| Description | Chapter Services - The Chapter's client services are designed to provide comprehensive and coordinated services to people with MS, their families and caregivers, including crisis assistance, access to and coordination of resources promoting independence, information and referral to/coordination with our intensive case management program when necessary. The Chapter provided 93 households with 156 instances of assistance for services such as auto and home modifications, durable medical equipment, home cooling units, eviction/foreclosure and utility disconnection prevention, emergency food needs, scholarships, transportation, occupational therapy home evaluations and chore services. The Chapter also served 37 families through its Care Management program this year via a contractual agreement with CareStar, Inc. | Description |
