Bad Debt FootnotePart III, Line 4: The Hospital provides a wide variety of health care services to patients within the community in order to ensure access to appropriate care for populations in need. The Hospital, grant credit without collateral to their patients, many of whom are local residents and are insured under third-party payor agreements. The Hospital provides care without charge or at amounts less than established rates to patients who meet certain criteria under the Hospital's charity care policies. Because the Hospital does not pursue collection of amounts determined to qualify as charity care, they are not reported as revenue.BadDebtFootnote
Shortfall As Community BenefitPart III, Line 8: Cost reporting principles are used to determine allowable and non-allowable costs based on rule and regulations. The Medicare shortfall is the subsidized amount of care provided to patients and should be considered a community benefit.ShortfallAsCommunityBenefit
Debt Collection PolicyPart III, Line 9b: The Hospital provides care without charge or at amounts less than established rates to patients who meet certain criteria under the Hospital's charity care policies. The Hospital's collection policy exempts these individuals and patients from any collection or billing procedures pursuant to the Commonwealth of Massachusetts regulations and policies. The Commonwealth of Massachusetts operates a 'health safety net' to reimburse hospitals for the cost of uncompensated care, defined as charity care, and bad debts associated with emergency services. Amounts are paid to the health safety net based on a percentage of private sector charges. Reimbursement from the health safety net is a flat amount per visit or discharge, determined prospectively from a Medicare-based reimbursement model.DebtCollectionPolicy
Needs AssessmentPart VI, Line 2: In accordance with the Massachusetts Attorney General's Community Benefits Guidelines for Non-Profit Hospitals, The Hospital conducted a Needs Assessment. Massachusetts law also requires that all non-profit hospitals and health plans submit an annual Community Benefits Report that details the extent to which these institutions provide charity care and support community health activities in their service areas. The Hospital conducts a periodic community health needs assessments that clarify the leading health concerns, service gaps, and barriers to access for their patients. The primary aim of these needs assessments are to help ensure that hospital and health plan community benefit activities are targeted appropriately. In the Spring of 2009, The Hospital engaged an outside consulting firm to assist with the community health needs assessment. The needs assessment compiled and analyzed secondary data drawn from existing federal, state, and local sources as well as analyzed primary data that the Hospital collected through surveys administered to patients in clinic waiting rooms. The needs assessment process explored issues related to: health care access and preventive services; maternal and child health status; the prevalence of chronic disease, mental illness, and substance abuse, and the extent to which patients are involved in risky behaviors. Then the information collected was analyzed and reported. The goal of the needs assessment was to address both quantitative and qualitative health care needs of the community.NeedsAssessment
Patient Education AssistancePart VI, Line 3: The Hospital's policy is that no individual will ever be refused care. Care is provided regardless of age, race, sex, religion, nationality, or ability to pay. The policy is posted on signs throughout the Hospital along with signs informing patients of the availability Public Assistance & MassHealth programs are posted at all Registration points, the Emergency Department, Cashier's Window, and Financial Counselor offices. All signs are large enough that they are clearly visible and legible by patients visiting these areas. Signs are in both English and any other language spoken by more then 10% of the population in the Hospital's service area. The Hospital also provides care to the elderly and the indigent under the Medicare and Medicaid programs, granted at a discount, which is generally less than cost. The proportion of services provided to Medicaid patients approximates the proportion of Medicaid beneficiaries living in the medical center's service areas. In addition, the Hospital pays into the Health Safety Net for hospitals within the Commonwealth of Massachusetts. Throughout 2010, the Hospital continued to enroll and provide referral and health care services for the indigent, underinsured and uninsured in our community. The Hospital recognizes the delivery of care to those in need as an integral part of our mission and we will continue to focus on meeting the health care needs of this population going forward.PatientEducationAssistance
Community InformationPart VI, Line 4: The designated geographic and patient populations focus on the hospital's immediate neighbors in Dorchester and Mattapan and extend to communities to the south and southeast of Boston. Target populations identified within these communities include a large number of African American, Haitian, Latino and Vietnamese individuals and their families. As provided in the Boston Public Health Commission Report on the Dorchester Community, which was based on Year 2000 census data, Dorchester has experienced a population growth since 1990 of 6.2 percent with the African American, Haitian, Latino and Vietnamese communities expanding most rapidly. In addition, children under the age of 17 are the fastest growing segments of the Dorchester population. As a result, Carney Hospital aims to increase services and capabilities to meet the needs of these growing populations.Dorchester's racial and ethnic make-up consists of the following: 45% Black, 14% Latino, and 8% Asian. Of foreign born residents, 41.9% are Caribbean, mainly Haitians who make up 13.2% of the total foreign born population, and 20.9% are Asians, of which the Vietnamese make up 17.2% of the total foreign born population. Carney Hospital's Community Benefits Advisory Committee consulted with the Carney Hospital Community Benefits Leadership Team and determined that Diabetes would be designated as the main priority for the FY 2010 Community Benefits Plan. The Committees determined that hospital resources should be devoted to addressing this chronic disease.CommunityInformation
Community Building ActivitiesPart VI, Line 5: Women @ Work Program: The Women @ Work Program at St. Mary's Women and Children's Center creates a supportive and nurturing environment that fosters personal growth and empowerment for program participants. Women @ Work is a job readiness program designed to transition women into jobs and careers. Participants are encouraged to invest in themselves and the program to reach the following goals: gain self-confidence and self-respect, learn and employ positive life and job readiness skills, provide support for family, and seek to live a fulfilling and independent life. In FY10, Carney Hospital served as an internship and job placement site. The women worked in a variety of departments, ranging from fiscal to medical records to human resources. The internship is generally twelve weeks in length and allows the participants to apply skills learned in the classroom.Workplace Skills and Education Program - Coordinated by the human resources department and three teachers, this program, endorsed by the Council on Higher Education, provides resources for career development. The program offers Carney Hospital employees, their families and members of the community instruction in English as a Second Language, pre-General Education Development (GED) and GED certificate preparation. It also teaches post-secondary school communication skills, writing skills for work and school, keyboarding, and specific-need tutoring (e.g. working on pronunciation). Learning skills are presented primarily by classroom instructors and through individual lessons via computer software. In addition to the specific education offered to participants, the program bolsters self-esteem and the confidence necessary to succeed in moving from low-wage or no employment to productive, higher-paying positions with added responsibilities and opportunities. The program provides students with the benefit of one-on-one tutoring and teaching. Participants obtain, among other things, college admission, job promotions and U.S. Citizenship.CommunityBuildingActivities
