Form990 Schedule H Part VI Form990ScheduleHPartVI (11 entries)

ExplanationExplanation
Part I, Line 7: The cost of providing charity care, means tested government programs, and community benefit programs is estimated using internal cost data, and is calculated in compliance with Catholic Health Association ("CHA") guidelines. The organization uses a costing accounting system that addresses all patient segments (for example, inpatient, outpatient, emergency room, private insurance, Medicaid, Medicare, uninsured, or self pay). The best available data was used to calculate the amounts reported in the table. For the information in the table, a cost-to-charge ratio was calculated and applied.
Part I, Line 7, Column (f): The Bad Debt expense included on Form 990, Part IX, Line 25, Column (A), but subtracted for purposes of calculating the percentage in this column is $ 29912533.
Part II: Saint Thomas Hospital continues to be a leader in Community health improvement and advocacy by supporting access to healthcare services through local clinics, the Mental Health Association, the Dispensary of Hope, community health events, rural area medical missions, and the YWCA of Nashville Domestic Violence Initiative. The partnership with the YWCA began in 2009 with a grant from the Office of Criminal Justice. The grant was used to pay for the subcontracting fees at the YWCA Domestic Violence Shelter, public service announcements, and educational materials so that the YWCA community educators could conduct monthly lunch and learns at all four hospitals in the Saint Thomas Health system. Though the grant ended in April 2011, Saint Thomas Hospital has committed to supporting the YWCA Domestic Violence Shelter and inviting the YWCA community educators to conduct lunch and learns through the STH Domestic Violence Awareness Task force. Saint Thomas Hospital also makes significant contributions to health care workforce development in collaboration with Metro-Davidson Public Schools and the PENCIL Program. Through donations to the PENCIL program, STH sponsors a number of activities at Maplewood High School Sciences Academy. STH also hosted Science Scholars again this year - a program offered to high school sophomores who show interest in health-related careers. Over the course of six Saturdays, these students get first hand experience with various medical specialties. Saint Thomas also hosts Jobs in Healthcare, a program providing education, training and employment opportunities to unemployed or underemployed individuals.Saint Thomas Hospital associates represent the hospital on community board for the American Diabetes Association, CCSI, and more
Part III, Line 4: The provision for bad debts is based upon management's assessment of expected net collections considering economic conditions, historical experience, trends in health care coverage, and other collection indicators. Periodically throughout the year, management assesses the adequacy of allowance for uncollectible accounts based upon historical write off experience by payor category, including those amounts not covered by insurance. The results of this review are then used to make any modifications to the provision for bad debts to establish an appropriate allowance for uncollectible accounts. After satisfaction of amounts due from insurance and reasonable efforts to collect from the patient have been exhausted, the organization follows established guidelines for placing certain past due patient balances with collection agencies, subject to the terms of certain restrictions on collection efforts as determined by the organization. Accounts receivable are written off after collection efforts have been followed in accordance with the Organization's policies. The organization's share of bad debt expense in 2011 was $29,912,533.
Part III, Line 8: Saint Thomas Hospital follows the Catholic Health Association ("CHA") guidelines for determining community benefit. CHA community benefit reporting guidelines suggest that Medicare shortfall is not treated as community benefit.
Part III, Line 9b: The organization has a written debt collection policy that also includes a provision on the collection practices to be followed for patients who are known to qualify for charity care or financial assistance. If a patient qualifies for charity or financial assistance certain collection practices do not apply.
Part VI, Line 2: Saint Thomas Health (STHe), which includes Saint Thomas Hospital ("the hospital") most recently surveyed the needs of its own communities in 2009, and finalized a Community Benefit Plan in 2010. This information was supplemented by focus groups and interviews conducted by Ascension Health the prior year, the national Catholic health care organization of which STHe is a constituent part. Consultants from Ascension Health conducted interviews with over 100 community based heath leaders. For the Nashville's broad service area, Ascension recommended that STHe focus on "expansion of clinic-based services for the poor," "engaging local and regional physicians to provide greater care for the poor and uninsured," and "expansion of the Middle Tennessee Safety Net Consortium (MTSNC) and the volunteer specialty panel." Then, consultants from Ascension Health conducted focus groups of 67 key community informants and literature review of safety net models in order to make recommendations regarding the expansion of the Middle Tennessee Safety Net Consortium. As a result, Ascension's consultants suggested that STHe focus on fortifying the Safety Net to include attention to chronic disease, support for the underlying causes of health needs, access to specialty care, adult dental care, mental health, respite care particularly for the homeless, access to pharmacy services for the uninsured and care coordination. Saint Thomas Health took these recommendations very seriously. In addition, associates from Planning, Mission Services, and Advocacy collaboratively gathered health information from national and state health organizations to understand the most prevalent unmet health needs of the state. Data from local health organizations were also collected regarding each of the three counties in which STHe has hospitals: Davidson, Rutherford and Hickman. In order to ensure that outreach efforts effectively address those health needs; a team of representatives from Planning, Mission Services, Advocacy, clinics, Physician services, the Foundation, Dispensary of Hope, Finance, and Marketing was convened at a half-day retreat to agree upon a set of recommendations from which each hospital would develop its community benefit plan. Having found the combination of focus groups and community health data to be fruitful, STHe associates in Mission services, Planning and Advocacy are once again compiling national, state and local community health data and planning focus groups to better understand the experienced health needs of communities.
Part VI, Line 3: Education of eligibility for assistance at Saint Thomas Hospital begins at registration with signage displayed at all registration points notifying patients that we have a Financial Assistance program, as well as contact information for the patient to use should they need assistance. Brochures containing this information are also available for patients at all registration points. Saint Thomas Hospital's registration associates discuss the estimated balance that will be due with both insured and self-pay patients. Should the associate identify that the patient may need assistance with that balance he/she will provide the patient with a Financial Assistance application and help them complete it, if needed. All self-pay and underinsured patients are screened by Saint Thomas Hospital for alternative coverage including Medicaid, SSI/SSD, Cobra, student coverage, and crime victims' compensation. If no alternative is identified then an associate will work with the patient to complete a Financial Assistance application. Also, contact information is listed on Saint Thomas Hospital billing statements for the patient to use in the event they need assistance with their balances. Collections associates have been trained to offer patients a Financial Assistance application if needed by the patient. Saint Thomas Hospital works with third party collection vendors who are required to follow the hospitals Financial Assistance policy. Third party vendors are also used to perform "charity scoring", a process that screens incomplete Financial Assistance applications to qualify patients for presumptive charity.
Part VI, Line 4: Located in Nashville, Tennessee, Saint Thomas Hospital primarily serves residents of Davidson County and adjacent surrounding counties of Williamson, Sumner, Rutherford, Cheatham, Wilson, Dickson, and Robertson in Middle Tennessee. About 650,000 people live in Davidson County alone. At least 15% of those persons are living in poverty according to federal agencies whose standards for measuring poverty are much lower than this hospital's. The majority of persons are non-Hispanic white (about 67%) and black (about 27%) and between the ages of 19-64 (67%). The most common causes of premature death in this age group are injuries and cancer. For those persons between 19-24 homicide is also a common cause of death. For those persons between 25-65 heart disease is also a cause. One quarter of the population are children 18 years of age and under. The 2009 CHNA noted that Tennessee is among the unhealthiest states ranking 44th out of 50 in the 2009 America's Health Rankings. In 2010, Tennessee moved up to 42nd, but still ranking among the states with the weakest health outcomes. The CHNA also found that the prevalence of preventable chronic disease such as heart attack, stoke, cancer, and diabetes in the communities that Saint Thomas Hospital serves were high.According to America's Health Rankings in 2010, Tennessee continues to be among states with the highest prevalence of smoking, obesity and diabetes. Prevalence of smoking is about 23% of non-Hispanic white and black, Hispanic, and multicultural adults. Prevalence of obesity has been increasing steadily over the past ten years to about 40% of non-Hispanic black and Hispanic adults, and 30% of non-Hispanic white and multicultural adults. Prevalence of diabetes is about 11% among non-Hispanic white and black populations. In addition, America's Health Rankings 2010 reports that Tennessee continues to suffer from high incidence of sick days per month and high violent crime rates. Other challenges to Tennessee's health outcomes include high infant mortality rates and a high rate of cancer deaths. The greatest concerns to Saint Thomas Hospital are the rates of obesity, which is a strong determinant for poor health outcomes, and smoking, which is related to the high incidence of cancer deaths and other poor health outcomes. Metro Social Services reports that workforce and economic opportunity, housing and related assistance, as well as food and nutrition continue to be the greatest unmet health needs in the Metro Nashville/Davidson County. However, Tennessee has shown the greatest decrease in cardiovascular deaths; greatest increases in cholesterol checks, and lowest incidence of high cholesterol according to supplemental measures. The state also demonstrated a slight improvement in the already low prevalence of binge drinking, 8.8%. Tennessee also showed a slight improvement in high school graduation, known to be a determinant for health outcomes. The state showed a slight decrease in violent crime, prevalence of smoking, and number of preventable hospitalizations of Medicare patients; all major challenges to health outcomes in the state. Tennessee was among the states that showed the greatest decrease in children living in poverty, and very high rates of immunization coverage for children, about 94%. America's Health rankings notes that Tennessee has a "ready availability of primary care physicians."
Part VI, Line 6: Saint Thomas Hospital operates an open emergency room open to all persons, regardless of ability to pay. The hospital also has an open medical staff with privileges available to all qualified physicians in the area. The majority of the hospital's governing body is comprised of independent persons who represent diverse aspects and interests of the community. The hospital participates in Medicaid, Medicare, TennCare, CHAMPUS, Tricare, and other government sponsored health care programs. Community organizations have access to the hospital's conference rooms at no charge. Patients and families in need have access to numerous support services to individuals and families who are unable to pay such as free, sliding scale and discounted overnight hotel lodging at the Seton Lodge and Inn at Saint Thomas for families who traveled long distances to visit loved ones. Saint Thomas Hospital makes available meal tickets, taxis, bus tickets and other transportation services to families in need. Patients and families also have access to the Irene and Melvin Lewis fund which assists patients who are financially unable to provide for needed medical-related expenditures. The Renewing Hope Fund supports victims of disaster both domestically and internationally, like the flood of May 2010. Understanding the importance of dental care, especially for cardiac and transplant patients, the Cecilia Nolan Roy Dental Fund provides dental care for needy patients. The Women's Health Services Fund supports prenatal classes for patients at the Saint Thomas Family Health Centers. There is also a fund available to employees of Saint Thomas Health who are experiencing hardship. This fund, the Sister Juliana Beurlein Employee Financial Assistance Fund offered over $100,000 in assistance in FY2011. Saint Thomas Hospital associates go above and beyond the standards of care to provide assistance with applications for access to public and private support services. Associates have made a habit of charity by frequently organizing drives and participating in local health awareness events. Associates collected carloads of household goods for women and families at the Domestic Violence Shelter. Other associates made knit caps for dozens of chemo patients. Our chaplains gave their time to funerals for long time patients whose families could not afford a service.
Part VI, Line 7: Saint Thomas Hospital is part of the Nashville Health Ministry of Ascension Health, the largest Catholic and not-for-profit health system in the United States and is known locally as Saint Thomas Health (STHe). STHe is dedicated to providing accessible, quality healthcare to all patients regardless of their ability to pay. STHe owns and operates four hospitals in Middle Tennessee including Baptist Hospital and Saint Thomas Hospital in Nashville, Middle Tennessee Medical Center in Murfreesboro, and Hickman Community Hospital in Centerville, Tennessee. In addition, STHe provides a variety of ambulatory services through the operation of an urgent care center, community primary care clinics, employment of and close alignment with physicians, and partnerships to provide diagnostic imaging, sleep lab, and ambulatory surgery services. As a system, STHe supports a variety of efforts specifically aimed at providing services for persons who are poor. For example, in fiscal year 2011 Saint Thomas Health organized another rural area mission. Over six hundred volunteers from all of the Saint Thomas Health entities participated and offered health screenings, referrals, consultations, dental care, eye exams, glasses, health education, and a health ministry presence to persons living in rural areas who have limited access to health care. STHe also operates primary care clinics and programs that provide many thousands of patient encounters each year. STHe also advocates for healthcare access and coverage for all persons through sponsorship of community events and presentations and booths to increase awareness and educate the public regarding healthcare reform and access. Saint Thomas Health helped to form the Safety Net Consortium of Middle Tennessee to address the needs of the uninsured residents of Middle Tennessee. Saint Thomas Health Foundation serves as the legal and fiduciary agent to the Consortium. The Board of Directors that governs the Consortium is comprised of health care providers and local business leaders who serve the poor and uninsured. STHe created The Dispensary of Hope, a national distribution center and pharmacy, to support a network of dispensaries and direct-to-patient prescription solutions that provided millions in available medications for patients unable to afford their prescriptions. Saint Thomas Health also supports a variety of efforts to address persistent health needs in the community. For example, STHe is a founding partner and primary supporter of Camp Bluebird. The director of the camp is an associate of Saint Thomas Hospital who draws volunteers from other hospitals in the Saint Thomas Health system as well. Camp Bluebird is the first camp in the Middle Tennessee area designed for adult cancer patients. For three days and two nights volunteers from Saint Thomas Health provide campers with education, support and encouragement in living life after a cancer diagnosis. Saint Thomas Health also partners with other health care organizations to address the persistent unmet health needs of the community. In fiscal year 2011, Saint Thomas Health made a significant contribution to a diabetes improvement project of Vanderbilt-Meharry.

Certain financial figures unique to non-profit healthcare organizations represent this commitment to delivering the highest quality of health care to those who are most poor and vulnerable, including those who are elderly and those who live below the federal poverty guidelines. Saint Thomas Hospital provides the highest quality of care to all persons, regardless of their ability to pay. STH cares for many persons who are elderly as well as those who are living with disabilities. Additionally, Saint Thomas Hospital cares for patients whose employers do not provide or cannot afford insurance, as well as those who are self employed or for some other reason, otherwise uninsured. In 2011, 65.74% of the value of services rendered were to patients who participate in government-sponsored insurance programs or were uninsured. - Medicaid (other than TennCare) - 0.51% - Medicare HMO - 12.01% - Medicare - 42.75% - Tenncare - 5.63% - Bridges to Care - 0.00% - Uninsured - 4.84% Much of their care is covered by Medicaid, Medicare and other means-tested programs. However, the payment that the hospital receives from these programs often does not cover the actual cost of those health care services. In some cases, hospitals incur losses caring for these patients -that is, the difference between the actual cost of services and how much these programs pay. Losses are also incurred when patients who are uninsured and underinsured, and qualify for a significant discount in the cost of their care under the health system's Financial Assistance Policy, do not have the means to pay the remaining cost of professional health care services. Altogether, these numbers demonstrate Saint Thomas Hospital's commitment to providing the highest quality care to all persons, particularly those who are poor and vulnerable, regardless of ability to pay.
