Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - Dana-Farber Cancer Institute ("DFCI"). In 2025, Dana-Farber's Board of Trustees approved the 2025-2028 Cancer-Focused Community Health Needs Assessment (CHNA) and Implementation Plan to ensure that our collective efforts have the maximum possible impact within the Institute's priority neighborhoods. Dana-Farber's Community Benefits Office retained Health Resources in Action (HRiA), a non-profit public health consultancy organization in Boston, to undertake a comprehensive assessment of the health needs within our priority communities, including how those needs are currently being addressed and where there are opportunities to address these needs in the future. The 2025 CHNA and CHIP build on previous efforts and examine the cancer burden across Boston, both overall and among specific population groups. The CHNA and CHIP are grounded in Dana-Farber's commitment to health equity and to addressing social determinants of health (SDoH). As such, this implementation plan aims to be actionable to better support individuals from all backgrounds across the cancer care continuum. The 2025 Dana-Farber CHNA identified multiple geographies and populations who are disproportionately impacted by cancer, access to screenings, and the social and economic factors that give rise to these health disparities. These include the neighborhoods of Dorchester, Mattapan, Roxbury, Mission Hill, Jamaica Plain and East Boston and communities of color, non-English speaking residents, immigrants, low-income residents, people with disabilities, and LGBTQIA+ residents. Dana-Farber is an active member of the Boston Community Health Collaborative (the Collaborative), which is a partnership of Boston health institutions, the Boston Public Health Commission, community health centers, and community organizations working to improve the health and well-being of Boston residents. The Collaborative conducted a large-scale city-wide CHNA in 2025 and many findings from this effort are integrated into this implementation plan particularly in relation to health equity and the social determinants of health. In June 2025, findings from the Boston Community Health Needs Assessment were formally presented to the Dana-Farber Trustees Community Programs Committee and the Dana-Farber Community Benefits External/DoN Advisory Committee. The presentations highlighted citywide trends, inequities, and priority community needs, and helped contextualize and inform Dana-Farber's community health planning and alignment with the upcoming CHNA and CHIP. This alignment ensures that Dana-Farber remains responsive to immediate community needs and supports flexibility and readiness to address issues that might arise in the future. This integrated approach provides a deeper context for cancer-related experiences across Boston's communities. The Collaborative's Needs Assessment and Improvement Plan can be found here: https://www.boston.gov/government/cabinets/boston-public-health-commission/racial-justice-and-health-equity/bostonchna The 2025 Dana-Farber Cancer CHNA uses multiple data sources including data already collected via existing sources (secondary data) and new information gathered for this specific effort (primary data). As noted, this report also incorporates data from the citywide Boston CHNA. The following provides an overview of the methods employed in the 2025 DFCI CHNA and a highlight of the Boston CHNA methods that were incorporated into this report. Please see the 2025-2028 DFCI CHNA Appendix A for a more detailed description of these methods and their limitations and Appendix B for more information on data analyses. Primary data are new data collected specifically for the CHNA. Primary data were collected using 2 different methods: community resident focus groups and a key informant survey. Focus groups for the Dana-Farber Cancer CHNA were carried out in collaboration with community partners in surrounding neighborhoods, including Mission Hill, Dorchester, and East Boston. 91 cancer patients, survivors, and caregivers engaged in 9 focused discussions from February - April 2025. Dana-Farber made introductions and connections for some of these groups. Focus groups were conducted both in-person (90 minutes) and virtually (60-90 minutes) and aimed to delve deeply into experiences with cancer screening, treatment and care coordination, and survivorship. Focus groups were conducted with the following population groups: English-speaking men who specifically are prostate cancer patients/survivors and their caregivers; Latinx residents who speak Spanish (2 groups); Asian residents who speak Cantonese; Asian residents who speak Vietnamese; Residents who speak English (3 groups); Residents who identify as LGBTQIA+. The following community partners assisted with recruiting participants and/or hosting focus groups: Asian Women for Health, DFCI LGBTQIA+ Patient Support Group, Prostate Health Education Network, Roxbury Tenants of Harvard, Union Capital Boston, and VietAID. Focus group notes were coded and analyzed using NVivo, a qualitative analysis software, to identify major categories and sub-themes that emerged across all groups, as well as unique issues specific to particular populations. The frequency and intensity of discussions on a topic were the key drivers used for extracting main themes. Selected de-identified paraphrased quotes are presented to further illustrate points within topic areas. Additionally, 24 providers, organizational leaders, and community residents completed an open-ended key informant survey. The Dana-Farber Key Informant Survey aimed to collect information about key stakeholders' perceptions of community resources and opportunities for improvement. The survey was offered between March - April 2025 to medical providers, navigators, staff from community-based organizations focused on cancer care, and community residents touched by cancer. These individuals were pre-selected by Dana-Farber for their cancer-related expertise. Targeted outreach was conducted primarily through email. The survey tool included 8 open-ended questions and was made available online through Qualtrics in English. In this report, people who completed the survey are referred to as "key informant survey respondents" (whereas those who were part of focus groups are referred to as "participants" for distinction). The final sample includes 24 key informant survey respondents. NVivo was also used to analyze the open-ended survey responses, employing a similar qualitative coding process used for the focus group data noted above. Secondary data are data that has already been collected for another purpose. Examining secondary data helps us to understand trends, provide a baseline, and identify differences by sub-groups. These quantitative findings help guide the methods and populations for primary data collection. As an example, populations for focus groups were prioritized among those groups with the largest cancer burden, lowest screening rates, and challenges with healthcare access. Secondary data for this CHNA were gathered to understand cancer outcomes, risk behaviors, and social determinants of health. Existing data were drawn from national, state, and city sources, including the Boston Behavioral Risk Factor Surveillance Survey (BBRFSS), Massachusetts Department of Public Health, Boston Resident Deaths, U.S. Census American Community Survey (ACS), and Healthy People 2030.
Schedule H, Part V, Section B, Line 5 Facility , 2Facility , 2 - Dana-Farber Cancer Institute ("DFCI"). Secondary data from the Massachusetts Cancer Registry, Boston Resident Deaths, and Boston Behavioral Risk Factor Surveillance Survey (BRFSS) were analyzed by the Boston Public Health Commission's Center for Public Health Sciences and Innovation. Other data were analyzed by the organizations cited in the data source. Analyses are presented as frequencies (percentages) and rates throughout the report. Data from the surveillance systems, such as the BBRFSS, are presented with 95% confidence intervals (indicating 95% confidence that true value is in the range; noted as error bars in the figures), where possible. Time series analyses examined cancer rates over time across multiple time points. When statistical significance testing was conducted, it is noted in figures or in text. Specifically, when the word "significantly" is used in the text it connotes statistical significance (p<0.05). The following recent presentations and articles from Dana-Farber provided detailed data on specific topics and populations: 2024 Massachusetts Comprehensive Cancer Coalition Statewide Cancer Summit, The Lancet Oncology Disabilities and Cancer Series, Cancer Disparities Experienced by People with Disabilities, Dana-Farber Cancer Institute Intro to Cancer Genetic Counseling, Dana-Farber Cancer Institute Prevent Cancer Dialogue, and Dana-Farber Cancer Institute Inclusion, Diversity, and Equity in Action (IDEA) Lecture Series. Key themes from these related materials were summarized and embedded into relevant sections in the report. The description below is an overview of the Boston CHNA methods, given many findings were included in Dana-Farber's CHNA report. The Boston CHNA Community Survey aimed to collect information about Boston residents' perceptions of community strengths, priority health issues, and access to care and vital resources contributing to health and well-being. The Boston CHNA survey was offered between September 2024 and January 2025 to individuals aged 14 and up living in Boston. The anonymous survey was made available online and in paper format in English and 8 languages in addition to English (Arabic, Cape Verdean Creole, Haitian Creole, Portuguese, Spanish, Simplified Chinese, Somali, and Vietnamese). Intensive community outreach was conducted. Outreach was conducted through various means, including in-person outreach, email distribution, social media and online survey promotion. Building on previous processes, the Boston Community Health Collaborative leveraged a large, existing network of community partners who distributed the survey in online and paper formats. In addition, targeted outreach and paper survey distribution were conducted at 43 community events and locations, including farmer's markets, health fairs, block parties, public libraries, food pantries, and community meetings. Promotional flyers were distributed in all 9 survey languages. The final survey tool included 28 questions. The final sample of the CHNA Community Survey comprises 1,866 respondents who were Boston residents. 8 focus groups were conducted with specific communities of focus from December 2024 through February 2025. A total of 62 community residents participated in focus groups. Focus groups were conducted with the following population groups: South Boston mothers (in Spanish); Chinese older adults (in Cantonese); Residents in active substance use recovery; New immigrants and/or English language learners; Residents who live in Boston Housing Authority housing; Somali parents of children with special healthcare needs (in Somali); Fathers and men of color; Refugee youth. Additionally, 5 sector-based focus groups were conducted with 28 organizational partners and direct service providers. The sectors and topic areas for these groups were: Climate Justice, Housing, Community Health Workers, Mental/Behavioral Health, and Economic Mobility. These discussions aimed to focus in particular on community needs, assets, and promising practices or recommendations for action.Additionally, a total of eleven key informant interviews were completed with 13 individuals between January and March 2025.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - Dana-Farber Cancer Institute ("DFCI"). The goals and strategies outlined in the 2025-2028 Implementation Plan are specifically designed to recognize the milestones we have reached thus far, address new findings based on emerging public health research, and continue the work of achieving the goals we have set to help meet the health needs of our surrounding communities. As part of our approach to implementing the identified strategies, we will continue our existing community-based programming, which includes assessing the most effective evidence-based interventions, collaborating with community partners to implement community health improvement efforts, and conducting ongoing evaluation of program effectiveness. Our expertise as a comprehensive cancer center leads us to focus largely on reducing cancer incidence and mortality among medically underserved populations through our clinical community screening and outreach programs, which include mobile mammography and breast health education, skin cancer education and screening, HPV education and vaccination, tobacco cessation and increasing awareness about survivorship in communities of color. The 2025-2028 Implementation Plan focuses on the following areas: 1) Cancer Screening (Routine, Underutilized, and Emerging): Increasing access to and uptake of cancer screenings, particularly among populations experiencing lower screening rates; 2) Access to Care: Improving awareness, navigation, and access to timely, high-quality care across the cancer continuum; 3) Cancer Care Coordination and Treatment: Enhancing patient navigation, care coordination, and support throughout diagnosis, treatment, and survivorship; and 4) Social Determinants of Health: Addressing upstream factors - including food access, economic mobility, healthy living, and housing - that influence cancer risk and outcomes. Dana-Farber is a comprehensive cancer care specialty hospital committed to supporting communities across the cancer continuum, with a particular focus on marginalized populations. While the CHNA revealed a broad range of needs, this implementation plan focuses on areas where Dana-Farber's expertise, clinical capacity, and resources can have the greatest impact. Dana-Farber's leadership is committed to sustaining the resources required to advance these Community Health priorities and to strengthening efforts that reduce cancer disparities and improve health outcomes in the communities we serve. Dana-Farber remains engaged in broader public health efforts. As an active leader in the Collaborative, Dana-Farber collaborates with partners to address root causes of health inequities and to advance shared goals that extend beyond cancer care. While needs such as childcare or substance use may fall outside Dana-Farber's primary cancer-focused scope, we recognize their connection to the cancer care continuum, and they will be addressed at the individual patient level. Considerable progress has been made in advancing the goals set in our 2022-2025 CHNA Implementation Plan. Community Health efforts increased preventive screening and improved treatment access, deepened culturally and linguistically responsive outreach and education, and advanced policy and systems changes that addressed root causes of health inequities. The highlights summarized below reflect key accomplishments and demonstrate the collective impact of our work. * Delivered 6,784 mammograms (4,513 via the mobile van; 2,271 at Whittier Street Health Center); and worked to increase the follow-up rate for those with abnormal screening results. * Conducted skin cancer screening and provided education for 2,526 people at 49 events; 416 abnormal findings referrals and 175 biopsy referrals made; digitized an updated interactive sun safety curriculum. * Provided 208 tobacco cessation counseling sessions to seventy-five patients; 84% reduced daily cigarette use and 41% quit; supplied nicotine replacement therapy and gift cards for healthy snacks. * Engaged 1,700+ residents via virtual cancer prevention sessions with Union Capital. * Launched AssessYourRisk (AYR), a free online tool in English and Spanish, to assess risk of breast and ovarian cancers and offer resources to reduce these risks; over 14,000 AYR completions in 16 months. * Hired four bilingual Community Outreach Specialists; presented in English, Spanish, Cantonese, Mandarin, and Haitian Creole to groups including low-income residents, immigrants and men. * Expanded outreach via Chinese and Spanish media (e.g. WeChat, El Mundo). * Distributed a 7-language HPV Dental Toolkit and created LGBTQIA+- tailored education materials with Team Maureen while leading the MA HPV Coalition. * Funded and evaluated four multi-year Greater Boston non-profit organizations through DoN grantmaking focused on environmental justice, food insecurity, and child homelessness; achieved legislative wins, strengthened resident power-building and advocacy, and shared evaluation learnings at national and local meetings. * Cancer Care Access Program provided streamlined diagnostic resolution and treatment for underserved medically complex patients with suspected malignancies at three Boston FQHCs. * Expanded patient navigation to Gynecologic, Lymphoma, Myeloma, and Genitourinary cancer centers. * Strengthened Dana-Farber patient demographic data and began routine patient SOGI collection. * Earned Joint Commission Health Equity Certification. * Secured state approvals for a new 300-bed inpatient cancer hospital.

Schedule H, Part III Section A, Lines 2 and 3 - Bad Debt ExpenseIN CALCULATING BAD DEBT EXPENSE, DFCI USES A COMBINATION OF ACTUAL BAD DEBT WRITE OFFS AND HISTORICAL BAD DEBT FACTORS IN DETERMINING WHICH CURRENT ACCOUNTS RECEIVABLE ARE NOT LIKELY TO BE COLLECTED. ADDITIONALLY, DFCI DETERMINES THE AMOUNT OF BAD DEBT ATTRIBUTABLE TO INDIVIDUALS WHO ARE ELIGIBLE FOR FINANCIAL ASSISTANCE BASED ON A HISTORICAL ESTIMATE OF THE PERCENTAGE OF PATIENTS THAT HAVE QUALIFIED FOR FINANCIAL ASSISTANCE, WHICH IS APPLIED AGAINST TOTAL BAD DEBT.
Schedule H, Part V, Section B, Line 20d Billing and collectionsDANA-FARBER DOES NOT ENGAGE IN PRESUMPTIVE ELIGIBILITY FOR ITS PATIENT FINANCIAL ASSISTANCE POLICY (FAP). THROUGH ITS PARTICIPATION IN THE MASSACHUSETTS HEALTH SAFETY NET, THE STATE SPONSORED UNCOMPENSATED PROGRAM, DANA-FARBER FIRST RELIES ON DEMONSTRATION OF INELIGIBILITY FOR MASSHEALTH PROGRAMS, SUCH AS HEALTH SAFETY NET, OR SIMILAR OUT OF STATE MEDICAID PROGRAMS, FROM PATIENTS APPLYING FOR ASSISTANCE. UPON DETERMINATION BY THE STATE AGENCY, DANA-FARBER WILL CONSIDER EACH CASE INDIVIDUALLY AND RELIES ON THE COMPLETION OF THE PATIENT FINANCIAL ASSISTANCE APPLICATION ("PFA APPLICATION") AND PROVISION OF NECESSARY SUPPORTING DOCUMENTATION.
Schedule H, Part V, Section B, Line 22 Charges to FAP eligible individualsDEPENDING ON A PATIENT'S FINANCIAL SITUATION, FAP-ELIGIBLE PATIENTS WERE CHARGED AMOUNTS RANGING FROM 0% OF BILLED CHARGES UP TO THE APPLICABLE AGB PERCENTAGE OF BILLED CHARGES, WHICH WAS NOT MORE THAN THE AGB. IN FY25, DFCI DETERMINED AGB ON AN ANNUAL BASIS USING THE "LOOK-BACK METHOD," DESCRIBED UNDER TREASURY REGULATION SECTION 1.501(R)-5(B)(1). SPECIFICALLY, DFCI'S ANNUAL AGB PERCENTAGE WAS EQUAL TO SUM OF ALL GROSS CHARGES DURING THE PRIOR FISCAL YEAR DIVIDED BY CLAIMS PAID IN CONNECTION WITH THOSE CHARGES. FOR PURPOSES OF THE AGB CALCULATION, CLAIMS PAID INCLUDED CLAIMS FOR EMERGENCY AND MEDICALLY NECESSARY CARE PAID BY BOTH MEDICARE FEE-FOR-SERVICE AND ALL PRIVATE HEALTH INSURERS AS PRIMARY PAYERS, TOGETHER WITH ANY ASSOCIATED PORTIONS OF THESE CLAIMS PAID BY MEDICARE BENEFICIARIES OR INSURED INDIVIDUALS IN THE FORM OF CO-PAYMENTS, CO-INSURANCE OR DEDUCTIBLES.

Schedule H, Part I, Line 7 Bad Debt Expense excluded from financial assistance calculation1646424
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteCONSISTENT WITH THE INSTITUTE'S MISSION, CARE IS PROVIDED TO PATIENTS REGARDLESS OF THEIR ABILITY TO PAY. THE INSTITUTE HAS DETERMINED IT HAS PROVIDED IMPLICIT PRICE CONCESSIONS TO UNINSURED PATIENTS AND PATIENTS WITH OTHER UNINSURED BALANCES (E.G. COPAYS AND DEDUCTIBLES). THE IMPLICIT PRICE CONCESSIONS INCLUDED IN ESTIMATING THE TRANSACTION PRICE REPRESENT THE DIFFERENCE BETWEEN AMOUNTS BILLED TO PATIENTS AND THE AMOUNTS THE INSTITUTE EXPECTS TO COLLECT BASED ON ITS COLLECTION HISTORY WITH THOSE PATIENTS. PATIENTS WHO MEET THE INSTITUTE'S CRITERIA FOR CHARITY CARE ARE PROVIDED CARE WITHOUT CHARGE OR AT AMOUNTS LESS THAN ESTABLISHED RATES. THE INSTIUTE HAS DETERMINED THAT IT HAS PROVIDED SUFFICIENT IMPLICIT PRICE CONCESSIONS FOR THESE ACCOUNTS. PRICE CONCESSIONS, INCLUDING CHARITY CARE, ARE NOT REPORTED AS REVENUE.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsTHE MEDICARE COST ON LINE 6 COMES FROM THE MEDICARE COST REPORT AND IS BASED ON MEDICARE'S COST FINDING PRINCIPLES AND METHODOLOGIES. WE STRONGLY BELIEVE THAT THE ENTIRE AMOUNT OF THE SHORTFALL OF $85,990,129 SHOULD BE TREATED AS A COMMUNITY BENEFIT AS THIS REPRESENTS THE INSTITUTE'S COMMITMENT TO THE ELDERLY AND DISABLED COMMUNITY BY PROVIDING SERVICES THAT ARE NOT REIMBURSED.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceWITHIN DFCI'S CREDIT AND COLLECTIONS POLICY THERE ARE PROVISIONS THAT DESCRIBE COLLECTION PRACTICES TO BE FOLLOWED FOR PATIENTS WHO ARE KNOWN TO QUALIFY FOR CHARITY CARE OR FINANCIAL ASSISTANCE IN ACCORDANCE WITH FEDERAL AND MASSACHUSETTS LAW.

Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceDFCI IS COMMITTED TO OFFERING FINANCIAL ASSISTANCE TO ELIGIBLE PATIENTS WHO DO NOT HAVE THE ABILITY TO PAY FOR THEIR MEDICAL SERVICES IN WHOLE OR IN PART. IN ORDER TO ACCOMPLISH THIS CHARITABLE GOAL, DFCI WIDELY PUBLICIZES ITS PATIENT FINANCIAL ASSISTANCE (PFA) POLICY WITHIN ITS FACILITIES, IN DIRECT COMMUNICATIONS WITH PATIENTS AND IN THE COMMUNITIES THAT IT SERVES, PARTICULARLY IN THE COMMUNITIES WITH THE GREATEST UNMET HEALTH NEEDS. THE PFA POLICY, A PLAIN LANGUAGE PFA SUMMARY AND THE PFA APPLICATION ARE POSTED ON DFCI'S WEBSITE IN ENGLISH AND SPANISH. IN ADDITION, INFORMATION ABOUT THE PFA IS PROVIDED TO ALL PATIENTS AT REGISTRATION AND/OR ADMISSION, AND IS AVAILABLE UPON REQUEST, WITHOUT CHARGE, THROUGH A DFCI FINANCIAL COUNSELOR, CUSTOMER SERVICE REPRESENTATIVE, OR BY MAIL. FOLLOWING CARE, ALL BILLING STATEMENTS SENT TO A PATIENT INDICATE HOW THE PATIENT CAN OBTAIN INFORMATION ABOUT THE PFA POLICY. IN COLLABORATION WITH DFCI'S COMMUNITY PARTNERS, INFORMATION ABOUT THE FAP IS SHARED WITH RESIDENTS OF DFCI'S PRIORITY NEIGHBORHOODS. EFFORTS TO NOTIFY COMMUNITY MEMBERS MOST LIKELY TO REQUIRE ASSISTANCE INCLUDE DISTRIBUTING PLAIN LANGUAGE SUMMARIES OF THIS POLICY TO LOCAL COMMUNITY HEALTH CENTERS AND OTHER NON-PROFIT ORGANIZATIONS.
Schedule H, Part VI, Line 4 Community informationDANA-FARBER SERVES THE COMMUNITY OF EASTERN MASSACHUSETTS, INCLUDING THE COMMUNITIES SURROUNDING ITS PRIMARY LOCATION IN BOSTON AND ITS SATELLITE LOCATIONS IN MILFORD, MASSACHUSETTS, WEYMOUTH, MASSACHUSETTS, BRIGHTON, MASSACHUSETTS, NEWTON, MASSACHUSETTS, METHUEN, MASSACHUSETTS, FOXBOROUGH, MASSACHUSETTS AND LONDONDERRY, NEW HAMPSHIRE. DANA-FARBER'S COMMUNITY INCLUDES ADULTS AND CHILDREN WITH CANCER OR AT RISK OF DEVELOPING CANCER, AND THEIR FAMILIES. Dana-Farber focuses its community outreach efforts on its priority Boston neighborhoods of Dorchester, East Boston, Jamaica Plain, Mattapan, Mission Hill, and Roxbury. The median incomes of Dana-Farber's priority communities are generally much lower than Boston overall, with Roxbury at a median income of $47,921, Dorchester at $58,681, and Mattapan at $67,206. Residents in Dana-Farber's priority neighborhoods appear to experience higher rates of poverty than Boston overall.
Schedule H, Part VI, Line 5 Promotion of community healthDana-Farber's 2025-2028 Community Health Needs Assessment and Implementation Plan provide a robust description of our comprehensive cancer control programs and services. The report also provides a summary of accomplishments from our last needs assessment and sets forth our commitment to addressing the new opportunities to reduce the cancer burden in medically underserved communities in Boston. Form 990, Part III, Line 1 Organization's missionTHE MISSION OF DFCI IS TO PROVIDE EXPERT, COMPASSIONATE, AND EQUITABLE CARE TO CHILDREN, ADULTS, AND THEIR FAMILIES, WHILE ADVANCING THE UNDERSTANDING, DIAGNOSIS, TREATMENT, CURE, AND PREVENTION OF CANCER AND RELATED DISEASES. WE TRAIN NEW GENERATIONS OF CLINICIANS AND SCIENTISTS, DISSEMINATE INNOVATIVE PATIENT THERAPIES AND SCIENTIFIC DISCOVERIES AROUND THE WORLD, AND REDUCE THE IMPACT OF CANCER, WHILE AT ALL TIMES MAINTAINING A FOCUS ON THOSE COMMUNITIES THAT HAVE BEEN HISTORICALLY MARGINALIZED.
Form 990, Part III, Line 4a Program Service AccomplishmentsCLINICAL CARE AS ONE OF THE LEADING CANCER CENTERS IN THE WORLD, DANA-FARBER CANCER INSTITUTE PROVIDES COMPASSIONATE, COMPREHENSIVE, AND PERSONALIZED CARE TO ADULTS AND CHILDREN WITH CANCER, BLOOD DISORDERS, AND RELATED DISEASES. IN FISCAL YEAR 2025, OUR EXPERT CLINICIANS AND CAREGIVERS AT OUR SPECIALIZED TREATMENT CENTERS WORKED TOGETHER TO DELIVER THE LATEST THERAPIES TO OUR PATIENTS THROUGH 1,295 INPATIENT ADMISSIONS, 417,867 OUTPATIENT DOCTOR VISITS, AND 242,140 INFUSION TREATMENTS. SINCE ITS FOUNDING IN 1947 BY SIDNEY FARBER, MD, DANA-FARBER HAS FOLLOWED HIS VISION FOR A CANCER CENTER THAT IS JUST AS DEDICATED TO DISCOVERIES IN CANCER RESEARCH AS IT IS TO DELIVERING EXPERT, COMPASSIONATE CARE. OUR INSTITUTE IS A FOUNDING MEMBER OF DANA-FARBER/HARVARD CANCER CENTER AND ONE OF 57 NCI-DESIGNATED COMPREHENSIVE CANCER CENTERS IN THE U.S. WE ARE THE ONLY HOSPITAL RANKED IN THE TOP THREE NATIONALLY BY U.S. NEWS AND WORLD REPORT IN BOTH ADULT AND PEDIATRIC CANCER CARE. IN 2025, WE OFFERED MORE THAN 1,200 CLINICAL TRIALS
Form 990, Part III, Line 4b Program Service AccomplishmentsRESEARCH Scientific work at Dana-Farber is based on the premise that basic and clinical investigation are complementary and reinforcing activities. To encourage this cross-pollination of ideas, the institute has developed an organizational framework that fosters collaborations among investigators from different disciplines. The result is an intense scientific impact on a group of diseases that continue to represent one of humanity's greatest health challenges. Some recent advances include: Belzutifan approved by US Food and Drug Administration (FDA) for kidney cancer: in a large phase 3 multicenter clinical trial, patients with metastatic clear cell renal cell carcinoma who received belzutifan had better survival outcomes than those who received standard therapy. Cabozantinib approved by us FDA for neuroendocrine tumors of the pancreas or elsewhere: in a phase 3 clinical trial, patients with previously treated, advanced disease who received cabozantinib had better survival outcomes than those who received a placebo. Dordaviprone was granted accelerated approval by us FDA for patients with recurrent rare fatal brain cancer: in a phase 2 clinical trial, dordaviprone, an oral, first in class targeted medicine, was well tolerated by patients with recurrent h3 k27m-mutant diffuse midline glioma and provided a durable response for some patients. Pembrolizumab approved by us FDA as up-front treatment for head and neck cancer: in a pivotal phase 3 trial, patients who received pembrolizumab before, during, and after steadard care had better survival outcomes than those who received standard care alone. Ruvemenib approved by us FDA for relapsed or refractory acute myeloid leukemia with common mutations: revumenib is a first-in-class oral menin inhibitor that was approved for the treatment of patients with kmt2a translocations in 2024. The 2025 approval was based on results of a phase 2 clinical trial for patients with npm1 mutations, giving up to 40% of patients with AML access to this medicine. Sunvozertinib approved by us FDA for egfr-mutated non-small cell lung cancer: in a phase 2 clinical trial, sunvozertinib showed positive results for patients with previously treated advanced non-small cell lung cancer with certain EGFR mutations. Antibody-drug conjugate (ADC) shows promise as first-line therapy for patients with advanced triple negative breast cancer who are not eligible for immunotherapy: a phase 3 clinical trial reported early results suggesting that patients who received sacituzumab govitecan had improved progression-free survival compared to patients receiving standard chemotherapy. Adc combination shows promise for metastatic her2-positive breast cancer: a phase 3 trial reported improved progression-free survival for patients receiving a combination of trastuzumab deruxtecan plus pertuzumab compared to current standard first line therapy. Ras inhibitor shows promise in advanced pancreatic cancer: early results of a phase 1/2 clinical trial supported the initiation of a phase 3 study comparing daraxonrasib to second line chemotherapy for patients with advanced pancreatic cancer. Personalized cancer vaccines continue to be improved and tested: early phase clinical trials reported promising results in kidney cancer and in melanoma. Natural killer (NK) cell therapies continue to be designed and tested: a new clinical trial testing NK cell therapies has begun in ovarian cancer. Direct cyclin inhibitors: direct cyclin inhibitors are a new class of medicine that disables a quality control checkpoint in the cell cycle. Laboratory evidence supported the launch of a nationwide phase 1 clinical trial of a first-in-class direct cyclin inhibitor for patients with small cell lung cancer, triple negative breast cancer and other cancers. Weight-loss intervention validated: clinical trials can now proceed to determine if weight loss through use of the validated intervention can help reduce the risk of breast cancer recurrence. Exercise and diet influence survival: clinical studies reported that after treatment for colorectal cancer, exercise and anti-inflammatory diets can improve survival. Acute leukemia diagnostic: a new diagnostic tool for acute leukemia could significantly improve patient care by allowing faster and more precise treatment decisions. Multiple myeloma blood test: a novel blood test for multiple myeloma enables diagnosis of multiple myeloma and its precursors using a blood test, offering an alternative to traditional bone marrow biopsy.
Form 990, Part III, Line 4c Program service accomplishmentsCOMMUNITY HEALTH The role of Dana-Farber's Community Health office is to support the Institute's goal to reduce cancer risk among medically underserved populations. We work with city and state health departments, community partners, and Boston-based coalitions to assess and monitor the needs of local residents with respect to cancer control. In collaboration with staff throughout Dana-Farber, we serve as a bridge with community organizations and establish evidence-based and sustainable outreach programs. We are on the front lines of supporting and collaborating on programs designed to eliminate disparities in breast, colon, and skin cancer; educate diverse populations about tobacco cessation, human papillomavirus (HPV) prevention and screening; and strengthen the support system for medically underserved populations. In short, we are committed to making Dana-Farber's care and research findings more accessible to everyone within and outside its walls. FY25 programmatic highlights are as follows: Dana-Farber's mammography services provided 2,836 mammograms on the van and at the mammography suite at Whittier Street Health Center in Roxbury. Dana-Farber's Sun Safety Program provided 16 in-person sun safety screening events with added health and safety measures. 860 participants were screened by a dermatologist and 123 people were referred for follow-up appointments. 52 participants were referred for a biopsy. Dana-Farber's Tobacco Treatment program received 28 referrals and provided 35 individual tobacco cessation counseling sessions to 13 patients. 9 participants were able to reduce their total number of cigarettes smoked per day, and 4 of these reported being able to quit completely. The Tobacco Treatment program continued to provide financial incentives for participants enrolled in the program. Stop and Shop gift cards were provided after counseling sessions to encourage individuals to reduce their tobacco usage and quit smoking. Continued to partner with the Breast Oncology Department in managing AssessYourRisk, a digital tool to help people of all ages better understand and manage their risk of breast and ovarian cancer. The survey tool was completed by over 10,000 users in FY25. Implemented and evaluated the fourth and final year of Dana-Farber's multi-year Systems Level Funding, awarded to three non-profit organizations in Greater Boston to address air quality, environmental justice, and child homelessness. Successes from the fourth year include advancing air quality legislation, launching community-owned data tools, strengthening cross-sector partnerships, and deepening resident and youth leadership - building momentum for continued policy, systems, and environmental (PSE) change beyond the grant-funded period. The Education and Outreach team held 65 events in 5 languages (English, Cantonese, Haitian-Creole, Mandarin, and Spanish), reaching a total of 4,868 participants. These events included cancer education sessions on topics including General Cancer Prevention & Resources, HPV-Related Cancer, Breast Cancer, and Colorectal Cancer, and Prostate Cancer, as well as community events including tabling at health fairs, farmers markets, and cultural celebrations such as Lunar New Year and Pride for the People, where we provide tailored education materials and speak to community members in our priority populations about cancer prevention. The Education and Outreach team expanded outreach in bilingual media, specifically for Spanish speaking and Chinese populations. In FY25, we published 44 posts in Simplified and Traditional Chinese on WeChat, a social media platform popular among Chinese populations. We also participated in 4 interviews on Spanish media platforms (Telemundo, Univision, La Mega Radio, Orale con Veronica) focused on breast and colorectal cancer screening. Lastly, we published 4 articles in Spanish in El Mundo newspaper. From late 2024 through early 2025, Dana-Farber's Community Health Office, in collaboration with the Community Benefits External/Determination of Need (DoN) Advisory Committee, conducted a series of community information sessions to solicit public input related to planning for Dana-Farber's proposed inpatient adult cancer hospital.

A FINAL VERSION OF THE FORM IS APPROVED BY THE CFO AND FILED ELECTRONICALLY BY ERNST & YOUNG LLP WITH THE IRS.
Form 990, Part VI, Line 12c Conflict of interest policyDFCI MONITORS AND ENFORCES COMPLIANCE WITH THE DFCI CONFLICT OF INTEREST AND CONFIDENTIALITY POLICY FOR BOARD OFFICERS, TRUSTEES, AND KEY EMPLOYEES ("COVERED PERSONS"). THE OFFICE OF GENERAL COUNSEL ANNUALLY DISTRIBUTES A CONFLICT OF INTEREST QUESTIONNAIRE AND CONFIDENTIALITY STATEMENT ("STATEMENT") REQUIRING ALL COVERED PERSONS TO DISCLOSE INTERESTS, ROLES, AND ACTIVITIES THAT COULD GIVE RISE TO A POTENTIAL OR ACTUAL CONFLICT WITH THE INTERESTS OF DFCI. ADDITIONALLY, DFCI REQUIRES COVERED PERSONS TO COMPLETE THE STATEMENT UPON INITIAL APPOINTMENT TO A COVERED OFFICE AND TO UPDATE THEIR DISCLOSURES ON A CONTINUING BASIS WHENEVER A COVERED PERSON BECOMES AWARE OF A NEW OR PREVIOUSLY UNDISCLOSED INTEREST. THE OFFICE OF GENERAL COUNSEL COMPARES THE DISCLOSURES TO INSTITUTIONAL INFORMATION IN ORDER TO IDENTIFY POTENTIAL OR ACTUAL CONFLICTS OF INTEREST. THE DFCI GENERAL COUNSEL REPORTS ON ALL DISCLOSURES TO THE PRESIDENT OF DFCI, CHAIR OF THE DFCI BOARD AND CHAIR OF THE DFCI BOARD GOVERNANCE COMMITTEE.
