DescHUD Project:The intervention, entitled Helping Chicagos Westside Adults Breathe and Thrive: A Health Homes Approach to Improving Respiratory Health (HCWABT), is the first to examine the effects of a Community Health Worker (CHW)-based intervention on improving asthma management among adults. HCWABT builds off of SUHIs Helping Children Breathe and Thrive (HCBT) program, which officially ended in August 2013. The program served both adults and children with asthma living in CHA developments on the Westside, and saw significant improvements in overall asthma management and reductions in asthma triggers found in the home environment among participants. HCWABT is a three year project and officially kicked off in November of 2013. HCWABT is SUHIs eighth asthma grant, and when completed, SUHI will have been intervening to improve asthma management in some of Chicagos most vulnerable communities for over 15 years. CHWs conduct 5-6 home visits over the course of the 12-month intervention, where they provide comprehensive asthma and healthy homes education. We have completed seven months of recruitment. Methods of recruitment that have been implemented include referrals from: Sinai Emergency Department, Sinai inpatient units (hospitalizations), Sinai physicians and the Chicago Housing Authority. As of June 30, 2014 343 referrals were received. Sixty percent of these referrals came from Sinais Emergency Department and since contact information is limited to patients medical files, 52% of ED referrals were never reached. Although only 32% of referrals came from the Chicago Housing Authority, over half of active enrollees (57%) came from this referral source since participants initiated their own referral. Nearly 22% of total referrals were ineligible for HCWABT, in large part due to their home zip code being outside of the service area. A few ineligible participants were given one-time teaching sessions since they could not be enrolled in the program. As of June 30, 2014 76 adults have completed their baseline visit and are enrolled in the program. We continue to anticipate many more referrals as recruitment from several outpatient clinics and other community partner organizations in the projects service area will begin soon.Community Health Worker Interventions in Chicagos Health Care InstitutionsCommunity Benefits FY2014:Health care reform calls for innovation and systems change. Provisions of the Affordable Care Act, such as hospital readmission reduction and patient-centered medical homes, well match Community Health Worker (CHW) roles and work. As such, the utilization of CHWs as valued members of health care teams is progressively being discussed by medical providers, researchers, and policy-makers as a strategy in complying with health care reform and meeting the goals of health cares triple aim. However, there is uncertainty among health care administrators and providers as to how to implement the CHW model to obtain the triple aim- better health, better quality, and lower health care costs. CHWs have been central to SUHIs work in eliminating health disparities in Chicagos most vulnerable communities since its inception. A wealth of experience has been amassed in recruiting, training, supervising and deploying the CHW workforce to deliver health education, assist clients in navigating the health care system, and connecting clients with appropriate social service needs. SUHI has implemented CHW projects in various health conditions, including asthma management, diabetes, breast health, HIV/STD and smoking cessation. SUHI has also conducted extensive research on the CHW model in general and in 2014, released a report entitled, Best Practice Guidelines for Implementing and Evaluating Community Health Worker Programs in Health Care Settings. The intention of this project is to create greater awareness for the value of CHWs and to build the health care systems ability to incorporate CHW work into their models of care. The project consists of the following three activities:Dissemination of the CHW Best Practice GuidelinesSUHIs CHW best practice report aims to provide information and solutions to a range of organizations struggling to provide quality care to vulnerable populations in the health care field. The guidelines can assist agencies with new uptake of the CHW model and also aid groups currently working on CHW issues in Chicago, Illinois, and the nation to better implement their work. This year, much time has been devoted to disseminating the guidelines through meetings, webinars, and conferences. The report is publicly available on SUHIs website and also sent via email to SUHIs more than 400 listserv members. Several CHW groups have featured a link to the report on their networking websites, and one organization (The National Peers for Progress) sent the report via its global distribution list of more than 1,000 contacts. Hosting an Inter-professional Conference on CHW Integration into HealthcareIn January 2014, SUHI and the Institute of Medicine of Chicago held an inter-professional conference entitled, Community Health Workers: The Potential for Enhancing Patient Care, Improving Health, and Reducing Costs. To our knowledge, this conference is the first of its kind in Illinois to bring together such diverse stakeholders to have a meaningful discussion of the changing healthcare system and the integration of CHWs as part of the healthcare delivery team. Of the over 125 in attendance, there was a diverse mix of physicians, CHWs, insurers, researchers, policy experts, health care administrators, other health or medical professionals, and representatives from state and local government. Providing Capacity-Building Support on the CHW ModelFollowing the conference, the project hosted on-site workshops with five health care centers to provide capacity-building support in conceptualizing the system-level changes needed to implement the CHW model. We believe that this work can advance Chicago as a leader in incorporating CHWs into health care settings, ultimately reducing health disparities and improving the health of some our most vulnerable communities. Most importantly, this work has the potential to make a lasting impact on the health and economic stability of underserved communities by improving preventative health services and chronic disease management, while empowering community members to serve as health care professionals. Health disparities are well documented; what is not well understood is how to eliminate health disparities in a manner that is cost-effective. The wide spread use of CHWs is one method that has been proposed as a venue by which health equity might be achieved in a manner that is sustainable in a difficult economic climate.Sinai Community Health Survey:In October 2013, the Sinai Urban Health Institute (SUHI) launched the second Sinai Community Health Survey (Sinai Survey) the largest, community-driven, door-to-door health survey ever conducted in Chicago. The survey will provide robust data about the health of local individuals and communities. The Sinai Survey was first implemented in 2002, leading to the generation of 31 different health intervention and approximately $17 million in funding to improve the health of Chicago communities. The key goals of the Sinai Survey are:1.Document the health status of various, diverse community areas in the City of Chicago2.Provide community-level survey data that can be used to develop public health interventions and policies3.Compare the findings of the 2014 Sinai Survey to those of the 2002 Sinai SurveyThe second Sinai Survey is being conducted in ten diverse Chicago community areas: Chicago Lawn, Gage Park, Hermosa, Humboldt Park, Lower West Side, North Lawndale, Norwood Park, South Lawndale, West Englewood, and West Town. The survey includes approximately 600 questions on adult and child health from 3,000 adults and 1,500 children. The survey questions were selected and approved by the Community Advisory Council, 20 individuals from the 10 surveyed communities. Surveyors will be in the field from Winter 2014 through Spring 2016. SUHI will work in concert with the Community Advisory Board to widely disseminate the survey findings, engaging community members to use the data captured in the Sinai Survey to help improve the health of Chicagos most vulnerable communities. Project Accomplishments: July 2013 through June 2014A fundamental aspect of the Sinai Survey is working with the community to ensure that the data gathered will benefit local efforts and assist residents in improving health and well-being. In an effort to establish a formal working collaboration with communities, we assembled a 20-member Community Advisory Committee (CAC) to guide the development of the survey instrument and the project. In a collaborative effort with the CAC and our partners at the Survey Research Laboratory at the University of Illinois at Chicago, over the course of eight months, final surDesc

DescThe Greater Humboldt Park Community Diabetes Empowerment Center at 2753 West Division Street, Chicago is a community-driven partnership of five medical providers (Sinai Health Systems, Northwestern Medicine, Norwegian American Hospital, Saints Mary and Elizabeth Medical Center, and the Public Health Institute) and three community organizations (The Greater Humboldt Park Community of Wellness, The Puerto Rican Cultural Center, and Pueblo Sin Fronteras). The Center is a highly visible community resource to prevent and control Type 2 diabetes for the residents of Greater Humboldt Park and surrounding neighborhoods. Since Type 2 diabetes is impacted by both physiological and social determinants of health, the Center is part of a multi-level community-driven approach that provides health education, physical activities, nutritional counseling, and social support to neighborhood residents in a community environment that facilitates collaborative learning about diabetes. The Center strives to inform, advocate, and empower residents -- regardless of literacy, language, insurance status, or income to reduce the health risks associated with diabetes. Sinai Health System is a strong and long-term partner with this work.Grant funds from the funders are used to support:1) operational infrastructure at the Diabetes Empowerment Center such as telephone, internet, alarm services, cleaning and maintenance supplies 2) the Muevete Physical Activity Program including salary of the Muevete Coordinator, Leony Calderon, stipends for community physical activity volunteers, printing of forms and outreach material, equipment, supplies and healthy snacks for physical activity events and meetings..The Muevete Program provides locally relevant physical activity programs in a variety of community-based settings throughout the project area. The programs are designed primarily to facilitate weight loss and improve glycemic control among people with diabetes as well as without diabetes. Daily individual and group exercise programs are organized in parks, field houses, community-centers, schools, church basements, etc. The Muevete Coordinator trains community volunteer facilitators to lead and administer the physical activity program at each location. To improve everybody's health and create a community-wide climate of wellness, everyone in the community is encouraged to participate in the physical activities programs. Muevete organizes physical activity oriented community events and has developed a Network of community programs and organizations doing similar work in the project area. Muevete offers free use of two treadmills, Dance Aerobics three days a week both day time and night time, Zumba two days a week, Pilates two days a weeks.3) health education and cooking classes in the form of healthy snacks, printing, equipment, supplies, teaching material, and incentives. Health education and nutrition classes are offered every Wednesday, 12 noon to 2:00 p.m. and Saturdays from 10 a.m. to 2:00 p.m.Programming remained consistent in FY2014, with solid participation and turnout from the community, including at the Muevete festival.Desc

DescIn October 2006, the Sinai Urban Health Institute first reported findings on widening disparities inbreast cancer mortality between Black and White women in Chicago. From 2005-2009, Black women were 48% more likely to die from breast cancer than White women. All women need and deserve equal access to breast cancer screening, diagnosis and treatment. The Helping Her Live (HHL) program was founded in November 2007 to ensure that women on the west and southwest side of Chicago do not die unnecessarily from breast cancer.Helping Her Live aims to:1.Empower women with knowledge;2.Help women who are 40 years and over obtain annual screenings;3.Work with women and health care facilities to eliminate delays in obtaining test results and ensuring follow up as necessaryHealth Educators are trained to conduct breast health workshops throughout the community. They also help women navigate breast health services by:Making appointments for screening mammograms;Getting free or low-cost mammograms for women with no insurance;Helping women understand their mammogram results;Following up with any abnormal or suspicious mammogram resultsSending reminder postcards for annual mammograms;Answering breast health questions or concerns.Outreach and Education ProductivityActive Outreach Efforts: July 2013 - June 2014Across this 12-month period, HHL staff presented 15 workshops, hosted or attended 84 health fairs and outreach activities such as job fairs and food pantries. In total, HHL encountered almost 2400 women. Of these 2400 women, 954 signed up for HHL navigation services (56% Hispanic, 40% African American and 4% Other).Requests for ServicesWomen who request either a doctor's appointment or a mammogram reminder are carefully tracked in the HHL database. Our goal was to complete 50 mammograms per month; we averaged 62 per month. Note that these are not all the same requests as those generated during this time period. Some of these clients will be the same women we met during this time and some will not. That is to say we do not close all the women we meet within the same month; as mentioned earlier, it typically takes about 75 days to successfully navigate a woman.Doctor's Appointment Requests:HHL closed 1,152 doctor's appointment requests (DRs) during these 12 months. Of these 1,214 DRs, 817 were completed (71%). Of these completed mammograms, 664 were screening mammograms (82%); 153 were diagnostic mammograms (19%) An additional 62 clients were information sent to out of area client. The remaining 335 DRs were incomplete (29%). Of the 335 incomplete DRs, 242 were unable to contact (72%); 34 refused services (10%); and 59 were closed for some other reason (18%).Desc
