PART V SEC B LINE 5Qualitative data was gathered during individual interviews with a total of 11 stakeholders of South Florida Baptist Hospital service area. These stakeholders were identified by a CHNA oversight committee including Hospital leadership and collaborating areas of other Hospital Facilities in BayCare Health System. Each stakeholder was interviewed by Tripp Umbach, a consulting firm representing the Hospitals, and lasted approximately 60 minutes. All respondents were asked the same set of questions (questions and responses can be found in the CHNA Report starting on Page 51) previously developed by Tripp Umbach and reviewed by the individual Hospitals facility leadership. 1. CEO of the Suncoast Community Health Center 2. Chief of Emergency Medical Services for Plant City 3. Coordinator of San Jose Mission, Catholic Charities Medical Mobile Services 4. Dean and Professor at University of South Florida College of Public Health 5. Director of the Hillsborough County Health Department 6. Executive Director of the YMCA of Plant City 7. Medical Director of an Emergency Department 8. Physician, with the HealthPoint Medical Group 9. Project Manager of the One Bay Healthy Communities 10. Regional Center Manager for Plant City Neighborhood Service Center 11. VP of Behavioral Health Department of BayCare Health System In addition to the specific Stakeholder's identified, South Florida Baptist Hospital gathered qualitative data from residents of the their community by conducting facilitated focus groups. The participants were able to provide direct input related to top community needs of themselves, their families, and communities. The goal of the focus group process is that each participant feels comfortable and speaks openly so that they contribute to the discussion. It was explained to participants that there are no wrong answers, just different experiences and points of view. This process ensures that each participant shares their experiences from their point of view, even if it is different from what others have said. Specifically, focus group participants were asked to identify and discuss what they perceived to be the top health issues and/or concerns in their communities. Focus group input is subject to the limitations of the identified target populations (i.e., vocabulary, perspective, knowledge, etc.), and therefore, is not factual and inherently subjective in nature. The focus group audience participants were: 1. Residents for whom English is a second language 2. Undocumented and/or migrant working residents 3. Obstetric professionals serving families that are at risk of poor birth outcomes
Part V Sec B Lines 6aCHNA was conducted with the following hospital facilities 1. Mease Countryside Hospital 2. Mease Dunedin Hospital 3. Morton Plant Hospital 4. Morton Plant North Bay Hospital 5. Part V Sec B Line 11Based on its most recent Community Health Needs Assessment, South Florida Baptist Hospital is committed to addressing the following significant community needs: 1) Improving access to affordable healthcare 2) Decreasing the prevalence of clinical health issues 3) Improving healthy behavior and environments. Progress was made to address each of the key health needs as prioritized within the 2013 Community Health Needs Assessment. Efforts were specifically dedicated to the following action step categories with highlighted activities as noted below: Improve access to and coordination of mental health services o BayCare is actively recruiting psychiatrists in an effort to expand access. o A management services agreement with Northside Mental Health Center is now in place and is working according to plan to increase access in Hillsborough County. o Adult and pediatric-specific 'Mental Health First Aid' classes are offered by BayCare Health System to increase the communitys awareness to recognize when someone is in mental health distress. Classes are offered through various forms of outreach in Hillsborough County. Continue advocacy outreach efforts to expand access o Navigators are in place across BayCare to facilitate enrollment in marketplace plans. Outreach activities continue (by navigators) to encourage enrollment in health insurance plans across various community locations. Team members are also available to interview uninsured patients in the hospitals and assist insured patients needing assistance for copays/deductibles (by phone or by appointment). The number of people served continues to increase. Enhance cancer prevention and treatment initiatives o Community outreach efforts continue across the health system to encourage the community to understand risk factors and to promote early detection of various types of cancer. o BayCare sites have signed the '80% by 2018' pledge with the American Cancer Society to promote appropriate colorectal cancer screening in 80% of age-appropriate community members by 2018. o The South Florida Baptist Hospital campus remains tobacco-free (and maintain the incentives offered to team members for not smoking). Various smoking cessation programs are offered throughout BayCare. o Collaborative efforts continue across BayCare to provide mammography services to the under-served. Improve care coordination and outcomes for heart failure (CHF) patients o South Florida Baptist Hospital established a CHF clinic in the 4th quarter of 2015. o Efforts to provide consistent care to treat heart failure across BayCare are ongoing. o Efforts are underway to provide consistent patient education materials as well as to develop clinical scorecards and evidence-based measure order sets across locations. o Various initiatives are underway to decrease readmissions across BayCare including daily inpatient rounds; post-discharge phone calls; utilization of faith community nurses; increased referrals to palliative care; establishment of heart failure support groups and nurse practitioner-led transitions of care programs. Sites continue to monitor, analyze and report CHF readmission & length of stay (LOS) metrics. Enhance stroke prevention and recognition initiatives o Hospitals are making significant progress to increase the percentage of patients appropriately receiving thrombolytic therapy (for eligible patients). o The first meeting of the newly formed 'BayCare Stroke Collaborative' took place on 8/15/2015 with defined goals to identify best practices; standardize education and increase BayCare's community presence. o In August 2015, a system-wide neuro-focused Facebook campaign took place which included the online stroke risk assessment; warning signs of a stroke; and the importance of a primary stroke center. o Enhance services to improve birth outcomes. o South Florida Baptist Hospital continues to partner with Healthy Start, March of Dimes, Florida Perinatal Quality Collaborative and Reach Up Hillsborough County to implement best practices and to improve birth outcomes. The hospitals providing obstetrical services throughout BayCare Health System actively participate in The Florida Perinatal Quality Collaborative initiatives to decrease morbidity and mortality in maternity and newborn patients. o BayCare hospitals continue to partner with the Attorney General's office and statewide agencies to address the needs of infants born with neonatal abstinence syndrome by improving education and outreach. Evaluate and align services offered to patients with diabetes to improve health outcomes for diabetic patients o South Florida Baptist Hospital offers the 'Journey for Control' diabetes management course. o The outpatient diabetes management collaborative continues to work together to standardize outpatient education and services across BayCare. o In building on a BayCare-wide initiative in 2014 to implement a diabetic ketoacidosis protocol, a 'Hyperglycemia Power Plan' is integrated within the Electronic Health Record. This will provide for a single standard of care for patients with hyperglycemia across BayCare. Improve care coordination and support initiatives to reduce readmissions o All hospitals continue to strive to reduce readmissions (specific efforts are underway to decrease readmissions from heart failure, diabetes, COPD and other pulmonary diseases). o Medication assistance was provided to 29 patients from January 2014 - March 2016 through the BayCare Indigent Drug Reimbursement Program at South Florida Baptist Hospital. The value of pharmaceuticals provided for free to patients/hospital bills credited for this time period was $92,037. Facilitate access to transportation services for those in need o South Florida Baptist Hospital continues to facilitate and/or directly provide transportation to patients who are not able to afford it post-discharge. The following is a list of those needs that were identified as not being met by the hospital: Chronic environmental stressors in the service area: While hospital leaders are interested in this issue and intend to re-evaluate the need, there are organizations offering services that address environmental stressors for residents in the service area. Improving the environmental stressors of residents in the service area is not directly related to the mission of South Florida Baptist Hospital. However, the hospital does address socio-economic issues through financial assistance and community benefits as it relates directly to healthcare and medical services of residents that are under/unfunded. Long-term acute care: While hospital leaders are interested in this issue, and are interested in further evaluating the barriers that residents experience when seeking long-term care, the South Florida Baptist Hospital does not currently have the expertise, resources, and/or provider base to provide these services. Because the primary needs within the community have dictated that financial and human resources of South Florida Baptist Hospital are utilized for diagnostic and therapeutic medical and surgical care, hospital leaders have determined that long-term care services could be better met by existing providers, allowing available resources to remain focused on the existing and planned health services. However, the need as identified has increased awareness and may be further evaluated. While hospital leaders are interested in this issue, and are interested in further evaluating the barriers that residents experience when seeking long-term care, the South Florida Baptist Hospital does not currently have the expertise, resources, and/or provider base to provide these services. PART I LINE 3CPatients who are uninsured or underinsured and cannot pay for hospital services are eligible for charity consideration. These patients are screened by designated team members in our Financial Assistance Department. The Agency for Health Care Administration (AHCA) defines charity eligibility at 200 percent of the federal poverty guidelines, unless the total hospital bill is more than 25 percent of the patient's annual income. Medicaid recipients who have exceeded their coverage limits are also considered for charity care. South Florida Baptist Hospital, Inc goes above and beyond the AHCA requirements by providing additional "hardship" charity for patients who are at 250 percent of the federal poverty guidelines. In addition, an uninsured discount of 40% is given to any patient who does not have insurance coverage or benefits. There is no income or asset test required for the uninsured discount. Patients receive an additional 10% discount if the account is paid within 30 days.
PART I LINE 6AThe Community Benefit Report is available to the public and was prepared by BayCare Health System Inc., a related organization.
PART I LINE 7Financial assistance and means-tested government programs costs (lines A through D) are determined using our cost accounting system, which captures all inpatients and outpatients, including emergency room patients. The system also captures all patient pay types - private insurance, Medicare, Medicaid, uninsured and self pay. The costs have been offset by any payments received from Medicaid or any other uncompensated care program. Other benefits at cost (lines E through J, as well as amounts reported in Part II) were compiled by the community health department using the Catholic Health Association guide for planning and reporting community benefits.
PART I LINE 7 COLUMN FBad Debt expense of $14,394,380 was included on form 990, Part IX, Line 25, Column (a), but subtracted for purposes of calculating the percentage in this column. Part II South Florida Baptist Hospital supports the health of its community through coalition building with the Plant City Kiwanis Club which provides support to the local elementary and high schools by providing over 2,000 children with supplies and backpacks.
PART IIILines 2 & 3 Bad debt expense is reported as total bad debt for the facility. The amount of bad debt expense attributable to patients eligible for financial assistance is calculated as a charge ratio, derived from data sampling. The resulting charge ratio is then applied to total bad debt accounts of the organization, which calculates the bad debt attributable to finance assistance. The state of Florida requires the patient to provide certain documentation in order to qualify for financial assistance. In cases where the patient has not responded to hospital requests or billing statement alerts, those accounts are processed as bad debt, if unpaid. Part III Line 4 The organization's financial statements include a footnote that describes bad debt expense on page 9 of the South Florida Baptist Hospital, Inc. and Subsidiary Notes to Consolidated Financial Statements.
PART III LINE 8Cost reports were used to report Medicare allowable costs. Medicare defines allowable costs as those appropriate and helpful in developing and maintaining the operation of patient care facilities and activities. It specifically excludes certain costs that are not directly related to patient care. The hospital incurs additional expense related to the provision of care to Medicare patients that Medicare has deemed non-allowable. This additional expense includes costs of physician services (emergency on-call fees, hospitalist program, recruitment, etc.), advertising costs, cafeteria costs for meals sold to visitors, etc. The hospital attempts to collect coinsurance and deductibles from Medicare beneficiaries. To the extent collection efforts are unsuccessful, Medicare reimburses the hospital at 70% of unpaid amounts. The following table reconciles the surplus or shortfall from Line 7 to the actual surplus or shortfall. The additional costs were allocated to Medicare based upon Medicare's percentage of total allowable costs. The unpaid coinsurance/deductibles were estimated using historical collection results. Any shortfall amounts have not been treated as community benefit. Line 7 Surplus or (Shortfall) ($1,587,339) Additional non-allowable costs and unpaid/non-reimbursed coinsurance/deductibles ($2,297,266) Total Surplus or (Shortfall) ($3,884,605)
PART III LINE 9BPatients who are unable to pay are encouraged by BayCare Health System representatives, via personal interviews, signage, on patient billing statements, brochures or Customer Service phone calls, to submit financial information to the Financial Assistance Department to determine eligibility for programs, such as County, Medicaid, Disability, Victims of Crime, Charity, etc. For those patients who provide all the necessary documentation and qualify for charity according to the Financial Assistance policy, (defined in Part I, line 3c), patients' account balance would be written off completely to charity and not billed to the patients.
PART VI LINE 2 - NEEDS ASSESSMENTSouth Florida Baptist Hospital, Inc. is committed to meeting the needs of the community it serves. Our quality philosophy is modeled around understanding our customers' needs in the communities it serves. In addition to the aforementioned comprehensive process, South Florida Baptist Hospital, Inc also informs and educates patients who may be billed for patient care, but may be eligible for charity or other programs, via patient billing statements and customer service representative calls. PART VI LINE 5 - PROMOTION OF COMMUNITY HEALTHSouth Florida Baptist Hospital was founded in 1953 as a result of a partnership between many generous, civic-minded citizens of Plant City and the dedicated financial support of the Florida Baptist Church Convention. During its inception, the entire Plant City community rallied behind the idea of building the city's first hospital in the post WWII years. Today, South Florida Baptist Hospital is a full-service 147-bed community hospital that offers high-tech services of a larger acute care hospital, but in a small, warm and friendly environment designed for personal attention and compassionate care. South Florida Baptist Hospital provides the surrounding Plant City community with services that include surgery, diagnostics, rehabilitation, acute and intensive care, pediatric care, obstetrics, interventional heart and vascular services, and emergency care. While much has changed during the past 60 years, our commitment to improve the health of our community through accessible, compassionate and family-focused health care services remains. Community Outreach & Partnerships Through education and a focus on prevention and early detection, we are building a healthier community while reducing suffering and total health care costs. South Florida Baptist Hospital participated in more than 100 community health improvement events, touching more than 6,000 lives in 2015. More than $430,000 was spent to support our strategic directions to foster and implement community relationships and partnerships to improve the health status of our community. We offer a variety of free services such as health screenings, physician lectures, support groups, health fairs and mobile clinics. To extend its reach, South Florida Baptist Hospital collaborates with several community partners. By sharing resources, we are able to find less expensive ways to make an even greater health impact without duplicating our efforts. The Faith Community Nursing program, sponsored by South Florida Baptist Hospital, was established to provide health and wellness information to members of local congregations of all denominations. The goal is to assist congregations in developing health ministries that compassionately and effectively address the emotional, physical and spiritual needs of individuals within the congregation and the people they reach out to in their communities. The hospital provides the infrastructure for the Faith Community Nurse Program and assists the faith community by providing recruitment, training, continuing nurse education and health screenings. In 2015, volunteer nurses worked with congregations in the Plant City, Brandon, Lithia, Zephyrhills and Lakeland areas. Through home and hospital visits, parish nurse office visits or other encounters at the church, educational programs, health fairs and other forms of correspondence, Faith Community Nurses directly touched the lives of more than 5,000 people. In total, more than 4,000 hours were served by South Florida Baptist Hospital. South Florida Baptist Hospital's Community Health team develops community partnerships with area agencies, creating collaborative efforts that bring health services directly into area neighborhoods. As a result, Community Health was able to promote better health to 6,017 people in 2015. Almost 50 percent of the events are outreach activities offered outside the hospital on the Wellness on Wheels mobile health unit. The 37-foot Winnebago is equipped to provide health screening that include cholesterol, blood sugar and blood pressure, body mass index and skin cancer checks, physical exams and wellness education. The unit was purchased through community fundraising. Annually, the RICOH Children's Classic Golf Tournament is held to support children's health services provided on the Wellness on Wheels bus. Health Education provided by Community Health in 2015 included CPR and AED certifications, Germ-Busters (hand hygiene program) and safety education games for children. Community Health also provided diabetes education for adults and children, and nutrition education for children. Through a partnership with Hillsborough County Schools, Community Health helped facilitate more than 2,900 elementary and middle school vision, scoliosis and height and weight screenings on school aged children. South Florida Baptist Hospital participates in activities surrounding the community's largest event, the Plant City Strawberry Festival. Hospital team members often volunteer in various functions during the event, and the South Florida Baptist Hospital Wellness on Wheels health unit participates in both the Strawberry Festival Grand Parade and the Youth Parade. These two events kick off the festival celebrations and are heavily attended by Plant City residents. South Florida Baptist Hospital also actively supports the Plant City Chamber of Commerce, which promotes the commercial, industrial, agricultural, and civic interests of Greater Plant City and East Hillsborough County. As a not-for-profit hospital, South Florida Baptist Hospital provides many services to the community at no charge or at a reduced charge. For example, Ladies Night Out is a creative seminar series featuring different health topics for women of all ages presented by SFBH professionals and physicians. With nearly 26 million children and adults afflicted with the disease in the U.S., and an additional 79 million at risk for type 2 diabetes, diabetes has reached epidemic proportions. Understanding how to manage this complex disease begins with education. To meet the community's need for diabetes education, South Florida Baptist Hospital offers The Journey for Control, a series of diabetes classes that teaches participants to build an action plan to make changes in their lifestyle choices and behaviors. The course consists of a series of four classes. Content is geared toward adults who are diabetic, pre-diabetic or are a caregiver for a diabetic. Part VI, Line 11b - Describe the Process used by Management &/or Governing Body to Review 990 The Form 990 is prepared by the organization and reviewed by the CFO, as well as the organization's paid preparer. Prior to filing with the IRS, a final copy of the Form 990 was provided to the entire Board via a web portal. Part VI, Line 12c - Description of Process to Monitor Transactions for Conflicts of Interest South Florida Baptist Hospital, Inc. has two separate conflict of interest procedures; one that relates to Board members and another that relates to non-board member employees. Both groups are required on an annual basis to complete, sign and file an annual disclosure statement detailing existing or potential conflicts of interests. For Board members, the review of conflicts or potential conflicts occurs at the Board or committee level. After disclosure of the Board Member's or Committee Member's actual or potential conflict, the following procedures for addressing the conflict of interest will be adhered to by each Board and all Committees with Board delegated powers, without exception: 1. The interested Director or Committee member shall leave the Board or Committee meeting while the conflict of interest issue is discussed. 2. The remaining Board or Committee Members shall decide if a conflict of interest exists. 3. If a conflict of interest is deemed to exist: a. The Chairperson of the Board or Committee shall, if appropriate, appoint a disinterested individual or committee to investigate the proposed transaction or arrangement. b. The Board or Committee shall determine whether the BayCare entity can obtain a more advantageous transaction or arrangement with reasonable efforts from an individual or entity that would not give rise to a conflict of interest. c. If a more advantageous transaction or arrangement is not reasonably available, the Board or Committee shall determine whether the transaction or arrangement is in the BayCare entity's best interest, and whether the transaction is fair and reasonable to BayCare. An interested Director or Committee Member shall not vote, participate in, influence or attempt to influence any determination or proceedings. The Director or Committee Member may, however, respond to questions posed by the Board or Committee regarding the contract or transaction. Any such contract or transaction must be authorized by a vote of at least two-thirds (2/3) of the Directors or Committee Members entitled to vote at a meeting at which a quorum was present. Any interested Director or Committee Member may not be counted in determining the existence of a quorum. For employees, the review of conflicts of interest or potential conflicts goes to the Conflict of Interest Determination Committee. This committee consists of BayCare Chief Compliance Officer, the Corporate Responsibility Officers, and the BayCare Vice President of Team Resources. This committee shall determine if an actual conflict exists and any action required to address the conflict of interest situation.
PART VI, LINES 15A & 15BProcess used for Compensation Review and Approval The filing organization does not directly compensate some of its top management employees; rather compensation is paid by a related organization that also follows the compensation policy of the Compensation Committee. The independent Compensation Committee is appointed by the Board of Directors. The Compensation Committee's purpose is to provide oversight for the organization's executive compensation program, review and approve compensation and benefits for all "disqualified persons" subject to the Intermediate Sanctions regulations issued under Section 4958 of the Internal Revenue Code (including the Chief Executive Officer, Chief Operating Officer & Chief Financial Officer, other system and entity executives, and other disqualified persons as defined in the Intermediate Sanctions regulations (i.e., voting members of the governing body, family members, former officers)), and establish the compensation philosophy for all other executives. This committee engages nationally recognized compensation consultants to assist them in review of executive compensation. The compensation consultants provide a review of each vice president and above in the system to determine if that employee's compensation is reasonable when compared against market standards. The data reviewed comes from compensation studies that include comparable compensation for similarly qualified persons in functionally comparable positions at similarly situated organizations. The organization keeps contemporaneous minutes of the compensation committees meetings and decisions. External consultants review compensation every other year, the last review occurring in 2015, but the compensation committee regularly monitors compensation and all other procedures are followed annually. Part VI, Line 19 - How and If the Governing Documents, Conflict of Interest Policy and Financial Statements are Made Available to the Public South Florida Baptist Hospital, Inc. publishes its financial statements with the Agency for Health Care Administration. Governing documents are available to the public via Sunbiz.org.
