Each board member completes a new conflict of interest statement which his kept on file at central office. Additionally at the commencement of each board meeting, the president asks if anything has changed to present a conflict and provides the opportunity for board members to disclose any new conflict or perceived conflict. Additionally, each appointed member of a council or committee completes a conflict of interest form which is likewise, kept on file in ADHA's central office. New employees sign and date a conflict of interest statement upon commencement of employment. Annually, all staff must sign a new conflict of interest statement. |
| Form 990, Part VI, Section B, line 15 | At the conclusion of each fiscal year, the Executive Committee completes the previously adopted evaluation instrument for the executive director's performance. After all the input of members of the committee is completed, the Executive Committee discusses the evaluation with the Executive Director and performance objectives for the coming year are established. The results of the process are shared with the entire Board of Trustees at the Fall Board meeting. Upon completion of the evaluation process, the Compensation Committee (President, Immediate Past President and Treasurer) meet with the executive director to determine a compensation package that is reflective of ADHA's financial position, market conditions and salary surveys from Association Forum of Chicagoland and American Society of Association Executives.
