IS A COMMUNITY HEALTH CENTER LOCATED IN LOUISVILLE, KENTUCKY THAT PROVIDES PRIMARY HEALTH CARE SERVICES TO PATIENTS IN THE LOUISVILLE AREA THROUGH SIX PRIMARY CARE CENTERS AND ONE SPECIAL HEALTH CLINIC FOR THE HOMELESS. DURING THE YEAR ENDED NOVEMBER 30, 2012, FAMILY HEALTH CENTERS SERVED A TOTAL OF 41,962 (INCLUDING 6,059 WHO WERE HOMELESS) PATIENTS WHO HAD APPROXIMATELY 162,000 VISITS BROKEN DOWN AS FOLLOWS: MEDICAL VISITS OF 130,694; DENTAL VISITS OF 11,540; MENTAL HEALTH AND SUBSTANCE ABUSE VISITS OF 9,222 AND ENABLING VISITS OF 10,338. OF THE 41,962 PATIENTS SERVED, APPROXIMATELY 22,700 (54%) WERE UNINSURED. IN 2012, FAMILY HEALTH CENTERS PROVIDED TOTAL CHARITY CARE OF APPROXIMATELY $12,453,000. OVER THE PRIOR THREE YEARS, CHARITY CARE TOTALED APPROXIMATELY $37,179,000.	Description
