Sch H, Part V, Section B, Line 5:Community Hospital was an active participant in the CHNA collaboration with the Mesa County Community Health Department and a number of other local hospitals and organizations in Mesa County. A Community Hospital representative worked with the CHNA planning committee and the Mesa County Community Transformation Group to determine what data needed to be gathered. Community Hospital provided aggregate health screening reports for its contracted employer groups to Mesa County Health Department. The healthcare partners of our community met regularly to put together a timeline and action plan to implement the Strategy Report and ensure no duplication of efforts. Community Hospital utilized previous assessments to guide interventions and collaborations. Primary data was collected from subject matter experts and community members through focus groups, informal interviews, and electronic surveys.
Sch H, Part V, Section B, Line 6a:The CHNA was conducted with the following other hospital facilities: Family Health West, Monument Health, LLC, St. Mary's Regional Hospital, and The VA Western Colorado Health Care System Center.

sch h, Part V, Section B, Line 11:A Community Health Needs Assessment (CHNA) is part of the required documentation of "Community Benefit" under the Affordable Care Act for 501(c)(3) hospitals and fulfills requirements for accreditation for many health and public health entities. However, regardless of status, a CHNA provides many benefits to an organization. At Community Hospital, we work to inspire and support people to be healthier in all aspects of their lives, building happier, healthier, and stronger communities. We actively participate in the development of a CHNA every three years with Mesa County Public Health and a number of other local hospitals and organizations. This effort demonstrates that Community Hospital makes an ongoing commitment to improve the health of Mesa County residents. The CHNA is a fundamental tool of public health practice which aims to describe the health of the community by presenting information on the widespread health status and health problems. The CHNA process helps Community Hospital and other local organizations to develop strategies aimed at making long-term, sustainable change. It also facilitates the strengthening of relationships and partnerships between health-related organizations that are working together to improve community health. develop strategies aimed at making long-term, sustainable change. It also facilitates the strengthening of relationships and partnerships between health-related organizations that are working together to improve community health.
Community Hospital's Implementation Strategies:This Implementation Strategy is intended to satisfy each of the applicable requirements set forth in proposed regulations released 2021 and serves as Community Hospital's planned response to the needs identified through 2024 Community Health Needs Assessment (CHNA) process. For a full copy of the CHNA report, please visit: https://cdn.yourcommunityhospital.com/files/Mesa+County+Community+Health+N eeds+Assessment+2024-26.pdf Community Health Needs Identified in the CHNA Report The findings from the 2024 CHNA detail five themes based on Social Determinants of Health (SDH). These Social Determinants of Health: economic stability, education, healthcare and access, neighborhood and built environment, and social and community context. Health Needs Community Hospital Plans to Address Building on the criteria used in the CHNA process, Community Hospital has evaluated the knowledge delivered, skills developed, and expertise within our existing service lines, and the knowledge of community efforts and entities that are currently, or may be, addressing the identified needs. Further, Community Hospital is leveraging collaborative initiatives that are addressing and impacting the identified needs. Based on this evaluation, Community Hospital has selected a number of needs from the outlined key determinants of health and from the broader areas of concern to address. This will allow us to maximize our ability to focus resources and have a meaningful impact on community health needs. Key CHNA data points for each inequity that Community Hospital plans to address are summarized below. 1. Economic Stability Community Hospital Services and Programs Committed to Respond to This Need: Community Health Partnership (CHP): A suite of services specifically tailored to improve employee health and control healthcare costs in collaboration with local businesses. This program offers convenient and affordable access for employees to employer-sponsored medical clinics, population health management, health coaching, and additional services. Local Employer: As one of the largest employers in Mesa County, Community Hospital provides stable, well-paying jobs across a wide range of skill levels for over 1,300 employees. Hospital operations generate significant local spending through employee wages, purchased services, and support of broader economic development. Future Goals and Objectives to Address this Significant Health Need: Goal: Strengthen the economic stability of Mesa County by expanding workforce opportunities, supporting local businesses, and enhancing access to cost-effective healthcare services. Objectives: Expand the CHP program to reach additional local employers to improve access to affordable healthcare for the regional workforce. Enhance workforce development through career pathway programs in partnership with local schools and community colleges to support high-demand healthcare roles. Support employee financial well-being through competitive compensation, benefits, and retention initiatives to reduce turnover and enhance long-term economic security for staff. Expected Outcomes: Improved access to affordable healthcare for the local workforce, and A stronger local workforce pipeline. 2. Access to Primary Care Community Hospital Services and Programs Committed to Respond to This Need: Convenient Access to Primary Care: Community Hospital offers multiple convenient locations with a range of providers that offer primary care services including: Grand Valley Pediatrics Grand Valley Primary Care - 28 1/4 Road Grand Valley Primary Care - Patterson Road Grand Valley Primary Care - Fruita Grand Valley Primary Care - Palisade Internal Medicine Associates of the Grand Valley Western Medical Associates of the Grand Valley Future Goals and Objectives to Address this Significant Health Need: Goal: Enhance access to primary care services across Mesa County to reduce unnecessary emergency department visits, increase early intervention, and improve community health outcomes. Crisis Intervention and Suicide Prevention: All emergency department patients are screened for suicide risk, and those identified as high-risk are placed in a safe environment for evaluation by a Behavioral Health Specialist. Staff coordinate with behavioral health facilities to secure appropriate inpatient placement while ensuring the patient remains safe during the interim. Community Hospital also shares suicide-related data with Mesa County Public Health and participates in the local suicide prevention coalition. Future Goals and Objectives to Address this Significant Health Need: Goal: Improve behavioral health outcomes in Mesa County by strengthening the local behavioral health workforce, crisis intervention services, and community education. Objectives: Expand outpatient behavioral health access through provider recruitment to support primary care integrated behavioral health and health coaching. Strengthen community partnerships to increase community education and awareness of local behavioral health services, stigma reduction, and early intervention. Expected Outcomes: Improved access to behavioral health providers and integration with primary care, and Reduction in suicide mortality through improved prevention and crisis intervention. 4. Health Needs Community Hospital Does Not Intend to Address Community Hospital is committed to addressing many of the priority health needs identified in the CHNA. However, this implementation plan does not focus on housing insecurity and childcare at this time. Based on a review of available resources and potential impact, the hospital has chosen to prioritize other areas of health inequity where it can make the most meaningful contribution. While Community Hospital offers services and support for housing and childcare, there are other community organizations better equipped to lead these efforts.
sch h, Part V, Section B, Line 13h:Community Hospital may make a presumption of eligibility on the basis of individual life circumstances that may include homelessness, patient deceased with no known estate, and other circumstances. The facility may use outside agencies in determining estimated income amounts for presumptive charity care eligibility.

sch H, Part I, Line 3c:In addition to FPL, the organization used an asset test in determining eligibility for financial assistance. The organization also took insurance status and medical indigency into consideration. Community Hospital may make a presumption of eligibility on the basis of individual life circumstances that may include homelessness, patient deceased with no known estate, and other circumstances. The facility may use outside agencies in determining estimated income amounts for presumptive charity care eligibility.
sch h, Part I, Line 7:Charity care expense was converted to cost on line 7a based on an overall cost-to-charge ratio addressing all patient segments. Line 7b and 7c were determined using the overall cost to charge ratio. Line 7i is actual expenses from the general ledger.
sch, Part III, Line 2:Allowance for bad debts is determined monthly by review of contractuals by payor using historical activity and expected collection rates. The amount is reviewed by the CFO and Board. Part III, Line 4: Footnote from Financial Statement: Please see Note 2, Financial Statement, Patient Service Revenue on page 17.
sch h, Part III, Line 8:The Medicare cost report was used to determine the amount of the Medicare shortfall. IRS Revenue Ruling 69-545, which established the community benefit standard for nonprofit hospitals, states that if a hospital serves patients with government health benefits, including Medicare, then this is an indication that the hospital operates to promote the health of the community. This implies that treating Medicare patients is a community benefit. The organization estimates 100% should be considered a community benefit. If these individuals did not qualify for Medicare, they would qualify for financial assistance, based on the demographics of the community in which we operate.
sch h, Part III, Line 9b:For patients who qualify for charity and who are cooperating in good faith to resolve their discounted hospital bills, Community Hospital may offer extended payment plans, will not send unpaid bills to outside collection agencies, and will cease all collection efforts. Community Hospital will not impose extraordinary collections actions such as wage garnishments, liens on primary residences, or other legal actions for any patient without first making reasonable efforts to determine whether that patient is eligible for charity care under this financial assistance policy.
sch h, Part VI, Line 2:Community Hospital develops its strategic plan both internally and supported by Quorum Health Resources to assess the healthcare needs of the community it serves. This plan includes market assessment of the service area and local demographic characteristics, clinical service line utilization and projections for inpatient, ambulatory, and provider practices. Clinical foci resulting form the same diabetes, obesity, and primary care access.
sch h, Part VI, Line 3:Part of the Hospital's written financial assistance and charity care policy provides for financial counselors to meet with all uninsured patients prior to discharge. The Hospital's billing team may also inform patients of the Hospital's financial assistance and charity care program.
sch h, Part IV, Line 4:The Hospital's primary service area is Western Colorado and the residents of Mesa County. Mesa County's population is approximately 150,000 with 15% of the population being over the age of 65. Community Hospital captures 15.9% of the area's healthcare market for those patients requiring inpatient stays.
sch h, Part VI, Line 5:The Hospital is governed by a 12-member Board of Trustees composed of seven Mesa County, Colorado, community members and five community medical staff members who are all committed to fulfilling the following mission statement: Community Hospital will improve the health and quality of life of the individuals and communities we serve. The community members use their unique experience and abilities to lead the Hospital in directions that support the needs of the community.
