Costing Method UsedPart I, Line 7: The cost to charge ratio as derived from worksheet 2 was used as the costing methodology used to calculate the amount used to calculate the amounts in the table of line 7.CostingMethodUsed
Bad Debt FootnotePart III, Line 4: The Hospital provides a wide variety of health care services to patients within the community in order to ensure access to appropriate care for populations in need. The Hospital, grant credit without collateral to their patients, many of whom are local residents and are insured under third-party payor agreements. The Hospital provides care without charge or at amounts less than established rates to patients who meet certain criteria under the Hospital's charity care policies. Because the Hospital does not pursue collection of amounts determined to qualify as charity care, they are not reported as revenue.BadDebtFootnote
Shortfall As Community BenefitPart III, Line 8: Cost reporting principles are used to determine allowable and non-allowable costs based on rule and regulations. The Medicare shortfall is the subsidized amount of care provided to patients and should be considered a community benefit.ShortfallAsCommunityBenefit
Debt Collection PolicyPart III, Line 9b: The Hospital provides care without charge or at amounts less than established rates to patients who meet certain criteria under the Hospital's charity care policies. The Hospital's collection policy exempts these individuals and patients from any collection or billing procedures pursuant to the Commonwealth of Massachusetts regulations and policies. The Commonwealth of Massachusetts operates a "health safety net" to reimburse hospitals for the cost of uncompensated care, defined as charity care, and bad debts associated with emergency services. Amounts are paid to the health safety net based on a percentage of private sector charges. Reimbursement from the health safety net is a flat amount per visit or discharge, determined prospectively from a Medicare-based reimbursement model.DebtCollectionPolicy
Needs AssessmentPart VI, Line 2: In accordance with the Massachusetts Attorney General's Community Benefits Guidelines for Non-Profit Hospitals, The Hospital conducted a Needs Assessment. Massachusetts law also requires that all non-profit hospitals and health plans submit an annual Community Benefits Report that details the extent to which these institutions provide charity care and support community health activities in their service areas. The Hospital conducts a periodic community health needs assessments that clarify the leading health concerns, service gaps, and barriers to access for their patients. The primary aim of these needs assessments are to help ensure that hospital and health plan community benefit activities are targeted appropriately. In the Spring of 2009, The Hospital engaged an outside consulting firm to assist with the community health needs assessment. The needs assessment compiled and analyzed secondary data drawn from existing federal, state, and local sources as well as analyzed primary data that the Hospital collected through surveys administered to patients in clinic waiting rooms. The needs assessment process explored issues related to: health care access and preventive services; maternal and child health status; the prevalence of chronic disease, mental illness, and substance abuse, and the extent to which patients are involved in risky behaviors. Then the information collected was analyzed and reported. The goal of the needs assessment was to address both quantitative and qualitative health care needs of the community.NeedsAssessment
Patient Education AssistancePart VI, Line 3: The Hospital's policy is that no individual will ever be refused care. Care is provided regardless of age, race, sex, religion, nationality, or ability to pay. The policy is posted on signs throughout the Hospital along with signs informing patients of the availability Public Assistance & MassHealth programs are posted at all Registration points, the Emergency Department, Cashier's Window, and Financial Counselor offices. All signs are large enough that they are clearly visible and legible by patients visiting these areas. Signs are in both English and any other language spoken by more then 10% of the population in the Hospital's service area. The Hospital also provides care to the elderly and the indigent under the Medicare and Medicaid programs, granted at a discount, which is generally less than cost. The proportion of services provided to Medicaid patients approximates the proportion of Medicaid beneficiaries living in the medical center's service areas. In addition, the Hospital pays into the Health Safety Net for hospitals within the Commonwealth of Massachusetts. Throughout 2010, the Hospital continued to enroll and provide referral and health care services for the indigent, underinsured and uninsured in our community. The Hospital recognizes the delivery of care to those in need as an integral part of our mission and we will continue to focus on meeting the health care needs of this population going forward.PatientEducationAssistance
Community InformationPart VI, Line 4: Saint Anne's is a 160-bed, acute-care community hospital, located in Fall River, MA. St. Anne's primary service area includes Fall River, Swansea, Somerset, and Westport, MA; and Little Compton, Portsmouth, and Tiverton, RI. The Hospital has a rich history of addressing the needs of our local communities by serving those without ready-access to health care and by providing a wide range of services that meet the needs of Fall River's urban poor. Our commitment to the individuals we serve is more important than ever given community statistics on unemployment and poverty which are among the highest in Massachusetts.Saint Anne's serves a disproportionate number of the impoverished, minority, working poor, and medically underserved income. The most recent census data reports that one in five people in Fall River live below the poverty line and the city's per capita income trails the state average by one-third. Fall River's poverty rate is 21.3 percent, up from 16.1 percent in 2005 and more than double the state average, which stayed relatively flat at 10.3 percent. In addition, 35.6 % of children under 18 live below the poverty line. In January 2010, Fall River's unemployment was the highest in the state at 17.2% and hovered at between 14-16% throughout the remainder of the year.CommunityInformation
Community Building ActivitiesPart VI, Line 5: St. Anne recognizes community building activities as part of it's mission to provide health care services in the local community it serves. St. Anne's building activities consist of the following:Leadership Development & training for community members: A bi-lingual Community Resource Liaison (CRL) continued the hospital's long-standing effort to improve access to healthcare for the working poor, elderly, those with language barriers, those without transportation and more. Through increased outreach to small businesses, adult daycare centers, senior centers, churches and health fairs and through appointments in her hospital office, the advocate assisted the uninsured and underinsured in enrolling in available insurance products by providing direct help with the many required application forms and the filing of these for enrollment. When appropriate, assistance in scheduling, coordinating interpreter services, and providing referrals or transportation to health care providers and services, is offered.Coalition building: St. Anne's program of "Promoting wellness of vulnerable populations" came from the results of their community health needs assessment which showed the leading cause of hospitalizations in our county to be mental health and substance abuse. For our PSA, Fall River had significantly higher rates for alcohol, heroin, injection drug use, and marijuana admissions. Hospitalizations for alcohol and substance abuse were significantly higher than the state. Saint Anne's commitment to connecting these at risk individuals with the services needed to aid in their immediate and long-term health and well-being is demonstrated though its Project Assert program. Through its Health Promotion Advocate, Project Assert provides screening, intervention, advocacy and referrals to treatment or services for patients and community members who are screen and detected for substance, alcohol and tobacco use and mental illness. The Advocate is based in the Emergency Department and screens emergency patients at various stages of their care. She is a direct liaison to a multitude of community agencies and services allowing her to provide a continuum of care and smooth referral, placement and follow-up for patients and community members. Referrals are made to detoxification facilities, substance abuse hospitals, intensive outpatient programs, individual counseling, shelters, health insurance providers, primary care physicians and state programs for food, fuel and housing assistance. Project ASSERT has become a catalyst in the community to linking behavioral health providers together in collaboration to provide a regional network of service. Physicians, Behavioral Health providers, The Department of Children and Families, shelters, and community partners are making direct referrals to SAH Project ASSERT. This has led to enhanced access for vulnerable populations seeking services related to the treatment of behavioral and medical conditions. Follow-up services on an ongoing basis have resulted in long term success stories which have been documented and supported by dozens of letters, cards, phone calls, and awards to the program.Other - Food shelter: The city of Fall River continues to have a lower median wage than the state average and a higher percentage of elderly and other individuals who are dependent on some form of public assistance. For many families and individuals, buying sufficient, nutritious food is often not possible. Recognizing that poor nutrition can lead to a host of health problems, the hospital helped to launch the Fall River Food Pantry more than a decade ago and the pantry remains community-wide initiative to feed the city's hungry. Open several times a week, at a church close to downtown Fall River, Food Pantry staff dispenses approximately 12,000 bags of groceries annually. In 2010, Saint Anne's was informed of a growing need for food assistance from individuals and families served by Marie's Place, a free clothing distribution center located in the south end of Fall River, an area known for higher levels of poverty compared to the city. Marie's Place was founded in 1987 by the Dominican Sisters of the Presentation and a group of volunteers. Rising unemployment and poverty created additional hardships for families who turn to Marie'sPlace for assistance. In an effort to ensure a wider distribution of its food subsidy and assist vulnerable individuals and families with receiving healthy and adequate food, Saint Anne's provides monthly subsides to The Fall River Food Pantry and Marie's Place. In addition, the Saint Anne's sponsors, prepares and serves dinner to those served through Third Baptist Church Soup Kitchen.CommunityBuildingActivities
