SchH_P02_S00_L00Schedule H, Part IICommunity Building Activities conducted by Connecticut Children's Medical Center promote the health of multiple communities served by the organization. The following are descriptions of how selections of our community building activities serve the immediate neighborhood, the city of Hartford, the state of Connecticut, and have influenced multiple states around the country. Southside Institutions Neighborhood Alliance (SINA) is a partnership between Connecticut Children's Medical Center, Hartford Hospital, and Trinity College. SINA's mission is to make improvements to our neighborhood, one of the three poorest neighborhoods in the city. The partners each pay dues to support the infrastructure of SINA, and its Board of Directors is made up of three individuals from each partner. There is also a REACH (Recognition, Education, Achievement, and Community Health) Committee that is made up of a combination of Board members, other hospital employees and community members. During the year, employees, through SINA, were involved in community clean-ups, public safety events, activities supporting the city-wide school system, as well as some of our neighborhood schools, and recognition activities that support community involvement by residents of Hartford. SINA has led neighborhood efforts to eliminate blight. A housing program has identified vacant, run-down housing stock, and working with city officials and other partners, developed one and two family homes, all sold to individuals who plan to occupy those homes. Additionally, SINA has acted as a support to many of the local non-profit organizations, providing professional development opportunities, and advocating on their behalf in order to better serve the residents of the neighborhood. All these efforts are part of a public health approach to neighborhood improvement and development and training for community members, community health improvement advocacy. An example of city-wide activities in which we take a leadership role is with the Hartford Childhood Wellness Alliance, a coalition of 30 programs and organizations around the city, whose mission is aimed at preventing and decreasing childhood obesity in Hartford and improving the overall health of the city's schools, early childhood educators, local community organizations, advocacy groups, and local government. The goal of the alliance is to provide linkage between programs and initiative that are currently in place, and to develop new approaches to tackle complex issues such as childhood obesity. Our work with the Alliance fits Part II of the schedule H in the areas of Community support, Leadership development and training for community members, Coalition building, and Community health improvement advocacy. Two examples of state-wide initiatives that fall into the Community Building category include our Injury Prevention Center and Lead Action for Medicaid Primary Prevention Project. Both have an impact at the city level as well. The Injury Prevention Program's mission is to reduce unintentional injury and violence among Connecticut's children, adolescents, and adults. The Center seeks to translate research into prevention programs and policies. Core activities include research, community safety programs, education and training, and public policy. There are many individuals programs that address issues specific to Hartford, and many which address issues on a state-wide basis. Multiple partnerships have been created with public officials, civic leaders, community representatives and residents. This program fits Part II of Schedule H in the areas of Community support, Leadership Development and training, Coalition building, and Community health improvement advocacy. Another community building program with a state-wide reach is our Lead Action for Medicaid Primary Prevention (LAMPP) Project. Connecticut Children's operates the project, and there are numerous partners across the state. This project targets 14 cities where large numbers of Medicaid enrolled children live. The components of the project include; Education and Risk Assessment for families whose children have been referred by providers as having elevated lead/blood levels, Planning for Reduction of Health Hazards in homes, Low-Level intervention to create Lead-safe housing, and Training in lead-safe practices for landlords, maintenance, and low-income workers who do maintenance, painting and repairs on rental properties. This project fits Part II in Schedule H in Physical improvements and housing, Community support, Environmental improvements and Leadership development and training. Help Me Grow is a project that has created a coordinated system of early identification and referral with children at risk for developmental or behavioral problems. It is a system that is integrated, comprehensive, and effectively connects families to services. This project has successfully implemented system-wide changes at the state level in order to support children at the local level. This model has been replicated in multiple states and it fits Part II of Schedule H in the areas of Community Support, Leadership Development, Coalition Building, and Community Health improvement advocacy.
SchH_P03_S0A_L04Schedule H, Part III, Section A, Line 4From Note 2 of the consolidated Connecticut Children's Medical Center and Subsidiaries financial statements, "The Medical Center records it provision for bad debt based upon a review of its outstanding receivables." This is effected by completing a historical collection percentage analysis of accounts written off and applying those percentages to the current receivable balance.
SchH_P03_S0B_L08Schedule H, Part III, Section B, Line 8These revenues and expenses relate to end stage renal disease patients who generally have nowhere else to go for treatment. Accordingly we would consider any shortfall generated under this program as community benefit. The source of information for line 5 and 6 is the Medicare Cost Report.
SchH_P03_S0C_L09bSchedule H, Part III, Section C, Line 9bConnecticut Children's Medical Center will only refer accounts to collection agencies when it has been determined that the individual has the means to pay the balance, either through insurance or with their own funds, and has chosen not to apply for or is ineligible for patient financial assistance.
SchH_P06_S00_L02Schedule H, Part VI, Line 2Connecticut Children's Medical Center entered into an agreement with the City of Hartford's Health and Human Services Department, Hartford Hospital, and St Francis Hospital to assess the health needs of city residents. The agreed upon format included surveys with residents, surveys with Community Key Informants, and information collected by the city's Health Equity index project, a pilot project support by the Connecticut Association of the Directors of Health. The assesment, was conducted in 2011, edited and released in 2012. Additional information that helps guide our community benefit activity direction comes from the following: We sit on the city of Hartford's Public Health Advisory Committee; We collect information about trends from our clinics and Emergency Department; We research national and local helath related issues; We participate on neighborhood, local, state-wide, and national committees, coalitions, and networks. Using those experiences to help guide our decision making; We respond to grant opportunities, which require us to assess specific needs with each specific grant proposal.
SchH_P06_S00_L03Schedule H, Part VI, Line 3As written in the Credits and Collections policy: Posted text in general public areas and other communications (in English and Spanish) will notify patient and their guarantor of the availability of hospital-based assistance and other programs of public assistance. If the hospital determines that a patient or guarantor is potentially eligible for the Medicaid or other government program, it will encourage the patient or guarantor to apply for such program and the Financial Counselors will assist patient guarantors in applying for Medicaid, hospital-based assistance, or other assistance and payment plan programs when appropriate. Connecticut Children's offers hospital-based assistance for medically necessary inpatient and outpatient services for those patients unable to pay who can demonstrate financial need according to Connecticut Children's Patient Financial Assistance eligibility determination methodology. It is available as a last resort after all other third party resources have been exhausted. Once approved, the duration for eligibility for financial assistance is 6 (six) months.
SchH_P06_S00_L04Schedule H, Part VI, Line 4Connecticut Children's main campus is located in one of the poorest neighborhoods in one of the state's poorest cities. Neighborhood - 2500 families reside in the 14 block SINA neighborhood. 75 % are Latino. 44% have household incomes below $20,000. 45% of the residents 18 and older have less than a high school education. (SINA Neighborhood survey 2005) Hartford - is made up of 124,512 residents, 38% of whom are African American or Black, while 40% are of Latino origin. The median household income is $24,820 and 60% of the population 25 and over have a high school diploma. Connecticut has a population of 3,518,288. 10% of the population is African American or Black, while 9.4% are of Latino origin. The median household income throughout the state is $68,294 and 84% of persons 25 and older are high school graduates.
SchH_P06_S00_L05Schedule H, Part VI, Line 5There are a number of additional vehicles used to promote community health. Our Corporate Communications department coordinated all external communications, many of which were designed for that purpose (public service announcements and various reports). Our Annual Report informed the community of CT Children's latest advancements in caring for and curing childhood illnesses and diseases. "Pediatric Matters" was a publication sent out 4 times to over 30,000 readers, and it contained information about services and programs at the medical center. "Connecticut Children's Medical News" offered a more clinical perspective on activities and programs and was sent to referring providers. Safe Kids sent out newsletters to hundreds of coalition members throughout the state, featuring safety tips and research based information on preventable childhood safety issues. Our Continuing Medical Education office held 41 Pediatric Grand Round lectures during the year. These were open to community providers, and we began a registration process for Grand Rounds online. To date over 300 physicians and mid-level practitioners were registered. Public Safety on a local level is promoted through our participation with Southside Institution's Neighborhood Alliance (SINA). Trough SINA we participate at our local Neighborhood Revitalization Zone (NRZ) and its Public Safety Committee which addresses many public health issues, both to inform the neighborhood, and develop strategies to address problems. Our Government Relations Department participated in Advocacy efforts to ensure that the voices of children and their families were heard on healthcare issues that directly affected them. The department functioned as an expert resource with regard to pediatric health care issues for policy makers. Staff, Doctors, and families were able to leverage their experiences at CT Children's to provide expert testimony whenever possible.
SchH_P06_S00_L06Schedule H, Part VI, Line 6Not applicable
SchH_P06_S00_L07Schedule H, Part VI, Line 7The State of Connecticut requires hospitals to file Community Benefit reports every other year to the Office of the Health Care Advocate. This is an on-line report, and it was filed in February 2011.

SchJ_P01_S00_L03Schedule J, Part I, Line 3Each year, TowersWatson conducts a market analysis of the organization's CEO, officers and other key employees. To augment their proprietary data and other data to which they have access, Connecticut Children's provides the results data from salary surveys in which we participate.
