Youth Transitions Initiative assists young people with disabilities and chroinc illness to move from school to adulthood in the world of work. | Description | HSC Home Care, LLC, a disregarded entity of the organization offers Skilled Nursing Services, Home Health Aid and Personal Care Aid Services, Respite Care, Rehabilitative Therapies, Licensed Clinical Social Workers, well mom and baby visits, intermittent intravenous services, client family, caregiver, and patient education. During 2010 HSC Home Care had a total of 11,094 home visits to primarily Medicaid pediatric patients in the DC Metropolitan area.
