CRITERIA FOR DETERMINING ELIGIBILITY FOR FREE OR DISCOUNTED CARESCH H, PART I, LINE 3CJHSMH uses HUD Geographic Very Low-Income Guidelines to determine eligibility for Charity Care. A patient qualifies for 100% assistance if their annual income is below 130% of the HUD Geographic Very Low-Income Guidelines for the Louisville, Kentucky metro region. Discounted Care is provided for patients with annual income greater than 130% but not in excess of 195% of the HUD Geographic Very Low-Income Guidelines. JHMSH's method for determining Free and discounted care provides Free care at approximately 200% of the Federal Poverty Level and Discounted care up to approximately 400% of the Federal Poverty Level. In situations where patients have other assets, liquid assets are defined as investments that could be converted into cash within one year; these will be evaluated as cash available to meet living expenses. Assets that should not be considered as available to meet living expenses include: the patient's primary place of residence; adequate transportation; adequate life insurance; and sufficient financial reserves to provide normal living expenses if the wage earners are unemployed or disabled.
BENEFITS AT COSTSCH H, PART I, LINE 7JHSMH is a single provider which includes several hospital facilities. Facilities that utilize a cost accounting system calculate amounts reported on Schedule H, Part I, line 7 by utilizing the cost accounting system. For those facilities that do not have a cost accounting system, amounts are determined by multiplying revenue by the cost to charge ratio calulated from Worksheet 2. Bad Debts included in Part IX, Line 25 totaled $50,997,220. This amount has been removed from total expenses calculated in Column f on Schedule H, Part II.
BAD DEBT EXPENSESCH H, PART III, LINEs 2 and 4JHSMH is a single provider which includes several hospital facilities. Facilities that utilize a cost accounting system calculate amounts reported on Schedule H, Part III, line 2 by utilizing the cost accounting system. For those facilities that do not have a cost accounting system, amounts are determined by multiplying revenue by the cost to charge ratio calulated from Worksheet 2. Patient receivables and net patient service revenue are derived primarily from patients who reside in Kentucky and southern Indiana. Patient receivables consist of amounts due from third-party payors, including federal and state programs, commercial insurance companies and individual patients, for health care services rendered. Management maintains an allowance for doubtful accounts to reserve for estimated losses based on the length of time the account has been past due and historical collection experience.
CALCULATION OF MEDICARE ALLOWABLE COSTSSCH H, PART III, LINE 8Costs reported on Line 6 are obtained from the Medicare cost report which are based on a cost to charge ratio.
COLLECTION PRACTICESSCH H, PART III, LINE 9BFor patients known to have Charity eligibility, an approval letter is sent to the patient, which describes JHSMH Policies regarding qualifying for Charity Care as well as contact information for assistance. Before accounts are turned over to collections, JHSMH utilizes a Risk-Assessment Scoring system which predicts the likelihood of a patient qualifying for financial assistance and charity care. This predictive model uses 9,000 database sources comprising more than 2 billion public record sources to provide a socioeconomic score for these underserved patients. JHSMH classifies patients as Charity Care if they qualify for Presumptive Charity based on their Charity Score.
NEEDS ASSESSMENTSCH H, PART VI, QUESTION 2During 2008 JHSMH initiated a community health needs assessment utilizing the Association for Community Health Improvement's Community Health Needs Assessment approach. JHSMH's approach includes six steps. 1. Establish the Assessment Infrastructure 2. Define the Purpose and Scope of Assessment 3. Collect and Analyze Data 4. Select Priorities 5. Develop Action Plan 6. Document and Communicate Results A community benefit oversight committee was established to coordinate the Community Health Needs Assessment. This committee along with input from JHSMH Leadership, JHSMH Board Mission Committee, and community leaders provided input into this process. As part of this process Key Health Findings were highlighted and communicated to the various groups above. Based on the assessment, as well as further discussions with various JHSMH team members, the following priorities were identified and action plans has been developed for 2009-2010. Cardiovascular Disease Diabetes Cancer Mental Health/Violence Prevention
PATIENT EDUCATION OF ELIGIBILITY FOR ASSISTANCESCH H, PART VI, QUESTION 31. Charity care policies and procedures will be in place at all JHSMH-owned facilities to address any instances in which patients may be eligible for financial assistance when accessing services. 2. Signs will be posted in the access areas advising patients of the availability of financial assistance. Staff members will communicate the contents of the signs to people who do not appear to be able to read. 3. Patients receiving emergency services will be treated in accordance with JHSMH emergency services policy, developed in accordance with EMTALA and other requirements. Upon registration, patients without Medicare/Medicaid, other local health care financial assistance or adequate health insurance will receive either (1) a packet of information that addresses the financial assistance policy and procedures or (2) immediate financial counseling assistance from staff. The packet will clearly indicate that JHSMH provides care, without regard to ability to pay, to individuals with limited financial resources, and explain how patients may apply for financial assistance. In instances where there are a significant number of patients not proficient in reading, writing or speaking English, additional information will be provided to complete necessary forms. 4. Upon registration, a non-emergency patient scheduling an admission or other procedure without Medicare/Medicaid, other local health care financial assistance or adequate health insurance shall receive either (1) a packet of information that addresses the financial assistance policy and procedures, including an application for such assistance, or (2) immediate financial counseling assistance from staff, including the presentation of the application for financial assistance. JHSMH's Patient Access personnel should begin the process to assess financial ability as soon as patients contact the hospital to schedule a procedure or when they register as a patient.

PROCESS FOR DETERMINING COMPENSATIONFORM 990, PART VI, SECTION B, LINE 15A & 15B------------------------- The following review and approval process is used by Jewish Hospital & St. Mary's HealthCare, Inc. (JHSMH) for all Disqualified Persons, which includes the CEO and all Senior Vice Presidents. The Management Review Committee consists of approximately 9 independent members of JHSMH's Board, and is responsible for review and approval of top management compensation. The full committee addressed the overall compensation philosophy and how the organization's goals can be furthered by the structure of compensation. The independent Management Review Committee and all independent members of the full Board approved maximum compensation levels for all Disqualified Persons. Non-independent directors did not participate in the discussion or vote on compensation levels for individuals. The Management Review Committee retained an outside compensation consultant from a national firm to advise it regarding reasonable compensation levels, and relied on the consultant's opinion that compensation levels are reasonable. The Committee receives a market review on each Disqualified Person from the compensation consultant. The Management Review Committee approved the peer group of similar entitles used to measure comparable compensation levels.
