK Prosper uses the framework of Accountable Health Communities, the elements of Collective Impact, and the principles of Results Based Accountability. Key organizations that comprise that comprise the NEK Prosper! Network that addresses the root causes of poor health are the regional mental health agency, housing organization, community action program, Federally Qualified Health Center, Council on Aging, the Vermont Foodbank, as well as many other state agencies and community-based non-profits make up the body of NEK Prosper!. NEK Prosper! has identified five outcomes for our community: well-nourished, well-housed, physically healthy, mentally healthy, and financially secure. Workgroups were formed in the NEK Prosper! structure to provide strategic direction and results-based focus in each outcome area and continue to support the grassroots efforts to improve conditions that impact health.NVRH's recent CHNA report identified priority health needs that will be addressed both by the hospital as well as NEK Prosper! organizations that are committed to centering and aligning all community health work and health equity initiatives in the health service area with this report to collectively improve these priority areas.

Part I, Line 7:The Organization used three different methods to allocate costs reported in the table: four physician clinics are rural health clinics: NVRH - Corner Medical, NVRH - St Johnsbury Pediatrics, NVRH - Kingdom Internal Medicine, and NVRH - Women's Wellness Center. Costs for these clinics were taken directly from the Medicare cost report. Costs for other reported programs were direct costs for the program plus an allocation of indirect costs based on a ratio of direct to indirect costs taken from the Medicare cost report. A cost to charge ratio was used to determine the cost of bad debts and free care for all programs reported on the table.
Part I, Line 7g:The Organization included as subsidized health services costs attributable to four physician clinics.1. NVRH - Corner Medical, a primary care physician clinic. Total subsidized costs related to this physician clinic were $1,676,244.2. NVRH - St Johnsbury Pediatrics, a pediatric physician clinic. Total subsidized costs related to this physician clinic were $1,360,955.3. NVRH - Women's Wellness Center, an obstetrician/gynecological physician clinic. Total subsidized costs related to this physician clinic were $681,065.4. NVRH - Kingdom Internal Medicine, a primary care physician clinic. Total subsidized costs related to this physician clinic were $1,153,463.
Part II, Community Building Activities:Physical Improvements/HousingNVRH Community Garden:NVRH has provided garden space on our campus for community use for 35 years. There are currently 28 garden spots. The garden spots are provided free of charge, The community benefit value is based on the staff time to coordinate the assignment of spots and other correspondence with the gardeners. Workforce development:NVRH is located in a rural area with limited access to public transportation. Poverty is also our primary health disparity. We have listened to our community and they tell us they need access to services that are close to home. NVRH takes the lead in recruiting both primary care and specialty physicians and mid-level providers to fill the need for access to services close to home. The value listed in this section is related to the costs associated with recruiting primary care and specialty providers; as well as loan repayment for many of our providers (MO, NP, PA, etc). We also support a Career Advancement Program to grow the nursing workforce, including Medical Assistants and nursing of all ranks. Additionally, NVRH supports LNA programming with instructor capacity and clinical rotations for both high school and adult learners, as well as other job shadow opportunities across all healthcare professions.
Part III, Line 4:Please refer to pages 11-14 of the attached audited financial statements.
Part III, Line 8:All Medicare allowable costs were taken directly from the Medicare cost report.
Part III, Line 9b:NVRH provides care to all patients regardless of their ability to pay for services. Patients who indicate they have no insurance receive information about NVRH's Patient Assistance program, including the income level eligibility guidelines, with their first bill and with all subsequent bills. Patient Assistance is provided to those patients who, through a written documentation process, are identified as financially needy. Patients who have qualified for 100% Patient Assistance are not billed for subsequent services for one year. After one year, those patients go through the qualification process again. Financial assistance applications are made readily available to patients, in addition, signs are posted throughout the hospital and clinics and linked on our website.
Part VI, Line 2:Assessment of community health needs is an ongoing process at Northeastern Vermont Regional Hospital (NVRH). The hospital promotes openness, inclusiveness, and public participation in strategic planning and decision-making through several formal and informal mechanisms.NVRH's governance structure includes the Northeastern Vermont Regional Corporation (NVRC) Corporators, a body of nearly 225 members representing towns throughout the hospital's service area. Corporators serve as important connections between the hospital and the community, providing valuable insight and perspective for hospital leadership. Quarterly Corporator meetings provide opportunities for two-way communication. During these meetings, Corporators share concerns and needs expressed by residents across the region, including those of families, friends, and neighbors. In turn, hospital leadership provides updates on hospital services, facility improvements, community health initiatives, and broader healthcare developments at the state and national levels.NVRH also participates in formal community health assessment activities conducted by regional and state partners, including the Northeast Prevention Coalition, regional housing coalitions, and the Vermont Department of Health through the State Health Improvement Plan (SHIP). Hospital staff actively participate in these collaborative efforts.Community health assessment activities typically begin with identifying a purpose or priority area, which may range from broad questions such as identifying the community's most significant health needs to more focused assessments aligned with the mission of specific partner organizations. Community partners conduct ongoing engagement through surveys, community forums, listening sessions, and other outreach activities designed to gather input from residents and stakeholders. To inform these assessments, NVRH utilizes both secondary and primary data sources. Secondary data is primarily obtained from the Vermont Department of Health and Vermont Agency of Human Services Community Profiles, which compile data from multiple sources including the Behavioral Risk Factor Surveillance System (BRFSS), the U.S. Census Bureau, the Vermont Health Care Cost and Utilization Project, and other state and national datasets.Primary data is gathered through engagement with key stakeholders, including community leaders, partner organizations, and residents. Methods for collecting primary data include individual interviews, surveys (including online tools such as SurveyMonkey), community forums, and stakeholder focus groups. Assessments may also include inventories of existing services as well as gap and asset mapping to better understand available resources and unmet needs. After primary and secondary data are compiled, a comprehensive analysis is conducted to identify key themes and emerging priorities. In some cases, findings from the data may lead to refinement of the assessment focus if new or more urgent health needs are identified.Reporting results back to the community is an essential component of the assessment process. NVRH makes every effort to share findings and summaries with community members, partner organizations, and other stakeholders. The hospital's most recent 2024 Community Health Needs Assessment and Implementation Plan is publicly available at:https://nvrh.org/community-health-needs-assessment/ Increasingly, NVRH and its partners apply Collective Impact principles when developing community health strategies and interventions. This approach emphasizes shared leadership, collaborative planning, and equitable participation among community members and organizations. In 2024, NVRH worked with multiple regional health and human service partners to complete a collaborative regional Community Health Needs Assessment, improving alignment across organizations and strengthening opportunities for coordinated community health improvement efforts.
Part VI, Line 3:A summary of NVRH's Patient Assistance Program and the income eligibility guidelines are included in the first and all subsequent bills sent to patients identified as self-pay. The phone number of a financial counselor is also provided. The counselor is available to discuss eligibility guidelines and the qualification process. The counselor also discusses with patients the availability of Medicaid and other State assistance programs. Information about the NVRH Patient Assistance Program is also available on the NVRH website. Patients can also learn about NVRH's Patient Assistance program via a link to the State's Green Mountain Care Board. The Patient Assistance Program is available to hospital patients as well as patients of all NVRH-owned physician practices.
Part VI, Line 4:NVRH serves over 30,000 people in Caledonia and Southern Essex Counties in the northeast corner of Vermont. The Hospital is located about 50 miles from the Canadian border to the north, and about 15 miles from the New Hampshire border to the east. Only 3 towns in our service area have a population of more than 2,000 people. Our service area population is divided relatively evenly between males and females. The population is predominantly White (approximately 94%), and English is the primary language spoken in most households. The median household income in the region is approximately $59,105, which is significantly below the Vermont statewide median of $74,014. Educational attainment also differs from statewide averages. Approximately 92% of adults age 25 and older have earned a high school diploma or equivalent, while about 27% have completed a four-year college degree, compared to approximately 44% statewide. These demographic and socioeconomic factors provide important context for understanding health outcomes and access to care in the region and are considered in community health planning and intervention efforts.Education, healthcare, and social services are the largest employment sectors, followed by retail and then manufacturing. There are three public school supervisory unions, and numerous private and independent schools. There are 3 colleges located in our service area.
