DescriptionMorristown-Hamblen Healthcare System ("Hospital") is a community hospital serving an eight county area in the heart of the Lakeway Area. The Hospital's heritage reflects a concern for each patient and family, as well as a sense of commitment to excellence in the provision of medical service. MHHS offers 167 acute care beds which includes 12 gero-psychiatric beds and is supported by approximately 188 active, courtesy, and consulting staff physicians. The Hospital is committed to providing quality medical healthcare regardless of race, creed, sex, national origin, handicap, age, or ability to pay. Although reimbursement for services rendered is critical to the operation and stability of the Hospital, it is recognized that not all individuals possess the means to purchase needed medical services. The Hospital also strives to serve the community by providing both healthcare services and health related education. Therefore, in keeping with Hospital's commitment to serve all members of its community the following are offered:- charity care and/or subsidized care;- care provided to persons covered by governmental programs; and- health related activities and programs to support the community. All the above services are provided when a medical need exists (non-elective) regardless of an individual's ability to pay. The Hospital's policy of providing needed care to people regardless of their ability to pay constitutes the greater part of our commitment of service to our local community and surrounding region. Other activities include community education programs, special programs for the elderly, a program for obese children, the training of medical professionals, financial support of various community non-profit organizations for underserved individuals, and support for a variety of other community activities. In an effort to provide a continuum of care for patients from diagnosis through recovery, the Hospital has developed six clinical service lines (Centers of Excellence) which include our: Heart, Lung and Vascular Center, Women's Center, Morristown Regional Cancer Center, Surgery Center, Joint and Spine Center, and Emergency and Critical Care Center. The Heart, Lung and Vascular Center offers patient access to various specialists including: cardiologists, a pulmonologist and a vascular surgeon. The Center offers diagnostic services consisting of electrocardiograms, 4-D Ultrasound services, stress testing, echocardiograms, transesophageal echocardiograms, heart catheterizations and angiography services. Specialized services include: emergent and non-emergent treatments with stent placement, cardio versions, pacemaker placement, and peripheral vascular services.Cardiac rehabilitation and pulmonary rehabilitation services are also provided to promote wellness and a healthy lifestyle. Patients are provided a customized three-phase rehabilitation program that begins the moment the patient is diagnosed or enters the hospital with a heart or lung condition. The Heart, Lung and Vascular Center also offers a Sleep Disorder Center to provide diagnostic testing and screening services for area patients who have health-threatening sleep disorders. The Center's staff works with teams of physicians including: a pulmonologist, neurologist, primary care doctors, and ENT specialists to provide diagnosis and treatment solutions for their patients. Additionally, the Sleep Disorder Center provides community programs such as smoking cessation classes and risk assessments for heart disease. The Women's Center provides services to both mother and baby in the same facility. The Center includes a labor and delivery unit with a Level III nursery, and a dedicated unit for gynecological and other female patients. Our Birthing Center is a secure unit that provides postpartum care, obstetrical services as well as childbirth, parenting and baby care education. A fetal monitoring system ensures that babies are being cared for even before they are born. Over 45,000 babies have been delivered at our current location. Gynecological and specialized care for women complete the services offered through this Center. The Morristown Regional Cancer Center offers services focused on the diagnosis, care and treatment of patients in a comfortable setting specially designed for cancer patients and their families. A multidisciplinary team of physicians, nurses, dietitians, pharmacists, and other support staff work with patients and their families to create an individualized plan. In addition, related specialists include: medical oncologists, a radiation oncologist, radiologists, pathologists, general surgeons specialized in oncology, a pulmonologist and gastroenterologists to assist in the care and treatment of cancer patients. A weekly tumor conference is all held to enhance communication among providers and to develop optimal treatment plans. The Center offers the latest treatments including High Dose Radiation and Chemotherapy services. The Center for Surgical Services includes both inpatient and outpatient surgical services. The Center encompasses 25,300 square feet of space and is equipped with technology to support general surgery, orthopedic and spine surgery, ophthalmologic, gynecologic, endoscopic, cystoscopic and vascular surgery services for the community. The Joint & Spine Center offers a full scope of care for orthopedic and spine patients. The Center offers patient education classes for both those who have made the decision to have surgery and those who have questions about surgery. The classes are offered at no charge to the public. The Center is designed for the specialized surgical and inpatient needs for patients who undergo orthopedic and/or spine surgery, and includes specially trained staff and facilities designed around the needs of these patients. Treatment and discharge plans are designed to enhance overall patient outcomes. The Emergency and Critical Care Center provides 24-hour care seven days a week to area patients for the treatment of injuries, illness, trauma, and various other conditions. Experienced physicians and nurses treat and respond to emergencies requiring immediate attention. The Center treats all types of general emergencies and maintains the only designated general pediatric emergency facility in the eight county region. A fast track area is available to care for patients in need of services for minor medical emergencies. Healthcare professionals with EPNC ("Emergency Nurse Pediatric Certification") and PALS ("Pediatric Advanced Life Support") also work in the Center. Additionally, aero medical services are available for those requiring advanced levels of care not otherwise available in our community. The Critical Care Unit and Intensive Care Unit include 20 inpatient beds with monitoring equipment and specially trained staff to care for patients in need of intensive care services. In addition to our Centers of Excellence, the Healthcare System also provides other needed services. The Radiology and Imaging Department features diagnostic imaging technologies which include general radiographic rooms, advanced 16-slice and 64-slice computerized tomography (CT) scanners, magnetic resonance imaging ("MRI") machines, ultrasound equipment, and a nuclear medicine camera. We also offer limited interventional radiology services. A Picture Archiving Computerized System (PACS) was added to transition the Hospital from film to electronic images to enhance the quality of care delivered by this clinical department. The Hospital has also updated its Breast Center to include digital mammography, an R-2 checker, ultrasound and breast MRI technologies. This area is staffed with a team of full time technologists and radiologists. The Center for Behavioral Health addresses the emotional, physical and spiritual needs of our senior population. The Center offers an inpatient gero-psychiatric program designed to meet the psychological and medical needs of older adults. An out-patient program is also available. Treatments are available for depression, dementia, delirium, generalized anxiety, panic, other disorders, as well as changes in behavior, function and memory. A senior assessment service is also available which provides a multidimensional assessment of a senior adult's functional ability, physical health, cognitive and mental health, and socio-environmental situation. Recently, the Center added Senior Connect to provide seniors in our community a support group designed to promote healthy living. A Senior Access Team, which is a team of trained professionals, also offers specialized assessments for in-patient's with behavioral issues or mental status changes.Description

Costing Method UsedPart I, Line 7: Cost is derived from the actual expense data accumulated within the hospital general ledger which addresses all patient segments (inpatient, outpatient, emergency room, private insurance, medicaid, medicare, uninsured, and self-pay). the Hospital allocates those expenses to all patient segments at the procedure level based on a cost-to-charge ratio.CostingMethodUsed
Percent Of Total Expense ExplnPart I, Line 7f: Bad debt expense included in FOrm 990, part IX, line 25, column (a) is $10,510,906. the Hospital excluded the full amount, $10,510,906, for purposes of calculating the percentage in column (f), on part I, line 7.PercentOfTotalExpenseExpln
Bad Debt FootnotePart III, Line 4: A cost-to-charge ratio was also used to determine bad debt cost. The total operating expense was divided by patient revenues to calculate an overall ratio that was then applied to the bad debt expense to arrive at cost.BadDebtFootnote
Shortfall As Community BenefitPart III, Line 8: Medicare allowable costs were reported using the hospital's filed Medicare Cost Report (C/R). The C/R uses a cost to charge ratio based on a step-down allocation methodology. In caring for the patient, there are several services that are considered non-allowable by CMS such as: transportation of a patient, comfort items to include a television or a telephone. Additional non-allowable costs include the recruitment of physicians, physician guarantees and a portion of the bad debt associated with the care of the patient.ShortfallAsCommunityBenefit
Debt Collection PolicyPart III, Line 9b: It is the policy of the hospital to pursue collection of patient balances from patients who have the ability to pay for these services. The Hospital's collection practices apply to all patients, charity care and non-charity care, consistently and fairly regardless of insurance status. Practices include no abusive phone calls. A patient account representative is assigned to each patient account. Self-pay accounts receive a discount at discharge. Additionally, any remaining balance in an account, after posting the remit amounts, automatically transfers to a self-pay classification. Self-pay accounts that have aged 61 days and have had no payment or account activity in 29 days are sent to in-house collections. Patient account representatives diligently work with patients and evaluate new information learned during the collection process to determine if collection activities should be ceased or if the patient qualifies for financial assistance. Self-pay accounts are written off to bad debt and sent to an outside collection agency, Tri-County Collections, an organization with substantial experience in collection of patient accounts, only after all established steps have been undertaken, without success.DebtCollectionPolicy
Needs AssessmentPart VI, Line 2: The hospital currently does not formally assess the health care needs of the community it serves. However, any new physician positions created are approved by the board of directors and evaluated for the overall benefit provided to the community.NeedsAssessment
Patient Education AssistancePart VI, Line 3: To communicate the hospital's financial policy to the patient, the hospital's website has a "Patients and Visitors" menu where patients can go to view information about payment policies, accepted insurance, and the hospital financial assistance (charity care) program. The website gives two phone numbers that patients can use to contact a financial counselor to discuss their individual financial alternatives. The hospital's charity care policy and financial assistance contact information is posted in the admissions areas, emergency areas, and other areas of the hospital's facilities in which eligible patients may be present. All staff with patient contact are knowledgeable about the charity care policy (admitting and billing clerks, nursing and medical staffs, social workers, chaplains, patient advocates, etc.).PatientEducationAssistance
Community InformationPart VI, Line 4: The hospital is located in Hamblen county. Hamblen County has a population of 61,412 and is in the Eastern portion of Tennessee. Hamblen County is part of the Appalachian region as defined by the Appalachian Regional Commission. 18.4% of individuals living in Hamblen County are below the poverty level.
