DescThe Children's Center, Inc. d/b/a Bethany Children's Health Center (Bethany Health Center) is an innovative leader in the field of pediatric rehabilitation and 24-hour complex care. The private, nonprofit hospital, located in Bethany, Oklahoma, offers inpatient and outpatient services for children. The Complex Care Unit provides specialized health care to children with complex medical needs. The patients served in this unit often present with chronic health conditions and require ongoing medical care. The Transitional Care Unit at Bethany Children's Health Center serves children who would benefit from an individualized plan of care for their unique medical and therapy needs. (continued on Schedule O)Each patient in the Transitional Care Program is evaluated by the interdisciplinary team and receives a comprehensive plan to address their specific functional and medical goals. Bethany Health Center offers 24-hour medical care, comprehensive rehabilitative therapies including, but not limited to, physical therapy, occupational therapy, and speech therapy, and can include other specialized services as directed by physicians. Respiratory care, nutritional support, complex respiratory care, complex post-operative care, palliative care support, and special education are offered to meet the medical, nursing and therapy needs of a variety of patients requiring post-acute, inpatient care before the transition home. Bethany Health Center is accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF) and the Accreditation Commission for Health Care (ACHC).Bethany Children's has firsthand experience with the growing need for outpatient services. In 2017, Bethany Children's outpatient services relocated to the newly constructed tower on campus, enabling significant growth through additional providers and expanded services. In 2024, Bethany Children's outpatient therapy department recorded 13,720 total patient visits, up from 11,466 in 2023, a growth of approximately 20%. The outpatient medical clinic visits increased by 22.5% from 9,130 in 2023, to 11,184 in 2024, and significant growth is expected in 2025 and 2026 as well. As demand for these services continues to grow, the space currently used by outpatient providers will not be sufficient to deliver the level of care and service to which Bethany Children's is committed. With the new outpatient tower scheduled to be open and fully occupied by early 2027, we anticipate serving significantly more children and families across our region. The Bethany Health Center provides health care services to patients who meet certain criteria under its charity care policy without charge or at amounts less than established rates. Since the Bethany Health Center does not pursue collection of these amounts, they are not reported as patient service revenue.Desc

The Children's Center, Inc.Part V, Section B, Line 5: Bethany Children's Health Center held four 1x1 interviews with community stakeholders to facilitate discussions and get input around the health needs and resources in the community. The invitees were selected from a broad range of individuals in the community including populations from medically underserved and low-income communities. Communities represented include a parent of a BCHC patient, BCHC Foundation Board Member, Oklahoma Complete Health, Referring Physician to BCHC, teacher at a local school, and Regional Department of Health.
The Children's Center, Inc.Part V, Section B, Line 11: The Children's Center conducted a community health needs assessment in 2024. The following are the needs identified along with the FY2025 initiatives.1. Mental/Behavior Health2. Access to Healthcare3. Health Education4. Number of PhysiciansMental/Behavioral HealthWe are developing a proactive, parent-focused education series to provide families with tools to build healthy routines at home.Understanding the complex needs of children in foster care or adoptive placements, we are planning a dedicated clinic that integrates behavioral and medical services in one location. As rates of autism diagnoses rise and access to timely evaluations remains limited, Bethany Children's is exploring the development of a comprehensive autism evaluation clinic to fill a critical service gap in early diagnosis and care planning. Our intensive feeding program is being evaluated for expansion to include conditions such as Avoidant/Restrictive Food Intake Disorder (ARFID) and other feeding challenges rooted in trauma or anxiety.Led by Dr. Cieminski, this clinic will guide teens with complex medical, developmental, or acquired disabilities as they transition into adulthood. Patients will be seen at ages 15, 17, and 19 by a coordinated team-physicians, therapists, psychologists/neuropsychologists, and social workers-who will help answer key questions about guardianship, adult providers, post-secondary planning, and state-based supports.Already operational and awaiting formal marketing rollout, the TEDDIES Clinic offers neuropsychological evaluations for medically complex children aged 0-6, with a goal of ensuring no child waits longer than six months for assessment. In collaboration with interdisciplinary teams, our behavioral health providers are participating in a care transformation initiative to evaluate driving readiness and safety for youth with brain injuries or developmental challenges.Access to Healthcare Bethany Children's Health Center is actively working to improve access to specialized pediatric care through the expansion of outpatient services and infrastructure. As the demand for pediatric rehabilitation and complex care continues to grow, we are responding with strategic investments that will ensure more families can receive timely, high-quality care -both in person and virtually. A centerpiece of this effort is our new 200,000-square-foot Outpatient Center of Excellence, currently under construction on our main campus. This state-of-the-art facility will significantly increase our capacity to serve children and families across Oklahoma by reducing waitlists for therapy and specialty services, supporting new clinical programs, anp enabling greater collaboration through shared learning environments and advanced technologies. The new center will house a comprehensive range of outpatient services - including physical, occupational, speech, recreational, and music therapies, as well as behavioral health and specialty medical clinics-all under one roof. These services are essential to helping children recover from life-altering injuries or illnesses and regain independence, function, and quality of life. In addition to physical expansion, we are continuing to grow our virtual care capabilities. Health EducationOur outreach is rooted in two major statewide programs-Healthy Schools Oklahoma (HSOK) and Safe Kids Oklahoma (SKO)-which together provide evidence-based education, safety training, and critical resources across urban and rural communities.HSOK supports 68+ elementary schools statewide, trains over 125 educators annually, and delivers comprehensive health education focused on nutrition, physical activity, and injury prevention.SKO works to prevent unintentional injury and death through education, resource deployment, and community service delivery.Number of PhysiciansAs identified through our 2025 Community Health Needs Assessment, the shortage of pediatric specialists across Oklahoma is a critical barrier to care for many families. Bethany Children's Health Center is committed to expanding access to high-quality, specialized pediatric care by growing our team of physicians in key areas that align with both our mission and the needs of our community. Child Psychiatry There is a need for child psychiatric services in Oklahoma, with only nine counties statewide currently having access to a child psychiatrist. Bethany Children's has just one child psychiatrist on staff, and we recognize the urgent need to expand this service. Addressing this gap is a top priority, and we are actively exploring opportunities to recruit additional child psychiatrists to meet growing demand and better support the mental and behavioral health of our patients. Child Neurology We are focused on expanding specialization within pediatric neurology, with a particular interest in sub-specialties such as neuromuscular disorders. As neurological diagnoses in children become more complex and prevalent, our goal is to enhance our capacity to provide targeted, expert care through strategic recruitment and development of new programs. Pediatric Physiatry Bethany Children's remains the only facility in the state offering inpatient pediatric rehabilitation led by pediatric physiatrists. Given the unique and essential nature of this specialty, we are continuously recruiting these highly trained providers. Expanding our psychiatry team will allow us to serve more children with complex rehabilitation needs and reduce wait times for families seeking this care. Developmental and Behavioral Pediatrics This is an area of increasing interest, and we believe the addition of developmental behavioral pediatricians could significantly benefit our patient population. Early discussions are underway to evaluate the best path forward in bringing this specialty to our campus, further strengthening our ability to provide comprehensive, family-centered care. By addressing these physician shortages, Bethany Children's is taking meaningful steps to reduce healthcare disparities and ensure that more children across Oklahoma have access to the expert care they need, close to home.
The Children's Center, Inc.Part V, Section B, Line 13b: Patients whose family income is above 400% of the federal poverty guidelines are eligible to receive services at amounts no greater than the amounts generally billed to Oklahoma Medicaid.
The Children's Center, Inc.Part V, Section B, Line 13h: There are instances when a patient may appear eligible for charity care discounts, but there is no financial assistance form on file due to a lack of supporting documentation. Often there is adequate information provided by the patient or through other sources, which could provide sufficient evidence to provide the patient with charity care assistance. In the event there is no evidence to support a patient's eligibility for charity care, Bethany Children's could use outside agencies in determining estimate income amounts for the basis of determining charity care eligibility. Once determined, due to the inherent nature of the presumptive circumstances, the only discount that can be granted is a 100% write off of the account balance. Presumptive eligibility may be determined on the basis of individual life circumstances that may include:1. State-funded prescription programs;2. Homeless or received care from a homeless clinic;3. Participation in Women, Infants and Children programs (WIC);4. Food stamp eligibility;5. Subsidized school lunch program eligibility;6. Eligibility for other state or local assistance programs that are unfunded (e.g., Medicaid spend-down);7. Low income/subsidized housing is provided as a valid address.Patients complete a Hospital Financial Assistance Application to help determine the discounted or free services in which they are eligible. The Application is reviewed with supporting documentation and based on internal guidelines a decision on the applicable discount is made.
The Children's Center, Inc.Part V, Section B, Line 16j: The policy is also provided to patients and families upon admission to the Hospital facility.

Part I, Line 7:There was no charity care as 90% of patients are covered under Medicaid. Unreimbursed Medicaid on line 7b did not result in a community benefit expense as it is reimbursed at cost. Community health improvement services on line 7e are reported based on actual expenses recorded in the Organization's general ledger.
Part III, Line 2:The amount reported on line 2 represents implicit price concessions. The Children's Center determines its estimate of implicit price concessions based on its historical collection experience with this class of patients.
Part III, Line 3:The Children's Center reasonably estimates the portion of implicit price considerations that could have qualified for charity care. An estimated 20.5% of implicit price concessions in 2024 are attributable to patients living below the federal poverty level in Bethany, OK.
Part III, Line 4:The footnote to the Organization's financial statements can be found on pages 13 and 14 of the attached audited financial statements.
Part III, Line 9b:Normal collection procedures will be followed for all patients unless Bethany Children's Financial Assistance Application Form is completed and submitted to Bethany Children's along with required documents listed on the application. The Children's Center does not pursue extraordinary collection activities for 120 days from the date of service while the patient is applying for financial assistance. A 30-day notice would be provided to the patient before any collection activities would begin. A patient has a total of 240 days to apply for financial assistance. After a determination of financial assistance any personal portion due would follow normal collection procedures.
Part VI, Line 2:In addition to the community health needs assessment, the Hospital Executive team has a strategic plan to address the needs of the community. Also, patient satisfaction surveys are distributed at various Hospital events.
Part VI, Line 3:Notification about charity care available from Bethany Children's shall be disseminated by various means, which may include, but are not limited to, the publication of notices in patient bills and by posting notices in hospital hall rooms, in the Conditions of Admission form, admitting departments, hospital business offices, and at other public places as Bethany Children's may elect. Bethany Children's also shall publish and widely publicize a summary of this charity care policy on facility websites, in brochures available in patient access sites and at other places within the community served by the hospital as Bethany Children's may elect. Such notices and summary information shall be provided in the primary languages spoken by the population serviced by Bethany Children's. Referral of patients for charity may be made by any member of the Bethany Children's staff or medical staff, including physicians, nurses, financial counselors, social workers, case managers, chaplains, and religious sponsors. A request for charity may be made by the patient or a family member, close friend, or associate of the patient, subject to applicable privacy laws. The majority of the Hospital's in-patients qualify for Medicaid assistance. In other cases, a social worker at the Hospital will work with the patient and family to educate them on eligibility for financial assistance.
Part VI, Line 4:The Children's Center, Inc. d/b/a Bethany Children's Health Center is a private, nonprofit health care organization located in Bethany, Oklahoma, that offers inpatient and outpatient services for children from birth to 21 years of age who have sustained traumatic injuries or illnesses, including brain or spinal cord injuries. Bethany Children's serves patients from across Oklahoma and other states with diverse backgrounds, ethnicities, and income levels. It is the only inpatient pediatric rehabilitation facility in Oklahoma. In Bethany, Oklahoma, 20.5% of individuals lived below the federal poverty level in 2024.
