Key sources and methods included: Community Survey: The 2023 Waukesha County Community Health Survey, conducted by JKV Research, LLC, collected information from 400 residents on health status, coverage, economic hardship, and mental health. Chronic disease, mental health, and substance use were identified as top issues. The full report is available at ProHealthCare.org. Focus Groups: Three groups comprising 28 participants highlighted barriers to care, including transportation, childcare costs, and long wait times, and emphasized the need for accessible services and community support. Key issues included food insecurity, addiction, mental health, and social isolation. Metopio Data Platform: Used to integrate public and proprietary data, providing curated insights on health behaviors, outcomes, healthcare utilization, and social determinants such as income, education, housing, and transportation. This supplemented primary data findings. Secondary Data Reports: Local and national data on social determinants of health, Waukesha County Health Rankings, and other compiled public health reports were included. Full summaries are available at ProHealthCare.org. Together, these data sources informed the assessment and prioritization of community health needs, ensuring representation of diverse populations and identification of key areas for intervention.

In the past fiscal year, leaders contributed more than 2,000 volunteer hours to 64 organizations, helping strengthen community capacity and foster collaboration around local priorities that influence health and well-being. The hospital is the major sponsor of Leadership Waukesha County, a program that develops emerging community leaders and promotes civic engagement throughout the county. ProHealth also supports neighborhood development initiatives that strengthen connections and improve local environments. In partnership with University of Wisconsin-Madison Extension and the Tower Hill Neighborhood Association, the hospital provides financial support and collaborates on initiatives that build community capacity in the Tower Hill neighborhood. These include educational programs that promote civic participation, organizational development assistance to strengthen leadership, and activities that foster healthy environments and stronger social connections. Neighborhood projects, such as the Tower Hill Neighborhood Garden, serve as hubs for community engagement, nutrition education, and resident leadership development. Extension educators also help connect residents with safety initiatives, home improvement resources, and employment opportunities, while supporting the Prairieville Neighborhood Revitalization Strategy Area strategic plan. ProHealth participates in the Housing Action Coalition of Waukesha County to address local housing needs. The hospital also serves as a host site for Project SEARCH in partnership with TransCen, Inc. Since the program began more than 10 years ago, ProHealth has provided internship opportunities for over 100 individuals with developmental or physical disabilities, helping participants develop job skills and pursue competitive employment opportunities. Through these partnerships and initiatives, ProHealth Waukesha Memorial Hospital strengthens the social, economic, and environmental conditions that support long-term community health and well-being.

ESTIMATES FOR EXPLICIT PRICE CONCESSIONS ARE BASED ON PROVIDER CONTRACT, PAYMENT TERMS FOR RELEVANT PROSPECTIVE PAYMENT SYSTEMS, AND HISTORICAL EXPERIENCE ADJUSTED FOR ECONOMIC CONDITIONS AND OTHER TRENDS AFFECTING THE CORPORATION'S ABILITY TO COLLECT OUTSTANDING AMOUNTS. FOR RECEIVABLES ASSOCIATED WITH SELF-PAY PATIENTS, WHICH INCLUDES BOTH PATIENTS WITHOUT INSURANCE AND PATIENTS WITH DEDUCTIBLE AND COPAYMENT BALANCES DUE FOR WHICH THIRD-PARTY COVERAGE EXISTS FOR PART OF THE BILL, THE CORPORATION RECORDS SIGNIFICANT IMPLICIT PRICE CONCESSIONS IN THE PERIOD OF SERVICE ON THE BASIS OF ITS PAST EXPERIENCE, WHICH INDICATES THAT MANY PATIENTS ARE UNABLE OR UNWILLING TO PAY THE PORTION OF THEIR BILL FOR WHICH THEY ARE FINANCIALLY RESPONSIBLE. THE CORPORATION'S PATIENT ACCOUNTS RECEIVABLE BALANCE CONSISTS OF AMOUNTS DUE FROM ITS PATIENTS AND THIRD-PARTY PAYORS. PATIENT ACCOUNTS RECEIVABLE ARE STATED AT EXPECTED NET REALIZABLE AMOUNTS. THE CORPORATION EVALUATES PATIENT RECEIVABLES TO DETERMINE THE ALLOWANCE FOR CREDIT LOSSES BASED ON SIMILAR RISK CHARACTERISTICS THAT EXIST WITHIN CERTAIN CLASSES OF PAYORS. THE CORPORATION CALCULATES THE ALLOWANCE USING AN EXPECTED LOSS MODEL THAT CONSIDERS THE CORPORATION'S ACTUAL HISTORICAL LOSS RATES ADJUSTED FOR CURRENT ECONOMIC CONDITIONS AND REASONABLE AND SUPPORTABLE FORECASTS. THE CORPORATION CONSIDERS MARKET TRENDS, FUTURE ECONOMIC CONDITIONS, AND CREDIT RATINGS OF ITS PAYORS WHEN MAKING ADJUSTMENTS FOR REASONABLE AND SUPPORTABLE FORECASTS. UNCOLLECTIBLE AMOUNTS ARE WRITTEN OFF AGAINST THE ALLOWANCE FOR CREDIT LOSSES IN THE PERIOD THEY ARE DETERMINED TO BE UNCOLLECTIBLE. RECOVERIES OF AMOUNTS PREVIOUSLY WRITTEN OFF ARE RECOGNIZED WHEN RECEIVED.

Schedule H, Part VI, Line 2 Needs assessmentA Community Health Needs Assessment (CHNA) is a disciplined, data-driven process used to identify barriers to health and well-being in the communities we serve. For the 2025 assessment, ProHealth Care collected and analyzed a variety of primary and secondary data, including results from community focus groups, key informant interviews, county and state public health databases, and other local surveys when available. This information was aggregated into a comprehensive data compendium, which was reviewed by our board-level Community Benefits Committee. Using this data along with community input, the committee applied criteria-based evaluation to identify the most pressing health concerns in Waukesha County. The resulting targeted implementation strategy guides the development of programs and resources where they are most needed and can have the greatest impact, ensuring that our efforts address the social, environmental, and behavioral factors that most influence community health. Following discussion and debate, three overarching themes emerged as priority areas: Healthy Aging - Waukesha County's 65+ population is expected to grow through 2040 and continues to make up a larger portion of the county's population distribution. Currently, 31.6% of Waukesha County households have at least one member who is 65+ years old. - Fall-related hospitalization and mortality is higher in Waukesha County than the state of Wisconsin for people 65 and older. - More than one third of adults age 45+ report feeling lonely; nearly one-quarter of adults 65+ are considered to be socially isolated. Social isolation significantly increases a person's risk of premature death from all causes. Substance Abuse - Drug-related deaths in Waukesha County nearly doubled from 2019 to 2020. - From 2007 to 2021, opioid overdose emergency room visits in Waukesha County tripled. - Alcohol continues to be the most used substance in Waukesha County. Mental Health - Prevalence of depression in adults over 18 is 21.6% - Mental health issues have been increasing across the county. - Approximately 3% of Waukesha County adult residents may have considered suicide in 2020. FOR MORE INFORMATION ABOUT OUR CHNA, PLEASE VISIT https://www.prohealthcare.org/about-us/community-involvement/chna/.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceEvery patient with a self pay balance receives a statement which conveys the availability of community (charity) care and a contact number for further information. Patients that are uninsured receive an uninsured discount which is listed on the first statement that they receive. Patients receive a total of four statements. Each statement contains verbiage referencing the hospital's community care program and a contact number for further information. If someone has been found eligible for some level of charity care, the eligibility period is three months so any subsequent accounts during that timeframe would be discounted at whatever level of charity care the patient is deemed eligible. Patients may reapply for Community Care when their eligibility period has expired. Self pay patients may also be informed of the hospital's community care program while they are inpatients. Social work and/or the business office may make contact with these patients during their stay to inform them of the community care program and assist them with completion of the application. Information is available at registration areas related to our community care program. This information contains contact information for the hospital's central business office where patients can receive assistance in obtaining and completing applications.
Schedule H, Part VI, Line 4 Community informationEstablished in 1914, ProHealth Waukesha Memorial Hospital serves the residents of Waukesha County and surrounding communities. ProHealth Care's service area is defined as a ZIP-code-based geographic region that accounts for 85 percent of the hospital's inpatient and outpatient activity. This area includes all of Waukesha County, as well as East Troy and Elkhorn in northern Walworth County, Waterford and Burlington in northeast Racine County, and Palmyra in southeast Jefferson County. Waukesha County has experienced consistent population growth in every federal census since 1950, increasing from 85,901 residents to over 400,000, and the upward trend continues. The county is large (580 square miles) and diverse, incorporating urban, suburban, and rural areas. Its population is aging, with a median age of 43.5 years, well above state averages, and projections indicate that adults age 65 and over will account for more than 30 percent of the population by 2040. Waukesha County is among the most affluent in Wisconsin, with a median household income of $101,639, yet a growing number of residents live below the poverty level, with the largest concentration in the City of Waukesha.
Schedule H, Part VI, Line 5 Promotion of community healthIn addition to community building, ProHealth Waukesha Memorial Hospital promotes community health through programs, services, and partnerships that improve access to care, preventive services, and health education. The hospital is governed by a community board and partners with a wide range of nonprofit health and human services organizations to strengthen the health of the communities it serves. We actively support two free clinics in the area, the Lake Area Free Clinic and the Waukesha Free Clinic at Carroll University, providing both financial and in-kind support to expand access to care for underserved populations. Through our leadership, the Waukesha County Community Dental Clinic was formed to meet urgent oral health needs; ProHealth continues to provide funding, clinical staff, and office space to support the clinic's mission. ProHealth also provides culturally and linguistically appropriate care and outreach through its Hispanic Health Resource Center, helping Spanish-speaking residents navigate the health system, access preventive services, and engage in health education programs that reduce barriers and improve outcomes. Hospital staff deliver services directly to the community. Trained therapists provide coverage free-of-charge at local high school athletic events to reduce injuries. Transportation assistance is offered to patients needing help getting to free clinic appointments. Staff also participate in community events, including National Night Out and local health fairs, providing screenings, health education, and resources. Staff are actively engaged in public health initiatives, including the Waukesha County Community Health Improvement Plan (CHIP), and serve on committees for the Department of Health and Human Services. Our partnerships extend across the community-from local housing, addiction recovery, and social service organizations to public safety initiatives-providing volunteers, financial support, and programmatic collaboration that improve population health. Through these initiatives, ProHealth Waukesha Memorial Hospital furthers its exempt purpose by expanding access to care, delivering preventive and educational programs, and addressing social, behavioral, and clinical determinants of health in Waukesha County.
