DescriptionThe Greater Humboldt Park Community Diabetes Empowerment Center at 2753 WestDivision Street, Chicago is a community-driven partnership of five medical providers(Sinai Health Systems, Northwestern Medicine, Norwegian American Hospital, SaintsMary and Elizabeth Medical Center, and Rush University Medical Center) and threecommunity organizations (The Greater Humboldt Park Community of Wellness, The PuertoRican Cultural Center, and Pueblo Sin Fronteras). The Center is a highly visible community resource to prevent and control Type 2 diabetes for the residents of Greater Humboldt Park and surrounding neighborhoods. Since Type 2 diabetes is impacted by both physiological and social determinants of health, the Center is part of a multi-level community-driven approach that provides health education, physical activities, and social support to neighborhood residents in a community environment that facilitates collaborative learning about diabetes. The Center strives to inform, advocate, and empower residents regardless of literacy, language, insurance status, or income to reduce the health risks associated with diabetes. The Center houses the staffs of the NIH funded Block-By-Block. The Greater Humboldt Park Community Campaign Against Diabetes of which Sinai Health System is also a partner helps to reduce the burden of diabetes and childhood obesity in the communities of North Lawndale (NL) and South Lawndale (SL), located on the west side of Chicago.Description

DescriptionThe Avon Safety Net grant and the Helping Her Live Breast Cancer Intervention aims to empower women with knowledge, help women who are 40 years and over obtain annual screenings, work with women and health care facilities to eliminate delays in obtaining test results and ensuring follow up as necessary.Health Educators are trained to conduct breast health workshops throughout the community, they also help women navigate breast health services by: making appointments for screening mammograms;getting free or low-cost mammograms for women with no insurance;helping women understand their mammogram results;following up with any abnormal or suspicious mammogram results, sending reminder postcards for annual mammograms and answering breast health questions or concerns.Active Outreach Efforts: January 2013 - December 2013Across this 12-month period, HHL staff presented 13 workshops, hosted or attended 19 events, and participated in 37 woman-to-woman activities. In total, HHL encountered almost 1600 women in 2013.Workshops:Between January and December 2013, HHL staff gave 13 workshops, reaching 330 women (about 25 women per event). Of the 330 women with whom HHL spoke to at workshops, 124 completed a pink sheet. Outreach productivity for workshops was thus 38% (124/330).Events:Between January and December 2013, HHL staff attended or hosted 19 events, reaching 538 women (about 28 women per event). Events provide a good opportunity for high volume education and outreach. Of the 538 women with whom HHL spoke to at events, 134 completed a pink sheet. Outreach productivity for events was thus 25% (134/538).Woman-to-woman:Between January and December 2013, HHL staff participated in 37 woman-to-woman activities, reaching 691women (about 19 women per activity). Woman-to-woman activities, such as table set-ups at events that are not health related (e.g., job fairs), provide a good opportunity to seek out women we may miss with other outreach efforts. Of the 391 women with whom HHL spoke to during these activities, 220 completed a pink sheet. Outreach productivity for woman-to-woman activities was thus 32% (220/691).Requests for Services:Women who request either a doctor's appointment or a mammogram reminder are carefully tracked in the HHL database. Our goal for 2013 was to complete 50 mammograms per month; we averaged 61 per month. Note that these are not all the same requests as those generated during this time period. Some of these clients will be the same women we met during this time and some will not. That is to say we do not close all the women we meet within the same month; as mentioned earlier, it typically takes about 75 days to successfully navigate a womanDescription

Form 990, Part III, Line 4dForm 990, Part III, Line 4d: Other Program Services DescriptionOTHER PROGRAM SERVICES 4: Support from the Aids Foundation of Chicago allows Sinai to provide Primary and HIVmedical specialty care to 85 uninsured clients in our Adult Infectious Diseaseclinic. Throuh the clinic, Sinai caregivers provide medical and social support casemanagement services to all qualified HIV infected patients. This includes foodvouchers, transportation back and forth to visits, support systems, linkage intocare, and referrals sources.Case manager case load is approximately 170 patients. OTHER PROGRAM SERVICES 5: This program includes the Stand Against Cancer Evaluation Support, Chicago Community Trust Evaluation Grant and Community Health Worker Programs in Chicagos Health Care Access Community Health Network (ACCESS) has implemented the Stand Against Cancer (SAC) program in partnership with the state of Illinois in order to address disparities in breast and cervical cancer outcomes by targeting low income, minority, and uninsured women in the Chicago area. SUHI has previously served as an external evaluation consultant for these efforts. In 2013, SUHI provided the following support and technical assistance:Completed SAC records of abnormal breast and cervical screenings using Sinais medical record system. Built a Microsoft Access database for the SAC abnormal data in order to streamline the data entry and analysis process for ACCESS abnormal cases moving forward.Provided a report detailing how SAC patients access mammography services at Sinai Health System in order to evaluate the capacity of Breast Imaging Services. In May 2010, The Sinai Urban Health Institute (SUHI) began work on an evaluation grant from the Chicago Community Trust (CCT). The purpose of this grant is to provide an evaluation framework for Trust-funded health programs, thereby allowing for critical program review by both grantee organizations and the Trust. This project was conceptualized because many community-based health programs funded by the Trust do not have a structured evaluation plan in place, making it difficult to discern program effectiveness. The primary aim of the Community Health Worker Programs in Chicagos Health Care Institutions project is to clearly delineate Community Health Worker (CHW) effectiveness and best practices in terms of evidence-based science. Information collected in the study will be synthesized and analyzed to: 1.) Create standardized process and outcome evaluation tools for CHW programs; and 2.) Inform the CHW model in health care settings. In 2013 the Sinai Urban Health Institute announces the release of a new report, Best Practices Guidelines for Implementing and Evaluating Community Health Worker Programs in Health Care Settings. This report is the culmination of a two-year project funded by the Lloyd A. Fry Foundation. The Community Health Worker (CHW) Best Practice Guidelines are intended to be a blueprint for health care organizations interested in implementing the CHW model. These practice guidelines were developed to address gaps in the CHW professional literature and assist health care administrators, public health professionals, health care providers, CHWs, and communities in designing and implementing CHW interventions grounded in evidence-based science.
