THE INSTITUTION IS THE CORPORATE NAME FOR THE HEALTHCARE SYSTEM COMPRISED OF LAKES REGION GENERAL HOSPITAL (LRGH) AND FRANKLIN REGIONAL HOSPITAL (FRH AND COLLECTIVELY WITH LRGH, THE HOSPITALS). LRGH WAS FOUNDED IN 1893 AND WAS ORIGINALLY KNOWN AS THE COTTAGE HOSPITAL. IN 1908, COMMUNITY NEED FOR THE HOSPITAL HAD OUTGROWN ITS LOCATION SO A NEW FACILITY WAS OPENED AT THE PRESENT SITE ON HIGHLAND STREET. THE FACILITY WAS RENAMED LACONIA HOSPITAL AND MAINTAINED SUCH NAME UNTIL 1966 WHEN THE NAME OF THE HOSPITAL CHANGED TO LAKES REGION GENERAL HOSPITAL. LRGH IS LOCATED IN LACONA, NEW HAMPSHIRE. LRGH, DESIGNATED AS A RURAL REFERRAL CENTER AND SOLE COMMUNITY HOSPITAL UNDER MEDICARE, IS A COMMUNITY AND REGIONAL ACUTE CARE FACILITY WITH A LICENSED BED CAPACITY OF 137 BEDS. FRH WAS FOUNDED IN 1909 AS A NOT-FOR-PROFIT CHARITABLE CORPORATION BY THE FRANKLIN LADIES HOSPITAL AID SOCIETY, WITH THE STATED CHARITABLE PURPOSE OF PROVIDING ACUTE HEALTHCARE SERVICES TO RESIDENTS OF THE CITY OF FRANKLIN AND SURROUNDING COMMUNITIES. iN 1909, THE FRANKLIN LADIES HOSPITAL AID SOCIETY PURCHASED A MANSION KNOWN AS THE AIKEN HOMESTEAD WHICH OPENED ITS DOORS AS THE SITE OF THE FIRST FRANKLIN HOSPITAL ON JANUARY 1, 1910. FROM THAT TIME TO THE PRESENT, THE FRANKLIN HOSPITAL GREW INTO FRH WITH A FOCUS OF SERVING THE GREATER FRANKLIN, NEW HAMPSHIRE COMMUNITY. FRH, DESIGNATED AS A CRITICAL ACCESS HOSPITAL UNDER MEDICARE, IS A 25 BED COMMUNITY HOSPITAL. ON JULY 1, 2002, LRGH MERGED WITH FRH TO BECOME LRGHEALTHCARE. THE INSTITUTION OFFERS A BROAD RANGE OF MEDICAL, SURGICAL, SPECIALTY AND SUB-SPECIALTY SERVICES TO ADULTS, CHILDREN AND INFANTS. Income Based CriteriaPart I, Line 3c: HealthLinkHealthLink is our community's front door to healthcare information and resources for everyone. It is an information and referral system that provides healthcare financial counseling and access to medical, dental, and wellness services for those in financial need.HealthLink is a partnership between LRGHealthcare, local physicians and medical providers, and area service agencies. Since our creation in 1993, we have helped to connect more than 20,600 people in the Lakes and Three Rivers regions to the services they need, both locally and through programs such as Medicaid, Medicare, Healthy Kids, and services available to veterans through the Veteran's Administration. In addition to the information and referral services that are available free-of-charge to everyone in the community, HealthLink also has three membership programs for which local residents may apply.HealthLink AccessThe HealthLink Access program is a program designed to provide people who are uninsured with access to the healthcare services they need. If a person qualifies, he or she will be given a co-pay responsibility or a set discount based upon a sliding fee scale, and will then have access to providers and services in the HealthLink network.Eligibility: Eligibility is based on several factors. Applicants must meet income requirements based on the Federal Poverty Guidelines. Local residency is required. In general, they cannot have access to health insurance, though consideration may be given based on income level in some cases in which insurance is available but unaffordable. Assets, including savings, property, investments, and vehicles will also be considered in determining eligibility. HealthLink AssistanceThe HealthLink Assistance Program is designed for people who may have access to insurance; but still meet HealthLink financial guidelines and face financial hardship in paying for their healthcare costs. Members may have difficulty meeting a high deductible or covering co-payments or other costs required by their plan. They often need one-time or short-term assistance meeting a healthcare financial obligation rather than needing long-term financial assistance. HealthLink Assistance will provide short-term supplemental support to an existing insurance plan; or short-term support paying a particular healthcare cost. This program was formerly known as Community Care.Eligibility: Eligibility is based on several factors. Applicants must meet income requirements based on the Federal Poverty Guidelines. Assets, including savings, property, investments, and vehicles will also be considered in determining eligibility. Applicants may have an insurance plan, but must have demonstrated difficulty paying their copays, deductibles, or other healthcare costs. HealthLink CatastrophicThe HealthLink Catastrophic program is available not only to HealthLink Access or Assistance members, but to anyone meeting financial guidelines who incurs an out-of-pocket expense of $10,000 or more for a single medically-necessary hospital stay or outpatient treatment. Costs incurred over time do not qualify as part of this $10,000 minimum unless they are all the result of a single injury or health episode. Once a patient qualifies for HealthLink Catastrophic, and discount eligibility is determined (ranging from 20% - 30%); that discount will apply for any additional costs incurred during the six months after approval.Eligibility: HealthLink Catastrophic eligibility is based solely on gross income. The Federal Poverty Level Guidelines will be used to determine eligibility and the percentage of discount provided. Our staff will determine whether costs resulted from a single, qualifying hospitalization or course of treatment. VetLinkThe VetLink Program is a LRGHealthcare program that assists all veterans and families of veterans with their questions and concerns about their healthcare. The VA system is complicated, and VetLink can help veterans navigate the system to access the benefits and services that they deserve. Since its creation, VetLink has helped more than 800 veterans access the healthcare services they need.Eligibility: All veterans who have earned an honorable discharge from military service are eligible. ________________________________________The Medication ConnectionThe Medication Connection Program helps qualified patients access reduced-rate prescriptions offered through pharmaceutical company programs. While these programs are available to the general public, they often involve a cumbersome application process, which can be confusing for patients to complete on their own. The Medication Connection Program is here to help make sure patients don't have to choose between prescription medications and other necessities of life.Eligibility: The Medication Connection Program is available to those who cannot afford to pay for their prescription medications, and who have no other source for assistance with medications. Many have too much income to qualify for state aid, but not enough to purchase a supplemental prescription plan. Depending on the medication needed, different pharmaceutical companies have individual guidelines for eligibility. Not all medications are available through this program; participation by pharmaceutical companies is voluntary. ________________________________________IncomeBasedCriteria

Subsidized Health Services ExplnPart I, Line 7g: Subsidized Health Services - $4,569,149LRGHealthcare provides a number of health services because they meet an essential community need, even though we realize a loss on them each year.Access to primary care services is one of the biggest challenges facing this, and other New Hampshire communities. LRGHealthcare subsidizes a number of practices to ensure ongoing access to our region not just in the communities where our hospitals operate, but throughout our service areas. Last year, we subsidized primary care practices in the amount of $2,325,031. In addition, $220,000 supported the operation of HealthFirst Family Care Center, a rural health clinic with locations in both Laconia and Franklin. HealthFirst alone saw 3,488 patients last year, nearly one third of whom had no insurance. Without these subsidies, many of these practices would not be able to sustain their operations.LRGHealthcare also provides access to a wide range of specialty services, providing our community with a much broader spectrum of specialty care than one would typically find in a rural healthcare system. In 1997, we founded our Dental Resource Center to ensure that dental care was accessible to those without dental insurance or the means to pay out-of-pocket for the care they needed. Today, that practice treats a patient mix of the uninsured and insured, in order to maintain viability. But even with the existence of the highly successful practice, access to dental care was still identified as a top concern by our community in the last assessment, so our work in this area continues. Last year, we invested $240,633 in the dental health of our community through the Center. In addition, we also subsidized a number of specialty physician services to ensure specialty access in our community, including vascular and orthopedic surgery, oncology, urology, general surgery, and anesthesia, at a total cost of $1,552,036.Chronic disease management programs have also been proven to not only provide better health outcomes and quality of life for those suffering from chronic diseases, but also to be fiscally wise investments. LRGHealthcare's outpatient Chemical Dependency Program meets a critical need in the State and the region, as there is a shortage of such programs to assist those with addiction issues, even though substance abuse was identified among the top three priority needs in our community assessment. Last year our Chemical Dependency Program counseled 504 patients either by phone or in person, and 66 individuals were admitted to the intensive, 18-week, outpatient program. Additionally, our Chemical Dependency Program now provides evaluation for the DOT and companies that provide safety sensitive jobs. Last year we invested $95,949 in Chemical Dependency services for our community.Breast cancer has long been one of the top causes of cancer death in our region, and LRGHealthcare has been aggressive in our efforts to fight this disease. In addition to providing a full-time Breast Health Educator, reduced-rate screening mammograms in the month of October, and participating in the State's Breast and Cervical Cancer Program (BCCP), this year we added a Mammography Bridge program. This program provides very low cost screening mammograms to women who do not qualify for either Medicaid or the BCCP, but have financial barriers to receiving screening mammograms. In FY 10, Mammography Bridge served 98 patients, and the BCCP program served 240 patients at our site. Last year, LRGHealthcare invested more than $22,500 in our Breast Health programs. The cities of Laconia and Franklin are Federally-designated as Medically Underserved Communities; and Belknap County is designated as a Physician Scarcity Area for Primary Care. LRGHealthcare works hard to combat these challenges. Physician recruitment is a primary job responsibility of both our CEO and our Vice President of Provider Relations, who continually seek primary care and needed specialty providers for our communities. In executives time alone, in FY 2010 LRGHealthcare expended more than $113,000 on these efforts.SubsidizedHealthServicesExpln
Percent Of Total Expense ExplnPart I, Line 7f: It is a well-established standard that 50 percent of bad debt can be considered community benefit, since it is estimated that at least half of bad debt represents individuals who were unwilling or unable to provide the information necessary to complete the financial assistance application. In accordance with DHHS reporting standards, LRGHealthcare reported $3,056,340 in uncollectable debt last year. In addition, through our Patient Advantage Program, we leveled the playing field by giving all patients at least a median discount. Through that program, we provided community benefit in the amount of $1,312,201 in FY 2010.PercentOfTotalExpenseExpln

HEALTHLINK IS PROVIDED TO PATIENTS WHO ARE UNABLE TO PAY FOR THE COSTS OF THEIR CARE, AND FOR WHICH THERE IS NO OTHER SOURCE OF FUNDING AVAILABLE. DETERMINATIONS OF FINANCIAL ASSISTANCE IS MADE PROSPECTIVELY FOR A ONE-YEAR PERIOD FOR HEALTHLINK ASSISTANCE AND HEALTHLINK ACCESS. Needs AssessmentPart VI, Line 2: For more than 100 years, LRGHealthcare, including Lakes Region General Hospital and Franklin Regional Hospital, has been caring for the healthcare needs of our community. We are guided by our mission to make healthcare available, affordable, and accessible to all, regardless of ability to pay.According to the New Hampshire Behavioral Risk Factor Surveillance System, in 2007-2008, significantly fewer adults in LRGHealthcare's service area had health insurance coverage than other areas in the state: While 89% of adults in the State of New Hampshire reported having health coverage, only 77.3% of adults in the LRGH service area, and 79.6% of adults in the FRH service area reported having any health coverage. The majority of LRGHealthcare's Community Benefit is composed of the provision of charitable care for the uninsured or patients who cannot afford care; as well as the unreimbursed costs for caring for patients with Medicaid coverage. While our Critical Access Designation at our FRH campus does help to offset this, we have a disproportionately high percentage of Medicaid patients throughout our service area, which creates significant financial challenges due to the shortfall in Medicaid reimbursementNeedsAssessment
Patient Education AssistancePart VI, Line 3: HEALTHCARE FINANCIAL COUNSELING AND ASSISTANCELRGHealthcare's mission is to make quality healthcare available, affordable, and accessible to all, regardless of ability to pay. In the spirit of our mission, we offer a number of different financial assistance and referral programs to meet the full range of need in our community. We also participate in Statewide efforts to keep quality healthcare accessible for all.Whether you are insured or uninsured, in need of extensive medical services or just a check-up from time to time, everyone deserves to have access to healthcare. Community Building ActivitiesPart VI, Line 5: Community Building Activities - $88,367LRGHealthcare works in partnership with a number of local organizations and initiatives to strengthen and develop our community. In addition to our financial contributions, senior executives and Board members are active participants in a number of economic development initiatives, including the Belknap County Economic Development Council and the Choose Franklin community building initiative.LRGHealthcare has been a key member of community safety and support systems enhancement efforts over the past year. We participate in Emergency Preparedness efforts in both Belknap and Merrimack Counties, and actively work with the Lakes Region Partnership for Public Health and Caring Communities Network of the Twin Rivers to address public health initiatives, including injury prevention and obesity prevention, in our community. Our contributions in this area totaled $63,079 in FY 10.We have undertaken a number of initiatives to lessen our environmental footprint through transitioning to environmentally-friendly sterilization materials, expanding our recycling program, and providing a sharps disposal program. We spent $24,688 on environmental improvements last year. Our environmental specialist is also an active participant in the City of Laconia Recycling Committee. For several years we have supported community leadership development through our participation in the Leadership Lakes Region program, which trains 20 future community leaders each year. We have not only sent several employees through each class to assist in their training and engagement in the community (with two in the program last year), but we also provide support for one program day each year at a cost of $600.CommunityBuildingActivities
Other InformationPart VI, Line 6: Community Health Services $976,997LRGHealthcare provides a variety of services designed to help our community access healthcare services, as well as provide education to help community members live healthier lives. We have an active Community Health Education Department, which offered 73 programs and special events and 107 car seat inspections last year, reaching 4,501 people. Our Programs are designed to address identified community needs, including chronic disease prevention and care, obesity (including childhood obesity), and community safety and injury prevention. Program categories include Asthma, Cancer Care, CPR/First Aid, Childbirth and Parent Education, Diabetes, Fitness, General Wellness, Injury Prevention, Nutrition and Weight Management, Seniors, Smoking Cessation, and Women's Health. Two of our staff members acquired special training to offer a Chronic Disease Management program, to empower those living with chronic diseases to achieve better health through lifestyle choices. Last year we also greatly increased our child and maternal health offerings, sending staff out into the community through a number of programs to reach young mothers. In addition to our traditional childbirth education and breastfeeding support programs, we worked with the local Boys & Girls Club to provide childbirth and parenting classes especially for teens. In 2010, both the Laconia and Franklin regions received Healthy Eating, Active Living grants from the Foundation for Healthy Communities. LRGHealthcare has been a vital community partner in these efforts, and staff members have been instrumental in the implementation of grant efforts focused on obesity prevention and reduction. Last year, the Mayor's Drug & Alcohol Task Force was formed, and in September, received a $650,000 Federal Drug-Free Communities grant, which runs over a five year period. LRGHealthcare is also pleased to partner and support this community collaboration.LRGHealthcare funds Ambulance Services for the City of Laconia. Last year we expended $309,457 to fund positions for five paramedics and the EMS liaison to the Laconia Fire Department, who also provides training to EMS providers across the region. We maintain four ambulances, and have one fully staffed ambulance available at all times, with a second available as backup. Last year we responded to 2,182 calls. In addition, the Regional Early Activation Care Team (REACT) program, a new collaboration with local EMS workers and area hospitals, created an integrated system for evaluating heart attack victims in the field, and transporting them directly to the facility that best meets their needs; whether that's a local hospital ER, or a facility with emergency cardiac catheterization capabilities. LRGHealthcare funded training for all EMS personnel, expending $9,835 on advanced cardiac monitoring training to support REACT this year.Healthcare Support Services are vital to our mission of ensuring that healthcare is available, affordable, and accessible to all in our community. Our largest contribution in this area is our HealthLink Program, which is our community's "front door" to health information and services. This program also ensures that those without insurance have access to the primary and specialty services they need. Last year we enrolled 3,713 people in the HealthLink program, and assisted nearly 500 walk-in customers per month. We expended $245,558 on enrollment staff, and another $101,959 on community care management through this program. LRGHealthcare also still actively participates in the New Hampshire Healthy Kids program. Our VetLink program is a unique program to help Veterans navigate the VA system, and access the benefits and services to which they are entitled. Last year, VetLink served 338 Veterans and their families. Operating under the umbrella of HealthLink, we supported this program at a cost of $27,900 last year. Our Medication Connection program ($24,919) helps patients in need access pharmaceutical company programs offering free or reduced-cost medications that those patients otherwise could not afford. During FY 2010, the Medication Connection served 400 clients. As the economic climate continued to decline, the number of referrals to the program greatly increased, and during FY 10 prescriptions ordered totaled 4,400. These prescriptions were valued at $1.8 million. In 2010, LRGHealthcare began participating in the New Hampshire Colorectal Screening Program. Through this program, a number of patients who met financial guidelines were able to receive free screening colonoscopies. While clinical services were grant funded, several of our staff members spent extensive time on both provider and community education, enrollment, and management of this program. Health Professions Education $378,093LRGHealthcare is committed to helping build the healthcare workforce of tomorrow not only for our organization, but for our society. As the largest employer in the region, we also take seriously our responsibility to help ensure that local workers can access the education they need to earn livable wages in healthcare fields. To these ends, we participate in a number of programs to provide opportunities for students interested or engaged in healthcare careers.LRGHealthcare provides clinical settings for undergraduate training in a number of fields nursing, medical imaging, pharmacy, physical therapy, emergency medical services, social work, medical assisting and phlebotomy. This requires extensive staff coordination time; 215 students participated in clinical activities at LRGHealthcare last year. We also continued to provide our popular Caring Student Interns (CSI) program, which allows teens to explore healthcare careers. Last year, 24 students participated in CSI, and another 39 participated in informational interviews or observations, to assist them in their career decision-making processes. LRGHealthcare invested $53,359 to make these opportunities available. We also provide funding for educational programs in a number of health professions. Our TuitionLink Program, which provides funding to those enrolled in healthcare education career programs in exchange for a commitment of employment, served 5 area students this past year at a cost of $46,698. Last year, we also partnered with Lakes Region Community College's Nursing program at a cost of $226,125. In 2010, we welcomed eight third-year osteopathic medical students from the University of New England, who completed multiple rotations in our hospitals and practices. While we were reimbursed for some program costs, we expended an additional $51,911.The city of Laconia is Federally-designated as a Medically Underserved community; and Belknap County is designated as a Physician Scarcity Area. LRGHealthcare works hard to combat these challenges. Physician recruitment is a primary job responsibility of both our CEO and our Vice President of Provider Relations.Looking ForwardDespite difficult financial times, LRGHealthcare has always been, and will continue to be, committed to our community benefit activities. Over the next year, added emphasis will be placed on the following programs:In 2010, LRGHealthcare, in partnership with local insurance broker Melcher & Prescott, launched the GPS Program. The goal of the program is to keep health coverage affordable in our community. GPS offers local businesses the opportunity to save money on their healthcare costs and offers their employees the opportunity to save on their co-pays and deductibles for care received at LRGHealthcare facilities and through our providers. In order to provide this program, which helps keep provision of health benefits affordable for local businesses, and ensures convenient access to quality, affordable care for community members, LRGHealthcare is passing along savings associated with the collection of these deductibles and co pays to the patient by securing payment at the time of service. We anticipate a growing investment in this program in 2011. LRGHealthcare has begun working with regional EMS departments to help support them in their education and re-certification, cost recovery and compliance, clinical CQI, and central purchasing efforts. With rising costs, communities throughout our service area are finding it increasingly difficult to maintain strong EMS programs. By leveraging group purchasing opportunities, providing other business operations assistance, and offering the expertise and support of personnel within LRGHealthcare's system, we are helping to maintain a vibrant emergency service throughout our service area.OtherInformation

SCHEDULE H PART VI 6Last year, LRGHealthcare began participating in the New Hampshire Colorectal Screening Program. Through this program, a number of patients who met financial guidelines were able to receive free screening colonoscopies. While clinical services were grant funded, several of our staff members spent extensive time on both provider and community education, enrollment, and management of this program. Given the prevalence of colorectal cancer in our community, we look forward to expanding our involvement in, and promotion of, this program to help more people access screening services. LRGHealthcare is in the process of building a 98,000 square-foot addition at our LRGH campus. This addition, projected to be completed in late 2011, will expand our ICU and eldercare capacity to help meeting the growing needs of our aging population; and will provide the space to enable us to transition to single-patient room care facility-wide. Single patient rooms will enhance privacy, quality, comfort, and enable us to better meet today's infection control standards for our community. In 2011, LRGHealthcare will be rolling out a newly revamped Speaker's Bureau, offering local civic groups and businesses speakers to address a broad range of healthcare topics. These programs will be made available free-of-charge for not-for-profit groups. This coming year, we anticipate a growing involvement in both the Healthy Eating, Active Living grants from the Foundation for Healthy Communities; and the Federal Drug-Free Communities grant received by the Franklin Mayor's Drug & Alcohol Task Force. LRGHealthcare is an instrumental partner in implementing programs in support of these efforts in both the Lakes and Three Rivers regions. In January, 2011, LRGHealthcare consolidated our Laconia-based pediatric services in order to provide expanded access to care for families in our service area. This consolidation will increase appointment availability, lowering emergency department utilization for children in our community.CONTINUATION
