Mission
NORTHWELL HEALTH PLANS HOLDING COMPANY, INC. IS THE PARENT COMPANY OF CARECONNECT GROUP HOLDING COMPANY, INC. AND NORTH SHORE-LIJ HEALTH PLAN, INC.("HEALTH PLAN"). HEALTH PLAN PROVIDES MANAGED LONG-TERM CARE SERVICES TO CHRONICALLY ILL OR DISABLED MEDICAID BENEFICIARIES WHO REQUIRE MORE THAN 120 DAYS OF CONTINUOUS CARE. IN ADDITION, HEALTH PLAN PROVIDES FULLY-INTEGRATED MEDICARE AND MEDICAID BENEFITS UNDER THE FULLY INTEGRATED DUALS ADVANTAGE DEMONSTRATION.
