DescSt. Luke's Cornwall Hospital ("SLCH") is a not-for-profit hospital organization dedicated to serving the healthcare needs of those in the Hudson Valley. On January 2016, The Montefiore Health System, Inc. acquired St Luke Cornwall Hospital to create an integrated healthcare delivery system, providing quality comprehensive healthcare services. With dedicated staff, modern facilities and state-of-the-art treatment, SLCH is committed to meeting the needs of the community and continuing to aspire to excellence. St Luke's Cornwall Hospital with main campuses in Newburgh and Cornwall, and off-site facilities throughout the community, is a 345 bed acute care hospital dedicated to providing for the health care needs of the community. St Luke's Cornwall Hospital is the regional center for neonatology, thanks to its Elaine Kaplan Neonatal Intensive Care Unit. The Hospital also offers other pediatric subspecialties, as well as a robust list of both inpatient and outpatient services including a cardiac catheterization lab, Intensive Care Unit, Birthing Center, Metabolic and Bariatric Surgery, Thoracic Surgery and much more. The St Luke's Cornwall campus is a thriving outpatient focused Center. This vital and growing community resource offers necessary healthcare access points to area residents for a variety of services including the Littman Center for Emergency Medicine, Radiation Oncology Services, Littman Cancer Center, Wound Care, Kreisberg Pain Management, Eastern Orange Ambulatory Surgery Center, Physical Therapy among others. St Luke's Cornwall Hospital provides medically necessary healthcare services to all individuals in a non-discriminatory manner regardless of race, color, creed, sex, national origin or ability to pay. St Luke's operates two active emergency departments (the Emergency Department in Cornwall closed in January 12,2017) for all persons which is opened 24 hours, 365 days of the year. The emergency rooms are one of the newest and busiest emergency departments between New York City and Albany. With a recent $11 million renovation and expansion, SLCH's emergency services were designed for patient comfort and convenience. General trauma bays and specialty treatments bays for cardiac, asthma, Women's Health and Pediatrics enable SLCH's highly skilled ER team to provide comprehensive, quality care in a Patient-centered environment. To accommodate growing outpatient services, several satellite facilities were opened, including the Dialysis Center, Rehabilitation Center at New Windsor, and the Outpatient Care Center located on Route 9W in the Town of Newburgh. St Luke's Cornwall Hospital offers centers of excellence in cancer service, cardiovascular care, angioplasty, cardiac rehabilitation, orthopedics, sport medicine, and metabolic and bariatric surgery. St Luke's was one of the first hospitals in the region to earn the distinction of being recognized as a New York Stroke Center. Additional outpatient services at SLCH include, but are not limited to; Imaging Services, Laboratory Services, Physical Therapy, Pain Management, Outpatient Nutrition Counseling, Sleep Center, Stroke Care, Wound Healing & Hyperbaric Medicine, Psychological Services, Epilepsy Monitoring and Home Care. SLCH recently Earned 'Top Performer' Recognition from The Joint Commission on key quality measures. SLCH has been recognized for its excellence in management of Heart Attack, Heart Failure, Stroke and Surgical Care. SLCH has recently achieved the Healthgrades Patient Safety Excellence Award, a designation that honors hospital performance in the prevention of thirteen serious, potentially preventable complications during hospital stays. The distinction makes SLCH the only hospital in New York State to receive this award for five consecutive years. This achievement also names SLCH as the only hospital in the state to be ranked among the top 5% in the nation for patient safety three years in a row, as evaluated by Healthgrades, the leading online resource for comprehensive information about physicians and hospitals. SLCH recently received its fifth straight "Get With the Guidelines Gold Plus Achievement Award" from the American Stroke Association. The goal of the program is to improve the overall quality of care for stroke patients by improving acute stroke treatment and preventing future strokes and other cardiovascular events. Hospitals that receive the "Get With The Guidelines Gold Plus Achievement Award" have reached an aggressive goal of treating stroke patients with 85% or higher compliance to core standard levels of care as outlined by the American Stroke Association for 12 consecutive months. SLCH was also named to the Target: Stroke Honor Roll for achieving Time to Intravenous Thrombolytic Therapy. For the third straight year, SLCH has been included on the list of Health Cares Most Wired hospitals, ranking among the nations best in using information technology to improve patient care. The survey, recently released by the American Hospital Associations (AHA) Health Forum, is a leading industry barometer measuring information technology use and adoption among hospitals nationwide. SLCH has also been designated as an Institute of Quality for Cardiac Medical Intervention by Aetna. SLCH was chosen as an Aetna designated IOQ facility because it demonstrates excellent in care, a commitment to continuous improvement and represent an exceptional value to our patients.Desc

ExplanationENDOWMENT FUNDS ARE USED CONSISTENT WITH INTENT AND IN FURTHERANCE OF THE ORGANIZATION CHARITABLE TAX-EXEMPT PURPOSES. ST. LUKE'S CORNWALL HOSPITAL has adopted investment and spending policies for endowment assets that attempt to provide a predictable stream of funding to programs supported by its endowment. AN INDEPENDENT CPA FIRM AUDITED THE CONSOLIDATED FINANCIAL STATEMENTS OF ST. LUKE'S CORNWALL HOSPITAL AND SUBSIDIARIES FOR THE YEARS ENDED DECEMBER 31, 2016 AND DECEMBER 31, 2015; RESPECTIVELY. THE FOLLOWING FOOTNOTE IS INCLUDED IN THE ORGANIZATION'S AUDITED CONSOLIDATED FINANCIAL STATEMENTS THAT REPORTS THE ORGANIZATION'S TREATMENT WITH RESPECT TO TEMPORARILY AND PERMANENTLY RESTRICTED NET ASSETS: TEMPORARILY RESTRICTED NET ASSETS ARE AVAILABLE TO SUPPORT PROGRAM ACTIVITIES AS STIPULATED BY DONORS. ST. LUKE'S CORNWALL HOSPITAL ("HOSPITAL") FOLLOWS THE REQUIREMENTS OF THE NEW YORK PRUDENT MANAGEMENT OF INSTITUTIONAL FUNDS ACT ("NYPMIFA") AS THEY RELATE TO ITS PERMANENTLY RESTRICTED CONTRIBUTIONS AND NET ASSETS, EFFECTIVE UPON NEW YORK STATE'S ENACTMENT OF THE LEGISLATION IN SEPTEMBER 2010. THE HOSPITAL HAS INTERPRETED NYPMIFA AS REQUIRING THE PRESERVATION OF THE FAIR VALUE OF THE ORIGINAL GIFT AS OF THE GIFT DATE OF THE DONOR-RESTRICTED ENDOWMENT FUND, ABSENT EXPLICIT DONOR STIPULATIONS TO THE CONTRARY. THE HOSPITAL CLASSIFIES AS PERMANENTLY RESTRICTED NET ASSETS THE ORIGINAL VALUE OF THE GIFTS DONATED TO THE PERMANENT ENDOWMENT AND THE ORIGINAL VALUE OF SUBSEQUENT GIFTS TO THE PERMANENT ENDOWMENT. ACCUMULATED EARNINGS OF THE PERMANENT ENDOWMENT ARE USED IN ACCORDANCE WITH THE DIRECTION OF THE APPLICABLE DONOR GIFT. THE REMAINING PORTION OF THE DONOR-RESTRICTED ENDOWMENT FUND THAT IS NOT CLASSIFIED IN PERMANENTLY RESTRICTED NET ASSETS IS CLASSIFIED AS TEMPORARILY RESTRICTED NET ASSSETS UNTIL THE AMOUNTS ARE APPROPRIATED FOR EXPENDITURE IN ACCORDANCE WITH A STANDARD OF PRUDENCE PRESCRIBED BY NYPMIFA. THE INTEREST IN THE FOUNDATION INCLUDED IN TEMPORARILY RESTRICTED NET ASSETS IS PRIMARILY RESTRICTED FOR CAPITAL EXPENDITURES.ExplanationTxt

SCHEDULE H, PART V, Section B, Line 5The process for preparing the 2016-2018 Community Health Needs Assessment was a inter-organizational and community collaborative process, initiated with the goal of developing an assessment that was reflective of the needs of the community. The assessment was conducted using survey data from the Orange County Department of Health in collaboration with input from community partners. SLHC leads the Population Health Coalition, a group formed in 2014 that includes more than 30 local organizations focusing on improving the health of the population served. The group is comprised of disease specific advocacy, local health care providers, including primary and specialty care practices, rehabilitation centers, emergency medical services, and federally qualified health centers, as well as organizations that focus on housing, the aging population, employment, mental health and special needs. The Population Health Coalition strives to identify gaps in care, consolidate resources and information and collaborate on projects that serve the community.

Schedule H, Part V, Section B, Line 11A review of the results from the data collection in the Hospital's most recent conducted CHNA illustrated that SLCH should continue to retain the two priority areas selected in 2013 (1) Preventing Chronic Diseases and (2) Promoting Healthy Women, Infants and Children. Across these two focus areas, various goals, with specific interventions, performance measures and time frames were delineated; - Increase screening rates for cardiovascular disease, diabetes, and cancer; - Increase access to high quality chronic disease preventive care and management in clinical and community settings; - Increase rates for infants exclusively breastfed in the hospital. In addition to working with the Population Health Coalition, St Luke's Cornwall Hospital has it own community education program that reaches a wide variety of audiences on a number of topics related to its two prevention agenda items. Education is provided at a number of local senior groups, workplace wellness programs, local businesses, food pantries, soup kitchens and public events. In 2016, SLCH provided over 100 educational and screening events to the community. Future plans will build upon this existing model by identifying other audiences in need of educational programming, as well as utilizing digital outreach through the internet and social media. The Population Health Coalition has also succeeded in compiling an interactive directory of local community resources that can be utilized to better connect the population to appropriate services to best fit their needs.
Schedule H, Part V, Section B, Line 13HThe hospital uses a credit assessment tool to help determine if certain patients can auto-qualify for a charity care discount based on presumption eligibility.

1. Part I, Line 7The cost-to-charge ratio methodology was utilized to calculate the amount included in the table. The calculation of this ratio was derived from ratio of patient care cost-to-charge.
1. PART III, Section A, LINES 2The cost of bad debt expense is estimated based on the bad debt provision at charge, applied to the ratio of total patient care expenses to total charges for all services rendered. Any payments or discounts are excluded from bad debt expense. 1. Part III, Section A, Line 3 The estimated amount of the organization's bad debt expense (at cost) attributed to patients under the organization's charity care policy was based on results of predictive analysis. Bad debt should be included as a community benefit because the organization provides much needed health care services indiscriminately to the community-at-large without regard to whether or not the patient has insurance or if the bill will ever be paid. 1. Part III, Section A, Line 4 As reported in St Luke's Cornwall Hospital and Subsidiaries audited financial statements, bad debt expense is described as follows: "Account Receivable are also reduced by an allowance for doubtful accounts. In evaluating the collectability of account receivable, the Hospital analyzes its past history and identifies trends for each of its major payer sources of revenue to estimate the appropriate allowance for doubtful accounts and provision for bad debts. Management regularly reviews data about these major payor sources of revenue in evaluating the sufficiency of the allowance for doubtful accounts."
1. PART III, Section B, LINE 8ST. LUKE'S CORNWALL HOSPITAL OBTAINED THE MEDICARE GROSS REVENUE FROM ITS INTERNAL COST ACCOUNTING SYSTEM. THE MEDICARE GROSS REVENUE IS MULTIPLIED BY THE RATIO OF COST TO CHARGES ("RCC"). THE RCC IS CALCULATED BY DIVIDING THE AUDITED EXPENSES BY TOTAL GROSS REVENUE. The Medicare Shortfall should be considered a community benefit since THESE SERVICES AND RELATED COSTS PROMOTE THE HEALTH OF THE COMMUNITY AS A WHOLE AND ARE RENDERED IN CONJUNCTION WITH THE ORGANIZATION'S CHARITABLE TAX-EXEMPT PURPOSES AND MISSION IN PROVIDING MEDICALLY NECESSARY HEALTHCARE SERVICES TO ALL INDIVIDUAL'S IN A NON-DISCRIMINATORY MANNER WITHOUT REGARD TO RACE, COLOR, CREED, SEX, NATIONAL ORIGIN, RELIGION OR ABILITY TO PAY AND CONSISTENT WITH THE COMMUNITY BENEFIT STANDARD PROMULGATED BY THE IRS. Providing CARE FOR THE ELDERLY AND SERVING MEDICARE PATIENTS IS AN ESSENTIAL PART OF THE COMMUNITY BENEFIT STANDARD. MEDICARE, LIKE MEDICAID, DOES NOT PAY THE FULL COST OF CARE. MANY MEDICARE BENEFICIARIES, LIKE THEIR MEDICAID COUNTERPARTS, ARE POOR. MORE THAN 46 PERCENT OF MEDICARE SPENDING IS FOR BENEFICIARIES WHOSE INCOME IS BELOW 200 PERCENT OF THE FEDERAL POVERTY LEVEL. MANY OF THeSE MEDICARE BENEFICIARIES ARE ALSO MEDICAID eligible.
1. Part III, Section C, Line 9BFor patients who are known to qualify for Financial assistance, all collection activity is halted and the patient is referred for financial aid. If an account is in collection and the patient requests financial aid or if the agency determines that the patient is eligible for financial assistance, the account is referred back to the hospital where the patient is provided assistance with completing an application for assistance.
2. Need AssessmentThe organization assesses community needs by working jointly with a broad spectrum of community partners. SLHC leads the Population Health Coalition, a group that includes more than 30 local organization focused on improving the health of the population served. The group is comprised of disease specific advocacy, local health care providers, including primary and specialty care practices, rehabilitation centers, emergency medical services, and federally qualified Health Centers, as well as organization that focus on housing, the aging population, employment, mental health and special needs. The Population Health Coalition strives to identify gaps in care, consolidate resources and information and collaborate on projects that serve the community. SLCH also uses Primary and secondary data collections to assess the health needs of the community. Patient survey data obtained and compiled by the Orange County Department of Health are used as a secondary data source to supplement input obtained directly at COMMUNITY AND TOWN COUNCIL MEETINGS, MEDICAL EDUCATION SEMINARS, PHYSICIAN PRESENTATIONS AND VARIOUS HEALTH SCREENING EVENTS.
3. Patient Education of Eligibility for AssistanceAll intake, registration and collection agency staff is trained on the Hospital's Financial Aid Policy and how to provide patients with assistance. When it is determined that a patient is having difficulty paying their bill, a referral is made to the Hospital's Financial Aid Office and a Financial counselor will help the patient apply for financial aid. The hospital makes its financial aid policy known to the public by providing written information at patient service areas, information posted on the internet and information sent out on patient's bills.
4. Community InformationST. LUKE'S CORNWALL HOSPITAL ("SLCH") in comprised of TWO HOSPITAL CAMPUSES IN CORNWALL, NY AND NEWBURGH, NY AS WELL AS VARIOUS OFFSITE FACILITIES THROUGHOUT THE COMMUNITY.
