ExplanationThe College is the recipient of Federal and non-Federal grant funds. Some of the grant awards are subawarded to qualified organizations (subrecipients) to assist or provide an intellectual contribution to the project based on the requirements of the prime award. To monitor the use of grant funds under the subawards, the College enters into signed subaward agreements which contain the terms and conditions of the subaward relationship. The subaward agreements require that for reimbursement of expenses the subrecipient provide a certified invoice to the College. Before approval of an invoice for payment, the College verifies that the invoice and supporting documentation submitted by the subrecipient match the submitted and approved subaward budget and that the cost detail presented provides reasonable assurance that costs to be reimbursed are allowable, allocable, and reasonable. Progress reports submitted by the subrecipient are reviewed to determine that progress is made toward the goals and objectives of the subaward. Foreign subrecipients are required to submit a statement of accountability which provides assurance that financial practices are in place to ensure that subaward funds are used in accordance with their country's accounting policies, rules, and regulations.Explanation

ExplanationMedical and graduate school scholarships are applied directly to studenTs' accounts at the College. The College maintains records to justify both awards and the financial aid eligibility of the recipients. The College is the recipient of Federal and non-federal grant funds in support of its research mission. Some of the grant awards are subawarded to qualified organizations (subrecipients) to assist or provide an intellectual contribution to the project based on the requirements of the prime award. The College's public and community health mission is complemented by the Advancing a Healthier Wisconsin (AHW) endowment under which funds are awarded to qualified organizations to support initiatives that improve the health of the residents of the State of Wisconsin. To monitor the use of BOTH TYPES OF awarded funds, the College enters into signed subaward agreements which contain the terms and conditions of the subaward relationship. The subaward agreements require that for reimbursement of expenses the subrecipient provide a certified invoice to the College. Before approval of an invoice for payment, the College verifies that the invoice and any supporting documentation submitted by the subrecipient match the approved subaward budget and that the cost detail presented provides reasonable assurance that costs to be reimbursed are allowable, allocable, and reasonable. Progress reports submitted by the subrecipient are reviewed to determine that progress is being made toward the goals and objectives of the subaward. The College obtains an annual OMB Circular A-133 audit report, or a financial audit report or subrecipient questionnaire for organizations not subject to OMB Circular A-133, to ensure that audit findings do not exist which would impact awarded funds.Explanation

ExplanationPer College policy, first class travel is not allowed except for medical necessity or other exceptions approved by the Senior Vice President, such as an urgent business need. During calendar year 2010 there was one instance of first class travel by the Dean for urgent business needs. THIS instance was a NON-taxable event. PAYMENT FOR COMPANION TRAVEL IS NOT ALLOWED UNLESS THE ATTENDANCE IS A REQUIREMENT OR EXPECTATION OF THE EVENT ORGANIZERS (IN WHICH CASE JUSTIFICATION MUST BE PROVIDED ACCORDING TO THE IRS REQUIREMENTS). THERE WERE NO INSTANCES OF COMPANION TRAVEL DURING THE CALENDAR YEAR 2010. The College reimburses one Board member and five officers/key employees for memberships at the University Club. Memberships are used for the monthly Board and committee meetings and for other business meetings. Any personal use of the University Club by these individuals is reported to the College and is treated as a taxable event to the individual. The College provides discretionary spending accounts to EIGHT officers/key employees and two of its highest compensated employees. The spending accounts are not part of an accountable plan and are fully taxable to these individuals. Schedule J, Part I, Line 4B The College has FIVE deferred compensation executive agreements, which comprise its supplemental nonqualified retirement plan (457(f) plan). FOUR agreements are currently deferring benefits and one is not. The College contributes 12% of each currently participating executive's salary. The amounts of employer contributions to this plan for currently participating executives in calendar year 2010 were as follows: JOHN R. RAYMOND, SR. MD - $39,000 (AGREEMENT STARTED 7/1/10) T. Michael Bolger JD - $35,100 (AGREEMENT ENDED 6/30/10) Douglas Campbell - $59,400 Jonathan Ravdin MD - $73,200 Participant Michael Dunn MD retired from his former position as Dean/Executive Vice President of the College but remains employed as a Professor. His current deferrals under this plan ceased as of 6/30/08. Participants JOHN R. RAYMOND, SR. MD, T. Michael Bolger, Jonathan Ravdin, MD, and Michael Dunn, MD vest on the earliest of the following conditions: employee's death, employment termination due to disability, involuntary termination by the College not for cause, or the date the employee satisfies the two year non-compete covenant and a minimum of three years of service. Participant Douglas Campbell vests on the earliest of the following conditions: employee's death, employment termination due to disability, the date the employee satisfies the two year non-compete covenant and a minimum of three years of service, or the payout date provided he remains in employ until that date (payout date of current agreement is 6/30/11). Schedule J, Part I, Line 6 Physician incentive payments are based on the profitability of their individual practice groups (NOT THE COLLEGE AS A WHOLE). SCHEDULE J, PART II JULIA UIHLEIN AND DAVID WARLTIER, MD, PHD ARE COMPENSATED FOR THEIR WORK AT THE MEDICAL COLLEGE OF WISCONSIN, INC. AS PROFESSORS OR PHYSICIANS, NOT FOR THEIR ROLES AS TRUSTEES. Michael Dunn, MD is compensated for his current role as a faculty member, not for his former role as dean and executive vice president. JOSEPH KERSCHNER, MD, Robert Kliegman, MD, James Tweddell, MD, and John Thometz, MD are compensated by the College for their physician and administrator roles both at the College and at Children's Specialty Group, Inc., a related organization.
