Deaconess Illinois Medical CenterPart V, Section B, Line 5: Deaconess Regional Healthcare Services Illinois engaged in a consulting partnership with ICAHN (Illinois Critical Access Hospital Network) to complete the community health needs assessment. Two focus groups met to discuss overall health and wellness in our service area. Members of the focus groups included representatives of healthcare providers, community leaders, community services providers, schools, faith-based organizations, local elected officials, public health, and others.
Deaconess Illinois Medical CenterPart V, Section B, Line 11: Deaconess Illinois Medical Center addresses the health needs identified in its 2024 Community Health Needs Assessment (CHNA), adopted June 24, 2024. This was the facility's first CHNA, conducted following its 2022 acquisition, its conversion to not-for-profit status and its integration into Deaconess Health System. The three significant needs identified through that process are: Access to Care; Community Collaboration; and Health Education. The hospital's implementation activities during FY25 (October 1, 2024 September 30, 2025) are described below by priority area.Priority 1: Access to CareThe 2024 CHNA identified access to care as the leading community need, with emphasis on recruiting and retaining specialty care, mental health and substance use treatment providers, and on improving internal hospital processes affecting the patient experience. Community participants ranked specialty care access among the top opportunities in the service area.Deaconess Illinois Medical Center implemented a formal Leader Rounding program during FY25 to strengthen patient engagement, identify concerns in real time and support timely service recovery. The program responds directly to the internal facility service opportunities named by the onsite community group, including registration processes and communication of results. The hospital monitored patient experience through Press Ganey survey results, leader rounding feedback, online reviews and social media engagement, using those inputs to identify trends and prioritize improvement opportunities.Nursing leadership reinforced bedside shift report and high reliability rounding practices throughout FY25 to improve communication, patient involvement in care, safety and overall experience. These activities are consistent with the adopted implementation strategy action to evaluate internal processes within Deaconess Illinois Medical Center to ensure the best customer experience.Specialty care access. On July 1, 2025, the hospital opened a dedicated 22-bed neuroscience unit, the only dedicated neuroscience unit in the region. The unit expands local access to specialty neurological care within the service area, responding directly to the specialty care access gap ranked among the top opportunities by community participants. Mental health and substance use access. The hospital worked with Take Action Today and Arrowleaf to establish a medical respite pathway providing up to 30 days of post-discharge care for patients with mental health and substance use needs who lack a safe place to recover. This activity carries out the adopted strategy action to evaluate mental health and substance abuse service gaps and to work with community partners to fill them.Priority 2: Community CollaborationThe 2024 CHNA identified community coordination as a priority, with food insecurity and fragmented social service referral pathways cited as leading concerns. Inter-agency coordination. Deaconess Illinois Medical Center hosted a monthly Coffee with Case Management session convening local providers and social service agencies to align on referral pathways and reduce duplicated effort. The hospital also participated in the Franklin Williamson Healthy Communities Coalition, representing Deaconess Illinois in ongoing regional health planning. These activities carry out the adopted strategy action to support or develop an inter-agency group that reduces service duplication and improves coordination. Shared resource directory. The hospital's case management team adopted FindHelpSI.org, an electronic community resource directory, as a shared referral tool for hospital staff, partner agencies and residents seeking local services. This activity carries out the adopted strategy action to investigate a shared resource guide for agencies and residents. Food insecurity. Deaconess Illinois Medical Center supported the Lighthouse Family Shelter through a recurring monthly meal program. On the second Wednesday of each month, hospital dietary staff prepared 15 to 25 meals for shelter residents, with the effort coordinated by the hospital marketing department and supported by a hospital budget allocation of $250. The hospital also hosted a weekly mobile farmers market on the hospital campus, open to both employees and the public, offering fresh fruits and vegetables. The market accepts Illinois SNAP benefits, improving affordability of fresh food for lower income households in the service area. Together these activities carry out the adopted strategy action to support local agencies that address food insecurity. Care coordination partnerships. The hospital established an outreach partnership with The Hub, the City of Marion community fitness center, including a referral pathway transitioning cardiac and pulmonary rehabilitation patients into ongoing community-based exercise programming following discharge from formal rehabilitation. This partnership advances the adopted strategy action to develop and maximize health service partnerships that reduce duplication and improve coordination across community agencies.Priority 3: Health EducationThe 2024 CHNA identified health education as a priority, driven by screening and prevention gaps in the service area. Patient education. Following the August 2024 transition from MedHost to Epic, nursing staff received education on using Epic patient education resources, including materials available in multiple languages to better serve diverse patient populations. The hospital standardized patient education workflows within Epic to support consistent delivery of evidence-based education across the care continuum, and continued its emphasis on teach-back methods and patient-specific education to support health literacy and patient understanding of conditions and care plans.Preventive visit outreach. The hospital promoted Medicare Annual Wellness Visits through a MyChart outreach campaign encouraging eligible patients to schedule their annual preventive visit, using the visit as a point of contact for age-appropriate screening and health education. Prevention and screening focus areas. The hospital aligned its education and outreach work with a defined set of prevention and screening priorities: depression screening and follow-up planning, diabetes control, breast cancer screening, colorectal cancer screening, tobacco screening and cessation, fall risk screening, blood pressure control, statin therapy for cardiovascular disease and kidney health evaluation. These focus areas guided the community-facing screening and education activities described below.Community health fair. Deaconess Illinois Medical Center hosted a community health fair on the hospital campus carrying out the adopted strategy action to hold a health fair in the community. The hospital has established the resource fair as a recurring annual event held each October.School partnerships. The hospital partnered with area schools to deliver health, diet and exercise education paired with exposure to healthcare careers. Multiple service lines and members of hospital leadership staffed activities and education at the Carterville Elementary School Health Day. Hospital staff from EMS, cardiopulmonary rehabilitation and occupational health delivered a four-visit health education series at Johnston City High School. The hospital also promoted healthcare careers and provided classroom education at Marion High School.Cancer screening access. The hospital conducted Breast Check Fridays across four Fridays in October, supported by a marketing social media campaign, themed activities coordinated by the imaging department and promotion of discounted self-pay pricing through MDsave. To expand radiology lung cancer screenings, the hospital ran a dedicated social media campaign in November and created an educational flier distributed at all community outreach fairs. These activities carry out the adopted strategy actions to increase free or low-cost cancer screenings in the community and to expand radiology lung cancer screenings.Smoking cessation. Hospital respiratory and cardiopulmonary staff provided smoking cessation education at Deaconess Illinois Night at the Ballpark in June 2025, advancing the adopted strategy action to pursue a smoking cessation partnership.Physical activity access. The hospital established a discounted gym membership at The Hub in Marion available to all Deaconess Illinois Medical Center employees, and hospital staff delivered health topic presentations at public forums hosted at the center. These activities support the adopted strategy action to create low-cost or free exercise programs in a community where park access is limited.
