IdentifierIdentifierReturn ReferenceReturnReferenceExplanationExplanation
Description of Intended Use of Endowment Funds:Part V, Line 4:Endowment funds are created to support the healthcare ministry of St. John Health System Hospitals. The distributions from an endowment provide a dependable source of income each year to help St. John continue to meet the community's healthcare needs.
Description of Uncertain Tax Positions Under FIN 48:Part X:The member health care entities of the System are primarily tax-exempt organizations under Internal Revenue Code Section 501(c)(3) or 501(c)(2), and their related income is exempt from federal income tax under Section 501(a). The System accounts for uncertainty in income tax positions by applying a recognition threshold and measurement attribute for financial statement recognition and measurement of a tax position taken or expected to be taken in a tax return. During the year ended June 30, 2011, the System recognized other revenue of approximately $9,000,000 related to medical resident FICA refunds that had been remitted to the Internal Revenue Service (IRS) during years ended prior to March 2005. Such amounts represent management's estimate of claims submitted to the IRS related to the IRS acceptance of residents' qualification as students for purposes of determining Medicare and social security tax.

Part I, Line 4bThese executives participate in a program that provides for supplemental retirement benefits. The payment of benefits under the program, if any, is entirely dependent upon the facts and circumstances under which the executive terminates employment with the organization. Benefits under the program are unfunded and non-vested. Due to the substantial risk of forfeiture provision, there is no guarantee that these executives will ever receive any benefit under the program. Any amount ultimately paid under the program to the executive is reported as compensation on Form 990, Schedule J, Part II, Column B (I) in the year paid. Amounts Paid: Diane Radloff - $6,129

Form 990, Part VI, Section B, line 11Management, including certain officers, works diligently to complete the Form 990 and attached schedules in a thorough manner. Management presents the form to the board, or a designated committee, to review and answer any questions. Prior to filing the return, all board members are provided the Form 990 and management team members are available to answer any board member questions.
The organization regularly and consistently monitors and enforces compliance with the conflict of interest policy in that any director, officer, key employee, or member of a committee with governing board delegated powers, who has a direct or indirect financial interest, must disclose the existence of the financial interest and be given the opportunity to disclose all material facts to the directors and members of the committees with governing board delegated powers considering the proposed transaction or arrangement. The remaining individuals on the governing board or committee will decide if conflicts of interest exist. Each director, officer, key employee, and member of a committee with governing board delegated powers annually signs a statement which affirms such person has received a copy of the conflict of interest policy, has read and understands the policy, has agreed to comply with the policy, and understands that the organization is charitable and in order to maintain its federal tax exemption it must engage primarily in activities which accomplish its tax-exempt purpose.Form 990, Part VI, Section B, line 12c
In determining compensation of the organizations' CEO, executive director, or top management official, the process included a review and approval by independent persons, comparability data and contemporaneous substantiation of the deliberation and decision. Management reviewed and approved the compensation. In the review of the compensation, the CEO, executive director, and top management were compared to the other organizations in the area that hold the same title. Wage bands are adjusted annually and wages paid for all positions are within those bands. In determining compensation of other officers or key employees of the organization, this question is most appropriately answered as N/A as there are no officers or key employees.Form 990, Part VI, Section B, line 15
The organization will provide any documents open to public inspection upon request.Form 990, Part VI, Section C, line 19

Charity care Nearly 11% of Michigan residents or 1.1 million people lacked health insurance in 2011. In Detroit, 22% of residents are uninsured; this figure remains high due to increased layoffs, fewer companies offering health insurance and a lessened ability of workers to afford coverage in the metro Detroit area. In 2011 at 11.5%, St. John Providence Health System's service area has one of the highest rates of unemployment in the nation. The uninsured are likely to seek care in hospital emergency rooms. St. John Hospital and Medical Center had 146,377 emergency department and urgent care visits in fiscal 2011. Increasingly, patients are experiencing limited abilities to pay for health care services or qualify for state and federally funded programs like Medicaid and Medicare programs that cover only a portion of the cost of services provided. St. John Hospital and Medical Center offers various free clinics. For many Michigan residents, finding someone to care for them when they are sick is not as easy as making an appointment. Lack of health insurance, transportation issues and language barriers combine to create challenges.COMMUNITY BENEFIT REPORT
